1.Treatment of femur fracture in neonates using harness

Objective To treat femur fracture in neonates using a new homemade harness designed and observe the effec of this harness. Methods Designed a new harness used in femur fracture in neonates,and used this harness in 7(8 femur fracture)csses,including 6 boys(7 femur fracture)and 1 girl,the age from 1 day to 12 days(average 4.7 days).Those cases included proximal thirds(2 cases)and middle thirds(6 cases)of femur fracture.The angle of fracture was from 44°to 83°(average 62.4°)before treatment. Results The angle of fracture was from 0°to 22°(average 14.0°)after treatment using the harness.Hespitalization was from 2 to 3 days.There were no skin sloughing or harness breaking off.All cases were followed up 6 to 36 months(average 21.3 months).All femur fracture healed in good alignment with leg-length discrepancy<1 cm.Movement of hip and knee was normal. Conclusions The harness is a better method to treat femur fracture of the proximal and middle thirds in neonates.Advantages of it include simply operation,minimal hospitalization,minimal cost,and easy nursing.
2.Emergency one-stage implantation of coralline hydroxyapatite for the treatment of limb fractures with osseous defect in 27 cases

Twenty-seven patients of limb fractures with osseous defect were selected from Department of Orthopedics in the 3rd Hospital of Hebei Medical University between April and October in 2003. They all agreed the informed consents and received emergency one-stage implantation of coralline hydroxyapatite. Postoperative restoration of limb fractures with osseous defect was investigated by using X-ray for average 186 days. Among 27 included patients, 25 ones were involved in the result analysis, and 2 ones lost. The implants and osseous tissue were not closely integrated in 1 case, were partial integrated in 2 cases, and 22 cases showed close integration. No case appeared whole or local adverse effects.
3.Effects Comparison of Rivaroxaban and Low Molecular Weight Heparin Calcium on Deep Venous Throm-bosis after Total Hip Arthroplasty

OBJECTIVE:To compare the clinical efficacy and safety of rivaroxaban and low molecular weight heparin calcium (LMWHA)in the prevention of deep venous thrombosis(DVT)after total hip arthroplasty(THA). METHODS:A total of 100 THA patients selected from orthopedics department of our hospital as research objects were divided into control group and observa-tion group according to random number table,with 50 cases in each group. Control group was treated with LMWHA injection 0.4 mL subcutaneously,qd;observation group was given Rivaroxaban tablet 10 mg orally,qd. Both groups received treatment on the first day after surgery,for consecutive 14 d. Coagulation indexes(PT,APTT,Fib,TT,D-D),VAS score,the incidence of DVT and PE were observed in 2 groups. The postoperative bleeding volume and ADR as hematoma and gastrointestinal bleeding were compared between 2 groups. RESULTS:Before treatment,there was no statistical significance in coagulation indexes or VAS scores between 2 groups(P>0.05). After treatment,PT,APTT,TT and D-D levels,VAS scores of 2 groups were decreased sig-nificantly,while Fib levels were increased significantly;VAS score of observation group was significantly lower than that of con-trol group,with statistical significance(P<0.05). There was no statistical significance in coagulation indexes between 2 groups (P>0.05). The incidence of DVT and PE in observation group were 8.00% and 7.50% ,which were significantly lower than 12.00% and 4.00% of control group,with no statistical significance(P>0.05). The postoperative bleeding volume of observation group was(298.31±52.18)mL,which was significantly lower than(327.40±54.20)mL of control group,with statistical signifi-cance(P<0.05). There was no statistical significance in the incidence of hematoma or gastrointestinal bleeding between 2 groups (P>0.05). CONCLUSIONS:Rivaroxaban and LMWHA can significantly improve coagulation state,prevent the generation of DVT after THA. While rivaroxaban is better in shortening pain time without increasing the risk of ADR.
4.Effect of exterior point application combined with herbal concentrate-granules on 60 cases of asthma

