Objective Adopt the method of brachial plexus block by a noodle biopsy and pressing the root of interscalene,to discuss the effective of this method on the operation of shower and upper limb.Methods Summarize 182 cases of patients who got the upper limb operation,compared the patients adopting a noodle biopsy and pressing the root of interscalene to block,to the cases which un-pressing the interscalene.Evaluate the anesthesia on sense and movement and record the complicating disease.Results It got a satisfied result from a noodle biopsy and pressing the root of interscalene to block.In this case,all of the sense and movement including shower has disappeared,and has absolutely effect on analgesia.All of the patients avoid to be used herbal supplements and however to complicating symptom.Good rate 97％,compared to 73％ cases of un-pressing.Pressing the interscalene to block is better than un-pressing,on feet nerve,axillary nerves,the median nerve and C4 nerve.Conclusion A noodle biopsy and pressing the root of interscalene to block is a effective and simple method,it needn't special equipment and has a absolutely and extensively block result on the shower and upper limb operation.

Objective To evaluate the effects on mechanical ventilation in ICU patients with dexmedetomidine merely and combined with dezocine, fentanyl, respectively.Methods Fifty-seven patients with mechanical ventilation in ICU were collected during Jun till Dec in 2016, 35 males and 22 females, aged 18-75 years, and classified into three groups with random number table method, among whom there were 17 patients treated with dexmedetomidine, 20 patients treated with dexmedetomidine combined with dezocine and 20 with dexmedetomidine plus fentanyl, the general condition, pain score, MAP and HR were compared before and after the treatment, ideal treatment time and mechanical ventilation time were also compared among the different groups.Results There were no statistical differences in the general condition, CPOT score, MAP and HR at different points and mechanical ventilation time.Whereas, the time of ideal sedation and analgesia was significantly faster in group C than that of groups A and B (P<0.05).Conclusion Dexmedetomidine combined with dezocine can reach the ideal analgesic effect more rapidly than dexmedetomidine solely or combined with fentanyl.

Objective To study the signal transduction pathway of norepinephrine (NE)-induced expressions of oncogenes c-fos and c-myc in the rat heart. Methods mRNA levels of c-fos and c-myc in the left ventricle of the Langendorff-perfused rat heart was quantitated by reverse transcription and polymerase chain reaction (RT-PCR). Results NE increased the expressions of c-fos and c-myc while the effects of NE on the expressions of c-fos and c-myc were blocked by ? 1-adrenergic blocker prazosin (5?10 -6 mol/L) completely. Neomycin (5 mmol/L) and ruthenium red (10 ?mol/L) decreased the NE-induced c-fos and c-myc mRNA levels. Conclusion NE can induce the expressions of oncogenes c-fos and c-myc in the ventricle mediated by ? 1-adrenergic receptor. The signal transduction pathway of NE-induced c-fos and c-myc expressions may be associated with inositol phosphates-Ca 2+ system and diacylglycerol-protein kinase C system.

Objective To investigate the feasibility of searching for proteins which interact with intracellular domain of hematopoietic growth factor receptors using yeast two-hybrid system. Methods RT-PCR method was performed to amplify the genes of intracellular domains of G-CSF receptor and EPO receptor in NFS-60 and BET-2 cells of mice. The genes were cloned into yeast expression plasmid pGBKT7 vector,and then transformed into yeast AH109. The yeast proteins were isolated and analyzed with Western blotting. Transcriptional activation was analyzed by the ?-galactosidase colony-lift filter assay. Results The intracellular domains of G-CSF receptor and EPO receptor genes were successfully cloned into pGBKT7 vector. The results of Western blotting assay showed that both proteins were expressed in the yeast cells. The ?-galactosidase colony-lift filter assay demonstrated that G-CSF receptor alone had no activity of transcriptional activation,while the EPO receptor alone could activate transcription. Conclusion The findings suggested that intracellular domain of G-CSF receptor could be used as a bait to find interacting proteins using yeast two-hybrid system,while that of the EPO receptor could not. Therefore the system could not be applied to all hematopoietic factor receptor.

Objective To evaluate the expression of Yes-associated protein (YAP) in colorectal carcinoma and analyze its influence on tumor cell proliferation.Methods The expressions of YAP in 94paired colorectal carcinomas and pericancerous normal tissues were detected by using immunohistochemistry method.The expressions of YAP in colorectal carcinoma cell line HCT116 were inhibited with a YAP-spe-cific siRNA.Cell proliferation was then determined by methyl thiazolyl diphenyl-tetrazolium bromide (MTT) assay.Results The positive rate of YAP in colorectal carcinomas was significantly higher than that in pericancerous normal tissues [69.1％ (65/94) vs 22.3 ％ (21/94),P ＜ 0.001].The expression of YAP was associated with tumor Node Metastasis(TNM) stage and lymph node metastasis(P ＜0.05),but not associated with gender,age,tumor location and histological grade(P ＞0.05).After YAP-specific siR-NA was transfected into HCT116 using lipofectamine,the expression of YAP mRNA and protein in the experimental group were reduced by (78.2 ±2.1)％ and (81.7 ± 1.5)％,respectively,with a statistically significant difference (t =67.55,91.601,P ＜0.01).The growth of HCT116 was significantly inhibited and the reduced rate of cell proliferation was (28.1 ± 1.6) ％,(34.7 ± 2.4) ％ and (24.7 ± 1.2) ％ at the time point of 48 h,72 h and 96 h,respectively.Conclusions Expression of YAP was upregulated in colorectal carcinomas and downregulation of YAP expression could inhibit growth of colorectal carcinoma cells.YAP can be used as a new candidate target for diagnosis and treatment of a colorectal carcinoma.

