Objective To investigate the image characteristics of Synovial Sarcoma, and improve the diagnostic accuracy. Methods The clinical materials and image findings of synovial sarcomas proved by pathology and were retrospectively analyzed in 13 cases. Results ①Most synovial sarcomas located in close proximity to joints, especially large joints of lower limbs . ②The mass diameter was usually more than 5cm ,and most masses presented moderate density, and some with calcification inside it on X-ray films. ③Most masses density was not homogeneous and lower than that of the muscle on CT, the demarcation was clear or not. ④On T2WI /STIR of MRI, most tumours presented slightly hyperintense "cobble" nodules , with hypointense septa among the nodules. After contrast, the nodules were unenhanced or enhanced slightly, the septa were markedly enhanced. Conclusion Some image features of synovial sarcoma ,especially on MRI, are helpful for the diagnosis of it.

Journal of Clinical Hepatology 2020;36(6):1382-1385
Recent studies have found that visfatin is involved in lipid metabolism in patients with nonalcoholic fatty liver disease (NAFLD), and about 25% of NAFLD patients have thyroid dysfunction. It has also been found that visfatin is involved in autoimmune regulation through the autocrine or paracrine pathways, which is associated with the involvement of the thyroid gland in autoimmune regulation of the body. This article preliminarily explores the association of visfatin with thyroid dysfunction in NAFLD patients and points out that the measurement of visfatin may help with the early identification of thyroid dysfunction in NAFLD patients.

Objective To evaluate specific clinical and imaging features of osteogenesis imperfecta V and to improve diagnostic accuracy of this disease. Methods Data of 15 patients with osteogenesis imperfecta type V were retrospectively analyzed for their clinical and imaging features. There were 10 males and 5 females, aged from 1 year and 30 years old (median age,12.5 years ). All 15 patients had plain X-ray, and 4 of 15 had CT. All data were analyzed by 3 experienced deputy chief doctors in OI according to OI V standard. Results X-ray:calcification of the interosseous membrane between radius-ulna was detected in all patients and calcification of the interosseous membrane between tibia-fibula was detected in 2 of 15 patients. Dislocation of the radial head was seen in 13 of 15 patients, bilateral in 9 and utilateral in 4.All patients showed restriction in the pronation and supination of the forearm and restricton in the flexion and extention of the elbow joint. Patients with dislocation of raidal head were associated with large coronoid process and olecranon of the ulna. Hyperplastic callus of the extremities were detected in 7 of 15 patients (7 at femur , 3 at humerus, 1 at tibia. In early stage, hyperplastic callus showed thin cortice, and clear boundares with the diaphysis showing and low density, irregular, mesh-like lamellation inside. In the later stage, there were dense calcification inside hyperplastic callus, and no difference in density with the diaphysis. Diaphysis surrounded by hyperplastic callus had clear boundaries with the hyperplastic callus. No cortical destruction was detected. CT:there were sparse needle-dot calcification inside hyperplastic callus, with the patterns of granular, ring-and-arch,irregular streaky mineralization. The cross section of proximal femoral shaft showed irregular shape , flat square shape and tiny medullary cavity, with no calcification on the edge of hyperplastic callus. CT value:-91 HU inside hyperplastic callus; 283 HU in femoral shaft. Conclusions Interosseous membrane between radius-ulna or tibia-fibula, hyperplastic callus ,dislocation of the radial head are specific features in osteogenesis imperfecta V. X-ray can make a definitive diagnosis of osteogenesis imperfecta V. CT scan is helpful in the differential diagnosis of osteogenesis imperfecta V from osteosarcoma.

Objective To investigate the safety of the occipital condylar screw with vertical position and evaluate the selection strategy of the posterior approach of the posterior occipital condylar screw in Chinese people. Methods The clinical imaging data of 60 outpatients from September 2013 to September 2015 were retrospectively analyzed,36 male and 24 female, the average age was 41.6±9.2 (range from 25-58),Excluded occipitocervical injury, tumor and deformity patients. We built a three-dimensional digital model and simulated placing screw by utilizing CT data on Mimics software, after that we took the occipital condyle posterior medial and lateral midpoint as the entry point, then made 2 points equidistantly to the midpoint in vertical direction. We put 3.5 mm diameter virtual screws in 4 different conditions:largest cranial angle, smallest cranial angle, longest screw path and shortest screw path. Then we assessed the anatomical relationship between the screw and the hypoglossal canal or the atlanto-occipital joint by a three-dimensional window and measured the cranial angle, medial angle and length of screw path, then calculated the safety angle of the cranial angle, the successful rate of setting screw, and compared the safety of different screw points by 3-Matic software. Results 120 occipital condyles were obtained from the CT data of 60 patients by Mimics software. There was no significant difference in the data of the cranial angle, medial angle, safety range and length between both left and right sides. The obtained safe cranial angle of each point respectively was 20.9°±6.0° (lowest point),17.0°±6.2° (middle point),and 11.6°±7.1°(top point),obviously the largest angle was in the lower point and the smallest was in the top point. The difference was statistically significant. We then acquired the successful rates of different cranial angle of each point, the highest successful rate was 99.17%,96.67%,74.17% in lowest, middle and top point when cranial angle were 3°or 4°,3°and 0°respectively.The successful rates of lower point and niddle point were significantly higher than the top point, and the difference was statistically significant. The medial angle parameters obtained were 34.41°±2.59°on left and 34.06°±2.44°on right, and there was no significant difference. The length parameters of the longest screw path acquired were 23.09± 1.47 mm,22.84± 1.40 mm and 23.15± 1.45 mm at top, middle and lowest entry point. The average value of shortest screw path of each point was 21 mm,and there was no significant difference among every entry point. Conclusion Among the occipital condyle posterior screw entering points, selecting the lower point can improve the success rate and safety;the change of nail enter point in the vertical direction has little effect on the length of the nail.We can increase the safety and reduce the risk of occipital condylar screw placement as far as possible through the three-dimensional digital technology.

AIM: The effect of low dose radiation (LDR) with different doses of X-rays on apoptosis and its related gene P53 expression were studied in spermatogenic cells of male Kunming mouse testis. METHODS: The different kinds of spermatogenic cells were separated using density gradient centrifugation and their apoptosis was measured by flow cytometry. At meantime, P53 protein and P53 mRNA was measured with immunohistochemical SABC and in situ hybridization, respectively. RESULTS: The apoptosis in all kinds of spermatogenic cells induced by LDR had a remarkable regularity. When the doses were 0 025 and 0 05 Gy, spermatogone apoptosis was domaint. With the increase in irradiation dose (0 075-0 2 Gy), spermatocytes also showed an apoptotic change, but the apoptotic percentage of spermatogonia was significantly higher than that of spermatocytes. Moreover, the apoptosis of spermatids and spermatozoa scarely occurred after LDR. P53 protein expressed in spermatogonia and spermatocytes in varying degrees, and the former was significantly higher than that of the latter after LDR. With the increase in irradiation dose, P53 protein expression showed a upregulated tendency, but that of spermatids and spermatozoa scarcely occurred. P53 mRNA primarily expressed in spermatids and spermatocytes when the dose was 0 025 Gy. With the increase in irradiation doses (0 05-0 2 Gy), that of spermatogonia also showed an enhancement. P53 mRNA expression in spermatogonia and spermatocytes showed a remarkable dose-effect relationship. CONCLUSION: The apoptosis of spermatogenic cells was selectively induced by LDR of X-rays, which had remarkable the dose-effect and time-effect relationships. The mechanism of the selective apoptosis in spermatogenic cells by LDR is closely related to the upregulation of P53 .
