1.Different concentrations of insulin affect the growth and proliferation of in vitro cultured cardiomyocytes from neonatal rats

BACKGROUND:Study on mouse models of hyperinsulinemia confirms that hyperinsulinemia can induce the occurrence of myocardial hypertrophy. OBJECTIVE:To study the effect of insulin on proliferation and hypertrophy of cardiomyocytes in neonatal mice in vitro cultured. DESIGN,TIME AND SETTING:The in vitro cytology experiment was performed at the Key Laboratory of Anatomy,Histology and Embryology of Xinxiang Medical University from January 2006 to March 2007. MATERIALS:Twenty Wistar neonatal mice aged 1-3 days were used in this study. Insulin was obtained from Sigma,USA. METHODS:Cardiomyocytes of neonatal mice were in vitro harvested,culture and purified by trypsin digestion and differential attachment technique. Cardiomyocytes were incubated in 10-8,10-7,10-6 mol/L insulin for 48 hours. Blank control group was set up. 10-7 mol/L insulin group was used for cell cycle analysis. MAIN OUTCOME MEASURES:Cell diameter,activity and counting were measured under a microscope. The expression of cyclin E in cardiomyocytes was measured using immunocytochemistry. RESULTS:Cardiomyocyte activity in each group was over 90%. Compared with the blank control group,long diameter and short diameter of cardiomyocytes in the 10-8,10-7,10-6 mol/L insulin groups were significantly increased(t=2.781 5,P 0.05) ,but the number of cardiomyocytes in the 10-6 mol/L insulin group significantly decreased(t=1.976 2,P
2.Rapid induction and expansion of HBV-specific CD8 T lymphocytes from a patient with acute hepatitis B

Objective To develop a rapid expansion assay and determine the predominant epitope for HBV-specific CD8 T cells from peripheral blood mononuclear cells(PBMC) of patients with acute hepatitis B(AHB).Methods PBMC were isolated from a patient with HLA-A2 positive AHB,and the frequency of HBV-specific CD8 T-cells was determined using 3 kinds of HLA-A2 antigenic peptide florescence-labeled pentamer agents,i.e.S183-191 and S335-343 derived from HLA-A2 restricted HBsAg epitope,and C18-27 derived from HBcAg epitope.Cytokines and corresponding epitopic peptides were used for induction of positive CD8 T-cell expansion and enrichment in PBMC for 1-2 weeks.T epitope-specific CD8 T-cells were quantified and sorted by flow cytometry.Results After cultivation with antigenic peptide stimulation and proper cultural condition,the frequency increased significantly in the pentamer positive HBV-specific CD8 T cells in total CD8 T cells.The peak value was observed in 2-week cultivation.At the time the HBV-specific CD8 T cell frequencies increased from 0.21% to 15.17% for S183-191,and from 0.44% to 1.79% for S335-343,representing 73.8-folds and 3.1-folds increase for each epitope respectively.However,the frequency of C18-27-specific CD8 T-cells was negative and without obvious proliferation after stimulation.Conclusions Under stimulation of HBV epitopic peptide in proper culture condition,the specific CD8 T lymphocytes may be induced and expanded in quantity via sort-term conditioned culture.CD8 T cells specific for S183-191 may play an important role in the control of acute HBV infection.
3.An analysis of the clinical features of 424 cases of hepatitis E

171.1?mol/L) was 25.0%, 37.7% and 46.3%, the percentage of the fulminating hepatitis was 3.0%, 5.9% and 14.0%, and the mean hospital stay days for the convalescent patients were 31.59?18.97, 31.13?13.70 and 37.51?18.33 days, respectively. All these variables were significantly higher in aged group compared with youth and middle-aged groups. The levels of alanine transaminase (ALT) lowered significantly with the advance of age, and they were 1 711.7?1 063.4, 1 423.0?913.2 and 1 162.7?792.5U/L, respectively. The same was true in serum albumin and choline esterase (ChE). On the other hand, the percentage of acute hepatitis without jaundice decreased to 17.9%, 6.4%and 2.5% from the youth to the aged. However, TBIL on admission (131.41?106.97 vs 169.60?136.11 vs 164.36?106.22?mol/L) and the elevation of the peak of TBIL (135.01?109.15 vs 186.08?150.64 vs 209.63?143.74?mol/L) of the middle-aged and the aged were significantly higher than those of the youth. There was no significant difference in the symptoms, the ratio of direct to total bilirubin, mortality to complications among them. Furthermore, the serum IgM and IgG antibodies against HEV were negative in about 8.1% of the patients with HEV. Conclusion With a higher incidence and more severe hepatic damage, the middle-aged and the aged patients need a longer duration for recovery than the young patients.

