1.UV radiation-induced DNA damage in keratinocytes and the protective effect of resveratrol

Objective To explore the effects of ultraviolet on DNA damage in keratinocytes and to observe the protective role of resveratrol for the cells. Methods Comet assay was employed to evaluate the damage after radiation with different doses of UV rays (UVA, UVB and UVC) of 0, 10, 30, 50, 70 and 90 mJ/cm2, and the effects after pretreatment with various concentrations of resveratrol under irradiation with 30 mJ/cm2. Results UVA irradiation (0 ~ 90 mJ/cm2) had no significant effects on HaCaT cells. However, TailDNA%, TailLength, CometLength, TailMoment and OliveTailMoment showed both UVB and UVC induced DNA damage in a dose-de-pentent manner. UVC was more harmful than UVB at the same dose. Conclusions The DNA breakage induced by UVB and UVC is dose-dependent. As compared with UVB, UVC is more harmful to HaCaT cells. Resveratrol exerts a protective effect in HaCaT cells irradiated by UVB or UVC.
2.Renal collecting duct carcinoma: imaging spectrum with CT and MRI

Objective To summarize the experience of using CT and MRI to diagnose the renal collecting duct carcinoma.Methods From February 2005 to January 2012,10 cases with renal collecting duct carcinoma,confirmed by pathology,were reviewed retrospectively.The data contained 6 men and 4 women,whose age ranged from 21 to 62 years (mean age 48 years).The flank pain was complained by 7 cases,waist discomfort was complained by 3 cases.In urine laboratory test,positive urine erythrocytes (++++) were found in 6 cases.In 10 cases,7 cases accepted CT examination and 3 cases accepted MRI examination.The growth pattern,lesion location,dynamic enhanced scan phase of the tumor and the way of spreading and metastasis were analyzed based on those CT and MRI images.Results The lesions were located in the left kidney in 6 cases,located in the right kidney in 4 cases.The size of tumors ranged from 4.4 cm×5.8 cm to 7.2 cm× 7.4 cm (mean size 5.7 cm× 6.4 cm).The mass,located in the center of renal parenchyma with irregular shape,showed infiltrative growth pattern.The shape of kidney was normal,whereas the border line between cortex and medulla was indistinct.The tumor involved the renal cortex and medulla in 4cases and involved the renal cortex,medulla,pelvis simultaneously in 6 cases.Among 7 patients who accepted the CT scanning,the solid mass was revealed in 6 cases.On CT plain scanning,the masses demonstrated slightly low or equal density within flaky or patchy low-density necrosis.Two cases showed small punctate calcification within the mass.One case was solid and cystic mass,which the cystic part of the mass showed irregular shape of the liquid-density.Among 3 patients accepted MRI scanning,all masses showed solid characters.The substantial part showed slightly hypointense on T1WI and low signal intensity on T2WI.The necrotic foci demonstrated low signal intensity on T1WI and high signal on T2WI.Dynamic enhanced scan revealed mild to moderate enhanced in the substantive part.The density of signal was lower than the renal cortex and slightly higher than the renal medulla in corticomedullary phase.It enhanced continuously in parenchymal phase,but still lower than the renal parenchyma.It enhanced continuously in the delayed phase,while the cystic or necrotic lesions were not observed the enhancement.Renal artery was surrounded by the mass in 2 cases.Tumor embolus was found in the renal vein in 2 cases,9 cases were noticed with renal hilum and paraortic hyperlymphonodus.The thoracic and lumbar spinal metastasis was found in 1 case and adrenal metastasis was found in another case.All patients underwent radical nephrectomy,that pathological diagnosis was renal collecting duct carcinoma.Conclusions The CT and MRI imaging characteristics of renal collecting duct carcinoma can be described as the mass located in the center of renal parenchyma with the infiltrative growth pattern.In MRI image,it demonstrates low signal intensity on T2WI.And a mild continuously enhanced can be observed on dynamic enhanced scanning.Moreover,the tumor often involves renal hilum,perirenal fat capsule,paraortic lymph node,and shows the tendency of distant metastasis.