BACKGROUND:Porcelain jacket crowns have achieved good results in the repair of porcelain ceramic crown and bridge colapse.
OBJECTIVE:To test the bond strength of single porcelain jacket crown, thus determining the feasibility of clinical application of repairing colapsed crown and fixed bridge by porcelain jacket crown, and meanwhile to discuss the influence between blastsanding and silane coupling agent treatment on the metal bonding strength in the repair of porcelain fracture.
METHODS: The porcelain jacket crown was cast in the same shape from disk-shaped waxpatterns. Twenty specimens were chosen without blowholes and casting defects and they were al routinely burnished and cleaned in an ultrasonic bath and dried using an oil-free air stream stand-by. Then we randomly divided the specimens into four groups: specimens were treated with nothing (control), sandblast, silane coupling agent and sandblast+silane coupling agent, respectively. Each of the specimen was bonded with Poly-F Plus. AG-10TA universal testing machine was used to test the tensile strength.
RESULTS AND CONCLUSION:The tensile bond strength of control, sandblast, silane coupling agent and sandblast+silane coupling agent groups respectively were (0.68 0.04), (1.00 0.02), (1.31 0.08), and (1.09 0.04) kN. There were significant differences between groups (P < 0.01). In the control group, the metal surface was smooth, showing some irregular smal superficial veins. In the sandblast group, the metal surface showed irregular scaly protrusions conducive penetrate adhesives. In the silane coupling group, the metal surface manifested a honeycomb structure in favor of chimeric adhesives. The sandblast+silane coupling agent group showed some metal projections ranging in size, on the rough metal surface, which was conducive to bond. Porcelain jacket crown applied to repair colapsed crown and fixed bridge is feasible. Sandblasting and silane coupling agent can improve bond strength of porcelain jacket crown. The bond strength of metal specimens treated with silane coupling agent is the best folowed by the sandblasting+silane coupling agent.

BACKGROUND:Artificial cervical disc prosthesis simulates range of motion and buffer shock function of normal intervertebral discs. Clinical experiments verify that artificial cervical disc prosthesis material has good biocompatibility and mechanical characteristics. OBJECTIVE:To evaluate artificial cervical disc replacement and zero-profile interbody fixation and fusion system for multilevel cervical disease in 2-year folow-up. METHODS:Artificial cervical disc replacement and zero-profile interbody fixation and fusion system were used to treat 42 patients with multilevel cervical disease. The patient presented typical symptoms and signs of spinal cord or nerve root compression. There were 18 cases of cervical myelopathy, 15 cases of nerve root cervical spondylosis and 10 cases of mixed type of cervical spondylosis. After treatment, mean operation time, blood loss and reoperation rate were measured. Postoperative complications, disability index of neck function, visual analog scale, function unit range of corresponding surgery segments of the cervical spine, Cobb angle of C2-C7 vertebral body, range of motion of adjacent segment of proximal and distal vertebral bodies were observed and clinical outcomes were evaluated. RESULTS AND CONCLUSION: Al cases finished the operation and were scored at various time points. After treatment, radiating pain of shoulder and neck and upper extremity were remarkably lessened. Numbness and sensory loss symptoms disappeared obviously. Quality of life elevated noticeably. Visual analog scale and the disability index of neck function score were decreased in final folow-up compared with pre-treatment (P < 0.001). C2-C7 vertebrae Cobb angle, FSU angle, range of motion of proximal surgery adjacent segment and range of motion of the distal surgery adjacent segment were elevated compared with pre-treatment (P < 0.001). These data indicate that cervical spondylosis was improved after treatment. Each index of cervical spondylosis after artificial cervical disc replacement and zero-profile interbody fixation and fusion system was reconstructed to different degrees.

