1.Construction and evaluation of the pharmaceutical affairs management system for externally dispensed medications in medical institutions

OBJECTIVE To establish a pharmaceutical management system for externally dispensed medicines in medical institutions, so as to improve medication safety and regulatory efficiency. METHODS Based on policy analysis and hospital practice, a pharmaceutical management system for externally dispensed medicines was constructed by integrating institutional frameworks, an information‑based platform and management procedures, and the management effectiveness was evaluated. RESULTS Our hospital formulated the Regulations on the Management of Externally Dispensed Medicines , which standardized catalogue selection, prescription issuance, acceptance and use, supervision and evaluation. An information‑based management platform for externally dispensed medicines was built relying on the hospital information system, enabling electronic prescription circulation, intelligent prescription review and whole‑process traceability. Institutional requirements and platform functions were embedded into three core management procedures, namely catalogue selection, prescription issuance, and medicine acceptance and use, forming a closed‑loop working mechanism. After the implementation of the management system, compared with pre‑implementation, the qualified rate of externally dispensed prescriptions increased from 85.2% to 99.3% ( P <0.001), the automatic prescription review pass rate exceeded 95%, the prescription review duration shortened from 7.2 min to 1.8 min ( P <0.001). All dimensional satisfaction scores of patients and medical staff were significantly improved, the standardization awareness of medical staff reached 100%. CONCLUSIONS The construction of a pharmaceutical management system for externally dispensed medicines with standardized institutional frameworks, informatized platforms and standardized procedures effectively enhances the rationality and management efficiency of externally dispensed prescription medicines, and provides a replicable practical pathway for the management of externally dispensed medicines in medical institutions.
2.Analysis of acupoint selection rules for acupuncture for autism spectrum disorder based on data mining technology.

OBJECTIVE:
To analyze the core acupoint selection rules and syndrome-based compatibility patterns of acupuncture for autism spectrum disorder (ASD) using data mining techniques.
METHODS:
Relevant literature of acupuncture for ASD was retrieved from CNKI, Wanfang, VIP, PubMed, and Web of Science. After applying inclusion and exclusion criteria, a prescription database was established based on the extracted effective data. Descriptive analysis was conducted on the frequency, meridian tropism, anatomical distribution, and specific point. High-frequency acupoints were visualized using Origin software. The Apriori algorithm in IBM SPSS Modeler 18.0 was used for association rule analysis of acupoint combinations. Cluster analysis of high-frequency acupoints was performed using IBM SPSS Statistics 26.0. The relationships between high-frequency syndromes and acupoints were visualized using Cytoscape 3.10.0.
RESULTS:
A total of 223 studies and 452 prescriptions were included, among which 223 were based on syndrome differentiation. A total of 205 acupoints were included with a cumulative frequency of 4 067. The top five most frequently used acupoints were Baihui (GV20), Sishenzhen, Zhisanzhen, Niesanzhen, and Neiguan (PC6). Acupoints were primarily from Jin's three-needle therapy, the governor vessel, scalp acupuncture, and the foot-taiyang bladder meridian, with a high proportion of acupoints located on the head and neck and the limbs. Among specific point, five-shu points, yuan-source points, and back-shu points were most frequently used. Association rule analysis revealed that the core acupoint group was Sishenzhen-Dingshenzhen-Zhisanzhen-Niesanzhen. Cluster analysis divided the top 20 high-frequency acupoints into four categories: governor vessel activation and brain awakening group, spleen strengthening and heart nourishing group, Jin's three-needle spirit-regulating group, and kidney-reinforcing and marrow-filling group. Clinically, the main syndrome patterns were kidney essence deficiency, hyperactivity of heart and liver fire, phlegm obstructing the heart orifices, dual deficiency of heart and spleen, and liver qi stagnation.
CONCLUSION
The core acupoint prescriptions of acupuncture for ASD are Sishenzhen, Dingshenzhen, Zhisanzhen, and Niesanzhen. The treatment emphasizes spirit regulation and mental tranquility, guided by the principles of harmonizing multiple zang-fu organs, regulating qi and blood, unblocking qi movement, and balancing yin and yang. Syndrome-based acupoint compatibility is recommended in clinical practice.

3.Single-cell and spatial transcriptomics reveals an anti-tumor neutrophil subgroup in microwave thermochemotherapy-treated lip cancer.