Purpose:To investigate the toxicities and res po nse of liver cancers to 3-dimensional conformal radiation therapy (3-DCRT) com bined with Trancatheter arterial chemoembolization (TACE) Methods :The histopathological confirmed liver cancer patients were treated by 3-DCRT co mbined with TACE, all patients received TACE before 3-DCRT The total irradiati on dose is 50 Gy to 58 Gy in daily fractions of 2 Gy Each planning target volu me (PTV) received a minimum of 90% of the prescribed dose Each mean liver dose not reached 30 Gy, V 30 Gy (the percentage of normal liver volume with ra diation dose≥30 Gy) less than 33% Results:Thirty patients were included in this study including 2 1 hepatocellular carcinoma Survivals at 1 year after 3-DCRT were 76%, with a median survival time of 8 months 2 patients developed Grade 1 acute liver toxi city and one patient experienced Grade 2 gastrointestinal complications No pat ient developed Grade 2 or greater liver toxicity Conclusions:The use of 3-DCRT technique for liver cancer is a safe and efficient method O ur experience indicated the total irradiation dose above 58 Gy is feasible in da ily fractions of 2 Gy, if the mean liver dose did not reach 30 Gy and V 30 G y
4.Literatures analysis of clinical significance of lymph node metastasis in gastrointestinal stromal tumor

ObjectiveTo explore the clinical significance of lymph node metastasis in gastrointestinal stromal tumor (GIST).Methods A literature search to identify studies relating to GIST cases reported between January 1998 and December 2010 was conducted.The associations between lymph node metastasis and GIST clinicopathological features, gene expression, GIST stage, and GIST prognosis were evaluated.Results 6063 literatures were screened and only 16 GIST patients (median age of 49.25 years,10females) with lymph node metastasis were enrolled in this study.Among them,12 tumors were located in the stomach, 3 in the small intestine, and 1 in the esophagus.Morphologically, 13 specimens were mainly characterized by spindle cells.Gene analysis was examined in 7 cases.Conclusions Lymph node metastasis in GIST might more frequently found in the stomach, tumor stage of high grade, and cellular morphology of spindle-shaped.Gene mutation may have a relationship with lymph node metastasis.However, its clinical significance remains unclear.
5.A Meta-analysis of capecitalbine or 5-Fu combined with oxaliplatin therapy for metastatic colorectal cancer

ObjectivesTo compare clinical outcomes and safety of 5-Fu or capecitabine combined with oxaliplatin therapy for metastatic colorectal cancer. MethodsLiterature search were performed by key words,such as metastatic colorectal cancer,capecitabine,oxaliplatin,5-Fu on all randomized controlled trails reported on 5-Fu or capecitabine combined with oxaliplatin therapy for metastatic colorectal cancer before Jun.2011.Two authors drew the details of trail design,characteristics of patients,outcomes and so on from the studies included. Data analysis was performed by RevMan 4.2. ResultsAccording to the same screening criteria,6 clinical studies were included in this Meta-analysis. Sample volume in this Meta-analysis was 2189 colorectal cancer cases.The baseline characteristics of 5-Fu group were similar to those of capecitabine group.The short-term outcome such as response rate,RR was 0.92 [95 %CI(0.82,1.02),P=0.12], and the long-term outcome such as median overall survival, median progression-free survival and the WMD was -0.19 [95 %CI(-0.73,0.35),P=0.49],-1.91 [95 %CI(-2.53,0.16),P=0.08],respectively,two groups weresimilar. In the comparison of Ⅲ-Ⅳ grade toxicity between the two groups, the incidences of neutropenic in 5-Fu group were higher than those in capecitabine group,and the RR was 0.24 [95 %CI(0.11,0.55), P=0.0007]. However, the incidence of wand-foot symptoms in 5-Fu group was lower than those in capecitabine group, the RR was 2.83 [95%CI (1.66,4.82), P=0.0001]. ConclusionBoth 5-Fu and capecitabine were similarly effective in the therapy of metastatic colorectal cancer. And the Ⅲ-Ⅳ grade toxicity such as neutropenic was easier to happen in 5-Fu group,while the incidence of hand-foot symptoms was on the other hand.
6.Clinical analysis of patients received pemetrexed therapy without the advised folic acid premedication

