1.Effect of buqi tongluo prescription on recovery of peripheral nerve conduction velocity of rats

BACKGROUND: Buqi tongluo prescription, which is characterized by invigorating qi and promoting blood circulation, dredging collaterals and dispersing stagnation, promoting tissue regeneration, and promoting blood circulation to remove obstruction in the collateral, can promote blood circulation at local injured part and ameliorate nutrien of nerve so as to accelerate axonal regeneration of peripheral nerve and functional recovery of nerve conduction.OBJECTIVE: To observe the effect of buqi tongluo prescription of various agents on recovery of nerve function after peripheral nerve injury and compare the results with the combination of vitamin B1 and B6.DESIGN: Randomized controlled animal study.SETTING: Department of Orthopaedics of the First Hospital Affiliated to Guangzhou University of Traditional Chinese Medicine.MATERIALS: A total of 48 Wistar rats of confined clean grade were selected in this study. After modeling on right sciatic nerve, rats were randomly divided into buqi tongluo capsule group, buqi tongluo parenteral solution group, vitamin group and blank group with 12 in each group.METHODS: All rats were cut their right sciatic nerve laterally, and then anastomosis in situ was used immediately to establish models of peripheral nerve injury. Administration began on the third day after operation once a day. ① Rats in buqi tongluo capsule group were perfused with powder of buqi tongluo capsule (including huangqi, renshen, danggui, chuanxiong and danshen which was diluted with saline) of 0.9 g/(kg·d) raw drugs (ig). ② Rats in buqi tongluo parenteral solution group were injected with 0.9 g/(kg·d) buqi tongluo parenteral solution (ip). ③ Rats in vitamin group were perfused with 15 mg/(kg·d) suspension (1.5 g/L) of vitamin B1 and B6.④ Rats in blank group were perfused with 0.01 mL/g saline.MAIN OUTCOME MEASURES: Four rats from each group were selected for 4, 8 and 12 week time points after administration to record bilateral nerve conduction velocity. Recovery rate was regarded as the ration between nerve conduction velocity at the injured side and nerve conduction velocity at the normal side.RESULTS: A total of 48 rats were involved in the final analysis. ① At each time point, nerve conduction velocity in two buqi tongluo prescription groups were faster than that in vitamin group and blank group (P < 0.05, 0.01), and that in vitamin group was faster than that in blank group (P < 0.05). ②At each time point, recovery rate of nerve conduction velocity in two buqi tongluo prescription groups was higher than that in vitamin group and blank group (P < 0.05, 0.01), and that in vitamin group was higher than that in blank group (P < 0.05).CONCLUSION: Both buqi tongluo capsule and buqi tongluo parenteral solution can promote recovery of nerve function after peripheral nerve injury, and the effect is superior to that of vitamin B1 and B6.
2.Research for GC fingerprint chromatography of Yujin Injection

AIM: To establish the fingerprint chromatography of Yujin Injection (Herba Houttuyniae, Flos Lonicerae) by GC. In the paper the authors examine the similarity among standard and sample chromatogra-phies. METHODS: GC was used to analyze the volatile ingredients, SGE 30QC3 colume (30m? 0.32mm? 0.5?m) was used with column temperature from 100℃ to 170℃ with 2℃?min -1 below 150℃ and 5℃?min -1 above 150℃, flow rate at 1.0mL?min -1 and detector temperature at 200℃. RESULTS: Standard fingerprint consisted of 17 marker peaks, the comparison of similarity's RSD to the injection of different batch had no more than 2%. CONCLUSION: According to the selected chromatographic conditions, a good fingerprint of the injection has been described. The method is simple, accurate with good reproducibility. It may be practical for the quality control of Yujin Injection.
3.Study on microRNA expression in endometrium of luteal phase and its relationship with infertility of endometriosis

