1.The sedation values of dexmedetomidine combined with midazolam in the management of appendectomy

Objective To investigate the sedation values of dexmedetomidine combined with midazolam in appendectomy.Methods 90 patients with acute appendicitis were studied prospectively.According to the principle of completely random digital table,all patients were randomly signed into the dexmedetomidine combined with midazolam group,single midazolam group or single dexmedetomidine group.The hemodynamic changes in the three groups were compared.Results There were no significant differences in the three groups in heart rate and mean arterial pressure at t0 (P >0.05).When compared with the single midazolam group or dexmedetomidine group,patients in dexmedeto-midine group got a significant reduction in heart rate at t1 [(79.73 ±10.36)beat/min vs.(93.30 ±19.32)beat/min and (90.85 ±18.23 )beat/min,F =5.796,P =0.004];at t3 [(79.51 ±11.21 )beat/min vs.(87.53 ± 18.35)beat/min and (95.03 ±14.64)beat/min,F =8.006,P =0.001];and at t5 [(79.73 ±12.24)beat/min vs. (91.80 ±17.71)beat/min and (89.62 ±14.26)beat/min,F =5.584,P =0.005];a reduction in heart rate at t2 and t4,though there was no statistical difference (P >0.05);a significant reduction in mean arterial pressure at t1, t2,t3,t4 and t5 (F =8.444,6.591,8.165,17.121 and 6.977,P =0.000,0.002,0.001,0.000 and 6.977);a sig-nificant reduction in visual analogue scales [(3.40 ±1.28 )points vs.(4.50 ±1.81 )points and (4.37 ± 1.71)points,F =4.131,P <0.05 ];a significant reduction in operation duration [(111.17 ±15.77 )min vs. (129.33 ±13.98)min and (132.50 ±14.67)min,F =0.000,P <0.05];and a significant reduction in bleeding vol-ume [(202.47 ±61.07 )mL vs.(246.87 ±52.06 )mL and (241.93 ±58.71 )mL,F =0.006,P <0.05 ]. Conclusion Dexmedetomidine combined with midazolam contributed to maintaining the hemodynamic stable and reducing the operation duration,intra -operation bleeding volume and visual analogue scales.
2.THE ARTERIAL ARCHITECTURE OF THE PALMAR PART OF THE HAND

The arteries of the palmar part of the hand were examined in 50 hands of adult under the operating microscope.The arteries of the hand are arranged at three main levels: the superficial palmar arteries, the deep palmar arteries and the dorsal network. The arterial anastomoses between the levels may be divided into 2 groups: the marginal and the central anastomoses, the former which is situated at the radial and ulnar borders of the palm can be subdivided into superficial and deep groups, the latter is located in the central part of the palm including the direct and indirect anastomostic branches, the proximal and distal perforating branches, and the intermetacarpal perforating branches. The deep arch and its branches, as the center in the total arterial architecture of the palmar part of the hand, closely links with the arteries of the superficial and dorsal ones, playing an important role in regulating the collateral circulation of the hand.The flow formula of the Poiseuille's law indicates that the significant correlation exists between the arterial diameter and the blood flow. For the variety of diameter of each portion of the anastomostic branches, we inferred that the main source of blood flow in the superficial arteries is the ulnar artery and in the deep is the radial artery. The blood within the marginal anastomoses flows from palmar to dorsal, the central mainly from the deep arch and its branches.The dominant parts of the collateral circulation of the hand were found, that is, the deep arch dominantes over the superficial arch and the dorsal network, the central anastomosis over the marginal, the deep branches of the marginal over the superficial branches.The arrangement of the digital arteries has been discussed.
3.Exploration on the construction of 'grid type' pharmacy teaching comprehensive laboratory in newly-built university