Objective To explore whether 5 to -7-day of folic acid supplementation prior to pemetrexed therapy is needed. Methods We retrospectively evaluate the outcomes of non-small cell lung cancer patients received less than the advised folic acid premedication. Seventy patients with Ⅲ-Ⅳ NSCLC were randomly divided into two groups: patients who initiated vitamin supplementation on 7 days before the first dose of pemetrexed (group A) and patients who initiated vitamin supplementation on the day of the first dose of pemetrexed (group B). Results In group A and group B, CR 0 and 0, PR 10 and 8, the response rates of 28.6% and 22.9% were observed, respectively. There was no statistically significant differences between these two groups. No significant differences were observed in the incidence of hematologic and non-hematologic toxicities. Conclusions The initiation of pemetrexed-based chemotherapy does not need to accommodate a vitamin supplementation schedule.
7.Value of Narrow-band Imaging Magnifying Endoscopy in Diagnosis of Chronic Atrophic Gastritis

Background:Currently,chronic atrophic gastritis( CAG)is diagnosed by endoscopy combined with pathological examination. Narrow-band imaging magnifying endoscopy( NBI-ME)has been widely applied to diagnose CAG. However, it is a issue how to optimize the examination technology. Aims:To investigate the clinical value of NBI-ME in diagnosis of CAG,intestinal metaplasia and intraepithelial neoplasia. Methods:One hundred CAG patients reexamined gastroscopy were enrolled,sequential examination was performd,i. e. the extent of disease was observed under convention white light endoscopy( C-WLI ) followed by NBI-ME mode to observe locally,morphological changes of gastric mucosa pit was analyzed,endoscopic findings and histological results was compared. Results:Sensitivity of sequential examination for the diagnosis of CAG was 89. 7%,specificity was 63. 3%;sensitivity and specificity for the diagnosis of intestinal metaplasia were 89. 4%,89. 3%,respectively,for intraepithelial neoplasia were 84. 2%,95. 9%,respectively. Conclusions:Sequential examination can significantly improve the diagnostic accuracies of CAG,intestinal metaplasia and intraepithelial neoplasia,and effectively guide targeted biopsy,and easily to operate.
8.Effect of Tongxinluo on the interaction between COX-2 and iNOS in vascular injury of rats with deficiency of vital energy or qi stagnation

AIM:To investigate COX-2 and iNOS protein contents and their interaction in vascular endothelium injury of rats with deficiency of vital energy or qi stagnation,and the prevetion and treatment of Tongxinluo.METHODS:The model of vascular endothelium injury of rats with deficiency of vital energy or qi stagnation was established by using high L-Methionine,with load-carrying swimming or being fastened respectively.Western blotting was used to analyze protein contents of COX-2 and iNOS,co-immunoprecipitation and laser confocal microscopy were used to analyze the interaction between COX-2 and iNOS.Optical microscope and electronic microscope were used to evaluate pathological changes in vascular endothelium.RESULTS:The protein contents of COX-2 and iNOS,and their interaction increased significantly in deficiency of vital energy group and qi stagnation group,in accord with injury of vascular endothelium.Compared with deficiency of vital energy group and qi stagnation group respectively,their protein contents decreased and their interaction was weakened in Tongxinluo groups.CONCLUSION:When protein contents of COX-2 and iNOS increase and their interaction enhance after vascular endothelium injury of rats with deficiency of vital energy or qi stagnation,initiate exacerbations,Tongxinluo could attenuate the alterations and protect vascular endothelium from injury.
9.CTA characteristics of diabetic lower extremities arterial disease in different Fontaine stage

