1.Influence of Jieyu Huoxue Decoction on rehabilitation of patients with depression after cerebral infarction

OBJECTIVE: To study the influence of Jieyu Huoxue Decoction on rehabilitation of patients with depression after cerebral infarction. METHODS: Fifty four patients suffered from the depression after acute cerebral infarction were randomly divided into three groups: depression control group, fluoxetine treated group and Jieyu Huoxue Decoction treated group. Eighteen patients of cerebral infarction without depression were enrolled in the no depression control group randomly. The Zung self-rating depression scale (SDS), the modified Edinburgh-Scandinavia stroke scale (MESSS) and the activities of daily living (ADL) were evaluated before treatment, 30 and 60 days after treatment. RESULTS: After 30 and 60 days of treatment, SDS evaluation of the Jieyu Huoxue Decoction treated group showed significant difference as compared with the condition before treatment and that of the depression control group (P < 0.05 or P < 0.01), MESSS evaluation and ADL evaluation also showed significant difference as compared with the condition before treatment and that of the depression control group (P < 0.05 or P < 0.01); there was no obvious difference between the fluoxetine treated group and the Jieyu Huoxue Decoction treated group, but the Jieyu Huoxue Decoction treated group showed fewer symptoms and less side effect. CONCLUSION: Jieyu Huoxue Decoction can not only relieve depression after cerebral infarction, but also improve neurological functions.
2.Endemic situation of schistosomiasis and control effects of implementation of comprehensive management measures in Hunan Province

Objective To analyze the current endemic situation of schistosomiasis and evaluate the control effects of the implementation of comprehensive management measures for 4 years,so as to provide references for making control strategy in the next stage. Methods The endemic villages were grouped into categories based on egg positive rates of fecal examinations among villagers. Praziquantel chemotherapy regimens were provided for villagers and domestic animals,respectively. Snail control on the basis of niclosamide was conducted in snail habitat areas in the hilly area and inside embankment in Dongting Lake,and in infected snail distributed areas outside the embankment in the lake. A slow releasing formation of niclosamide were applied for cercariae killing in transmission areas outside the embankment in the lake in the flood season. The modification of snail habitat areas was integrated into the water conservancy and agricultural development projects. The varied health education,safe water supply and disinfestation toilet facilities were provided for residents in the community. Results The infection rate of schistosomiasis in residents dropped by 49.88%,and the estimated patients reduced by 39.62%. Acute schistosomiasis cases significantly decreased. The total number of advanced schistosomiasis cases slightly increased. The infection rate of schistosomiasis in livestock dropped by 66.36% and snail areas reduced by 33.31% as well as no infected snails were found inside the embankment. Yet,the total areas of snail habitats and distributed areas of infected snails slightly increased,with a high density. The first scale of villages and the second scale of villages were lowered into the third or fourth scale of villages. Out of 31 endemic counties (districts,farms),7 reached the criteria of transmission control,24 reached the criteria of infection control. Conclusions The implementation of the comprehensive management measure has achieved the anticipated results of reducing the infection rate of schistosomiasis among villagers and dropping the incidence of acute schistosomiasis. Yet the snail area on beaches outside the embankment slightly increases,with a high density of infected snails and so there is an infection danger there.
3.Study on risk factors of newly emerging advanced patients with schistosomiasis japonica in Dongting Lake regions