Objective To study the different expression of microRNA(miRNA) including mir-29c,mir-200a,mir-145 in the mid-secretary endometrium and its relationship with infertility of endometriosis.Methods From August 2011 to February 2013,36 infertile cases with endometriosis confirmed by laparoscopy and pathology and excluded the other infertile factors in Department of Reproductive Medicine in Peking University Shenzhen Hospital were enrolled in this study,which were divided into 17 cases with stage Ⅰ-Ⅱ,19 cases with stage Ⅲ]-Ⅳ according to the revised classification American Fertility Society.Forty-four healthy women with male factor infertility were chosen as control group.The relative expression levels of mir-29c,mir-200a,mir-145 in the endometrium of women in two groups were detected by using real-time quantitative polymerase chain reaction.Those women were followed up for pregnancy outcome of endometriosis group after assisted reproductive techniques (pregnancy and nonpregnancy group respectively).Results (1) The expression of miRNA between endometriosis and control groups:the average expression level of mir-29c,mir-200a,mir-145 in the endometrium of endometriosis group was 2.46 ± 1.98,3.20 ± 2.45,6.378 ± 3.275,which were significantly higher than 1.36 ± 1.05,2.04 ±1.16,4.548 ± 1.885 in control group (P =0.026,0.027,0.041,respectively).(2) The expression of miRNA between Ⅰ-Ⅱ stage and Ⅲ-Ⅳ:the average expression level of mir-29c,mir-200a,mir-145 in the endometrium was 0.53 ± 0.51,0.33 ± 0.26,0.048 ± 0.021 in patients with of stage Ⅰ-Ⅱ,while 0.26 ±0.18,0.28 ±0.12,0.045 ±0.016 in stage Ⅲ-Ⅳ respectively,there were no statistically significant differences between the two groups(P =0.191,0.661,0.753,respectively).(3) The expression of miRNA between pregnancy and non-pregnancy groups:the average expression level of mir-29c,mir-200a,mir-145 in the endometrium were 0.60 ± 0.30,1.23 ± 0.48,0.886 ± 0.238 in pregnancy group,while 2.64 ± 1.73,4.39 ± 2.58,7.199 ± 3.945 in non pregnancy group,there were statistically significant differences between the two groups(P =0.030,0.039,0.028,respectively).Conclusion Up-regulation of mir-29c,mir-200a,mir-145 in the endometrial tissue might play a role in endometriosis associated infertility.
4.Assessment of transcatheter closure of coronary artery fistula by transthoracic echocardiography

Objective To investigate the efficacy of transcatheter closure of coronary artery fistula (CAF) by transthoracic echocardiography(TTE) and the role of TTE in this therapy. Methods CAF were occluded with transcatheter closure techniques in 17 patients. TTE was performed before and after the treatments. The key points were retrospectively analyzed including: the sites of CAF, the position and diameter of CAF,the shape and position of the devices after the intervention,the residual shunt,and cardiac chamber size. Results Before the therapy,TTE made definite diagnosis of CAF. All patients had lesions in single coronary artery, and the diameter of CAF was 2-14 (6.4 ± 3.5)mm. All patients underwent transcatheter closure successfully. TTE revealed the shunts disappear 1-4 days after the occlusion. During a follow-up period of 1- 29 (11.7± 7.9)months,no residual shunts, no complications, and normal size of thecardiac chambers were recognized by TTE. Conclusions The transcatheter closure of CAF has emerged as a less invasive, safe and effective strategy. Echocardiography has important role in primary screening of patients and the follow-up after the treatments.
5.Evaluation of stroke prognostication using age and National Institute of Health Stroke Scale index for outcome after early endovascular treatment for anterior circulation large vessel occlusion

Objective To evaluate the value of stroke prognostication using age and National Institute of Health Stroke Scale index (SPAN) for outcome after early endovascular treatment for anterior circulation large vessel occlusion.Methods The patients who underwent early endovascular treatment were prospectively,sequentially collected in Yijishan Hospital of Wannan Medical College from December 2014 to September 2017 and Jinling Hospital from March 2014 to March 2017.Individuals whose age in years plus NIHSS score was greater than or equal to 100 were designated as SPAN-100-positive patients,while those with a score less than 100 were designated as SPAN-100-negative patients.We compared the baseline data and perioperative data between the two groups.The 90 days modified Rankin Scale score≤2 was regarded as favorable outcome.Single factor and multivariable Logistic regression analyses were used to determine the association between SPAN-100 and outcomes.Results One hundred and ninety patients were enrolled,20 (10.5%) of which were SPAN-100 positive,and 170(89.5%) were SPAN-100 negative.There were no significant differences between the two groups on postoperative intracerebral hemorrhage and 90 days mortality.Ninety days independence rates were higher in SPAN-100-negative patients (77/170,45.3%) than in SPAN-100 positive patients (4/20,20.0%;x2 =4.681,P =0.030).Multi-factor Logistic regression analysis showed that the higher preoperation systolic pressure (OR =1.030,95% CI 1.008-1.052,P =0.007),the lower Alberta Stroke Program Early CT Score (OR =1.609,95% CI 1.056-2.453,P =0.027) and poor collateral circulation(OR =5.714,95% CI 1.668-19.570,P =0.006) were the independent risk factors of outcomes.Conclusion SPAN-100 is not an independent predictor of favorable outcome after adjusting for factors of outcomes in patients with anterior circulation large vessel occlusion.
6.Endoscopic radiofre-quency ablation combined with metal stent implantation and simple metal stent implan tation forunrespectable cholangiocarcinoma

