1.Microsurgical treatment of hypothalamic gliomas in children

Objective To evaluate the efficiency of microsurgical methods for treatment of hypothalamic gliomas in children and the management of postoperative complications Methods Twenty-one tumors were classified into two types according to their location, size and extensive direction The tumors were removed with microsurgical technique by various approaches, such as transcallosal-interforniceal or translongitudinal fissure, transubfrontal methods Results Twenty-one cases of hypothalamic gliomas were verified by surgery and pathology The total and subtotal removal was 18 cases Partial removal was 3 cases There was no mortality in this series Major neurological complications included diabetes insipidus, electrolyte disorder and hyperthermia Surgical complications included subdural fluid collections, hydrocephalus, epilepsy Seventeen cases were followed up, and showed satisfying results Conclusion Microsurgical operations through appropriate approaches and reasonable management of complications are important methods for improving the outcome of hypothalamic gliomas in children
2.A case of peripheral T cell lymphomas——unspecified in vertebra canal

SUMMARY Peripheral T cell lymphomas-unspecified(PTCL-U) is an uncommon malignant tumor, accounting for 5%-7% of non-Hodgkin’s lymphoma. Clinical feature of a case of PTCL-U was investigated and the optimal treatment protocol was proposed. The clinical presentation of this case was progressing limbs weakness with sudden paralysis. Multiple superficial lymph nodes swelling and decreased sensation in all modalities below T10 level were found in physical examination. Bilateral leg paralysis (Grade 0/5) with high muscle tension, overactive knee reflex, bilateral Babinski sign (+) were present. Magnetic resonance imaging(MRI) showed the mass, which was around spinal cord located in T6-T8 vertebra canal, with intense enhancement after the administration of gadolinium diethylenetriamine penta-acetic acid. Abnormal signal in multiple centrums and accessories could also be found in MRI scan. Initial diagnosis was lymphoma, multiple systems involved. Laminectomy and epidural tumor resection were performed immediately. The dural sac was narrowed obviously by surrounding tumor. The result of pathological examination was PTCL-U. After operation, the level of pain sensation was descent, and touch sensation recovered. The patient received chemotherapy and his muscle strength was partly recovered in 4 months. Early diagnosis of peripheral T cell lymphomas-unspecified was difficult and easily misdiagnosed with poor prognosis. A review of literature showed central nervous system lesions occurred in advanced stage. However, the report of representation of intraspinal mass as initial symptom like the case was rare. For acute paraplegia, laminectomy and tumor resection should be performed as soon as possible. Regular radiotherapy and chemotherapy should be considered after operation. However, the value of local chemotherapy need to be further investigated.
3.Suicide Prevention, Suicide Attitude and Related Factors among Undergraduates in Changsha

Objective:To understand suicide prevention knowledge, suicide attitude and related factors among undergraduates in Changsha.Methods:Using multistage stratified cluster sampling, a total of 1664 undergraduates from six universities in Changsha were tested with Suicide Awareness Questionnaire, Suicide Attitude Questionnaire (QSA), Adolescent Self-Rating Life Events Check List (ASLEC), Social Support Questionnaire (SSQ) and Symptom Checklist 90 (SCL-90). Results:The mean score of suicide awareness among undergraduates was 10.8?0.9 and the correct response rate was 54.5%. The result of suicide awareness score showed, female undergraduates had a lower score than males (9.2?0.8, 12.3?0.8;t=2.33, P=0.020), medical undergraduates had a higher score than ones of humanities and social science or engineering (13.9?0.8, 10.0?1.0, 9.7?0.7;F=10.47, P=0.000), undergraduates from one-child families had a higher score than ones from non-one-child families (11.9?0.7, 9.1?0.9;t=3.25, P=0.034), and college students from nuclear family had a higher score than ones from extended families or one-parent families (14.0?0.9, 10.3?0.8, 9.4?0.7;F=7.49, P=0.000). The major factors related to suicide awareness were gender, specialty and family type (?=0.087, 0.342 and 0.084, respectively). And the major factors related to attitude towards attributes of suicide behaviors were specialty, gender, suicide ideation and mental health status (?=0.688, -0.055, 0.810 and 0.731, respectively). Conclusions:College students in Changsha have severe misunderstands about suicide prevention. It is necessary to pay more attention to the education of suicide knowledge and to cultivate the correct suicide attitude among college students.
4.Surgical Treatment of 660 Consecutive Cases of Spinal Cord Tumor