Objective To explore the risk factors of newly emerging advanced schistosomiasis patients in endemic areas.Methods The study areas were selected in two counties of Dongting Lake regions and a 1 :2 match case-control study was designed.Sixty schistosomiasis patients,who newly evolved into advanced schistosomiasis from 2006 to 2007,were selected into the case group,and 120 cases with chronic schistosomiasis into the control group.Questionnaires including potential risk factors of advanced schistosomiasis were designed and the information was collected based on face to face interviews.SPSS 12.0 was used to analyze the simple factors and multi ones (logistic regression) attributable to the development of advanced schistosomiasis.Results The history of hepatitis B (OR = 10.729),models of water contact (OR = 3.919) ,yearly exposure days to the infested water (OR = 5.457) and times of chemotherapy in the nearly 10 years(OR = 1.578) were the risk factors of development of advanced schistosomiasis.The times of examinations with positive schistosome eggs were protective factors.No association was found between yearly income,education degree,times of checking for schistosomiasis,times of examination with sera positive results and the emergence of advanced schistosomiasis.Conclusion The high frequency of exposure to the infested water,repeated infections,incomplete diagnosis and treatment are the risk factors of advanced schistosomiasis.The concurrent infection with hepatitis B is associated with the acceleration of development of advanced schistosomiasis.
4.Results of schistosomiasis epidemic monitoring and discussion of schistosomiasis control strategies in Dongting Lake regions

Objective To analyze the schistosomiasis epidemic of 4 national pilots in Dongting Lake regions during 5 years period, and provide references for control strategy and policy making. Methods Four longitudinal monitoring pilots were set up in Dongting Lake regions from 2000 to 2004, and the field work in pilots was carried out based on “National Scheme on Schistosomiasis Epidemic Monitoring”. Results During the monitoring period, the infection rate of Schistosoma japonicum kept at 4% in Dongjia, 5% in Xinshang and over 12% in Quanxing, but in Changjiang, the rate was 8.92% in the first year of monitoring period, and stayed at 4% in the next 4 years. For all pilots, 20-49 age group had the highest infection rate, with the rate in male being higher than that in female and centering on the fishermen, farmers and students in occupation. There was the occurrence of acute and new advanced cases. The infection rate in domestic animals was fluctuated at 20%. There was extensive distribution of infected snails on marshlands outside embankments. Conclusion Schistosomiasis is seriously endemic in Dongting Lake regions, and intensive control measures are necessary.
5.Expression of TRAP1 in esophageal carcinoma and its influence on the prognosis

Purpose To investigate TRAP1 expression in esophageal cancer and its relationship with clinicopathological features and prognosis.Methods Expression levels of TRAP1 in 60 pairs of cancer and adjacent normal tissues were detected by immunohistochemistry,and the relevance between TRAP1 and clinicopathological features was evaluated.Kaplan-Meier and Cox proportional regression analyses were performed to determine the association of TRAP 1 expression and survival of the patients.Results The positive expression rate of TRAP1 was 55.0%,and the amount of relative protein transcript level was 2.7 ± 1.1 in the cancer tissue.The positive expression rate of TRAP1 was 11.7%,and the relative expression protein was 0.5 ± 0.4 in the adjacent normal tissue.The expression level of TRAP1 in cancerous tissue was significantly higher than that of tissue adjacent to carcinoma.There was no statistically significant difference between the expression levels of TRAP1 with sex,age,location,and degree of differentiation (P > 0.05).There was statistically significant relationship between recurrence,metastasis and TNM stages with the expression of TRAP1 levels (P < 0.05).The average survival time of TRAP1-negative patients was 69.0 months (95% CI:60.2-77.9) while the TRAP1-positive patients was 34.2 months (95% CI:24.4-44.1).High levels of TRAP1 were correlated with decreased survival of postoperative esophageal cancer patients (P < 0.05).Conclusion TRAP1 is overexpressed in esophageal cancer and associated with the progression and prognosis of esophageal cancer.
6.Experimental research on local renal injury of dog with microwave ablation guided by DSA