Objective To retrospectively analyze the therapeutic effect of endoscopic ablation combined with metal stent implantationon biliary obstruction caused by unrespectable cholangiocarcinoma. Methods Fourty-seven patients with unrespectable cholangiocarcinoma were enrolled in this study ,who were treated in our department from June 2013 to June 2016. Patients in the experimental group(n = 24)underwent ERCP combined with metal stent implantation for biliary tract ablation. Patients inthe control group(n=23)underwent ERCP with simple metal stent implantation. Liver function indicators,complications,patency rate and survival rate were analyzed between the two groups. Results A total of 51 patients were satisfactorily performed the operation of metal stent implantation. One week after operation,the levels of total bilirubin(TBIL),alkaline phosphatase(ALP),gamma-glu-tamyltransferase(GGT)and alanine aminotransferase(AST)in the experimental group were much lower than those in the control group(P<0.05,respectively). After 3-month follow-up,the patency rate in the experimental group was significantly greater than that in the control group(P<0.05). The patency and survival time of patients in the experimental group were significantly higher than thosein the control group ,with 8.56 ± 1.12 vs 5.73 ± 0.81 months and 13.88±1.45 vs 9.48±1.38 months,respectively(P<0.05). No postoperative bleeding,perforation and other se-rious complications were found in all cases,8 patients with postoperative biliary tract infection and the increased blood amylase received anti-infective treatments,such as inhibition of symptomatic improvement after treatment. Conclusion The results of endoscopic radiofrequency ablation combined with metal stent implantation for the treatment of unrespectable cholangiocarcinoma were significantly better than those of simple metal stent implantation.
7. The clinical characteristics and outcomes of Maisonneuve fractures

Objective:
To investigate the clinical characteristics and outcomes of Maisonneuve fractures.
Methods:
Data of 21 cases of Maisonneuve fractures from February 2015 to December 2017 were retrospectively analyzed. There were 16 males and 5 females with an average age of 38.8 years (range, 21 to 61 years). The fractures occurred on the left side in 11 patients and on the right side in 10 patients. The causes of injuries were traffic accident in 4 patients, sprain injury in 9 patients and falling injury from height in 8 patients. There were 16 cases of medial malleolar fractures and 5 cases of ruptures of deltoid ligament (4 entirely and 1 partial). There were 17 cases of fractures of the posterior malleolus, among which there were 5 of typeⅠ, 8 of typeⅡ and 4 of type Ⅲ according to the Bartonícek classification of posterior malleolus. There were 4 cases without fracture of posterior malleolus including 1 complete disruption of posterior inferior tibiofibular ligament. Four cases were overlooked of Maisonneuve fracture at the first time. The interval between injury and operation was 2-12 days (mean, 4.9 days).
Results:
Stabilization of proximal fibular fractures were achieved with plate in 3 cases. There were 16 cases of medial malleolar fractures, and the fixation were achieved with cannulated screws in 13 cases and with anti-glide plates in 3 cases. The entirely rupture of deltoid ligament was repaired in 4 cases with suture anchors, the partial rupture of deltoid ligament was not repaired. There were 17 cases of posterior malleolar fractures, 12 cases treated with open reduction and internal fixation including cannulated screws in 9 cases and anti-glide plates in 3 cases. Stabilization of syndesmosis was achieved with syndesmotic plate in 1 case; the medial and posterior malleolar fractures were stabilized and anterior inferior tibiofibular ligament was repaired with suture anchor in 1 case; the other 19 cases were stabilized with syndesmotic screws, 2 screws in 11 cases and 1 screw in 8. Twenty-one patients were followed up for 13-48 months with an average of 25.6 months. The time of bony union was from 3 to 6 months with an average time of 4.9 months after operation. All patients received anatomical reduction without postoperative complications such as incision infection, reduction lose, breakage of screw and posttraumatic arthritis. In 13 cases, the syndesmotic screw was removed at the mean time of 15.38 weeks postoperative (range, 13-25 weeks). At the latest follow up, AOFAS score was from 84 to 100, with excellent in 13 cases, good in 8 cases, and the excellent and good rate was 100% (21/21). Baird-Jackson score was from 83 to 100, with excellent in 11 cases, good in 8 and fair in 2, and the excellent and good rate was 90.48%(19/21).
Conclusion
The diagnosis of proximal fibular fracture of Maisonneuve fracture is easily missed. The complete rupture of deep deltoid ligament and displaced obviously of posterior malleolar fracture should be reduction and stabilization. The accuracy of reduction of the syndesmosis is of great concern. The outcome of operation is satisfied.
8.Targets and validation of Salvia miltiorrhiza in myopia through network pharmacology

