Objective To compare the effectiveness of Pregabalin given by fixed dose method and flexible dose method on treating central post-stroke pain.Methods Patients with central post-stroke pain were consecutively enrolled and randomized into fixed group and flexible group.The patients in fixed group were given pregabalin 300 mg per day for 8 weeks.The patients in flexible group were given pregabalin as follows:150 mg per day for 2 weeks, 300 mg per day for 2 weeks and 450 mg per day for 4 weeks.The primary outcome was numerical rating scale ( NRS) . The secondary outcomes included adverse effects, hospital anxiety and depression scales ( HADS ) and Athens insomnia scale ( AIS) .Results A total of 132 participants were enrolled with 65 in fixed group and 67 in flexible group from Jul 2009 to Dec 2014.Two patients in fixed group gave up the treatment while 7 patients gave up in flexible group though no significant difference was observed.Finally, 63 patients in fixed group and 60 patients in flexible group were included into analysis.There was no significant difference on gender, age, medical history between two groups. Compared with baseline, the NRS、HADS-A、HADS-D and AIS score were significantly decreased ( all P<0.05 ) .However, there was no significant difference of these scores between the two groups. Conclusion Two regimens of pregabalin showed the similar efficiency and safety to treat CPSP, however, fixed dose seemed to be more acceptable.

Objective To evaluate the correlation between fluid balance and myocardial function and the mortality in patients with septic shock.Methods Ninety two septic shock patients enrolled between January 2013 and January 2015 were divided into survival group and non-survival group based on 28-day mortality.Fluid balance,haemodynamic parameters and B-type natriuretic peptide (BNP) were collected on days1,3,7 after admission.Results Forty four patients survived the crisis while 48 died.The 7 day average fluid balance volume in survival group was lower than those in non-survival group [(55 ± 9)ml/kg vs.(187 ± 14)ml/kg,t =54.260,P ＜0.01].The initial left ventricle ejection fraction (LVEF) in survival group was lower than that in non-survival group [(42％ ±4％) vs.(54％ ±6％),t =10.100,P ＜0.01].Serum BNP level was significantly lower than those in non-survival group [(267 ± 54) ng/L vs.(681 ± 22) nig/L,t =48.580,P ＜0.01].Multivariate logistic regression analysis showed that fluid positive balance,LVEF and acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) were the independent predictors for 28 day mortality (P ＜ 0.05).Conclusions Fluid balance and myocardial function can serve as risk predictors for septic shock patients.

Purpose:To detect the expression of a novel i nh ibitor gene of apoptosis,survivin,in gastric cancer and in gastric carcinoma MGC -803 cell line,also to analyze its correlation with the expression of COX-2. Objective To evaluate the value of retroperitoneal laparoscopic adrenalectomy for benign adrenal tumors.Methods From March 2003 to May 2008,78 patients with adrenal tumors received retroperitoneal laparoscopic adrenalectomy in our hospital. During the operation,three trocars were introduced into the lumbar region to establish retroperitoneal space by blunt dissection with the camera. Afterwards,the tumor was removed by using a harmonic scalpel. Results The procedures were successfully completed in all the 78 cases without blood transfusion,the operation time ranged from 45 to 180 min (mean,90 min). Drainage tubes were removed 24 hours after the operation. And the patients were discharged from hospital in 3 to 5 days postoperation. One patient showed regional subcutaneous emphysema after the surgery and was cured spontaneously. The patients were followed up for 3 to 24 months with a mean of 10 months,during which 36 patients achieved normal blood pressure in 3 months,while in the other 7 patients,oral antihypertension drugs were still needed. No patients had recurrent tumor during the follow-up. Conclusions Retroperitoneal laparoscopic can be the first choice for adrenal tumors,as it is safe and effective with minimal invasion,quick recovery,and short postoperative hospital stay.

Objective To investigate the early diagnostic value and prognostic significance of serum presepsin in patients with sepsis.Methods A prospective observational study of 80 patients from ICU was carried out between January 2014 and June 2015.According to sepsis criteria,the cases were divided into sepsis group (n =50)and noninfectious systemic inflammation syndrome group (SIRS group,n =30), and another 25 healthy volunteers were collected as control group.Meanwhile,sepsis group was further divided into survival and death subgroups according to 28-day mortality.The dynamic changes of serum presepsin,procalcitonin (PCT),soluble myeloid cells expressing triggering receptor-1 (sTREM-1 ),C-reactive protein (CRP),interleukin 6 (IL-6)were monitored on the 1st,3rd and 7th day after admission. Results (1)The serum presepsin level of sepsis group was higher than SIRS group and control group on the 1st day,and the difference was statistically significant [912.0 (563.5,1514.8)pg/mL vs.462.0 (334.0,556.0)pg/mL vs.120.0 (70.2,320.1)pg/mL,P <0.05];(2)The area under the ROC curve (AUC)of presepsin for the sepsis diagnosis was greater than PCT,sTREM-1,CRP,IL-6.As cut-off point set at 672.5 pg/mL,presepsin measurements showed a sensitivity of 80%,specificity of 96.7%, combination of presepsin,PCT and sTREM-1 could offer better diagnostic confirmation;(3)The level of serum presepsin,PCT and sTREM-1 in death group were higher than those in survival group.The levels of serum presepsin,PCT,sTREM-1 and IL-6 in survival group gradually decreased to normal level following the therapy,but the levels of above markers in death group didn’t decrease significantly;(4)The AUC of presepsin in prognosis of sepsis prediction was greater than that of PCT,sTREM-1,and combination of the above three markers could offer better prognostic value.Conclusions Serum presepsin provided superior diagnostic and prognostic accuracy to PCT and sTREM-1,and the combination of presepsin,PCT and sTREM-1 can offer the reliable guide for sepsis diagnosis and death risk prediction.

