1.Detection of Telomerase Activity in Gastric Mucosa by Improved TRAP Assay

Telomerase activity in 43 biopsy of gastric mucosa was detected with the modified telomeric repeat amplification protocol(TRAP) assay to elucidate pathogenesis of gastric cancer.The detected positive rates were as follows:87%(20/23) in gastric cancer,28 6%(2/7) in moderate and non-typical gastritis ,0%(0/10 in atrophic and superficial gastritis,respectively.These results suggest that the detection of telomerase activity may play a significant role in the diagnosis of gastric cancer.
2.Detection of CK19 mRNA and CK20 mRNA in peripheral blood cells of patients with early cervicalcancer and its clinical significance

Purpose:To study the expression of CK19 and CK2 0mRNA in peripheral blood patients with FRGO Stage ⅠA to ⅡA cervical carcinoma and to investigate its clinical significance. Methods:Using the reverse transcription polymerase chain reacti on(RT-PCR),CK19 and CK20mRNA expression was examined in peripheral blood fro m 250 patients with early cervical carcinoma before operation,50 Patients with benign gynecological tumors and 18 healthy volunteers. In 250 patients,possible correlations between clinical pathological factors were analyzed. Results:The positive expression rates of CK19 and CK20mRNA were 36% and 24% in 250 cervical carcinomas respectively,in comparison with 3.0% an d 0% with benign gynecological tumors and all subjects in healthy volunteers wer e negative; The expression of CK19 and CK20 mRNA were significantly correlated w ith lymph vascular space involvement,but was not associated with prognostic fac tors including stage,differentiation,pathological types ,lymph node metastasis ,bully tumor size . 3.Treatment and prognostic factors for the patients with yolk sac tumors of the ovary

Background and purpose:To evaluate the treatment regimens used for patients with yolk sac tumors of the ovary and prognostic factors in our hospital. Methods:Clinical data for the patients with yolk sac tumors of the ovary were analyzed retrospectively. Patients were divided into four groups: patients in A group received nonsparing surgery and platinum-based chemotherapy; in B group received nonsparing surgery and non-platinum-based chemotherapy; in C group received fertility-sparing surgery and platinum-based chemotherapy; in D group received fertility-sparing surgery and non-platinum-based chemotherapy. 5-year survival rate and prognostic factors were analyzed.Results:The overall 5- year survival rate was 69.2 %. The 5- years survival rates were 81.8 %, 40.0 %, 82.6% and 50.0% for A, B, C and D group, respectively. Platinum-based chemotherapy achieved significant survival benefit (82.4% vs. 44.4%, P0.05). Clinical stage, the size of residual tumor and Platinum-based chemotherapy were the mainly prognostic factors in multianalysis.Conclusions:Fertility-sparing surgery with a postoperative plainum-based chemotherapy should be the ideal modality for the treatment of yolk sac tumors of the ovary. Staging and tumor-reductive surgery were most associated with the prognosis of the patients.
4.Primary testicular non-Hodgkin's lymphoma

Objective To summarize the clinical presentation, pathology features, and treat-ment principle for primary testicular non-Hodgkin's lymphoma. Methods Twelve patients were di-agnosed with primary testicular lymphoma. The mean age was 62 years (36-78). Of the patients, unilateral primary testicular tumors were found in 11 cases and bilateral tumors were found in 1 case. All cases had swollen testes, 3 cases had mild pain and 1 had low-grade fever. Ultrasonic examination detected solid mass in all 12 cases. CT scan revealed retroperitoneal enlarged lymph nodes in 3 cases. Nine patients were diagnosed with disease of stage Ⅰ E, 2 of stage Ⅱ E, and 1 of stage Ⅲ E. All of the patients underwent radical orchiectomies. Postoperative treatment included: CHOP chemotherapy for 10 cases, radiotherapy after chemotherapy for 5 cases, and surgery alone for 2 cases. Results Post-operative pathology results were non-Hodgkin's lymphoma in all cases. One patient lost in follow up, one died within 2 years because of other disease. The 1, 3 and 5 year actual survival rates were 82% (9/11) ,40%(4/10),20% (2/10), respectively. The relapsed organs included contralateral testis(3/ 11), central nervous system(3/11), liver(1/11)and retroperitoneal lymph node(1/11). Conclusions The prognosis of the primary testicular non-Hodgkin's lymphoma is very poor. Chemotherapy must be used after surgery for any stage. Stage Ⅰ E and Ⅱ E patient should be treated by surgery combined with radiotherapy and chemotherapy. Contralateral testis should be irradiated prophylactically. Pa-tients beyond stage Ⅱ E should accept chemotherapy after surgery and radiotherapy according to the patient's status.
5.Predictive value of serum cTnI and NT-proBNP for patients with chronic stable heart failure