Objective:To explore and validate the targets of Salvia miltiorrhiza in myopia using network pharmacology and molecular docking technology. Methods:The TCMSP database was used to extract the targets of Salvia miltiorrhiza.GeneCards, DisGeNET, Malacard and OMIM databases were used to extract the myopia-related targets.The target intersection was taken, and the intersecting targets were selected to extract the corresponding active ingredients of traditional Chinese medicine (TCM) and construct the pharmacological regulatory network of TCM using Cytoscape.The protein interaction network map for the key target genes was constructed using the String database, and the relevant proteins were selected to download the three-dimensional structures of the active ingredients from the PubChem database, and molecular docking was performed using AutoDockvina software.Twelve 3-week-old guinea pigs were induced with lens-induced myopia (LIM) in the right eye and randomly divided into normal saline group and sodium danshensu group, with 6 animals in each group.During the maintenance of LIM, periocular injection of 1 ml normal saline or sodium danshensu was performed daily.The contralateral eye was used as a negative control.On days 0, 14, and 28 of the experiment, the axial length of both eyes was measured by A-scan ultrasonography, and the refractive status was assessed with a streak retinoscope.To avoid individual differences, relative spherical equivalent (treated eye-contralateral eye) and relative axial length (treated eye-contralateral eye) were compared.On day 28, the relative expression levels of hypoxia-inducible factor-1α (HIF-1α) and transforming growth factor-β1 (TGF-β1) proteins were determined by Western blot.The feeding and use of laboratory animals followed the 3R principle, and the research program was approved by the Ethics Committee of Experimental Animal Center of Zhengzhou University (No.ZZU-LAC 202320405[02]). Results:Sixteen intersecting key targets were screened for myopia and TCM components derived from Salvia divinorum.A TCM network pharmacology map and protein interaction map were constructed with Salvia divinorum as a drug candidate, and the corresponding proteins of target genes, such as MMP2, TGFB1, and MMP9 were screened to perform molecular docking with the active ingredients, such as lignocellulosic acid, danshensu, tanshinone ⅡA, and so on.After 14 days of induction, the relative spherical equivalent and relative axial length were (-4.67±1.03)D and (0.67±0.26)mm in sodium danshensu group, and (-6.30±1.22)D and (1.08±0.34)mm normal saline group, indicating slower myopia progression and axial elongation in sodium danshensu group, and the differences were statistically significant ( t=2.412, P=0.039; t=2.750, P=0.049). The relative expression levels of HIF-1α protein were 0.20±0.01, 1.29±0.05 and 0.63±0.02, and the relative expression levels of TGF-β1 protein were 0.93±0.05, 0.25±0.01 and 0.74±0.05 in the negative control, normal saline and sodium danshensu groups, respectively.The expression of HIF-1α protein was higher in sodium danshensu group than in negative control group but lower than in the normal saline group, and the expression of TGF-β1 protein was lower in sodium danshensu group than in negative control group but higher than in the normal saline group, showing statistically significant differences (all at P<0.05). Conclusions:Natural compounds extracted from Salvia divinorum extracts may serve as potential drug candidates to combat scleral hypoxia and improve scleral extracellular matrix remodeling.
9.Current status of comorbidity in elderly patients with coronary heart disease and effect of comorbidity on hospitalization costs

Objective:To gain a deeper understanding of the comorbidity status of elderly patients with coronary heart disease and the effect of comorbidity on their hospitalization costs.Methods:Using the convenient sampling method, a total of 8 334 elderly hospitalized patients diagnosed with coronary heart disease in Beijing Hospital from January 2018 to December 2021 were selected as the research objects. General Inforamtion Questionnaire and Charson comorbidity index (CCI) were used to investigate the comorbidity of patients.Results:Among the 8 334 elderly patients with coronary heart disease included, 88.18% (7 349/8 334) had comorbidity, and there was a statistically significant difference in hospital costs among patients with different CCI scores ( P<0.01). Elderly patients with coronary heart disease with comorbidity had higher hospital costs. The results of the binomial logistic regression analysis showed that comorbidities increased the hospitalization costs of elderly coronary heart disease patients in both the unadjusted model and the gradually adjusted model for age, gender, occupation, education level, admission department, admission status, admission year, admission mode length of stay and disease classification ( P<0.05) . Conclusions:The comorbidity in elderly patients with coronary heart disease is serious, which is an important factor affecting hospitalization costs. In medical practice, it is necessary to strengthen the comprehensive management of elderly patients with coronary heart disease, pay attention to the influencing factors of hospitalization costs, optimize prevention and treatment strategies, control the occurrence and progression of comorbidities among patients, deepen the reform of medical insurance payment methods such as grouping payments based on disease diagnosis, so as to accurately reduce hospitalization costs and promote the sustainable and healthy development of the medical and health system.
10.Effect of immune function on prognosis of patients with sepsis