Objective The purpose of this study was to evaluate the outcome of surgical treatment for spinal cord tumors. Methods We retrospectively analyzed a series of 660 patients with 676 intra-spinal tumors who were treated surgically from February 1993 to December 2007. The tumors located at the cervical level in 231 patients,thoracic in 202,lumbar in 159,sacral in 9,cervico-thoracic in 16,thoracico-lumbar in 42,and lumbar-sacral in 17. Totally 699 operations were performed,including posterior bilateral laminectomy approach in 599,unilateral laminotomy in 71 and laminoplasty in 6 cases.Besides,23 additional operations via lateral or ventral-lateral were performed for resection of extra-spinal component of dumbbell shaped tumors. Results In this series,Schwannomas and meningiomas were the most common extramedullary tumors accounting for 76.5% of the cases [517/676,rate of total resection: 92.3% (477/517)]. The intra-medullary tumors accounted for 23.5% (159/676),in which astrocytomas and ependymomas were the most often observed [rate of total resection: 67.3% (107/159)]. The peri-operative mortality was 1.7% (11/660). The motor and sensory functions were found to be improved after operation in 77.8% (357/459) and 79.2% (397/501) respectively. The sphincter function was improved in 72.4% (215/297). The rate of pain relief was 93.3% (361/387). The follow-up periods ranged from 0.5 to 15.1 years in 582 patients with an average of 5.9 years;during the period,87.8% (499/568) of the patients had an ASIA impairment scale E neurological function. Conclusions Intra-spinal tumors locate mostly in the subdural-extramedullary space,with a majority are Schwannomas and meningiomas. The widespread application and availability of MRI investigation provide the physician with accurate localization of the lesion thereafter ensure a minimally invasive surgical treatment strategy keeping the stability of the spine while removing the tumors with an satisfactory outcome.
5.EFFCT OF POLYSACCHARIDE FROM THE SPIRULINA PLATENSIS CULTIVATED IN CHINA'S YUNNAN PROVINCE ON THE D-GALACTOSE AGING MODEL MICE

With the dose of D-galactose 80mg/kg by sc administration to the female NIH mice for 6 weeks,the sub-acute aging model was established. In order to observe the effect of the polysaccharide (PSP) on anti-aging, the mice were received PSP 200mg/kg^ 100mg/kg respectively by ig administration at the same time of injection D-galactose. It was found that PSP could remarkably antagonise the effect of D-galactose on the mice. The results showed that PSP could significantly(1)decrease the content of MDA in heart,liver and brain of the mice;(2)increase the SOD activity of RBCJiver and brain in the mice;(3)increase the GSH-Px activity and GSH content of the blood and liver of the mice; (4)inhibit the MAO-B activity of liver and brain in the mice;(5)increase the hydroxyproline content of dermal tissue in aging model mice. In short,it is suggested that PSP could obviously improve the aging effect of D-galactose on the mice.
6.Diagnosis of bleeding aneurysm with three-dimensional computed tomographic angiography

Ojective To evaluate the value of computed tomographic angiography (CTA) in the diagnosis of intracranial aneurysml with acute subarachnoid hemorrhage and in making a surgical plan. Methods CTA was performed using Helical CT in sixty three cases of acute subarachnoid hemorrhage Results Twenty four patients with intracranial aneurysms were found by CTA, one case of aneurysm was found by DSA. Diagnostic results were confirmed by operation. Conclusion CTA is a rapid?safe and accurate method in the diagnosis of intracranial aneurysms, particularly for acute and severe patients.
7.Correlation between overexpression of matrix metalloproteinase-9 with acute graft injury after small-for-size liver transplantation

Objective Portal hypertension and ischemia/reperfusion (I/R) have been implicated in small-for-size liver graft dysfunction. Matrix metalloproteinases-2 (MMP-2) and MMP-9 are critically involved in hepatic I/R injury. The goal of this study was to investigate the role of MMP-2 and MMP-9 in acute small-for-size graft injury. Methods 108 rats were divided into three groups:100 % (full-size), 50 % (half-size) and 25 % (quarter-size) liver transplantation groups. Blood and liver samples were collected to assess liver function, hepatic malondialdehyde (MDA) content, tissue myeloperoxidase (MPO) activity and histological changes. ELISA, real-time PCR, gelatin zymography, and immunohistochemistry were used to determine the expression pattern of MMP-2 and MMP-9 in liver grafts. Results The expression levels of MMP-9 were significantly higher in quarter-size and half-size grafts than those in full-size liver grafts 6, 12, and 24 h after reperfusioa And theelevated levels of MMP-9 were related to graft size inversely. However, MMP-2 was expressed and remained in all groups invariably. MMP-9 overexpression was accompanied by extensive liver I/R injury, as evidenced by significant increases in hepatic microscopic damage scores, MDA content,MPO activity and liver function levels. Furthermore, MMP-9 was found mainly to locate around periportal area. The presence of the active form of MMP-9 was significantly higher in small-for-size grafts, which was correlated with sinusoidal dilatation, congestion and hemorrhage. Conclusion These results support critical function of MMP-9 in acute small-for-size liver graft injury. Moreover,portal hypertension may be a crucial trigger for expression and activation of MMP-9.
8.Expression of signal transducer and activator of transcription 3 protein in development of embryo liver in mice