Objective To observe the effect of combined therapy of external point application and herbal concentrate-granules on patients with asthma (hot wheezing in TCM).Methods 60 patients suffering from onset period of bronchial asthma were randomized to a control group and a treatment group.The control group was given budesonide inhaler and theophylline sustained release tablets.The treatment group was given combined therapy of external point application and herbal concentrate-granules.The course of treatment was 10 days.To observe the value ofFEV1%,ACT scores and TCM syndrome scores etc.Results After treatment,the value of FEV 1% [treatment group:(82.83 ± 11.35) %,control group:(83.85 ± 16.72) %] and ACT [treatment group:(19.86±2.32),control group:(19.66±2.54)] in both groups were markedly increased with statistical significance (P<0.01); and the difference between the two groups were also significant; TCM syndrome scoring [treatment group:(4.27±3.65),control group:(5.05±4.14)] was notably decreased in both groups compared with the values before the treatment with statistical significance (P<0.01); but the difference between the two groups was not significant (P>0.05).Conclusion The combined therapy of external point application and herbal concentrate-granules can treat bronchial asthma in the acute clinical course and improve lung function.
5.Early clinical features of severe cases with hand-foot-mouth disease

The clinical features of 120 severe cases with hand-foot-mouth disease (HFMD) were retrospectively analyzed compared with those of the regular cases. Clinical data showed that the age of mostsevere cases ranged from 1-3 years. Sustained high fever ( 55% ) , hypertension ( 40% ) , fewer rash (73.3%) were important clinical features. Lethargy, somnolence or restlessness (70. 8% ), vomiting (30. 8% ) were early signs of central nervous system ( CNS) ; and the CNS involvement in severe cases was characterized by body trembling or muscle spasm (80% ). High white cell count(75. 8% ) and high blood glucose levels(32. 5% ) were important makers to diagnose the severe HFMD at early stage. There were significant differences in presentation of above features between severe cases and regular cases of HFMD ( P < 0. 01 or P < 0. 05 ) . Early treatment with mannitol, gamma globulin and glucocorticoids were effective, no death was reported and no side effects were found in this case series.
6.Jinlong-Gubenmixturefor the treatment of asthma at the remission stage

Objective To observe the effect of treating asthma in remission stage(Latent phlegm in lung syndrome) withJinlong-Gubenmixture.Methods 108 patients of asthma on remission stage were randomized into a control group and a treatment group. The treatment group was givenJinLong-GuBen mixture. The control group was given nothing for treatment. Both groups were treated for 120 days and follow-up period were 360 days. The change of ACT scores and FEV1 was observed.Results After treatment, ACT scores(t=2.931, 2.618) and FEV1(t=2.011, 2.680) of treatment groups were markedly increased than before the treatment(P<0.05), and the effect of the treatment group was better than the control group(P<0.01). After treatment, the effective rate of treatment group was 93.75%(45/48), and the effective rate of control group was 78.72%(37/47),there existed statistical significance between the two groups(U=-0.4531,P<0.01). Conclusion The theory of latent phlegm in lung has a good guiding function in treating asthma in remission stage(Latent phlegm in lung syndrome).
7.Clinical Study of Zoledronic Acid Combined with Percutaneous Vertebroplasty for Osteoporotic Vertebral Compression Fracture