Background:With the development of capsule endoscopy,small intestinal injury induced by non-steroidal anti-inflammatory drugs( NSAIDs)has become an issue of growing concern. Although there are a variety of drugs used for NSAIDs-induced gastric mucosal injury,small intestinal injury caused by NSAIDs is lack of effective prevention and treatment modalities. Aims:To investigate the protective effect and possible mechanism of rebamipide on human colon cancer cell line Caco-2 injury induced by aspirin. Methods:In aspirin group,Caco-2 cells were treated with aspirin 10 mmol/L;in rebamipide group,Caco-2 cells were treated with aspirin and different concentrations of rebamipide(0. 1, 0. 5,1. 0 mmol/L),and a negative control group was established. Cell proliferation inhibition was measured by MTT assay. Cell apoptosis was determined by flow cytometry. Morphological changes of cells were observed under inverted phase contrast microscope. Permeability of cells was assessed by Transwell assay. Expressions of tight junction proteins occludin and zonula occluden-1(ZO-1),as well as mitogen activated protein kinase(MAPK)signaling pathway-associated proteins including extracellular signal-regulated kinase(ERK)1/2,phosphorylated ERK1/2(p-ERK1/2),p38,p-p38,c-Jun N-terminal kinase( JNK),and p-JNK,were determined by Western blotting. Results:Proliferation inhibition rate,apoptosis rate and permeability of Caco-2 cells in rebamipide 0. 1,0. 5,1. 0 mmol/L groups were significantly lower than those in aspirin group in a dose-dependent manner(P<0. 05). Injuries of Caco-2 cells were seen in aspirin group by inverted phase contrast microscope and rebamipide could reduce these injuries. Expressions of occludin,ZO-1 and p-JNK were significantly higher and expressions of p-p38 and p-ERK1/2 were significantly lower in rebamipide 0. 1,0. 5,1. 0 mmol/L groups than those in aspirin group in a dose-dependent manner(P<0. 05). Conclusions:Rebamipide have a protective effect against aspirin-induced Caco-2 cell injury,probably through regulating MAPK signaling pathway( inhibiting p38 and ERK1/2 phosphorylation,stimulating JNK phosphorylation),and subsequently up-regulating the expressions of tight junction proteins and decreasing the permeability of cells.

Objective To investigate the value of application of magnetic resonance imaging (MRI) in diagnosis and treatment of leptomeningeal metastases from malignant solid tumors.Methods A retrospective analysis was conducted depend on the radiological data of 63 patients who had been diagnosed as leptomeningcal metastasis of malignant solid tumors.Results All patients received brain MRI examination.26 patients underwent cervical or lumbar spine MRI examination.The MRI images showed nodular enhancement in the gyri and sulci,leptomeningeal linear enhancement,dura mater thickening and enhancement,ependymal enhancement,intraventricular metastasis,intraspinal metastatic nodules,communicating hydrocephalus,subdural hydroma and negative imaging.Ncuroimages of ependymal enhancenent,intraventricular metastasis and intraspinal implantation metastasis appeared in 5,7 and 9 patients with small cell carcinoma,respectively,which were higher than that of the other pathological types (P =0.002,P =0.009,P ＜ 0.000 1).Seizure occurred in 7 patients after intrathecal chemotherapy,of which 5 patients had pia mater linear enhancement.Among 33 patients treated with radiotherapy combined with intrathecal chemotherapy,3 patients had poor prognosis,of which 2 patients had negative imaging findings.The conditions of 25 patients were improved obviously after treatment,of which 21 patients showed different imaging remission in MRI re-examination.Conclusions The contrast MRI has an important assistant effect in diagnosis of leptomeningeal metastasis of malignant solid tumors.The MRI findings and clinical features are related with the pathological types.The pia mater linear enhancement indicates a high possibility of seizures after inthathecal chemotherapy,and it suggests that the preventive treatment should be given for such patients.MRI is not applicable to evaluate severity,prognosis and curative effect of the disease.

Objective To observe the changes of arm equilibrium pressure (Parm) in different occlusion time,the reference range of Parm in hemodynamic stable patients,and to explore the relationship between Parm and systemic hemodynamic parameters.Methods Mechanically ventilated postoperative abdominal surgery patients who admitted to ICU with stable hemodynamic status were enrolled.After hemodynamic data were recorded,arm stop-flow maneuvers were performed to measure Parm.At 10,20,30,40,50,60 seconds after occlusion,arterial pressure were measured twice within 5 minutes and recorded as the average value.Results Thirty patients were included.The Parm decreased gradually with the prolongation of the occlusion time.The value was not stable within 60 s,but the reducing extent was not obvious after 40s.The 95％ reference range of Parm 30 S was 23-44 mmHg(1 mmHg =0.133 kPa) and Parm 60 S was 19--35 mmHg.Parm at 30 s and 60 s were positively correlated with systolic arterial pressure,diastolic arterial pressure,mean arterial pressure,central venous pressure (all P ＜ 0.001),but not heart rate and pulse pressure.Linear regression analysis showed that Parm at 10 s,20 s,30 s only had linear correlation with diastolic arterial pressure (10 s,β =0.504,P =0.001;20 s,β =0.297,P =0.005;30 s,β =0.231,P =0.015),and Parm at 40 s,50 s,60 s were linear correlation with diastolic arterial pressure (40 s,β =0.220,P =0.004;50 s,β =0.210,P =0.004;60 s,β =0.213,P =0.004) and central venous pressure (40 s,β =0.516,P =0.018;50 s,β =0.513,P =0.01;60 s,β =0.472,P =0.023).Conclusion In mechanically ventilated postoperative abdominal surgery patients with stable hemodynamic status,Parm decreases when occlusion time is prolonged,which is not stable within 60 s occlusion.Arterial blood pressure and central venous pressure are positively correlated with Parm.