Objective To explore the efficiency, complications and probability of preserving part renal function by local renal microwave ablation. Methods The fresh pig renal pelvis full filled with 30% diatrizoate meglumine and the dogs kidney taken arterial pyelography were both ablated with microwave. Dogs were divided into three groups: measuring temperature after ablation group, single point ablation both on the two kidneys group and double points ablation on unilateral kidney group. In measuring temperature after ablation group, DSA and pathology were performed immediately after ablation. In the other groups, DSA with blood and urine samplings were taken for routine tests including renal function right after the ablation and 10 days later. Results Experiment in vitro showed conspicuous renal pelvic contraction and convolution. The group under power rate of 70, 3 min produced urine leak easily. Preliminary test in vivo with DSA showed the disappearance of local kidney blood supply. The residual renal function was related to areas of necrosis. Acute stage pathology revealed acute renal cortex medulla and pelvic cells injury. DSA of chronic stage showed no change in size of the area of ablation. The blood supply of necrotic areas was not restored. The residual kidney possessed the excretion contrast medium with no urine leaks. Upper pole of right kidney adhered with adjacent tissue, together with thickened covering. Pathology revealed fibrous proliferation around the coagulative necrosis. Conclusion Microwave ablation can inactivate the local renal tissue, and, effectively preserve the big blood vessels and function of residual kidney. No urine leaks occurred in chronic stage but easily to produce adhesions with adjacent tissue.
7.Percutaneous Radiofrequency Ablation of Pulmonary Metastases in the Patients with Colorectal Carcinoma under CT Guidance

Objective To explore the clinical effects and safety of treatment for colorectal cancer to pulmonary metastases with percutaneous multiple electrodes radiofrequency-2000 (RF-2000) under CT-guided.Methods 26 pulmonary metastatic tumors in 11 cases were treated with RF-2000. The following extents were observed: (1) blood pressure,pulse rate and oxygen saturation during the treatment;(2)the variety of the liver function,the lung function and renal function after treatment;(3) the variety of the volume and low dense shadow of tumors after treatment;(4)the survival rate for 5 years. Results (1) The blood pressure,pulse rate and oxygen saturation were not changed during the treatment;(2) the liver function,the lung function and renal function after treatment were also not changed;(3) the volume of tumors were increased one month after treatment,three months later,23 lesions of 26 tumors were disappeared,2 decreased to more than 80% and 1 to 50%~80% in volume,and the low dense shadow within the tumors appeared as Ⅰtype (less than 50%) in one,Ⅱtype (50%~80%) in 3 and Ⅲ type (more than 80%) in 22;(4)The survival rate for 5 years was 81.82%(9/11). Conclusion The clinical effects and safety of treatment of colorectal cancer to lung metastases with percutaneous radiofrequency-2000 (RF-2000) under CT-guided is excellent.
8.The DSA characteristics and the expression of VEGF of primary hepatocellular carcinoma: a correlative study

Objective To discuss the DSA characteristics of primary hepatocellular carcinoma (HCC) and to investigate the relationship between the DSA findings and the expression of tumor’s vascular endothelial growth factor (VEGF). Methods Fifty-four patients with pathologically-proved primary HCC were enrolled in this study. The DSA findings, including tumor’s size, lesion’s number, lesion’s margin, the degree of blood supply, the presence or absence of artreiovnous shunting (AVS), etc. were observed and analyzed. The qualitative test of VEGF of HCC tissue was made by using Elivision TMplus method, and based on the stain degree and the percentage of the positive cells a comprehensive analyse was carried out. With HE stain the pathologic classification was conducted by using Edmmond method. In this study, grade I and II were regarded as well-differentiated group and grade III and IV were regarded as poorly-differentiated group. The data were processed by software SPSSS13.0 and the results were statistically analyzed. Results The expression degree of VEGF in HCC tissue was related to the blood supply pattern, the lesion’s number, the integrity of the tumor’s border and the degree of differentiation (P 0.05). Conclusion Some DSA manifestations of primary HCC bear a certain relationship to the VEGF content in the tumor tissue. Through observing the DSA characteristics, the assessment of the invasion and metastasis of HCC can be well made, which will afford the clinical physicians a useful reference in making individualized therapeutic schedule and in estimating the prognosis.
9.Effects of hypertonic sodium chloride hydroxyethyl starch solution on cerebral vasospasm following subarachnoid hemorrhage and its mechanism

