1.Preparation Technology Optimization of Hongteng Decoction Hollow Suppositories by Orthogonal Test

Objective:To establish an optimum preparation process for Hongteng decoction hollow suppositories. Methods:An orthogonal design was performed to screen the proportion of drug and base(A),the temperature of mold filling with drug and base(B)and stripping time(C),and the appearance and melting time were used as the indices ,the best preparation technology of the hollow suppositories was optimized. Results:The optimum preparation technology of the hollow suppositories was as follows:the proportion of drug and base was 1 ∶2,the filling temperature was 40℃,and the stripping time was 30 min. Conclusion:The optimum preparation technology of Hongteng decoction hollow suppositories is simple and feasible.
2.Expression of CapG in gastric cancer and its relationship with clinicopathological features and prognosis

Purpose To investigate the expression of gelsolin-like actin-capping protein (CapG) in gastric cancer and adjacent normal gastric tissues and its relationship with clinicopathological features and prognosis.Methods The immunohistochemical (IHC) assay was performed to detect the expression of CapG protein in 125 specimens of gastric cancer (study group) and 30 specimens of matched adjacent normal gastric tissues (control group).No patients had received chemotherapy or radiotherapy before surgery.Immunostaining was scored semiquantitatively by two independent observers who were blinded to the patients' outcome and other clinicopathological parameters.The correlation of the expression of CapG with clinicopathological characteristics and prognosis was analyzed.Results Cytoplasmic and nuclear CapG protein staining was detected in all gastric cancer tissues.CapG protein expression was absent or low in matched adjacent normal gastric tissues but significantly higher in gastric cancer tissues.Expression of CapG in the tumor was not significantly associated with gender,age,tumor size,tumor location (P > 0.05).However,elevated CapG expression was strongly correlated with the depth of invasion (P =0.044),lymph node metastasis (P =0.026),distant metastasis (P =0.001),and AJCC stage (P =0.012).Significant poorer overall survival was observed in the high CapG expression group compared with that of the low CapG expression group (P <0.001).Multivariate analysis showed overall survival correlated with the patients' tumor location,lymph node metastasis,distant metastasis,AJCC stage and the expression of CapG protein.Conclusion CapG is highly expressed in gastric cancer.Tumors with CapG up-regulation tend to have poorer prognosis,suggesting overexpression of CapG may contribute to the prediction of poorer prognosis and may be a potential therapeutic target for gastric cancer.
3.TCM topical and enema in the treatment of 35 patients with recurrent endometrial shift disease

Objective To investigate the clinical effects of TCM herbs and enema in the treatment of recurrent uterine endometriosis and control of endometriosis recurrence.Methods The recurrent endometriosis patients were randomly divided into two groups,35 cases were treated with TCM herbs and Chinese medicine enema as the observation group,35 ninhydrin treatment of patients with pregnancy-ene as the control group,two groups of patients were treated for six months,the clinical efficacy and adverse reactions were observed.Results After treatment,VRS scores compared with the corresponding group before treatment was significantly lower.VRS scores in the observation group after treatment was significantly lower than the control group.The amenorrhea rate was significantly higher than control group,the complex tidal time at the end of treatment compared with the control group was significantly shorter,the observation group,no case of adverse reactions,the control group,patients experienced adverse events,observation group 4 cases(8.57%) recurrence,in the control group of 10 cases(28.57%) relapse,the difference between the two groups were statistically significant,The patients treatment of 30 cases of lesions decreased in size,the control group,had 28 cases of lesions smaller in size,there was no significant difference.Condtusion Chinese herbal enema and topical treatment of recurrent uterine endometriosis had significant efficacy,no adverse reactions,late recurrence is low.
4.Research progression on preoperative mechanical bowel preparation for elective colorectal surgery.

Preoperative mechanical bowel preparation (MBP) has been established as a standard procedure for elective colorectal surgery in most surgical centers since 1950s. However, the routine use of MBP for elective colorectal surgery is still in debate, as the researches on the evaluation of MBP have been carried out in recent 20 years. By searching and analyzing the existing evidence, we conclude that MBP should be routinely abandoned for elective open colorectal surgery, but should be routinely used for elective laparoscopic rectal surgery, and it still awaits large-scale RCTs for further evaluation of MBP for elective laparoscopic colonic surgery.

5.Exploration of digestive tract reconstruction with totally laparoscopic total gastrectomy for gastric cancer.