BACKGROUND: Signal transducer and activator of transcription 3 (STAT3) protein is widely expressed in cells and tissues of different type and. It is the important component of signal transduction and transcription chain that participates in the regulation of various physiological functions of cells like proliferation and differentiation. However, there is no literature reported regarding if there is STAT3 expression and whether it participates in embryo liver development both at home and abroad.OBJECTIVE: To explore the effects and mechanism of STAT3 signal transduction route on development of embryo liver by. Studying the expression of STAT3 protein during embryo liver development in mice.DESIGN: Randomized controlled study based on STAT3 protein.SETTING: Department of histology and embryology in a military medical university of Chinese PLA.MATERIALS: The study was completed in the Department of Histology and Embryology, Third Military Medical University of Chinese PLA during January to June 2004. Kunming mice were provided by Animal Centre of Third Military Medical University of Chinese PLA. Ten male mice and 20 female mice with body mass during 25 to 30 grams were randomly chosen.METHODS: Serial frozen sections of mouse embryo was performed in the 13.5 days ( n = 8), 14.5 days ( n = 9) and 15.5 days ( n = 7) after mating respectively. One of the every 2 slices was used as negative control by replacing first antibody of STAT3 with 0. 01 mol/L PBS. Immunochemical technique and immunofluorescence method were used to display the expression of STAT3 protein in embryo liver development in mice.MAIN OUTCOME MEASURES: Expression of STAT3 protein in serial frozen sections of mouse embryo.RESULTS: There was strong expression of STAT3 protein in liver cells in E13.5 days mouse embryo while the expression was decreased in E14.5 days and E15.5 days mouse embryos.CONCLUSION: STAT3 protein has participated in the development of embryo liver. The expression of STAT3 in liver development can be used to explain the mechanism of liver regeneration and provide experimental data for organ repair.
9.Endovascular interventional embolization for the treatment of aneurysms located at the bifurcation of middle cerebral artery

Objective To evaluate the therapeutic efficacy of endovascular embolism in treating aneurysms located at the bifurcation of middle cerebral artery. Methods During the period from Jan. 2009 to May 2013 at authors’ hospital, a total of 21 patients with intracranial aneurysms located at the bifurcation of middle cerebral artery were treated with interventional embolization. Of the 21 cases , steel coils were used in 7, stent-assistant embolization was adopted in 12, and dual-microcatheter technique was employed in 2. The results were analyzed. Results Postoperative angiography showed that complete embolization of the aneurysm was achieved in 14 patients, residual neck was detected in 2 patients and subtotal obliteration of the aneurysm was seen in 5 patients. Procedure-related complications occurred in 2 patients, including cerebral bleeding during operation (n = 1) and postoperative small area cerebral infarction. Seventeen patients were followed up for 6 months, and follow-up angiography showed that no recurrence of aneurysm was seen. Conclusion For un-ruptured, or ruptured with no big hematoma, intracranial aneurysms located at the bifurcation of middle cerebral artery, endovascular interventional embolization is the treatment of choice.
10.Therapy for epidural hematoma in children

Objective To explore the clinical characteristics of epidural hematoma in children. Methods A total of 120 children with epidural hematomas within recent three years were reviewed. Results The main cause of injury in infants and preschool children was falling or sliping, but traffic accident was the predominant cause in children over seven years old. About 65.8% children were complicated by skull fractures, with average Glasgow Coma Scale (GCS) score of 13.6. Except for acute hematoma treated with emergency surgical operation, the other hematoma was rechecked with CT scan at days 1 and 3 or so after it was found for the first time. Patients receiving operation accounted for 57.5% and those with hematoma due to diploe bleeding for 43.9%. Conclusions The primary cerebral injury is not severe relatively in children with epidural hematoma, in which the incidence of skull fracture is lower than that in adults. The main cause for hematoma formation is diploe bleeding. Sound prognosis can be obtained through recheck of CT scan and suitable therapy.