The construction of on campus practice base is crucial for improving the quality of practical teaching.We put forward a construction pattern of‘grid type' pharmacy teaching comprehensive laboratory.It covers three-leveled experimental platform and professional teaching laboratory according to the research and development process of new drug.It also forms the communication network between learning and working demand of students.This provides new ideas for the construction of pharmacy teaching laboratory in newly-built university.
4.The Inhibitory Effect of Dihydro-?-erythroidine on Nicotine Induced-current in Cultured Sympathetic Neurons

AIM:To investigate the inhibitory effect of dihydro-?-erythroidine on nicotine induced-current in cultured superior cervical ganglion of neonatal rats.METHODS:Pneumatic pressure administration of drug and whole-cell recording techniques were performed to compare induced-current amplitude.RESULTS:Dihydro-?-erythroidine competitively antagonized nicotinic effect,and EC50 was about 0.015mmol/L.CONCLUSION:Dihydro-?-erythroidine is a competitive antagonist of nicotinic receptors in sympathetic neurons.

Chronic obstructive pulmonary (COPD) has significant extra pulmonary effects, which could induce atrophy of peripheral skeletal muscle and respiratory muscles, and significantly influence the respiratory function and quality of life. Muscle nuclear factor (NF)-κB activation plays a key role in the skeletal muscle atrophy. This article discussed the potential mechanisms on how NF-κB signaling pathway increasing protein breakdown and reducing muscle regeneration. On this basis, we explored the role of NF-κB in skeletal muscle at-rophy in patients with COPD during exercise.
7.Study on p38 mitogen activated protein kinase in vascular endothelial cells dysfunction in preeclampsia

Objective To study expression and activation of p38 mitogen activated protein kinase (MAPK) in vascular endothelial cells dysfunction in preeclampsia. Methods From Sept. 2009 to Mar.2010, 54 pregnant women underwent deliveries in the First Affiliated Hospital of Chongqing Medical University were enrolled in this study, including 20 patients in mild preeclampsia group, 16 patients in severe preeclampsia group and 18 women with term cesarean section without perinatal complications as control group. Placental endothelial cells were labeled by CD34 to assay microvessel density (MVD) of each group. Immunohistochemical SP and western blot were used to detect localization and expression of p-p38 MAPK protein, respectively. The levels of sera soluble fms-like tyrosine kinase-1 (sFlt-1)and soluble endoglin(sEng) were measured by ELISA. Results ①The MVD of placenta were 103 ± 3 in control group, 81 ±5 in mild preeclampsia group and 63±4 in severe group, respectively, which showed statistical difference among each group (P<0.05).②The expression of p38 MAPK protein were 0.84±0.05 in control group,0.90±0.14 in mild group and 0. 86 ±0.18 in severe group, which did not reach remarkable difference among each group (P>0.05). The expression of p-p38 MAPK protein were 0.13±0.05 in control group,0.59±0.12 in mild group and 1.16±0.18 in severe group, which show statistical difference among each group(P<0.05).(3) The localization of p-p38 was in trophoblast, endothelial cells and a few (5.2±0.3)and(10.9±0.4)μg/L in control group,(12.5±1.2) and (20.4±5.3)μg/L in mild group and (19.3±3.0) and (29. 5 ±3.7) μg/L in severe group. When drawing paired comparison in those p-p38 MAPK protein levels and the concentrations of serum sFlt-1, sEng in preeclampsia groups (r=0.68,P<0.05;r=0.87,P<0.05). Conclusions The remarkable activation of the p38 MAPK in the placenta of patients with preeclampsia induced the increased levels of sFlt-1 and sEng in maternal serum, which confer the injury of vascular endothelial cells that caused the significant decline of MVD in placentas. p38 MAPK signaling might be one of the key pathways in vascular endothelial cell dysfunction in preeclampsia.
8.Clinical research of rhGH on pregnancy rate of polycysticovary syndrome