Compared to that with laparoscopic assisted approach, intracorporeal anastomosis with totally laparoscopic radical total gastrectomy has the advantages of smaller incision, and better vision for operation, and may also be fit for patients with large size lesion, high-located lesions, or obesity. It remains controversial though several surgeons have reported the safety and feasibility of intracorporeal anastomosis with totally laparoscopic total gastrectomy. This review describes the recent technical advances in intracorporeal anastomoses with totally laparoscopic total gastrectomy, focusing on the reconstruction skills and indications. Current data on totally laparoscopic total gastric resection for gastric carcinoma revealed that all digestive tract reconstructions were performed with esophagus-jejunum Roux-en-Y anastomosis, and different reconstruction techniques of such Roux-en-Y anastomosis have certain advantages and disadvantages. Surgeons should make choice based on tumor location, esophageal diameter and personal skills in order to achieve maximal benefit to patients.

The laparoscopic approach is rapidly becoming the preferred method of treatment for patients with early gastric cancer due to advantages of minimally invasive surgery. As laparoscopic experience has accumulated, laparoscopy-assisted distal gastrectomy (LADG) with D2 lymphadenectomy has become a valuable alternative for the treatment of patients with advanced gastric cancer. However, laparoscopic gastric surgery is demanding from a technical point of view, especially when a D2 lymphadenectomy is performed. Surgeons seeking to undertake LADG are concerned about unpredictable intraoperative bleeding that may occur during LADG. Comprehensive knowledge of the perigastric vascular anatomy is essential for LADG with D2 lymphadenectomy.

7.Preparation of a recombinant tumor-targeting ribosome inactivating protein luffin-α-NGR and evaluation of its antitumor activity.

Loofah seeds ribosome inactivating protein luffin-α was fused with a tumor-targeting peptide NGR to create a recombinant protein, and its inhibitory activity on tumor cells and angiogenesis were assessed. luffin-α-NGR fusion gene was obtained by PCR amplification. The fusion gene was ligated with pGEX-6p-1 vector to create a recombinant plasmid pGEX-6p-1/luffin-α-NGR. The plasmid was transformed into E. coli BL21, and the target protein was isolated and purified by GST affinity chromatography. The luffin-α-NGR fusion gene with a full length of 849 bp was successfully obtained, and the optimal soluble expression of the target protein was achieved under the conditions of 16 ℃, 0.5 mmol/L IPTG after 16 h induction. SDS-PAGE and Western blotting confirmed the recombinant protein has an expected molecular weight of 56.6 kDa. Subsequently, the recombinant protein was de-tagged by precision protease digestion. The inhibitory effects of the recombinant protein on liver tumor cells HepG2 and breast cancer cells MDA-MB-231 were significantly stronger than that of luffin-α. The Transwell and CAM experiment proved that the recombinant protein luffin-α-NGR also had a significant inhibitory effect on tumor cells migration and neovascularization. The inhibitory activity on tumor cells and angiogenesis of the recombinant luffin-α-NGR protein lays a foundation for the development of subsequent recombinant tumor-targeting drugs.

8.Expression and clinical significance of Flotillin-2 in gastric cancer tissues

Objective: To investigate the expression of Flotillin-2 (Flot-2) protein in gastric cancer tissues and its relationship with clinicopathological features and prognosis of gastric cancer (GC) patients. Methods: 112 samples of gastric cancer tissue and the corresponding paracancerous tissue that resected at the gastrointestinal surgery department of the Second Affiliated Hospital of Nanchang University between January 2009 andApril 2010 were collected for this study. The expression of Flot-2 protein in tumor tissues was detected by immunohistochemistry. The survival data were analyzed by Kaplan-Meier and Log-Rank test, and the survival curve was plotted. Spearman correlation analysis was used to examine the relationship between Flot-2 protein expression and clinicopathological characteristics and prognosis of GC patients. Results: In gastric cancer tissues, Flot-2 was primary stained in cytoplasm. Level of Flot-2 was significantly higher in gastric cancer tissues compared with that in paracancerous tissues (53.57% vs 46.43%, P<0.05). Expression of Flot-2 in tumor tissues was significantly associated with tumor size, depth of invasion, lymph node metastasis, distant metastasis and AJCC stage (all P<0.01), but not with gender, age, differentiation degree and tumor location (P>0.05). Moreover, survival analysis showed that the overall survival of patients with low Flot-2 expression was significantly higher than that of the patients with high level (P<0.01). Cox regression analysis indicated that distant metastasis, AJCC stage and Flot-2 expression were the independent risk factors for the prognosis of GC patients. Conclusion: Flot-2 protein was highly expressed in gastric cancer tissues and closely correlated with the poor prognosis of GC patients; Flot-2 is an independent risk factor for GC prognosis and may be served as a potential therapeutic target for gastric cancer.
