1.Blood flow dynamic changes after mitral valve replacement operation in patients with mitral valve stenosis by color Doppler echocardiography

Objective To study the blood flow dynamic c hanges after mitral valve replacement(MVR) operation in patients with mitral valve stenosis by color Doppler echocardiography. Methods Thirty-seven- patients with mitral stenosis after MVR operation were analyzed with color Doppler echocardiography retrospectively, the observed items including dimension of left atrium, tricuspid valve backflow velocity, pulmonary artery systolic pressure(PAPS) and left ventricular ejection fraction(LVEF).Relativity analysis was conducted between PAPS and trans-mitral valve pressure(MV?P). Results All the patients survived healthy, the average follow-up time was 15.8- months. The follow-up data suggested that the dimension of left atrium, tricuspid valve backflow velocity and PAPS decreased significantly while LVEF increased significantly to the pre-operation. PAPS was in positive correlation with MV?P and the difference between PAPS and MV?P increased according to MV?P.Conclusions MVR can ameliorate flow dynamics effectively in patients with mitral stenosis, PAPS is in positive correlation with MV?P, senior stenosis existed within pulmonary circulation in patients with severe pulmonary hypertension.
2.Quantitative study of the cellular nuclear DNA contents in odontogenic cysts and ameloblastoma.

OBJECTIVETo investigate the proliferative potential of the epithelial cells in odontogenic keratocyst, radicular cyst, dentigerous cyst and ameloblastoma.
METHODSDNA contents and ploidy of basal and spinous cells in keratocyst, radicular cyst, dentigerous cyst, and the peripheral column cells and central reticular cells in ameloblastoma were analysis respectively.
RESULTSThe more and higher DNA contents and the proliferating ploidy of keratocyst and ameloblastoma than those of radicular cyst and dentigerous cyst indicate the active proliferating potential. The spinous cells showed more active proliferating growth than the basal cells of keratocyst. The higher DNA contents of radicular cyst are related to the stimulus of the inflammation. The dentigerous cysts have more di-ploidy cells without active growth potential.
CONCLUSIONSThe active cell proliferating growth in keratocyst and ameloblastoma is probably the pathological basis of their local aggressive biological behavior.

3.Analysis of 112 cases of infectious endocarditis.

OBJECTIVETo analyze the clinical manifestations, epidemiological features, pathogens, treatment and outcomes of 112 patients with infectious endocarditis.
METHODSThe clinical data were reviewed for all the patients with the diagnosis of infectious endocarditis in Nanfang Hospital over the period from 2002 to 2012.
RESULTSThe average age of disease onset was 46∓17.1 years among these cases. Seventy patients (62.5%) had underlying cardiac conditions, including, in the order of frequency, rheumatic heart disease, congenital cardiovascular anomalies, and senile valve degeneration. Clinically the patients presented with fever, anemia, congestive heart failure, and embolism events, etc. Echocardiography found endocardial neoplasms in 105 cases, and the mitral valve was often involved. Blood culture yielded positive results in 61 cases, and the most common causative bacteria were Streptococcus (20.5%), Staphylococcus (19.6%) and Enterococcus (5.4%). Surgical intervention was performed in 78 cases. Ninety-one patients were cured or improved after comprehensive treatment, and 10 discontinued treatment for financial reasons; 11 died of cardiac failure (8 cases), pulmonary embolism (2 cases) and central nervous system events (1 case).
CONCLUSIONInfectious endocarditis shows a significant change in the clinical profiles and its mortality remains high despite improved diagnostic and therapeutic procedures.

4.Ultrastructural changes of TMJ articular cartilage and synovial membrane following occlusal trauma in rabbit.

OBJECTIVE: To study the effects of occlusal trauma on the ultra-structure of synovial membrane and articular cartilage in rabbit's temporomandibular joints (TMJ). METHODS: TMJs from six rabbits with occlusal trauma and three control rabbits were studied by transmission electron microscopy. RESULTS: Degenerative changes in synovial membrane and articular cartilage of TMJ were induced following occlusal trauma. The structure of the articular surface was damaged, and chondrocytes in cartilage showed signs of degeneration. The synovial lining cells contained dense accumulations of vimentin intermediate filaments (IFs), which were especially prevalent in the cellular processes as well as paranuclearly. Microvilli on the synovial cell membrane were commonly seen. The "vermiform bodies" in the deeper interstitium of the synovial tissue were also found. Our findings of the punctate adherens between synovial lining cells were described in detail. CONCLUSIONS: The occlusal trauma is really a factor inducing degenerative changes of the TMJ.
5.Composite mandibular allografts in canines.