Objective To investigate the influence of immune function and its changes on the prognosis of patients with sepsis. Methods 393 patients who met the diagnostic criteria of Sepsis-3 admitted to General Hospital of Southern War Zone of PLA from April 2003 to April 2017 were enrolled. Clinical data were collected and analyzed retrospectively. According to the initial immune status, patients with more than 4 days course of disease were divided into the initial immune suppression group (219 cases) and the initial immune function normal group (174 cases). According to the changes of immune function, patients with more than 7 days course of disease were divided into persistent inhibition group (113 cases), persistent normal group (96 cases), first normal inhibition group (22 cases) and first inhibited normal group (59 cases). In addition, the patients were divided into the elderly group (≥ 65 years old) and the young group (< 65 years old). Acute physiology and chronic health evaluation Ⅱ(APACHEⅡ), sequential organ failure assessment (SOFA), procalcitonin (PCT), C-reactive protein (CRP), aspartate aminotransferase (AST), alanine aminotransferase (ALT), serum creatinine (SCr) within 24 hours after diagnosis of sepsis, whether respiratory failure and circulatory failure occur, hormone, immunomodulatory drugs and high-volume hemofiltration treatment within 28 days, the absolute value of lymphocyte counts for 4 consecutive days after diagnosis and 4 consecutive days before the end point event (death or survival within 28 days or more than 28 days) were collected and compared between each group. Results Among 393 sepsis patients, 174 cases had normal initial immune function, of whom 85 cases were older than 65 years old;219 cases had depression of initial immune function, of whom 118 cases were older than 65 years old. Compared with the initial immune function normal group, the levels of PCT, CRP, ALT, AST and SCr in the initial immunosuppressive group were significantly increased [PCT (μg/L): 9.32 (2.13, 34.01) vs. 4.28 (1.02, 19.02), CRP (mg/L): 89.00 (26.00, 142.00) vs. 65.25 (19.88, 119.04), ALT (mmol/L): 39.0 (39.0, 99.0) vs. 27.0 (16.2, 73.0), AST (mmol/L): 55.0 (31.0, 148.0) vs. 39.0 (23.0, 100.8), SCr (μmol/L): 132.00 (74.75, 245.00) vs. 100.25 (61.00, 182.54)], the mean absolute value of lymphocyte counts for 4 consecutive days was significantly decreased [0.615 (0.380, 0.810) vs. 1.442 (1.217, 1.742)], SOFA and APACHEⅡ were significantly increased (SOFA: 9.25±4.19 vs. 6.87±4.66, APACHEⅡ:22.27±8.96 vs. 18.25±9.47), the incidence of circulatory failure (66.2% vs. 50.0%), the incidence of respiratory failure (87.7% vs. 69.0%) and 28-day mortality (65.3% vs. 33.9%) were significantly increased, with statistically significant differences (all P < 0.05). When combined with immunosuppression, there was no significant difference in 28-day mortality between the elderly group and the young group (26.3% vs. 15.8%, P > 0.05); when the immune function was normal, the 28-day mortality of the elderly group was significantly higher than that of the young group (48.2% vs. 20.2%, P < 0.01). The 28-day mortality of the persistent inhibition group and the first normal inhibition group were significantly higher than those of the persistent normal group and the first inhibition normal group [83.2% (94/113), 81.8% (18/22) vs. 26.0% (25/96), 40.7% (24/59), all P < 0.05]. The incidence of immunosuppression in elderly patients [33.3% (14/42) vs. 10.5% (8/76)] and the incidence of persistent immunosuppression [77.0% (67/87) vs. 54.1% (46/85)] were higher than those in young patients (all P < 0.01). Conclusions Immune function is closely related to the prognosis of sepsis patients. Elderly patients with sepsis are more likely to have immunosuppression or persistent immunosuppression than young patients, and the prognosis is worse.