Objective To investigate antimicrobial resistance of Escherichia coli (E.coli )isolated from patients with bloodstream infection,and provide evidence for rational use of antimicrobial agents in clinical practice.Methods BacT/A-lert automated blood culture system and VITEK 2 automated identification system were used for bacterial culture and identi-fication.Antimicrobial susceptibility testing and detection of extended-spectrum β-lactamases (ESBLs)-producing strains were performed by Kirby-Bauer method.Results From 2009 to 2011 ,a total of 235 strains of E.coli were isolated from patients with bloodstream infection,90 (38.30%)of which were ESBLs positive strains.The resistant rates of ESBLs-producing strains to ampicillin,cefotaxime and ceftriaxone were all 100%,but susceptibility rate to imi-penem/cilastatin and meropenem were all 100%,to cefmetazole and amikacin were >90%.The resistant rate of non-ESBLs-producing strains to ampicillin was the highest (70.63%),susceptibility rate to imipenem/cilastatin and meropenem were both 100%,to amikacin,cefotaxime,and cefmetazole were all >95%.The resistant rate of ES-BLs-producing strains was significantly higher than that of the non-ESBLs-producing strains.Ofβ-lactamase inhibi-tor,only susceptibility rate of ESBLs-producing E.coli to cefoperazone/sulbactam was>90%,susceptibility rates to piperacillin/tazobactam and ticarcillin/clavulanate were both<80%.Conclusion Antimicrobial resistant rate of ESBLs-producing strains causing bloodstream infection is high,individualized treatment strategies should be made according to antimicrobial resistance of bacteria causing infection in patients.
6.Contrasted study between thin coronal sectional anatomy of the pineal region and MRI image

Objective To investigate the morphology and relationships with the adjacent structures in the pineal region on the thin sections and to provide anatomic data for imaging diagnosis and surgical treatment of diseases in this region. Methods By CT and MRI examination, one normal head specimen was selected for this study. Using the computerized freezing milling technique, the specimen was sliced from anterior to posterior. The in vivo MR images were obtained from ten normal Chinese male adult volunteers using a 3.0 T GE scanner. The base lines of the sectioning and the MR scan were perpendicular to the AC-PC line. Then primary sections were contrasted with the corresponding MR images. Results By the appearance of the pineal peduncle and the disappearance of the pineal gland, the pineal region could be divided into three parts from anterior to posterior, and the shape changed from an inverted triangle to a trapezoid and a triangle gradually. The first interspace was getting wider in the anterior and middle parts of the pineal region, while in the posterior part of the pineal region, it was getting narrower and disappeared finally. From anterior to posterior, the bilateral internal cerebral veins were always in the midline of the pineal region and descended gradually.Conclusion By the computerized freezing milling technique, the anatomic details and adjacent relationships of the pineal region could be exhibited clearly in the thin serial sections, which could help the imaging diagnosis and surgical treatments for minute diseases in this region.
7.Clinical study of bean bag in lateral position placing in pulmonary operations