Objective To investigate the protective effect and potential mechanisms of hypertonic sodium chloride hydroxyethyl starch solution (HSH) against the cerebral vasospasm (CVS) following subarachnoid hemorrhage (SAH).Methods Twenty-four male Sprague-Dawley (SD) rats were randomly assigned to four groups according to the random number table,with 6 rats in each group.The SAH-CVS model was reproduced by injection of the blood twice through the cisterna magna.Rats in both model and HSH treatment groups received 8 mL/kg normal saline (NS) or HSH treatment everyday via caudal vein.Rats in sham group were injected with 1.5 mL/kg NS into cisterna magna followed by 8 mL/kg NS treatment.Rats in normal group received no treatment.Rats were sacrificed to harvest basilar artery after 7 days.The thickness of vessel wall and lumen area were measured using hematoxylin-eosin (HE) staining.The rate of apoptosis of vascular smooth muscle cell (VSMC) was assessed using flow cytometry.Caspase-3 activity was measured by a fluorometric assay.The expressions of Bax and Bcl-2 were determined by Western Blot.Intracellular reactive oxygen species (ROS) was detected by H2DCFDA.Results Compared with normal group,increased thickness of vessel wall (μm:27.72 ± 1.94 vs.18.30 ± 1.10,P<0.05),decreased lumen area (μm2:26 115 ± 1 991 vs.55 080 ± 2 091,P<0.05),and elevation of rate of apoptosis of VSMCs [(35.05 ± 5.54) % vs.(5.93 ± 1.53) %,P< 0.05] were found in model group.Compared with model group,decreased thickness of vessel wall (μm:22.55 ± 1.50 vs.27.72 ± 1.94,P<0.05),increase of lumen area (μm2:48 115 ±2 460 vs.26 115 ± 1 991,P<0.05),and depressed rate of apoptosis of VSMCs [(16.54 ± 5.94) % vs.(35.05 ± 5.54) %,P< 0.05] were found in HSH treatment group.Caspase-3 activity,intracellular ROS level,Bax and Bcl-2 expressions in model group were (188.40 ± 19.35)%,(163.50 ± 17.02)%,(208.71 ± 26.04)% and (44.52 ± 9.61) % of those of normal group,and the differences of these parameters between model and normal groups were statistically significant (all P<0.05).Caspase-3 activity,intracellular ROS level,Bax and Bcl-2 expressions in HSH treatment group were (135.05 ± 19.52)%,(119.44 ± 11.50)%,(139.20 ± 18.04)% and (85.35 ± 13.12)% of those of normal group,respectively,and the differences of these parameters between HSH treatment and model groups were statistically significant (all P<0.05).The differences of all measurements between sham and normal groups were not statistically significant.Conclusion The current results demonstrate that HSH attenuates the SAH-induced CVS,alleviates thickness of vessel wall,and increases lumen area via inhibition of VSMCs apoptosis.
10.Evaluation on effect of treatment and assistance to advanced schistosomia-sis patients in Hunan Province from 2004 to 2013

Objective To comprehensively evaluate the effect of the program of treatment and assistance to advanced schis-tosomiasis patients in Hunan Province from 2004 to 2013. Methods The fund investment of the program,the profits of hospi-tals and the improvement of the patients’health were investigated by data collection and questionnaire survey. The evaluation index system of treatment and assistance to advanced schistosomiasis in Hunan Province was constructed by the Delphi method and analytic hierarchy process,and the program was assessed comprehensively. Results The evaluation index system includ-ing 6 primary indices and 33 secondary indices was established. Among all the primary indices,the score of the treatment and assistance(22.25)was the highest,and that of the satisfaction assessment(8.15)was the lowest,and the score of the compre-hensive assessment was 87.06. The average cure rate of the patients was 13.08%from 2004 to 2013. More than 60%of the pa-tients’disease condition got better,and nearly 70%of the patients’psychological condition improved,and more than 70%of patients’self-help ability and social contact improved,as well as family happiness increased. In addition,the annual average cost for caretakers decreased by 2000 Yuan,and the profits of all the fixed-point hospitals for treatment and assistance in-creased. Conclusion The effectiveness and efficiency of the treatment and assistance to advanced schistosomiasis patients in Hunan Province is obvious,and the government should continuously invest in the program.