OBJECTIVE: To evaluate the feasibility of transplanting composite mandibular allografts to repair large mandibular defects. METHODS: Three composite mandibular trans-plantation models were established. The first model consisted of hemimandible with the attached teeth, muscle and skin,and oral mucosa. The second model was transplanted in the same way with the first one excluding oral mucosa and some teeth, and third one excluding the oral mucosa and all dental crowns. Fourteen transplanting operations were performed in canines. Cyclosporine A and methylprednisone were given for immunosuppression. RESULTS: The composite mandibular organs had an effective and closed return circuit. Transplantation of vascularized allograft of mandibular compound organs was feasible. Two longest time survivors of 67 d and 76 d were in the third model group. Cyclosporine A was successful in suppressing rejection of transplanted composite allograft and prolonging survival time of transplantation models. CONCLUSIONS: The composite mandibular allografts were available with large block of living composite tissue, and helpful in restoration of appearance and function for severe mandibular defects.
6.Plane measurement of three-dimensional CT images on the diagnosis of zygomatic fractures.

OBJECTIVETo investigate an available method for quantitative diagnosis of zygomatic fractures to quantitatively classify and diagnose the deformities of zygomatic fractures.
METHODSTraditional X-ray, two and three dimensional CT images of 96 patients with zygomatic fractures were measured and analysed by computer-assisted measuring system before and after operation to diagnose the displacement and deformity of the fractures.
RESULTS1. Using anthropology points and computer assisted measuring system, a new method was established to assess the displacement of the zygomatic fractures. 2. Displacement of the zygomatic fractures was approximately diagnosed by measuring different angle of 3D CT photographs. 3. A classification was proposed based on the deformities of the zygomatic fractures.
CONCLUSIONSComputer-assisted 3D CT measuring system is accurate, reliable and feasible in clinic for diagnosing zygomatic fractures.

7.The research and development of CAD-CAM system in restorative dentistry.

OBJECTIVETo develop a dental CAD/CAM system with the autonomic intellectual property.
METHOD28 standard teeth crowns were scanned using a newly 3-D laser scanner. As a development platform the Matlab 5.3 were used to process the acquired data, also be used to define the characterized areas on the surfaces of the crowns and to change the crowns form. The software of Surfacer 10.5 to develop a new CAD software for fixed prosthetics, and the 3.5 axis numerical controlled machine to manufacture the prosthetics were used.
RESULTIt is the first time, the 3D graphic data bank of Chinese teeth crowns with a standard form was established. A software of the occlusal adjustment and the form modification were developed. It is also the first time, the authors realized the whole process to use the CAD-CAM for the manufacture of a crown.
CONCLUSIONThe successful result shows, that we have already mastered well the base theory, the mathematics method, the technology of a CAD-CAM system. It provides the basics for the future development.

8.Clinical progress of acupoint application in the treatment of cirrhotic ascites

As an important part of the external treatment of Traditional Chinese Medicine (TCM), acupoint has the advantages of simple operation, remarkable effectiveness and safety. Recently, it has been widely used in the treatment of patients with cirrhosis ascites, either alone, or in combination with other external treatments. It can improve ascites symptoms and reduce recurrence rates. Physicians select acupoint based on the meridian acupoint theory or their own methods. The acupoints are mainly Shenque (CV 8), Qihai (CV 5), Zusanli (ST 36), Guanyuan (RN 4), Zhongwan (CV 12) and other Ren meridian acupoints, supplemented by the bladder meridian, the liver meridian, the stomach meridian, and the spleen meridian. The Chinese medical drugs, mostly in terms of compounds, have function for relieasing stasis water, regulating qi and tonifying deficiency. In the future, it is necessary to strengthen basic research, treatment based on syndrome differentiation and improve the quality of clinical researchs.