Objective:To explore the correlation among serum cardiac troponin I (cTnI),N terminal pro brain natri-uretic peptide (NT-proBNP)and main endpoint events of heart in patients with chronic stable heart failure.Meth-ods:The present study enrolled 95 patients with NYHA cardiac function class III~IV from Feb 2010 to Feb 2011.According to levels of cTnI and NT-proBNP,the patients were divided into cTnI negative group (n=60)and cTnI positive group (n=35);NT-proBNP negative group (n=40)and NT-proBNP positive group (n=55),all patients were followed up for two years,and the main endpoint events were cardiogenic sudden death and rehospitalization caused by acute aggravation of heart failure.Results:Compared with negative group,the hazard ratio (HR)of end-point events was 2.69 and confidence interval (CI)was 1.54~ 4.72,P = 0.002 in cTnI positive group;HR was 2.54 and CI was 1.35~4.78,P =0.003 in NT-proBNP positive group;further interclass crossover analysis found that,when patients'cTnI and NT-proBNP were both positive,the hazard ratio of cardiac endpoint events was the highest (HR=6.34,CI 2.26~17.9,P <0.001).Conclusion:In patients with chronic stable heart failure,serum elevated levels of cardiac troponin I and N terminal pro brain natriuretic peptide are important predictors reflecting prognosis of patients with heart failure.
6.Clinical characteristics and prognostic of uterine body invasion of stage Ⅰ_b and Ⅱ_a uterine cervix carcinoma

Background and purpose:Uterine body invasion of carcinoma of the uterine cervix is not in the International Federation of Gynecology and Obstetrics(FIGO)staging system.Some reports suggested that uterine body invasion of carcinoma of the uterine cervix was an important prognostic factor.The aim of this study was to investigate the clinical characteristics and prognosis of the uterine cervix carcinoma with uterine body invasion.Methods:The clinicopathologic records of 406 patients with carcinoma of the uterine cervix of stage Ⅰb~Ⅱa who underwent radical hysterectomy and pelvic lymphadenectomy were retrospectively analyzed,the number of uterine body invasion was 69 in all cases.On the contrary,the negative number was 337.We studied the general condition(age of onset)and tumor conditions,including clinical stage,tumor diameter,pathological type,pathology differentiated degree,positive pelvic lymph nodes,deep stromal invasion,and lymph vascular space involvement.Results:In univariate survival analysis,the incidence of stage Ⅱa,non-squamous cell cancer(non-SqCC),tumor size≥4 cm,deep stromal invasion and positive pelvic lymph nodes were signifi cantly higher in carcinoma of the uterine cervix with uterine body invasion than those in control group(P
7.Optimization of pretreatment methods for N-nitrosamine compounds in ready-to-eat aquatic products

Objective:
To optimize the pretreatment method of N-nitrosamine compounds in ready-to-eat aquatic products.
Methods:
Market-sold ready-to-eat aquatic products were collected, homogenized and distilled by steam. The samples were extracted for 10 minutes using dispersive liquid-liquid microextraction (DLLME) with ethanol, trichloromethane and sodium chloride (3.0 g). After centrifugation, the organic phase in the lower layer was collected and subjected to gas chromatography-tandem mass spectrometry (GC-MS/MS). The six common N-nitrosamine compounds were determined in ready-to-eat aquatic products using multiple reaction monitoring mode (MRM) and quantified by the internal standard method.
Results:
The optimized method exhibited a good linear relationship at concentrations of 10.0 to 500 μg/L for determination of 6 N-nitrosamine compounds (correlation coefficient of greater than 0.999), with 0.05 to 0.60 μg/kg limit of detection, 0.15 to 1.60 μg/kg limit of quantitation, mean spiked recovery rates of 71.8% to 108.9%, and relative standard deviations of 1.4% to 8.6%. N-Nitrosodimethylamine showed the highest detection rate in 20 market-sold ready-to-eat aquatic products (90%), and the detection rates of N-Nitrosopyrrolidine, N-Nitrosodiethylamine and N-dibutylnitrosamine were 15%, 10% and 10%, respectively.
Conclusion
Steam distillation combined with DLLME may optimize the pretreatment method of N-nitrosamine compounds in ready-to-eat aquatic products and meet the measurement requirements.
8.Diagnosis, therapy and follow-up in infants with subclincal hypothyroidism--A clinical analysis of 107 cases

