1.Risk factors of complications after laparoseopic gastrectomy: a Logistic analysis

ObjectiveTo analyze the risk factors of complications after laparoscopic gastrectomy.Methods The clinical data of 76 patients who received laparoscopic gastrectomy at the Tenth Hospital of Tangji University from April 2009 to July 2011 were retrospectively analyzed.All patients were divided into complication group (13patients) and non-complication group (63 patients).Seventeen variables,including gender,age,abdominal surgery history,comorbidities (cardiovascular disease,chronic obstructive pulmonary disease,diabetes mellitus,anemia,hypoproteinemia,pyloric obstruction),palliative operation,operative data ( operation time,blood loss,method of alimentary tract reconstruction),postoperative TNM staging,vascular or nerve invasion and number of lymph nodes dissected were analyzed by using the univariate and Logistic regression analysis to screen out the risk factors of postoperative complications.All data were analyzed using the t test or chi-square test.ResultsThere were 67 patients received laparoscopic curative gastrectomy and 9 received laparoscopic palliative gastrectomy.Sixty-three patients received distal gastrectomy ( including 49 received BillrothⅠgastrectomy and 14 received Billroth Ⅱ gastrectomy) and 13 patients received total gastrectomy + Roux-en-Y esophagojejunostomy).The mean operation time,blood loss,number of lymph nodes dissected were ( 263 ± 72) minutes,( 200 ± 191 ) ml and 17 ±8,respectively.There were 25 patients in TNM stage Ⅰ,18 in stage Ⅱ,27 in stage Ⅲ and 6 in stage Ⅳ.The incidence of complications was high in the old patients,but there was no effect of gender and age on the incidence of complications ( x2 =0.68,2.32,P > 0.05 ).The operation time of the complication group was longer than that of non-complication group,but no significant difference was observed ( t =1.44,P > 0.05 ).Preoperative comobidities (cardiovascular disease,chronic obstructive pulmonary disease,diabetes mellitus,anemia,hypoproteinemia,pyloric obstruction),blood loss and number of lymph node disseeted were not the risk factors of postoperative complications ( x2 =3.20,0.58,0.13,0.26,0.01,0.19,t =0.15,0.83,P > 0.05).The results of multivariate Logistic regression analysis showed that Billroth Ⅱ alimentary tract reconstruction,more comorbidites,and advanced TNM stage were correlated with postoperative complications ( OR =5.54,7.02,2.33,P <0.05 ).The accuracy rate of multivariate Logistic regression analysis was 8 i.6%.Conclusion Billroth Ⅱ alimentary tract reconstruction,more comorbidities,and advanced TNM stage are the independent risk factors of complicatioas after laparoscopic gastrectomy.
2.The effect of lornoxicam on the expression of growth associated protein-43 and nerve growth factor in the dorsal root ganglion of rats during the development of neuropathic pain

Objective To investigate the effect of lornoxicam on the.neurological behavior change and the expressions of growth associated protein-43 (GAP-43) and nerve growth factor (NGF) in dorsal root ganglion (DRG) on the peripheral nerve chronic constriction injury (CCI) model of rats.Methods Fifty Wistar rats were randomly divided into four groups:normal control group (n =5),CCI model group (CCI group),normal saline control group (NS group),and lomoxicam therapy group (L group) ; CCI,NS,and L groups were subdivided into 3 groups according to the different postoperative interval:3,7 and 14 days (n =5 each subgroup),respectively.The right sciatic nerve of rat was to be chronic constriction injure.Group L was given the rat 1.3 mg/kg of lomoxicam every 12 hours; then,the mechanical withdrawal threshold (MWT) and thermal withdrawal latency (TWL) of the rats of CCI,NS and L groups were measured at the different postoperative interval (3,7 and 14 days).After that,the DRG to the injured sciatic nerves were harvested.Immunohistochemistry was used to examine the expressions of GAP-43 and NGF.Results Comparing with the normal group,the latencies of MWT and TWL at CCI,NS and L groups were significantly declined at the third,seventh and fourteenth day after surgery (P < 0.05),and the expressions of GAP-43 and NGF in DRG of CCI,NS and L groups were significantly increased after surgery (P < 0.05).Comparing with the CCI group,the MWT and TWL of group L were significantly increased at the same time subgroup (P < 0.05).The expressions of GAP-43 and NGF in DRG of group L were significantly declined at the same time subgroup (P < 0.05).Conclusions Lornoxicam could relieve the symptoms of heat and mechanical hyperalgesia after sciatic nerve chronic constriction injury.It proved that lornoxicam was effective in the therapy of neuropathic pain.
3.Study of molecular genetic on diffuse large B -cell lymphoma

