1.Application of hand-assisted laparoscopic surgery through “Lapdisc” device in abdominal surgery:A report of 78 cases

Objective To explore the clinical feasibility of hand-assisted laparoscopic surgery (HALS) through the “Lapdisc” device. Methods HALS was performed in 78 cases. After the establishment of a CO_2 pneumoperitoneum, trocars and a “Lapdisc” device were placed appropriately according to lesion’s location and operative demand. The performance of the “Lapdisc” device during the HALS was examined and clinical effects of the device were evaluated intra- and post-operatively. Results HALS through the “Lapdisc” device was successfully performed in 70 cases. The operation time was 60~240 min (mean, 140 min), the blood loss was 100~300 ml (mean, 186 ml), and the length of hospital stay, 9~15 d (mean, 10.2 d), respectively. Conversions to open surgery were required in 8 cases because of difficulties of laparoscopic performance. Conclusions The “Lapdisc” hand-assisted device has advantages of simplicity of performance, comfortable handling, excellent protection of incision, and stable pneumoperitoneum. and perfect protection to incision. The device can simplify the traditional laparoscopic surgery and be applicable to most abdominal HALS.
2.Effects of inhaling N_6-phenyl-2R-isopropyl-adenosine on the experimental asthmatic guinea-pigs

AIM:To evaluate the role of inhaling N 6-phenyl-2R-isopropyl-adenosine (R-PIA) on the airway resistance and pulmonary compliance of experimental asthmatic guinea-pigs. METHODS: Experimental model of asthmatic guinea-pigs were made. Inhaling R-PIA 5 mg/mL( 5 mg R-PIA in 1 mL 0.9% saline). The airway resistance, pulmonary compliance and NO- x, cyclic adenosine monophosphate (cAMP), cyclic guanosine monophosphate (cGMP),tumor necrosis factor (TNF) in bronchoalveolar lavage fluid (BALF) were observed. RESULTS: Compared with asthma group, after inhaling R-PIA, the airway resistance of experimental asthmatic guinea-pigs increased ( P 0 05). CONCLUSION: Inhaling R-PIA could constrict the airway of experimental asthmatic guinea-pigs.
3.Laparoscopic Total Extraperitoneal Repair for Inguinal Hernia: Reports of 269 Cases

Objective To summarize the clinical experiences and efficacy of laparoscopic total extraperitoneal repair(TEP) for inguinal hernia.Methods From April 2004 to August 2006,294 TEP operations was performed on 269 patients with inguinal hernia by using laparoscopy under epidural or general anesthesia.Among the patients,175 had indirect hernia,40 direct hernias,25 bilateral hernias,and 29 recurrent hernias.Results The operation was completed without converting to open surgeries in all the cases.The mean operation time was(63.1?37.5) min,blood loss(12.3 ?8.2) ml,and hospital stay(3.7 ?1.4) d.Of the patients,24 had peritoneal tearing(8.2%,24/269),4 had groin hematoma(2.4%,4/269),and 2 had foreign body sensation(0.7%,2/269).The patients were followed up for 6-12 months(median,9 months),the recurrence rate was 1.0%(3/294). Conclusion Laparoscopic TEP is safe and effective for patients with inguinal hernia.
4.Application of domestically made endoscopic stapling instrument for laparoscopic assisted rectal cancer resection.

OBJECTIVETo investigate the safety and feasibility of domestically made endoscopic stapling instrument in laparoscopic assisted rectal cancer resection (Dixon).
METHODSSixty-four patients with rectal cancer were randomly divided into the research group (35 cases) to receive laparoscopic assisted rectal cancer resection using ENDO RLC general endoscopic linear cutter and single-use loading unit and circular staplers with staples (from REACH medical equipment co.LTD) and the control group (29 cases) to receive surgery with the corresponding products widely used (fom Johnson and Johnson Medical Euipment C.Ltd). The clinical data of the two groups were compared.
RESULTSSatisfactory therapeutic effects were obtained in all the cases. The two groups showed no significant differences in the operative time, intraoperative anastomosis success rate, or postoperative complications (anastomotic bleeding, leakage, or stricture) between the two groups (P>0.05), but the average cost of endoscopic stapling instrument was significantly lower in the research group (6604.31 ± 699.95 vs 7822.28 ± 576.98 RMB Yuan, P<0.05).
CONCLUSIONThe domestic endoscopic stapling instrument is safe, effective and less costly for laparoscopic assisted rectal cancer resection.

6.Laparoscopic disc hand assisted laparoscopic splenectomy for patients with ruptured spleen

Objective To investigate the feasibility and clinical efficacy of laparoscopic disc hand assisted laparoscopic splenectomy(LDHALS) for patients with ruptured spleen.Methods LDHALS was performed in 14 patients with ruptured spleen,and among them,8 cases were combined with other visceral trauma.Results All cases were performed successfully by using LDHALS.The median operation time was 70 min(45~100 min).The average hospital stay was 7 days in 6 cases with only spleen trauma and 13.5 days in 8 cases combined with other injuries.No major operative complications and mortality occurred.Conclusion The LDHALS is safe,effective,simple and flexible performed for patients with ruptured spleen.
