A survey was conducted among 121 health care managers and health workers in community health care institutions with questionnaire and interview.Data on health care conditions in the community and health needs of elderly people were collected and analyzed using principle component analysis method.The priority order of 17 community health care projects for elderly people was ranked with this analysis method and the development strategy of community health service was suggested.

Objective To study improvement of cognitive function impairment of vascular dementia rats induced by a permanent bilateral ligation of common carotid arteries (2VO) after administration of leptin in brain. Methods Hippocampal neurons was isolated and cultured from SD rats. At oxygen-glucose deprivation condition, protection role of leptin on hippocampal neurons was observed and expression of leptin receptor was detected. Animal model of rats was established by 2VO. Pre-treatment VaD model of leptin was established by administration leptin into hippocampus region. The Morris water test was performed to detect difference in the cognitive function between VaD group and control group. Neuron apoptosis in hippocampus tissue were determined with TUNEL. Results Leptin receptor expression could be seen in hippocampal neurons. After oxygen-glucose deprivation cultured for 12 h , plenty of apoptotic cells were seen in hippocampal neurons, apoptosis rat was up to (72.96 ± 6.25) % , while apoptosis rate was (46.33 ±7.85)% and (23.58 ±5.08)% in 1 ( μg and 5μg leptin treatment group,respectively. Compared leptin treatment group with control group,difference had a statistical significance(P<0.01). Compared with untreated VaD group, latency time was shorter and average velocity was increased in leptin-treat-ment VaD group. Neuron apoptosis in hippocampus tissue of leptin-treated group were different significantly from those of untreated group (P < 0.01). Conclusion Leptin could protect hippocampal neurons from apoptosis in vitro. Cognitive function impairment could be improved by administration of leptin into brain in VaD rats.

Objective To develop a quantitative method to determine vitexin in Cajanus cajan (L.) Millsp. Methods The chromatographic conditions were as follows: column C18(250 mm? 4.6 mm, 5 ? m) , mobile phase being MeOH ∶ 1 % HAC (25 ∶ 75) for 20 mins and then being MeOH ∶ 1 % HAC (30 ∶ 70) after 20 mins, flow rate at 1.0 mL/min, and wavelength at 339 nm. Results The good linearity of this method was in the range of 0.104 4～ 0.522 ? g (r=0.999 4), and the average recovery of vitexin was 100.11 % ( RSD =0.63 % ).Conclusion The method is simple and sensitive with good stability, it can be suitable for the quality control of vitexin in Cajanus cajan (L.) Millsp..

Objective To investigate the expression and clinical significance of p63, CD_(44v6) and human papilloma virus (HPV) 16/18 in carcinoma of cervix and precancerous lesion and to study their role in the pathogenesis of the infiltrative carcinoma of the cervix(ICC). Methods 145 patients were selected, among whom were 60 cases of ICC, 55 cases of cervical intraepithelial neoplasia (CIN) and 30 cases of normal cervical epidermis (NCE). Immunohistochemistry streptavidin peroxidase (SP) was used to detect the expressions of p63 and CD_(44v6) protein and hybridization in situ was used to measure HPV 16/18 gene on tissue microarray. Results HPV 16/18, p63 and CD_(44v6) in ICC were significantly higher than those in CIN and NCE groups (P < 0.05). The expressions of HPV 16/18 gene in CIN I 、CIN II and CIN III were 27.3 % , 43.8 %, 70.6 % respectively. p63 protein was mainly expressed in squamous cell carcinoma, but not in adenocarcinoma. p63-positive rate was related to the grade of squamous cell carcinoma and clinical stage. CD_(44v6) positive rate was related to the pathological grade and clinical stage. Moreover, the positive rate of lymph node metastasis was significantly higher than the cases without metastasis (P <0.05). The expression of HPV16/18 was positively correlated with that of p63 in ISCC (P <0.05, Cp =0.49). Conclusion HPV16/18 possibly participate in the pathogenesis and progress of cervical squamous carcinoma. p63, as the cancer gene, may participate in the occurrence and development of cervical cancer caused by HPV 16/18. p63 could be a differentiation indicator of cervical squamous carcinoma, and it could be one of the markers which would be differentiated into squamous cell in malignant tumour. CD_(44v6) could be used as one of the indicators of distant metastasis of cervical cancer.

Objective To investigate the expressions and clinicopathological significance of 3q27-3q29-related p63 protein in diffuse large B-cell lymphoma (DLBCL). Methods An immunohistochemical Envision~(TM) method was used to detect the expressions of p53 and 3q27-3q29-related p63 protein in 102 cases of DLBCL and 15 cases of reactive hyperplasia of lymph node (RHL). Results The tumor cell expressions of p53(62 %) and p63(56 %) in DLBCL were significantly higher than that in RHL (0 and 13 % P < 0.05). The expressions of p53 and p63 were significantly different (1) between stage Ⅰ + Ⅱ (the positive rate 48.3 % and 41.4 %, respectively) and stage Ⅲ+Ⅳ(the positive rate 79.5 % and 75 %, respectively; P <0.05), (2) between GCB type (the positive rate 28 % and 28 %, respectively) and non-GCB type(the positive rate 72.7 % and 64.9 %, respectively; P <0.05). The expressions of p53 and p63 had no relationship to gender, age, B symptoms and locations. The expression of p53 was positively correlated with that of p63 in DLBCL (P <0.05, Cp=0.629). p53 and p63 protein expression in negative group the 5-year overall survival rate is higher than that in positive group (38 % and 6 %, 51% and 4 %, respectively), the difference was statistically significant (P <0.05). Conclusion It was likely that p63, as the oncogene, participated in the occurrence and development of DLBCL together with p53. Also, p63 and p53 might play a synergistic role in the occurrence DLBCL. Combined detection of 3q27-3q29-related 1963 protein and p53 protein in DLBCL, might be one of the indicators to the prognosis of DLBCL.

Objective To investigate the significance of orthotopic liver transplantation in the management of primary hepatocellular carcinoma (PHC) ; To evaluate the survival and prognostic factors of liver transplantation for PHC.Method The clinical data from 194 consecutive PHC patients who underwent OLT were retrospectively analyzed.Result The 1-,3-and 5-year cumulative survival rate of the 194 patients was 92.78％,78.07％ and 76.80％ respectively.There were 37 cases of recurrence or metastasis of the tumor with the incidence rate being 19.1％ (37/194).The tumor TNM stage (P=0.026 667),tumor diameter (P =0.0045),tumor number (P =0.0318),vascular invasion (P =0.006 059),and AFP level before transplantation (P =0.003 426) were significantly related with tumor recurrence.Multivariate analysis revealed that tumor diameter (P =0.0081,OR =0.789,95％ CI:2.1233-6.124),vascular invasion (P＜0.001,OR =5.928,95 ％ CI:1.173-86.789),and AFP level before transplantation (P =0.019,OR =2.601,95％ CI:2.196-5.658) were independent risk factors for tumor recurrence.There was statistically significant difference in survival rate and tumor-free survival rate between Milan criteria group and beyond-UCSF criteria group (P＜0.01),and between UCSF criteria group and beyond-UCSF criteria group (P＜0.01).There was no significant difference in survival rate and tumor-free survival rate between Milan criteria group and UCSF criteria group (P ＞0.05).Conclusion Liver transplantation is an effective treatment for PHC.The tumor diameter ＞8 cm,vascular invasion and AFP level significantly affect the survival of PHC patients.