Objective To investigate antiproliferative activity of endostatin(ES), the combining effect of Endostar and radiation on VEGF and apoptosis of the A549 human lung adenoearcinoma cell line.Methods The cell of exponential phase of growth were divided into the control, radiation alone, ES alone,radiation after ES(ES→RT), ES after radiation(RT→ES), radiation and ES at equal paee (RT+ES), γ-ray radiation at a dose of 2 Gy, single fracination irradiation, YH-16 10 μg/ml in the cell culture bottle. The combining effect was quantified by the survival curve. ELISA was used to observe the VEGF of radiation with ES on A549 cell lines. Apoptosis was observed by Hoeehst staining. Results There showed that both of ES and combining treatment had the function of antiproliferative. But the union treatment's function was more obvious (P＜0.05). The expression of VEGF in ES and the combining treatment group was more lower than others (P＜0.05). On apoptosis, the rate of apoptosis in ES, RT, combining treatment group was higher than others (P＜0.05). Conclusion On antiproliferative activity and rate of apoptosis, the combining of ES and radiation is better than to use them alone. To reduce VEGF of the A549 human lung adenocareinoraa cell line by ES could enhance radioresponse. There is no evidences to show that the expression of VEGF is concerned with the different choronological order of radiation and ES.

Objective To analysis the DNA sequence of HEV ORF3 cDNA and express in HepG2 cell .Methods Recombinant vector carrying HEV ORF3 cDNA was constructed and identified by DNA sequencing .Expression of recombinant vector in HepG2 cell was determined by immunofluorescence technique .Results The recombinant vector was constructed and expressed in HepG2 cell successfully .Conclusion The research laid experimental foundation for exploring the biological function of ORF3 protein in the pathogenesis of viral hepatitis E .

BACKGROUND:The differentiation of neural stem cells (NSCs) is a hot research. Whether brain-derived neurotrophic factors (BDNF) can induce the differentiation of NSCs into neurons has not been detailed studied. OBJECTIVE:To investigate the effect of different concentrations of BDNF on the differentiation of NSCs from adult rat hippocampus into neurons in the medium with epidermal growth factor. DESIGN:A controlled study. SETTING:Department of Human Anatomy, China Medical University. MATERIALS:The experiment was performed at the Neurotomia Laboratory of China Medical University in August 2007. NSCs isolated from hippocampus of adult SD rats were inoculated in a serum-free medium. Three clean adult SD rats (200-250 g each) were provided by Laboratory Animal Department of China Medical University. Dispositions to the rats were consistent with ethical standards of animals. Epidermal growth factor (EGF) and BDNF were bought from R&D Company. METHODS:①NSCs obtained from rat hippocampus were cultured in the serum-free medium containing basic fibroblast growth factor (bFGF), EGF and B27. ②Single cell clone was obtained via limited dilution for the fourth passage of cells. Identification of NSCs for the passage of cells from monoclonal spheres was performed by Nestin immunocytochemistry. Neurons and astroglial cells were identified by immunocytochemistry for neuron specific enolase (NSE) and glial fibrillary acidic protein (GFAP) a week after differentiation. ③Differentiation for the monoclonal spheres was done to appraisal the proportion of neurons. NSCs were divided for several groups containing 0 μg/L, 50 μg/L, 100 μg/L, 150 μg/L, and 200 μg/L groups according to the doses of BDNF. EGF was added in the media of each group. Immunocytochemistry for NSE was done a week later to count the positive cells. MAIN OUTCOME MEASURES:Proportion of neurons differentiated from NSCs.RESULTS:①NSCs immunocytochemical staining showed that Nestin was positive in monoclonal spheres, and NSE and GFAP were positive in differentiated cells. ②The proportion of differentiation from NSCs into neurons was significantly higher in groups treated with 50 μg/L and 100 μg/L BDNF compared to other groups（t=2.502-5.025, P < 0.05）, but there was no significant difference between 50 μg/L and 100 μg/L BDNF groups（P > 0.05）. Moreover, no significant difference was detected among 0 μg/L, 150 μg/L and 200 μg/L BDNF groups (P > 0.05). CONCLUSION:The optimal concentration of BDNF is 50 μg/L for differentiation from NSCs into neurons while the concentration of EGF is 20 μg/L.