Microwave thermochemotherapy (MTC) has been applied to treat lip squamous cell carcinoma (LSCC), but a deeper understanding of its therapeutic mechanisms and molecular biology is needed. To address this, we used single-cell transcriptomics (scRNA-seq) and spatial transcriptomics (ST) to highlight the pivotal role of tumor-associated neutrophils (TANs) among tumor-infiltrating immune cells and their therapeutic response to MTC. MNDA+ TANs with anti-tumor activity (N1-phenotype) are found to be abundantly infiltrated by MTC with benefit of increased blood perfusion, and these TANs are characterized by enhanced cytotoxicity, ameliorated hypoxia, and upregulated IL1B, activating T&NK cells and fibroblasts via IL1B-IL1R. In this highly anti-tumor immunogenic and hypoxia-reversed microenvironment under MTC, fibroblasts accumulated in the tumor front (TF) can recruit N1-TANs via CXCL2-CXCR2 and clear N2-TANs (pro-tumor phenotype) via CXCL12-CXCR4, which results in the aggregation of N1-TANs and extracellular matrix (ECM) deposition. In addition, we construct an N1-TANs marker, MX2, which positively correlates with better prognosis in LSCC patients, and employ deep learning techniques to predict expression of MX2 from hematoxylin-eosin (H&E)-stained images so as to conveniently guide decision making in clinical practice. Collectively, our findings demonstrate that the N1-TANs/fibroblasts defense wall formed in response to MTC effectively combat LSCC.

Radiopharmaceuticals operate by combining radionuclides with carriers. The radiation energy emitted by radionuclides is utilized to selectively irradiate diseased tissues while minimizing damage to healthy tissues. In comparison to external beam radiation therapy, radionuclide drugs demonstrate research potential due to their biological targeting capabilities and reduced normal tissue toxicity. This article reviews the applications and research progress of radiopharmaceuticals in cancer treatment. Several key radionuclides are examined, including 223Ra, 90Y, Lutetium-177 (177Lu), 212Pb, and Actinium-225 (225Ac). It also explores the current development trends of radiopharmaceuticals, encompassing the introduction of novel radionuclides, advancements in imaging technologies, integrated diagnosis and treatment approaches, and equipment-medication combinations. We review the progress in the development of new treatments, such as neutron capture therapy, proton therapy, and heavy ion therapy. Furthermore, we examine the challenges and breakthroughs associated with the clinical translation of radiopharmaceuticals and provide recommendations for the research and development of novel radionuclide drugs.

6.Digital biopsy for liver diseases: A review of technological advances and application prospects

Digital biopsy for liver diseases is characterized by the deep integration of artificial intelligence (AI) technologies and large-scale liver disease data, through which intelligent analytics are applied to support clinical decision-making and full-cycle management. This article reviews the AI technical framework based on standardized data governance and centered on multimodal large medical models, covering the application of natural language processing, knowledge map, generative AI, and large language models in the establishment of databases for specialty diseases, diagnosis, prognosis prediction, treatment, and automated medical documentation. This article also discusses the application prospects of this framework in medical education, scientific research, and healthcare management. Although this technique shows broad application potential, it still faces challenges in areas such as multi-center data integration, model interpretability, ethics, and data security. In the future, a smart ecosystem with closed-loop optimization and human-AI collaboration should be established to promote the comprehensive implementation of digital biopsy in the whole process of medicine, education, research, and management, thereby providing help for the precise prevention and control and holistic health management of liver diseases.
7.Association between inflammation-related dietary patterns and cognitive impairment in older adults aged 65 years and above in longevity areas of China: a reduced rank regression analysis

Objective:To analyze the association between inflammation-related dietary patterns and the risk for cognitive impairment in older adults aged ≥65 years in longevity areas in China by using reduced rank regression (RRR) analysis.Methods:This study used cross-sectional data from the 2021 Healthy Aging and Biomarkers Cohort Study, including the information about study participants' demographic characteristics, lifestyles, daily life activities, and disease histories. Dietary intake was obtained by using a simplified food frequency questionnaire. Cognitive impairment was evaluated based on the Mini-Mental State Examination Scale combined with years of education. Fasting venous blood samples were collected to detect inflammatory markers, especially high-sensitivity C-reactive protein (hs-CRP) and the platelet-to-lymphocyte ratio (PLR). RRR analysis was used to obtain inflammation-related dietary patterns using hs-CRP and PLR as response variables. Multivariate logistic regression model was used to analyze the association between dietary pattern score and the risk for cognitive impairment. Restricted cubic spline was used to explore the dose response relationship, and mediation analysis was used to quantify the mediating effects of hs-CRP and PLR.Results:Two dietary patterns were identified with RRR. The primary pattern was characterized by higher intakes of flour, red meat, and dairy products, and lower intake of fresh vegetables, explaining 6.84% of the variance in food intake and 0.50% of the variance in inflammatory markers. Compared with the T1 group, the T3 group had significantly higher risk for cognitive impairment ( OR=1.242, 95% CI: 1.034-1.491). Each one standard deviation increase in the dietary pattern score was associated with an 8.7% increase in the risk for cognitive impairment ( OR=1.087, 95% CI: 1.008-1.172), with a significant linear trend (overall-model P<0.001, non-linear P=0.295). Mediation analysis indicated that hs-CRP mediated 6.2% of the association between the dietary pattern and the risk for cognitive impairment. Conclusion:The inflammation- related dietary pattern characterized by higher consumption of flour, red meat, and dairy products and lower consumption of fresh vegetables is associated with an increased risk for cognitive impairment in older adults, and hs-CRP partially mediates this association.
8.Association between dietary behavior and sarcopenia in older adults aged 65 years and above in longevity areas of China: a latent class analysis

