Objective To investigate the effect and safety of bronchial balloon positioning combined with autologous blood bronchial occlusion in the treatment of intractable pneumothorax.Methods 36 cases of hospitalized patients with refractory spontaneous were collected as the research object,based on the bronchial balloon positioned upstream bronchoscopy combined with autologous blood bronchial occlusion therapy.The efficacy and adverse reac-tions were followed up for one year.Results In the 36 cases of spontaneous pneumothorax in patients with refractory, 30 patients were successfully positioning balloon (83.33%),6 cases of patients who were not successfully positio-ning,were continued with conservative treatment of indwelling thoracic drainage.In 30 patients underwent successful positioning,autologous blood bronchial occlusion were used,and a total of 24 cases (80.0%)patients successfully reduced bubbles escape;6 patients relapsed,who were continued to retainthoracic drainage conservative treatment. Conclusion Bronchial balloon positioning combined with autologous blood bronchial occlusion in the treatment of intractable pneumothorax has accurate positioning and high blocking success rate,and also is safe and effective.It is worth promoting in clinical practice.

Objective:By analyzing clinical data, discuss the clinical efficacy of balloon dilatation for the treatment of tuberculous scarred airway stenosis and factors affecting efficacy. Methods:Selected 13 cases airway stenosis caused by tracheobronchial tuberculosis.Airway stenosis was confirmed and measured by bronchoscopy and CT before balloon dilation. Balloon dilatation with forming expansion was conducted once a week. We evaluated the shortness of breath scores, measuring FEV1, FVC and the diameter of airway before and after dilation. The long-term outcome and lung function improvement were evaluated. Results:Thirteen cases were treated by balloon dilation with fiberoptic bronchoscopy. The airway diameter before and after the treatment, shortness of breath score, FEV1, FVC have improved significantly, the difference was significant(t=15.596, t=-27.657, t=-14.604, t=-41.766;P<0.05). Conclusion:Balloon dilation treatment for tuberculous scarred airway stenosis can achieve better results.

Objective To investigate the role of Heme oxygenase-1 in the effect of hyperbaric oxygen preconditioning (HBOP) against the brain edema formation after experimental intracerebral hemorrhage in rats.Methods The study was carried out by animal experiment in two steps by using 54 Spradgue-Dawley rats weighting from 300-350 g.In the first step,rats were treated with HBOP (HBOP group,n =3) or with sham pre-conditioning (Sham pre-conditioning group,n =3).All the rats were sacrificed 24 h after the preconditioning,and basal ganglion of brain tissue was taken for detect HO-1 level by using western blot analysis.In the second step,rats were divided into 4 groups (n =12 in each group):HBOP +ZnPP group,in which rats had a micro-pump intra-peritoneally implanted containing a specific HO-1 inhibitor ZnPPⅨ (Zinc protoporphyrin IX,0.01 mg/kg),Sham pre-conditioning + Znpp group,HBOP + DMSO group,in which rats with a intra-peritoneal micro-pump containing 2 mL Dimethyl sulfoxide (DMSO,a solvent vehicle) and Sham pre-conditioning + DMSO group before HBOP.At 24 hours after the pre-conditioning,rats received an infusion of 100 μL autologous blood into the caudate nucleus to form a simulated intracerebrum hemorrhage (ICH),and were sacrificed 72 h later for brain water content measurements.All data were analyzed by using Stata 7.0 software and statistical analyses were carried out by two-tailed Student t test.Results Compared with the Sham pre-conditioning group,the HBOP group had significant higher level of HO-1.Compared with the Sham pre-conditioning + DMSO group,the HBOP + DMSO group had a significant lower level of water content in the ipsilateral basal ganglion [(81.4 ± 0.9) ％ vs.(82.6 ± 0.8) ％ (P ＜ 0.05)],however,peritoneal infusion of ZnPP Ⅸ before HBOP abolished HBOP-induced protection against brain edema formation after experimental ICH [(82.8 ± 0.9) ％ vs.(82.6 ± 0.7) ％ (P ＞ 0.05)].Conclusions Hyperbaric oxygen preconditioning attenuate brain edema formation after experimental ICH in rats,and this protection is attributed to the activation of HO-1.