Objective To study the molecular genetic characteristics and immunophenotype of diffuse large B -cell lymphoma (DLBCL),thus to provide evidence in further research.Methods 92 patients with DLBCL were selected.Immunophenotype was classified by Hans method,bcl -2 and bcl -6 were detected by fluorescence in situ hybridization(FISH).The relationship between immunophenotype and genes expression was analyzed by SPSS 19.0. Results There were GCB with 28 cases(30.43%)and non GCB with 64 cases(69.57%).Bcl -2 gene abnormality was more found in GCB(χ2 =38.39,P <0.05),there was no difference of bcl -6 abnormality between GCB and non GCB(χ2 =0.56,P >0.05).There was no correlation between bcl -2 translocation and bcl -6 translocation(r =0.095,P >0.05),but there was significant correlation between bcl -2 amplification and bcl -6 translocation(r =0.750,P <0.01).There were no correlations between the location and immunophenotype,bcl -2,bcl -6(r =0.174, r =0.205,r =0.188,all P >0.05 ).Conclusion Bcl -2 gene abnormality was more found in GCB,but the relationship between bcl -2 and bcl -6 is not clear.There may be no correlation between location and molecular genetic.
4.Glycoprotein I_(1-2-1) from Irpex lacteus

Objective To study the chemical structure of a pure glycoprotein I_(1-2-1) from Irpex lacteus.Methods Based on chemical and spectral analysis,the structural characterization of I_(1-2-1) was investigated.Results I_(1-2-1) was composed of Ara,Xyl,Man,Gal,and Glu with its mean molecular weight of(40 000.) Methylation analysis showed that the main chain of I_(1-2-1)was all 1→2,1→6 linked Manp.Conclusion I_(1-2-1) is a complicated glycoprotein obtained for the first time from I.lacteus.
5.Research in influence of group intervention on quality of life of community elderly population with depression tendency

Objective To explore a new type community nursing mode which can improve the life quality of community elderly population with depression tendency.Methods In the chosen community of Jilin City,we used Geriatric Depression Scale (GDS) to identify the elderly population with depression tendency.78 elderly people with depression tendency who met the conditions of study were assigned to the intervention group and the control group with 39 people in each group randomly.8-week group-intervention was given to the intervention group,and no intervention was given to the control group.For the intervention group,the total intervention time was 8 weeks,once a week for continuous 8 times together and intervention time was 90 minutes for each time.The Geriatric Depression Scale (GDS) and the world health organization quality of life (WHOQOL-BREF) were used to measure the quality of life of the two groups.Results After intervention,the total scores of the intervention group were significantly lower than those of the control group,the difference was statistically significant.Average scores of each dimension in the life quality of the intervention group were obviously higher than those of the control group.Conclusions Group intervention mode for the community elderly population with depression tendency was feasible,effective,and reasonable.It can be applied to mental health care of community elderly people,and can also provides basis for early discovery and early intervention of the community elderly people with depression.
6.The effects of different modes of mechanical ventilation under general anesthesia on children undergoing removal of foreign body with bronchofiberscopy

Objective To explore the effects of different ventilation modes of tracheobronchial foreign body in children with fiberoptic operation under general anesthesia.Methods Sixty children (1ys≤ age≤3ys) undergoing fiberoptic bronchoscopy tracheal foreign body removal according to the combinations of different ventilation modes during and after fiberoptic bronchoscopy (FOB) procedures were divided into group A [volume control ventilation (VCV) + VCV,n =20],group B [pressure control ventilation (PCV) + VCV,n =20] and group C (PCV + PCV,n =20) randomly.The P mean,Pmax,and PetCO2 during and after fiberoptic bronchoscopy procedures were monitored.The SpO2,PaO2,and PaCO2 after mechanical ventilation 1.5 hours were recorded.Results Compared to group A,groups B and C had lower P max and P mean (P < 0.05) during the FOB procedures.Compared to groups A and B,group C had a lower P max and P mean (P <0.01) after the FOB procedures.At the 1.5 hours after the procedure,all the children showed significant increase in SpO2 and PaO2 (P < 0.05) and decrease in PaCO2 (P < 0.05) in groups A,B,and C.Conclusions When fiberoptic bronchoscopy in tracheobronchial foreign body operation is applied in children undergoing general anesthesia,the pressure control ventilation (VCV) mode can decrease the pressure of airway (Paw) and PaCO2 than volume control mode during procedure.
7.Preliminary mechanism of paclitaxel enhanced radiation sensitivity for nasopharyngeal carcinoma cells.