Objective To investigate the feasibility and safety of Bean Bag in lateral position placing during pulmonary operations,evaluate its effects by comparing with the routine placing of lateral position by sandbags and side shields, and to provide scientific evidences to solve the existing clinical problems in lateral position placing. Methods One hundred patients with pulmonary surgeries who needed lateral position placing were divided into Bean Bag group and routine method group randomly according to random digit table,50 patients in each group.Bean bag was used in lateral position placing in Bean Bag group,while sandbags and side shields were used in routine lateral position placing in routine method group. The required time for positioning and skin pressing condition, the activity of both upper limbs during postoperative follow-up were recorded.The satisfaction of operating surgeons on this position was acquired by self-designed questionnaire when operation was finished. Results The required time for positioning in Bean Bag group was(178.36±24.27)seconds,and that for positioning in routine method group was (282.06 ± 29.34) seconds, there was statistically significant difference between two groups (t=19.254,P<0.01).There were 14 patients who appeared skin injury and press red in Bean Bag group and 27 patients who appeared skin injury and press red in routine method group, and there was statistically significant difference between two groups(χ2=6.986,P=0.008).In Bean Bag group,the total score of seven items in satisfaction questionnaire of operating surgeons on patient's position was 38.34±1.36,while that in routine method group was 29.34±1.29,there was statistically significant difference between two groups(Z=33.924,all P<0.01). Conclusions Our study indicated that it was feasible and safe to use Bean Bag to place lateral position.The exposure of operating field was good and the stability of position was strong. Compared with routine position placing method, lateral position placing by using Bean Bag could save time, the operating procedures were more simple and convenient, and it could protect the physiological function of all aspects in patient's body more effectively, the satisfaction of surgeons for this method in lateral position placing was higher. It might deserve to popularize this method in the clinical practice in the future.
8.Hepatic VX2 tumor in rabbits: treated with radio frequency ablation and evaluated with enhanced CT.

OBJECTIVETo explore and evaluate the effect of radio frequency ablation (RFA) in treatment of hepatic VX(2) tumor in rabbits.
METHODSThe hepatoma was treated with RFA in rabbits. The complete necrotic rate of the tumors, pathological changes, CT images and the animals' survival time were observed.
RESULTS(1) ALT in serum increased significantly on the first day and decreased to the control level during 4th-7th day following RFA. (2) On the second week, CT scans showed that complete necrotic foci became larger, and the density of which was asymmetrical. Enhanced CT scans showed no obvious intensification inside; however, ringed intensification appeared along edges of the foci. Biopsy showed that the dark necrotic tissue was surrounded by ringed granulation tissue. Incomplete necrotic tumor foci resembled the complete necrotic foci in no enhanced CT images; however, asymmetrical intensification was observed in enhanced CT scans in the incomplete necrotic foci. Macroscopic observation showed irregular tumor tissue between the necrotic tissue and the peripheral normal tissue. Biopsy showed tumor recurrence in it. (3) Compared with the control, survival time of the animals was longer, and metastases rate in lungs and the fatality rate were lower in the treatment group.
CONCLUSIONSRFA, with little injury, is an effective method in the treatment of hepatoma. Enhanced CT scans are consistent with pathological morphometrics of the tumor foci after RFA. The enhanced CT scan is a valuable check-up, which could be used to observe the therapeutic effect after RFA.

9.Computational omics biology model predicts therapy response of refractory acute myeloid leukemia:report of 1 case and review of literature

Objective:To investigate the value of computational omics biology model (CBM) in treatment of refractory acute myeloid leukemia (AML) patients.Methods:The clinical data of a refractory AML patient who received personalized therapy regimen predicted by Cellworks tumor response index (TRI) test in November 2018 were retrospectively analyzed. The diagnosis, treatment and the therapeutic efficacy were summarized. The literature related to CBM in AML was reviewed.Results:The patient, a 43-year-old female, was diagnosed as AML accompanied with t(6;11)(q27;q23). She failed to respond after 2 courses of induction therapy, and had poor tolerance of chemotherapy. And then the Cellworks TRI test recommended the 3-drug combination regimen of cladribine, trametinib and cytarabine as the optimal chemotherapy regimen. After 1 course of treatment, the patient achieved complete remission and minimal residual disease negative. After remission, the patient successfully underwent haplo-hematopoietic stem cell transplantation. She experienced a prolonged disease-free survival of 19 months and relapsed in November 2020, and passed away in April 2021. The overall survival time was 28.5 months.Conclusions:Cellworks TRI test based on CBM provides a new therapeutic approach for refractory AML patients, and its personalized treatment regimen based on genomics may improve the survival of patients.