Objective To discuss the value of analysis of serum myocardial enzyme spectrum and cardiac troponin I(cTnI) in neonatal hypoxic ischemic encephalopathy(HIE) .Methods Serum level of myocardial enzyme spectrum in each people was deter-mined with Hitachi 7602 ,Serum levels of cTnI was determined by chemoluminescence method .Serum levels of myocardial enzyme spectrum and cTnI were collect from 65 neonates with HIE(HIE group) and 68 cases of helthy volunteers(control group) .Results The levels of glutamic oxalacetic transaminase(AST) ,creatine kinase(CK) ,creatine kinase MB isoenzyme(CK-MB) ,lactic dehy-drogenase(LDH) and alpha-hydroxybutyric dehydrogenase(HBDH) in HIE group were(98 .9 ± 9 .6)U/L ,(385 .4 ± 15 .5)U/L , (89 .3 ± 9 .6)U/L ,(300 .8 ± 19 .8)U/L ,(399 .1 ± 17 .9)U/L ,which were higher than those in control group(P<0 .05) .The level of cTnI was(1 .4 ± 0 .4)ng/mL in HIE group ,which was higher than that in control group(P<0 .05) .CK-MB sensitivity was 88 .6% , cTnI speciality rate was 86 .3% .Conclusion The assay of myocardial enzyme spectrum and cTnI is significant in diagnosis of neo-natal HIE .

Objective To investigate the clinical value of combined detection of the serum levels of CA125,CA72-4 and tumor specific growth factor(TSGF)in the diagnosis of ovary cancer and the curative effect.Methods Serum levels of CA125,CA72-4 and TSGF in 68 patients with ovary cancer,53 patients with benign ovary rumor and 50 normal controls were measured by chemiluminescent immunoassay(CLIA)and chemical colorimetry.Results and clinical data were statisticaly analyzed.Results The levels of CA125,CA72-4 and TSGF in ovary cancer group were higher than that in the benign ovarian tumor group and healthy controls(all P ＜0.01).When use three tumor markers united detection,sensitivity,specificity and accuracy were 91.2％,83.0％ and 87.4％ respectively.The sensitivity and accuracy was higher than any single detection.The specificity with single CA125 testing was consistently.The levels of CA125,CA72-4 and TSGF in patients with ovary cancer were significantly different after surgical treatment for five days(all P ＜0.05).Conclusion The combined detection of the three markers may increase the positive rate in the early diagnosis of ovarian cancer,and help to differentiate the benign and malignant ovarian tumor,but also is valuable to observe the curative rate and postoperative monitor of the ovary cancer.

Objective To investigate the effects and mechanism of deoxypodophyllotoxin on cell proliferation and mi?gration of human lung cancer NCI-H358 cells in vitro. Methods CCK-8 assay, flow cytometry assay, wound healing assay and DCFH-DA assay were used to detect the effects of deoxypodophyllotoxin on the proliferation, cells cycle, apoptosis, mi?gration and reactive oxygen species (ROS). The protein expressions of Cyclin B1, Cdc25c, CDK1, Caspase-3, p53, Bcl-2, MMP9, ERK1/2, p38MAPK and JNK were measured by Western blot assay, respectively. Results Deoxypodophyllotoxin inhibited cell proliferation and reduced migration in human lung cancer NCI-H358 cells. Flow cytometry analysis showed that treatment with deoxypodophyllotoxin resulted in cell cycle G2/M and S phase arrest, cell apoptosis and ROS production. The result of Western blot assay showed that protein expressions of Cyclin B1, Cdc25c, CDK1, Bcl-2 and MMP9 were down-regulated while Caspase-3 and p53 were up-regulated. Moreover, Deoxypodophyllotoxin treatment decreased the phosphory?lated levels of ERK1/2, p38MAPK and JNK obviously. Conclusion Deoxypodophyllotoxin could suppress the proliferation and migration of human lung cancer NCI-H358 cells in vitro, which is a potential anti-tumor drug.

Related researches found that there exist relative specific microRNA (miRNA) expression patterns in the blood or urine of patients with tumor,and miRNA in the blood and urine are stable and reproducible.Therefore,miRNA can serve as non-traumatic biological markers for the early diagnosis,individualized treatment,prognosis monitoring or follow-up of cancer.

Objective To investigate the long time prognosis of liver resection or transcatheter arterial chemoembolization(TACE) of Barcelona clinic liver cancer stage C(BCLC-C) patients who have portal vein tumor thrombsis.Methods Totally 86 BCLC-C patients who satisfied our including criteria from our surgical database of People's Hospital of Yichang City from January 2000 to September 2015 were selected as the research object.According to different treatment,86 patients were divided into liver resection group(n=50) and TACE group(n=36).The general information of two groups were compared.Cox multi-factors analysis and overall survival rate were calculated.ResultsThe long-term prognosis of liver resection group was better than that of TACE group(5-year OS:26% vs.0,P<0.001).Multi-factors analysis revealed that multiple tumors (HR:1.510,95%CI[1.120,2.316],P=0.020),AST>40 IU/L(HR:0.615,95%CI[0.488,1.206],P=0.013) as well as HBV-DNA>1 000(HR:1.204,95%CI:[0.920,2.540],P=0.038)were adverse factors for prognosis.ConclusionLiver resection is better than TACE for BCLC-C patients with portal vein tumor thrombosis.However,randomized controlled trial still need to be used to further confirm our conclusion.