OBJECTIVE:
To study the radiation-sensitizing function and preliminary mechanism of paclitaxel in radiation-resistant nasopharyngeal carcinoma cells.
METHOD:
X-ray dose fractionated irradiation technology to build radiation-resistant subline of nasopharyngeal carcinoma; CNE-2S1 was treated with paclitaxel alone or combined with radiation therapy, while control group treated with radiation therapy; cell colony formation assay was used to observe sensitizing effect of paclitaxel on radiotherapy; flow cytometry analysis was used to analyze cell cycle distribution and apoptosis ratio of different treatment groups; immunoblotting was used to analyze SHP-1 expression levels of different treatment groups.
RESULT:
Nasopharyngeal carcinoma cells resistant to radiation was successfully established; cell colony formation assay showed that paclitaxel has obvious sensitizing effect on radiotherapy; FACS results showed that: CNE-2S1 treated by paclitaxel were arrested in G2M phase; paclitaxel and radiotherapy treatments significantly improved the CNE-2S1 apoptosis ratio; Western blot results showed that paclitaxel and combined radiotherapy can reduce the CNE-2S1 cells SHP-1 expression levels.
CONCLUSION
Paclitaxel enhanced radiation therapy for nasopharyngeal carcinoma cells resistant to radiation, and SHP-1 may be involved in this progress.

8.An experimental study of promoting peripheral nerve regeneration through slow-releasing diaphragm with FK506 after end-to-side neurorrhaphy

Objective To explore the effect for nerve regeneration with immunosuppresssant FK506 a-round the cut after end-to-side neurorrhaphy of peripheral nerve. Methods There were 40 adult SD rats and were divided into two groups randomly by means. In the experiment group, the right peroneal nerve of SD rat was cut off,then an 1mm epineural window was created on the neighboring tibial nerve,the distal end of peroneal nerve was sutured to the windowed tibial nerve by means of end-to-side attachment, application of slow-releasing diaphragm which was implanted around the nerve anastomosis with FK506. In the contrast group,there were not the FK506 around the cut after end-to-side neurorrhaphy. The tibial and peroneal nerve trunks were labeled by being in jected with fast blue ( KB) and fluorescence golden ( FG) respectively. The labeled cells in the dorsal root ganglia (DRG) and spinal cord were observed by fluorescence microscopy. Results There were a lot of FB labeled cells in the experiment group than in the contrast group in the DRG and spinal cord. Conclusion Immunosuppresssant FK506 could promote nerve regenerative speed and quality after end-to-side neurorrhaphy of peripheral nerve.
9.Toe pulp free flap to repair defects in the clinical application of the fingers

Objective To evaluate the toe flap repair of finger pulp defect of the clinical effects.Methods Of finger pulp defect using the first or second toe in 25 cases of free flaps, the flaps were cut size of 2.0 cm x 3.0 cm-3.5 cm x 4.5 cm, to toe at the end of artery-means the artery, subcutaneous veins-ve-nous anastomosis reconstruction of dorsal skin flap blood circulation, plantar digital nerve-refers to the inherent sensory nerve reconstruction.Results Twenty-five patients with flaps all survived, after vascular crisis occurred in 2 cases,surgical exploration and re-anastomosis of vascular survival, 3 months after flap reconstructive surgery in 12 cases.All patients were followed up for 2 months to 2 years, an average of 10 months,the fingers were satisfied with function and appearance, pulp full, two- point discrimination was 4-6 mm.Conclusion The toe plantar free flap repair of finger pulp defect may be a better clinical effects.
10.Effects of percutaneous radiofrequency ablation and repeat hepatectomy for the treatment of solitary recurrent hepatocellular carcinoma with the diameter no more than 3 cm

Objective To compare the efficacy of percutaneous radiofrequency ablation (PRFA) and repeat hepatectomy for solitary recurrent hepatocellular carcinoma (HCC) with the diameter≤3 cm. Methods The clinical data of 151 patients with recurrent HCC (diameter≤3 cm) who were admitted to the Cancer Center of Sun Yat-Sen University from January 1999 to December 2009 were retrospectively analyzed. Of all the patients, 79received PRFA (PRFA group) and 72 received repeat hepatectomy (repeat hepatectomy group). The survival rate, morbidity and recurrence of the tumor between the two groups were compared. All data were analyzed using t test, chi-square test or Log-rank test, and the survival of the patients were analyzed using the Kaplan-Meier method. Results The mobidities of the PRFA group and repeat hepatectomy group were 13% (10/79) and 36%(26/72), respectively, with a significant difference between the two groups (x2=11.411, P<0.05). The cumulative 1-, 2-, 3-, 4-, 5-year survival rates were 89.7%, 75.2%, 67.1%, 61.5%, 56.6% in the PRFA group, and 86.0%, 67.6%, 53.6%, 44.1%, 40.2% in the repeat hepatectomy group, with no significant difference between the two groups (x2=1.610, P>0.05). The cumulative 4-, 5-year survival rates of the PRFA group were significant higher than those in the repeat hepatectomy group (x2=4.682, 4. 196, P < 0.05). The local tumor recurrence rate of the PRFA group was 5% (4/79), and the incisal margin recurrence rate was 3% (2/72) in the repeat hepatectomy group, with no significant difference between the two groups (x2=0.565, P>0.05). Conclusion As a less invasive treatment method, PRFA is superior to repeat hepatectomy for solitary recurrent HCC with the diameter≤3 cm.