The serum TSH levels of 107 infants with subclinical hypothyroidism (SH) were > 20 mIU/L after 1-8 check-up, along with FT4, TT4, within low normal range. After given small dosage L-T4, for 4 weeks, blood TSH level obviously descended while FT4, TT4, ascended (all P <0.01). Seven cases of thyroid hypogenesis and 7 strumas were found by ultrasonography. It seems appropriate to use dosage of 3-4 μg·kg·-1·d-1 L-T4 in treating SH.
9.Treatment of renal calyceal stricture and atresia through antegrade percutaneous nephrostomy by Holmi-um laser

Objective To evaluate the clinical efficacy and safety of Holmium laser in the treat ment of calyceal stricture and atresia through antegrade percutaneous nephrostomy. Methods Ante grade percutaneous nephrostomy was performed in 68 patients with calyceal stricture and atresiathrough a rigid 8/9.8 F ureteroscope. The stricture and atresia was incised in a linear fashion by theHolmium laser with a 550 mm fiber. After completion of the incision,a double J ureteral stent wasplaced for 6-8 weeks and nephrostomy tube was kept for 7 days thereafter. Patients were then fol lowed up with IVU and/or ultrasound at 3-6 month intervals. Results The mean operative timewas 90 min,ranged from 80 to 120 min. The mean postoperative hospital stay was 8 d(7-9d). Hy dronephrosis was significantly improved in 38 cases in an average follow up of 9 months (4-26 months). Repeated laser incision was performed to 4 treatment failures and all turned out to be suc cessful. Conclusions The Holmium laser treatment through antegrade percutaneous nephrostomyfor calyceal stricture and atresia has characteristics of minimal invasion,short hospital stay,good effi cacy in short term and repeated cases. This procedure to be used as the first choice for patients withgood renal function and mild hydronephrosis,especially accompanied with renal calculus.
10.The follow-up study of therapeutic effects of alendronate sodium on osteoporosis

Objective To study early changes in biochemical markers of bone turnover for prodicting the bone mineral density(BMD) response to alendronate therapy in Chinese men with osteoporosis. Methods Seventy-eight men aged 60 to 82 years with osteoporosis [mean age (69.8 ±11.8) years]were treated with alendronate sodium 70 mg/week for 12 months. Serum bone gla-protein (BGP) and serum pyridinoline-eross-linked earboxyterminal telopeptide of type I collagen (ICTP) level were measured by chemiluminescence (equipment is Roche E170). Serum BGP, ICTP and BSALP levels were measured before and 3 months and 12 months after treatment. BMD in lumbar spine and femoral neck were measured with dual energy X-ray absorptiometry (GE PRODIGY Inc. ) at L1-4 and at the left hip(total hip,trochanter,Ward's area and femoral neck)before and 12 months after treatment. Results After 3-months and 12-months treatment, the percent reductions of serum ICTP levels were 45.8% and 51.6%, serum BGP level 32.0% and 37.5%, serum BSALP level 35. 3% and 39.9% ,respectively. After 12-months treatment, the percent increase of lumbar BMD was 11.8%, femoral neck BMD 11.4 %. The percent reductions of serum ICTP level at months 3 and 12 after treatment were positively correlated to the percent increase of lumbar BMD (r=0.28, 0.295,P <0.05 and P<0.01)and of fermoral neck BMD at 12 months after treatment(r=0.262, 0.333, P<0.05 and P<0.01)respectively. The percent reductions of serum BGP level at months 3 and 12 after treatment were positively correlated to the percent increase of lumbar BMD (r=0. 322, 0.401,all P<0.01) and of fermoral neck BMD at 12 months after treatment (r=0.277,0.284, all P<0.05)respectively. The percent reductions of serum BSALP level at months 3 and 12 after treatment were positively correlated to the percent increase of lumbar BMD (r=0.133,0.231,all P<0.05) and of fermoral neck BMD at 12 months after treatment(r=0.248, 0.317, all P<0.01)respectively.Conclusions Early changes in biochemical markers of bone turnover rates can predict BMD response to alendronate in Chinese men with osteoporosis.