Objective To investigate the effect of recombinant human growth hormone (rhGH) action on pregnancy rate of polycystic ovarian syndrome(PCOS).Methods The PCOS who were client in our hospital in February-August,2012 were investigated.The rhGH group (41 cases) were that all patients took rhGH at the same time ovulation.The control group (41 cases) were no medication group.The two groups had sex action after ovulation.Ovulation rate,pregnancy rate and endometrial score of the two groups were compared.Results Ovulation rate of rhGH group was 90.2% (37/41),the control group was 68.2% (28/41),the difference was statistically significant (x2 =3.459,P =0.036).Clinical pregnancy rate of RhGH group was 32.7% (12/37),the control group was 25.0% (7/28).The difference was statistics significance (x2 =4.319,P =0.032).Endometrial score of rhGH group was (14.96 ± 2.18),the control group was (12.77 ± 2.60) points,the difference between groups was statistically significant (t =-3.356,P =0.032).Conclusion Giving rhGH ovulation cycle specification can promote ovulation PCOS patients and improve the adverse factors affecting pregnancy,improving the pregnancy rate.
9.Purification of monoclonal antibody against PGRP and its activity for small cell lung cancer in vitro

Objective To explore the effect of purification on monoclonal antibody (MAb) against PGRP by Protein A-Sepharose affinity chromatography, and to provide some based data for the purification of other antibody using the same method. Methods The ascites which include MAb was purified by Protein A-Sepharose affinity chromatography. The purity and activity of MAb was tested by SDS-PAGE and ELISA. The biological function was identified by flow cytometer and immunohistochemistry. Results The average concentration of protein in ascites before purification is 23.62 mg/ml. Before and after purification, the total protein is 148.79 mg and 146.67 mg, respectively. The recovery coefficient of protein is 98.58%. The concentration of MAb in ascites is 5.21 mg/ml averagely. The MAb purity is more than 95 %. The immunoactivity of purified antibody is higher than that of unpurified antibody. Conclusion The purity of MAb against PGRP purified by Protein A-Sepharose affinity chromatography is very high. The immunoactivity of purified antibody is higher than that of unpurified antibody. So the ProteinA-Sepharose affinity chromatography is a rapid, convenient and reliable method for the purification of MAb Against PGRP.
10.Extended trochanteric osteotomy combined with long-stem cementless prosthesis in hip revision

BACKGROUND:Extraction of a wel fixed cementless femoral stem or a cemented stem is difficult during revision of total hip arthroplasty. The extended trochanteric osteotomy provides excellent methods, and combining with long-stem cementless prosthesis can better reduce postoperative complications.
OBJECTIVE:To retrospectively analyze the recent clinical curative effect after extended trochanteric osteotomy combined with long-stem cementless prosthesis in hip revision.
METHODA total of 25 patients undergoing hip revision were selected from the Department of Osteoarthrosis, Pingdingshan First People’s Hospital, China from May 2008 to May 2013. Of them, 17 patients were subjected to the extended trochanteric osteotomy combined with single-incision and double-approaches. The femurs were reconstructed with long-stem cementless prosthesis. Al patients were fol owed-up for 12 to 60 months. Pre-operative and post-operative Harris hip scores, the post-operative complications, the bone cutting length, the length of stem in contact with cortical bone, the length of the end of bone cutting to the end of the prosthesis were recorded.
RESULTS AND CONCLUSION:17 patients were fol owed-up. The mean length of bone was 17 cm (15-23 cm). The mean length of stem in contact with cortical bone was 6.4 cm (4-11 cm). The mean length of the end of bone to the end of the prosthesis was 11.5 cm (8-18.5 cm). After fol ow-up, the mean Harris hip scores were improved from (39.0 ± 13.6) scores preoperatively to (75.0 ± 14.9) scores postoperatively (P<0.01). Subsidence occurred in two hips. The cutting sites of 17 cases of extended trochanteric osteotomy were fixed wel . These data suggested that the operation through single-incision and double-approach to the hip, while preserving the attachment of the external rotators and posterior capsule, wil strength rear hip stability and prevent postoperative dislocations. Recent effects were satisfactory, but the long-term outcomes should be further investigated.