[ Abstract] Objective To investigate the effect of IL-8 on the viability and migration of HepG2 cells and the potential effect of integrins (αsubunits ) on the migration of HepG2 cells.Methods HepG2 cells were stimulated by IL-8 at different concentrations ranging from 0 to 125 ng/ml in vitro.MTT assay was preformed to detect the viability of HepG2 cells.Scratch wound migration assay was used to explore the effect of IL-8 on the migration of HepG2 cells at the time points of 0, 4, 8, 12 and 24 h,respectively.Transwell assay was conducted to detect the vertical migration of HepG2 cells after stimulation with IL-8 for 24 h.The F-actin of HepG2 cells was observed by immunofluorescence analysis after treatment with IL-8 at different concentrations.The expression of αsubunits of integrins in HepG2 cells treated with IL-8 for 8h was detected using Western boltting assays.Results Compared with the control group, HepG2 cells treated with IL-8 showed improved proliferation activities, but there was no significant difference between the groups of HepG2 cells treated with IL-8 at different concentrations.The cell scratch healing assays and Transwell analysis both indicated that IL-8 promoted the migration of HepG2 cells in a dose-dependent manner.Meanwhile, the cytoskeleton of HepG2 cells was rearranged and the number of filopodium-like protrusions (FLPs) was increased rapidly after treatment with IL-8.Western blotting analysis showed that IL-8 up-regulated the expression of αsubunit of integrins and there was a concentration difference betweenαsubunits.Conclusion IL-8 promotes the migration of HepG2 cells by up-regulating the expression ofαsubunits, and different αsubunits may play different roles.

Objective By analyzing the cesarean delivery rate (CDR) and the indications of cesarean section in Jiangsu Province from 2012 to 2014, to find the strategy to reduce CDR.Methods Data of 40 hospitals whose number of delivery was over 2 400 per year from the database of the Quality Control Center of Gynecology and Obstetrics of Jiangsu Province from 2012 to 2014 were selected.The cohort was divided into two groups, one was specialized hospital including 11 maternal and child health hospitals and the other included 29 general hospitals.After analyzing the current status of cesarean section of the whole province, the average CDR and the distribution of the first indications of cesarean section in two groups were compared using Chi-square test.Results The average CDRs at year 2012, 2013 and 2014 were 53.8％(96 347/178 970), 51.8％ (97 509/188 142) and 48.8％(106 323/217 877), which had a trend of decrease (x2=1 026.2, P ＜ 0.01).CDR without medical indications for each year were 15.7％ (28 098/178 970), 15.5％ (29 162/188 142) and 12.8％ (27 888/217 877).The rate had decreased apparently from both 2012 and 2013 to 2014 (x2=217.6, P ＜ 0.01;x2=341.3, P ＜ 0.01).The CDRs of specialized hospitals for year 2012, 2013 and 2014 were 51.2％ (45 496/ 88 916), 50.6％ (45 853/90 607) and 49％ (55 404/113 105), while the CDRs of general hospitals for each year were 56.6％(50 851/90 054), 53.0％(51 656/97 535) and 48.6％(50 919/104 772).Both showed significant decreasing trend (x2=105.8 and 1 215.7, both P ＜ 0.01).The average CDR of general hospitals was obviously higher than that of specialized hospitals in 2012 and 2013 (x2=505.6 and 104.3, both P ＜ 0.01).However, the difference disappeared in 2014 (x2=3.2, P ＞ 0.05).The three main first indications of cesarean section were associated and postoperative pregnancy complications, and no medical indications.Interestingly, from 2012 to 2014, the CDR for scarred uterus in specialized hospitals raised from 5.4％ to 20.3％, and that in general hospitals increased from 6.7％ to 21.6％, which became the first.Conclusions The average CDR in Jiangsu Province tends to decrease each year, so does the CDR without medical indications.The first indication of cesarean section is associated or postoperative pregnancy complications, while scarred uterus becomes the most common in cesarean section.
