1.Mechanism of kidney lesion in rats suffered from obstructive jaundice

Objective To explore the lesion mechanism and the apoptosis related protein changes of kidney in rat suffered from obstructive jaundice. Methods Choledocho-ligation methods was used to create rat obstructive jaundice model. Six rats in the normal control group and thirty rats of experiment group were observed at day 1, 3, 6,10 and 14 after rat model establishment. HE staining for kidney pathological changes, TUNEL staining for cell apoptosis, SABC for apoptotic related proteins of survivin and bax, xanthine oxidase method for SOD activity and thiobarbiturate method for MAD content were carried out. Results Renal tubular epithelial cell necrosis, cell shedding and aggregation were the main pathological changes in the experiment group. In the experiment group, the apoptosis ratio increased significantly, and reached the peak of (25.2?2.8)% on postoperative day 14. The relative gray value of survivin in the experiment group decreased significantly and of bax increased significantly. The plasma SOD activity in the normal group was (193.6?23.5) U/L, but it in the experiment group decreased significantly. The plasma MDA content and the endotoxin level in the experiment group increased significantly. Conclusion Cell apoptosis is one of the lesion mechanisms of kidney in rats suffered from obstructive jaundice. The down-regulation of survivin and the up-regulation of bax may be the biological factor in the occurrence of apoptosis. The increasing of free radical and endotoxin are important factors in the kidney lesion.
2.Treatment of polypoid lesions of gallbladder by Traditional Chinese Medicine

This article reviewed literatures on the treatment of polypoid lesions of gallbladder(PLG) with Traditional Chinese Medicine(TCM) in recent years. The research showed that TCM has satisfied therapeutic effects on PLG, which can avoid unnecessary operation. But the systemic theoretic and experimental studies have been insufficient, and further studies are needed.
3.Nutritional Support for Acute Pancreatitis

Objective To investigate the clinical advantages of enteral nutrition (EN) for acute pancreatitis(AP)comparing with parenteral nutrition (PN) and its prospect. Methods Literatures using MESH Browser in Medline were collected and reviewed. Results Being of much higher cost and complications, total parenteral nutrition (TPN) ever regarded as most importantly for AP nutritional support, is now challenged by EN. Clinical evidence suggests enteral (jejunal) nutrition may diminish intestinal permeability to endotoxin and diminish bacterial translocation, thus reducing the cytokine drive to the generalized inflammatory response and preventing organ dysfunction, as well as achieving “pancreatic rest” equivalent to the TPN. Conclusion Early enteral nutrition should be used preferentially for patients with severe acute pancreatitis without paralytic ileus.
4.Localization of adult cerebral central sulcus on the coronal MR imaging

Objective To localize precisely cerebral central sulcus on the coronal MR imaging. Methods The serieses of 4 mm thickness coronal sections and coronal MR images of brain were prepared on 30 cadaver heads. The preparation of the brain slices in correspondence with MR scan took the line perpendicular to the midpoint of intercommissural line as base one. The important cerebral gyri of cadaver were painted different colors. The coronal MR images of 6 volunteers were also prepared. The localization of central sulcus on the coronal sections was studied by the comparison of MR images with serial coronal sections of the brains. Results On the section of the central sulcus appearing for the first time, the appearing rate of dorsal thalamus was: left side 93%, right side 97%; the appearing rate of pyramidal tract was: left side 20%, right side 13%. The central sulcus almost disappeared on the section through the posterior border of splenium of corpus callosum, the appearing rate of the central sulcus on the superolateral surface was 33% on the both sides. Conclusion The central sulcus appears for the first time on the 1-2 sections which the dorsal thalamus appears on but the pyramidal tract doesn′t appear. On the section through pyramidal tract, the superior frontal, precentral and postcentral gyri can be identified according to the three medullary branches on superolateral surface of cerebral hemispheres. The central sulcus has almost disappeared on the section through the posterior border of splenium of corpus callosum.
5.Development and application of network information system for laboratory equipment management

Objective To develop a set of network information system for laboratory equipment management. Methods According to the requirement of laboratory equipment management provided by laboratory section, a professional laboratory software company developed the software by using SQL Server 2000 as database and Delphi 7.0 as program language. Results Data of laboratory equipment in hospital were shared and communicated. Laboratory equipment management was standardized and work efficiency and quality were improved. Conclusion The software is convenient and practical with high popularization and application value.
6.Best Operation Mode of AP-960 Full-automated ELISA System