OBJECTIVE:To explore the clinical effect and safety of Zoledronic acid injection combined with percutaneous ver-tebroplasty for osteoporotic vertebral compression fracture(OVCF). METHODS:A total of 130 OVCF patients selected from our hospital during Jan. 2014-Dec. 2015 were divided into observation group and control group according to random number table,with 65 cases in each group. Control group was given bone cement by percutaneous vertebroplasty. Observation group was additionally given Zoledronic acid injection 5 mg,ivgtt,once a year(dripping time ≥15 min),on the basis of control group. Both groups were given calcium and vitamin D orally after surgery. VAS scores,Oswestry disability index(ODI)and ADL scores were ob-served in 2 groups 1 week and 6 months after treatment. Bone density of lumbar vertebra L1-L4 and femoral neck were detected in 2 groups after one year of treatment. RESULTS:Before treatment,there was no statistical significance in VAS scores,ODI or ADL scores,bone density of lumbar vertebra LI-L4 and femoral neck between 2 groups(P>0.05). VAS scores and ODI of 2 groups were decreased significantly,while ADL scores were increased significantly 1 week and 6 months after treatment;obser-vation group were significantly better than control group,with statistical significance(P<0.05). One year after treatment,bone density of lumbar vertebra L1-L4 and femoral neck in observation group were significantly higher than control group,with statis-tical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups as fever,dizziness, bone and joint pain,muscle soft tissue pain and new fracture(P>0.05). CONCLUSIONS:Zoledronic acid combined with per-cutaneous vertebroplasty in the treatment of OVCF can significantly relieve pain,improve dysfunction and enhance bone density with good safety.
8.Partial splenic artery embolization for the treatment of patients with hypersplenism at high altitude region: clinical analysis

Objective To investigate the clinical curative effect and significance of partial splenic artery embolization (PSE) for the treatment of patients with hypersplenism at high altitude region. Methods The clinical data of 66 patients with cirrhosis complicated by portal hypertension and hypersplenism, who lived in Xining City of Qinghai Province, the high altitude region in China, and were admitted to authors' hospital during the period from March 2015 to December 2016 to receive PSE, were retrospectively analyzed. White blood cell (WBC) count, red blood cell (RBC) count and platelet (PLT) count were calculated at one day before operation as well as at one, 7, 30 and 90 days after operation. Results The technical success rate of PSE was 100%. The mean WBC count determined at one, 7, 30 and 90 days after PSE was obviously different from that determined at one day before PSE, the differences were statistically significant (P<0. 05). Conclusion For the treatment of patients with cirrhosis complicated by portal hypertension and hypersplenism, who live at high altitude region, PSE has reliable curative effect, therefore, this therapy is worth promoting in clinical practice. Objective Toinvestigatethemethod,safetyandefficacyoftranscatheterarterialchemoembolization(TACE)combined withargonheliumknifecryoablationintreatmentofadvancedprimaryhepaticcancer.Methods FiftyGfourpatientswithadvanced primaryhepaticcancerunderwentTACEfirstly,andfollowedbytheargonhelium knifecryoablationunderCT/ultrasoundguiding percutaneouspunctureafter1-2weeks.2-3cyclesofcryotherapywereperformedduringtheoperation.Afteroperation,enhanced CT/MRIwasperformedtofollowGup.Results Themediansurvivaltimewas17.6months.The6Gmonthsurvivalratewas100%,the 12Gmonthsurvivalratewas89.34%,thetumorprogressiontimewas9.3 months,andtheshortestsurvivalperiodwas8 months.Recent curativeeffectevaluationshowedCRin9patients,PRin34patients,SDin6patients,PDin5patients(RR=79.62%,DCR=90.74%). Conclusion TACEcombinedwithargonheliumknifecryoablationisasafeandeffectivetreatment,whichprovidesanewtreatment planforpatientswithprimaryhepaticcancer.
10.Superselective cystic arterial perfusion embolization for the treatment of invasive bladder cancer with hemorrhage

Objective To investigate the efficacy of transcatheter selective cystic arterial infusion chemotherapy embolization in the treatment of invasive bladder cancer with hemorrhage.Methods 81 cases of with invasive bladder cancer and hemorrhage treated by superselective intervention in hospital were selected,as well as postoperative complications and tumor volume changes were recorded in followG up.Results The success rate of intubation and embolization was 100%,the immediate hemostasis rate was 97.53%,and the preoperative bladder tumor volume (4.08±1.66)cm was significantly larger than that of the six months after surgery (3.45±1.33)cm.Conclusion Superselective cystic arterial perfusion embolization is a safe and effective treatment for patients with invasive bladder cancer complicated with hemorrhage.