Objective To explore CTA characteristics of diabetic lower extremities arterial disease (LEAD) with different Fontaine stage.Methods Data of 100 patients clinically diagnosed as diabetic LEAD and underwent lower limbs arterial CTA were analyzed retrospectively.The patients were classified into grade Ⅰ-Ⅳ according to Fontaine classifications.Lower extremities arteries were graded according to CTA.Stenosis degree of lower extremity arteries in different Fontaine stages were analyzed.Results The grades of lower extremity arteries stenosis in different Fontaine stages had statistically significant difference (x2 =186.24,P<0.001).There had statistically significant difference of stenosis degree in different lower extremity arterial levels among different Fontaine stages (superior genicular artery:x2 =69.24,P<0.001;inferior genicular artery:x2 =111.59,P<0.001;dorsalis pedis and arteriae plantaris:x2 =94.15,P<0.001).Grades of stenosis between superior genicular arteries and inferior genicular arteries,superior genicular arteries and dorsalis pedis and arteriae plantaris had statistically significant differences (Z=12.59,P<0.001;Z=10.47,P<0.001).There was statistically significant difference of stenosis grades between inferior genicular artery and dorsalis pedis and arteriae plantaris (Z=12.66,P<0.001).Occluded inferior genicular arteries usually associated with collateral vessels.Conclusion CTA can reflect the clinical stages of LEAD,which is the ideal method in diagnosing of diabetic LEAD.
10.The effect of antidepressant treatment on the working memory of Parkinson's disease patients with depression

Objective To investigate the impaired characteristics of working memory in Parkinson's disease patients with depression and the recovery status of working memory after antidepressant treatment.methods Totally 60 cases were enrolled in this study and divided into three groups:30 Parkinson's disease patients with depression (PD-D),15 Parkinson's disease patients without depression (PD) and 15 persons without Parkinson's disease as normal controls.All subjects were evaluated with Wechsler Memory Scale (WMS-R) audio-visual span,Tower of Hanoi (TOH) and Wisconsin Card Sorting Test (WCST).30 PD-D patients were randomly divided into two subgroups:routine treatment subgroup (8 weeks levodopa therapy) and antidepressant combined treatment subgroup (8 weeks levodopa + citalopram therapy).The evaluation of Hamilton Depression Scale (HAMD),WMS-R audio-visual span,TOH and WCST were performed on these two groups before and after treatment.Result s① There were significant differences comparing PD-D and PD groups with the normal control group in scores of WMS-R,TOH and WCST indicators (P<0.05).There were significant differences in WMS-R visual span,accuracy rate and speed of TOH,as well as the percent errors and percent perseverative responses of WCST comparing PD-D group(11.88±5.91,0.420±0.345,0.408±0.334,0.882±0.253,0.565±0.229) with PD group (15.87±5.21,0.768±0.167,0.634±0.232,0.493±0.161,0.327±0.122) (P<0.05).② Before and after treatment in PD-D routine treatment subgroup,there were significant differences in digital memory and audio-visual memory of WMS-R((6.73±3.72,5.95±3.13) vs (3.77±2.16,1.91±1.58)),accuracy rate of TOH(0.45±0.26 vs 0.23±0.13),as well as percent errors((-0.58±0.17) vs (-0.37±0.14)),percent perseverative responses((-0.32±0.15) vs (-0.14±0.09)),percent conceptual level responses(0.38±0.09 vs 0.13±0.07) and number of categories completed(3.27±1.56 vs 1.06±0.91) of WCST(P<0.05).③ After 4-week treatment and 8-week treatment,there were significant differences in HAMD score comparing PD-D antidepressant combined treatment group(16.33±2.72,10.27±2.66) with PD-D routine treatment group(21.73±2.28,18.4±2.47) (P<0.05).Conclusion Comparing Parkinson's disease patients without depression,the impaired working memory is more serious and extensive in Parkinson's disease patients with depression.Antidepressant treatment can improve the working memory of Parkinson's disease patients with depression.