Objective To compare the operation modes of AP-960 full automated ELISA system.Methods Two models were tested: beginning with adding specimens(model 1) and beginning with adding enzymes(model 2).The following factors were compared: the consumption of suckers,usage of time,result of test and the usual alarm during the test.Results The consumptions of suckers were 564 and 84 respectively.The needing time of the former mode was 125 minutes and that of the latter mode was 90 minutes.Conclusion The mode beginning with adding enzymes(model 2) is better for the current status of our clinical laboratory.
7.Influence of local-anaesthetic combined with dezocine on brachial plexus block and postoperative analgesia

Objective To observe the influence of local-anaesthetic combined with dezocine on brachial plexus block and postoperative analgesia.Methods 60 patients scheduled for unilateral upper limb operation were randomly divided into two groups of C group and D group with 30 cases in each group.The brachial plexus block was performed under ultrasound guiding.Two groups received a bolus injection of local anesthetic 30ml,but the local bolus injection of D group consisted of Dezocine 10mg.The tine of sensory blocked producing effect,anesthesia effect,sensory blocked duration were observed.Two hours after sensory nerve recovered,the scores of VAS pain and Ramsay Sedation were recorded.Adverse reactions of perioperation were also observed(nausea,vomiting,dizzy,respiratory,urine retention,pruritus).Results The sensory blocked duration in D group was(7.1±1.1)h,which was significantly longer than that of C group[(4.5±0.8)h](t=10.47,P<0.01);Two hours after sensory nerve recovered,the score of VAS pain in D group was(1.7±0.7)points,and significantly lower than that of C group[(3.9±1.0)points](t=9.87,P<0.01).Both two groups had no adverse reactions.Conclusion Addition dezocine 10mg to local anesthetic solutions for interscalene brachial plexus block,it could prolong the sensory block duration,provide satisfactory postoperative analgesia,have no adverse reactions.
8.Clinical application of 2000 ArthroCare System in knee arthroscopic surgery

Objective To evaluate the clinical application of 2000 ArthroCare System in knee arthroscopic surgery. Methods 221 cases of knee problems were treated with 2000ArthroCare System. The disorders of the 221 cases diagnosed by the arthroscopy were as follows: 73 cases of osteoarthritis, 49 meniscus tear, 29 degenerative cartilage injury, 11 plica synovitis, 11 Kaschin Beck disease, 8 ACL, 5 osteochondritis dissecans, and 2 TKA brisement. The operative procedures, such as meniscectomy, meniscoplasty, fitting of cartilage and ligament, synovectomy, and release of lateral patellar retinaculum, were done with 2000ArthroCare System and arthroscopy. Results The Lysholm Knee scores were 43.92 preoperatively, 81.96 three months postoperatively, and 92.06 six months postoperatively. Conclusion Knee problems can be effectively released with 2000 ArthroCare system vaporization under arthroscopic guidance. The advantages of this procedure are very limited tissue damage, mild reaction, less blood loss, early rehabilitation, and fine functional recovery.
9.Hospital Fungal Infection: Distribution and Drug Sensitivity

OBJECTIVE To understand distribution of fungi and condition of drug sensitivity to provide information for preventing nosocomial fungal infection,rational administration and approaching therapeutic strategy.METHODS CHROMagar coloration medium,API20C AUX systemic identification breed,identifying drug selected test chart of Hema-biotech BACT-IST,and susceptibility test were used according to broth micro-dilution antifungal susceptibility test(Mi-AFST) of CLSI M27-A2.RESULTS The major strains isolated from hospital infection samples were Candida albicans(60.04%)and C.tropicalis(22.22%),and mainly from sputa(69.03%),and the key departments were internal respiratory medicine(38.29%),intensive care unit(ICU)(31.91%) and senior cadre ward(16.54%) orderly.According to the sensitivity of antimycotic drugs the flucytosine(97.39%) and amphotericin B(95.27%) were relatively sensitive,the effective rate and resistance rate of antimycotic drugs were 91.00% and 44.00%,respectively,the rate of fungus detected without clinical symptoms was 10.63%.CONCLUSIONS The clinical diagnosis and therapeutic strategy are determined by clinical symptoms and laboratory results of drug sensitivity;and the prevention of fungal infection is also important.