Objective:To investigate the relationship between dietary behavior and sarcopenia in older adults aged ≥65 years in longevity areas of China based on latent class analysis.Methods:A total of 4 358 older adults aged ≥65 years were selected from the 2021 Healthy Aging and Biomarkers Cohort Study. The information about their demographic characteristics, lifestyles, and chronic disease histories were collected. A simplified food frequency questionnaire was used to collect information about their dietary intake in the last month. The food intake frequency and food category score were calculated, and the higher the food category score, the richer the dietary intake. Latent class analysis was used to identify the latent classes of the dietary behavior. Sarcopenia was diagnosed using the SARC-CalF. Multivariate logistic regression model was used to analyze the association of food category scores and different latent classes of the dietary behavior with the risk for sarcopenia.Results:In 4 358 older adults, 1 841 (42.24%) had sarcopenia. The frequencies of intakes of cereals and potatoes, vegetable and fruit, meat and bean products were lower in the sarcopenia group than in the non-sarcopenia group. The risk for sarcopenia decreased with the increase of food category score in older adults ( OR=0.850, 95% CI: 0.796-0.907). Latent class analysis identified 4 latent classes of the dietary behavior. Compared with those with class 1 (frequency of intake of all 5 food species was higher probability in T3 group), those with class 2 (frequency of intake of vegetables and fruits and energy-only foods were less likely to be in the T3 group) and class 3 (frequency of intake of all 5 food species was lower probability in T3 group) had significantly increased risk for sarcopenia ( OR=1.377, 95% CI: 1.131-1.676) and ( OR=1.354, 95% CI: 1.091-1.680), 37.7% and 35.4% increased risk for sarcopenia, respectively. Conclusion:Increasing dietary intake category and sufficient intake of various foods for a balanced dietary pattern can reduce the risk of sarcopenia in older adults.
9.Reliability and validity of the Chinese Version of Apathy Evaluation Scale-Clinician

Objective:To translate and adapt the Apathy Evaluation Scale-Clinician (AES-C) into Chinese and test its reliability and validity in patients with Alzheimer 's disease (AD). Methods:Following Brislin′s translation model, the English version of the AES-C was translated, back-translated, cross-culturally adapted, and pre-surveyed, ultimately forming the Chinese version of the AES-C. Clinical data were prospectively collected from October 2022 to April 2024 for AD patients aged 55-85 years who were treated at the outpatient and inpatient psychiatric departments of Sir Run Run Shaw Hospital, Zhejiang University School of Medicine. A total of 111 AD patients were recruited. Two weeks later, using systematic sampling, one patient was randomly selected from the first three cases as the starting point, followed by selecting patients at intervals of 3 and 4 alternately until 30 cases were selected. Item analysis of the scale was conducted using the critical ratio(CR) method. Content validity was assessed by calculating the item-level content validity index(I-CVI) and the scale-level content validity index (S-CVI). Structural validity was examined using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). Reliability was examined by internal consistency reliability (Cronbach′s α coefficient) and test-retest reliability.Results:The CR values of each item analysis were>3.000 ( P<0.01). Low-relevant entries were deleted after a unanimous expert discussion of content validity, and the I-CVI and S-CVI were both 1.000. The Kaiser-Meyer-Olkin (KMO) value was 0.722, Bartlett′s test of spherical χ 2=621.286 ( df=55), P<0.01. Four common factors were extracted, with variance contribution rate of 33.0%, 15.3%, 15.1%, 15.1%, and 14.9%, for a cumulative variance contribution of 78.4%. CFA showed χ2/ df=1.778, Root Mean Square Error of Approximation (RMSEA)=0.084, Tucker-Lewis Index (TLI)=0.928, Goodness-of-Fit Index (GFI)=0.906, Comparative Fit Index (CFI)=0.950, and Incremental Fit Index (IFI)=0.952, and all the indexes of the model were in line with the model fit standards. Cronbach′s coefficient α was 0.809, and the retest reliability coefficient was 0.928. Conclusion:The Chinese version of the Apathy Evaluation Scale-Clinician (AES-C) demostrates good reliability and validity in AD patients.

The emerging paradigm of tumor microecology has reshaped our understanding of carcinogenesis. Colorectal cancer(CRC),a prevalent malignancy worldwide,is not only associated with gut microbial dysbiosis but is also linked to the oral microbiome. Notably, Fusobacterium nucleatum(Fn)can translocate from the oral cavity to the colon via cross-organ migration,colonize tumor tissues,and promote CRC initiation,progression,and therapeutic resistance. This review synthesizes evidence on the association between Fn and CRC,focusing on its promise as a biomarker for early detection,risk stratification,and disease monitoring,to provide novel insights for developing Fn-based strategies for the prevention,diagnosis,and management of CRC.
