1.Expression of 14-3-3 sigma gene in patients with breast cancer and its clinical significance

Objective To investigate expression of 14-3-3 sigma gene in patients with breast cancer and its clinical significance.Methods Expression of 14-3-3 sigma gene was semi-quantitated by using RT-PCR and western-blot in 40 specimens of breast cancer and 18 specimens benign breast disease tissue.Results Of 40 cases of breast cancer 35(87.5%)were negative for 14-3-3 sigma gene in RT-PCR,32(80%)were negative in Western-blot,and 31(77.5%)were negative in both RT-PCR and western-blot.Besides,the expression in 2 cases was down-regulation in both the 2 method.In 18 specimens with benign breast disease tissue the expression of 14-3-3 sigma gene was detectable,which was demonstrated by RT-PCR or western-blot.Conclusion Inactivation and down-regulation of 14-3-3 sigma gene is a frequent event in breast cancer,and it may contribute to diagnosis of breast cancer.
2.Applications of three newer contrast-enhaced MRI sequences in the diagnosis of intracranial tumors

Purpose:To investigate the values of three newer contrast enhanced MRI sequences including gadolinium-enhanced FLAIR MRI(CE FLAIR), dynamic contrast-enhanced MRI (DCE MRI) and perfusion- weighted MRI(PWI) by comparing their advantages and disadvantages respectively in the diagnosis of intracranial tumors. Methods:43 patients with intracranial tumors underwent DCE MRI, CE FLAIR and PWI respectively. The gadolinium-enhanced FLAIR, dynamic enhanced MR and perfusion-weighted MR images were evaluated independently by two radiologists for the number of examinations with one or more enhancing lesions, the number and location of enhancing lesions per examination, the detectability for different lesions in different locations, size and extent of the lesions. Results:Perfusion-weighted MR images showed poor quality and could not give a diagnosis in 5 of 43 cases because of heavy susceptibility artifacts. There were 47 lesions in the 38 cases. However, 41 lesions were found on CE FLAIR MR images and 42 on DCE MRI and 45 on PWI. 3 lesions(2 located in the subcortical area and 1 in paraventricle) were only revealed on the CE FLAIR images. 4 lesions in the basal ganglia area were only found on dynamic enhanced images. 7 lesions in the cerebral hemisphere were only found on perfusion-weighted images. AIM: To observe the growthinhibiting cell cycle-modifying and apoptosis-inducing effects of satraplatin on human ovarian carcinoma cell line A2780, and to explore its possible mechanism. METHODS: The effect of satraplatin on A2780 cells proliferation was determined using MTT, and the change in cell cycle was analyzed using PI staining. Morphologic change was visualized by fluorescence and electron microscopy. AnnexinV-FITC/PI staining multiparameter flow cytometry and immuno- histochemical TUNEL assay were used to detect apoptotic cells. The activity of caspase-3 and the effect of pan-caspase inhibitor on cell viability were measured as well. RESULTS: The growthinhibiting and apoptosis-inducing effects of satraplatin were dose-dependent and similar to those of cisplatin. Satraplatin mainly caused A2780 cell accumulation in S phase accompanied by minor accumulation in G2/M phase. Cells treated with satraplatin exhibited typical morphology of apoptosis. Satraplatin-induced increase in caspase-3 activity of A2780 cells was concentration-dependent. The viability of A2780 cells was affected by pan-caspase inhibitor z-VAD-fmk in a dose-dependent manner under certain concentration of z-VAD-fmk. CONCLUSION: Satraplatin-induced apoptosis in A2780 in vitro was observed. Caspase-dependent and independent pathways were involved in apoptosis induced by satraplatin, and the latter included caspase-3 dependent and non-caspase-3 dependent pathways.
4.Usefulness of contrast-enhanced FLAIR MRI in the diagnosis of intracranial tumors

Objective To clone express and purify Schistosoma japonicum fructose?1 6?bisphosphate aldolase SjFBPA in E. coli and observe its expression in different developmental stages of S. japonicum. Methods FBPA gene was amplified from S. japonicum adult worm cDNA by using PCR. The amplified product was recombined into pET28a plasmid and inducibly expressed with IPTG in E. coli BL21. SDS?PAGE and Western blotting were employed to analyze and identify the recombinant protein SjFBPA rSjFBPA . Then rSjFBPA was purified by chromatographic purification and its purity was analyzed by SDS?PAGE. The protein concentration of rSjFBPA purified was measured by the BCA method. Furthermore SjFBPA mRNA was ana?lyzed in different developmental stages of S. japonicum by RT?PCR. Results SjFBPA was successfully amplified by using PCR and identified by restriction enzyme digestion and sequencing. The Western blotting analysis confirmed that the recombinant pro?tein could specifically reactive to the anti?His?tag monoclonal antibody. The concentration of the purified recombinant protein was about 4 mg/ml. The result of RT?PCR showed that SjFBPA mRNA was expressed in cercaria schistosomulum adult worm and egg of S. japonicum. Conclusion SjFBPA is successfully recombined and expressed in a prokaryotic system and SjFBPA mRNA is expressed in cercaria schistosomulum adult worm and egg of S. japonicum.
6.The potential effects of linalool on enantioselective skin permeation of norgestrel.

The purpose of this study is to investigate the enantioselectivity of norgestrel (NG) transdermal permeation and the potential influence of linalool and lipids on the enantioselectivity. In vitro skin permeation studies of NG across the excised rat skins were performed with Valia-Chien diffusion cells, and the permeation samples were analyzed by enantioselective HPLC. The possible enantioselective permeation of NG across intact rat back skin and lipids extracted rat back skin and the influence of linalool were evaluated. The skin permeation rate of dl-NG was two times higher than that of l-NG when donor solutions (EtOH/H2O 2 : 8, v/v) containing l-NG or dl-NG. It may be mainly attributed to the solubility discrepancy between enantiomer and racemate. The enantioselective permeation of dl-NG across intact rat skin was observed when the donor solutions containing dl-linalool. The permeation flux of l-NG was 22% higher than that of d-NG. But interestingly, the enantioselective permeation of dl-NG disappeared under the same experimental condition except that the lipid extracted rat skin was used. Attenuated total reflection-fourier transform infrared spectroscopy analysis of stratum corneum showed that the wave number for asymmetric CH2 stretching vibrations of lipids treated with dl-linalool was greater than that of the control. The results indicated that the enantioselective permeation of NG may be contributed by the interaction between dl-linalool and lipids. More than half of lipids were composed of ceramides. The stereospecific interaction maybe existed among chiral enhancer (linalool), lipids (ceramides) and/or chiral drugs (NG).
7.Docetaxel plus prednisone versus mitoxantrone plus prednisone for metastatic hormone-refractory prostate cancer in chinese patients:experience of a single center

Background and purpose:Chemotherapy with docetaxel has become the first line treatment for hormone refractory prostate cancer.To preliminarily investigate the efficacy of docetaxel plus prednisone and mitoxantrone plus prednisone for treating metastatic hormone-refractory prostate cancer and to further evaluate its adverse events in Chinese patients. Methods:83 patients with metastatic hormone-refractory prostate cancer were candidates for the trial and given a combination of docetaxel 75 mg/m2 intravenously on d 1 or mitoxantrone 12 mg/m2 on day 1 plus prednisone 5 mg twice daily on d 1~21, 21 days a cycle. Serum PSA level, relief of bone pain, myelosuppression, and vomiting were recorded and calculated.Results:Docetaxel plus prednisone were administered to 44 patients, 13.6% (6/44) of them got a complete response; 29.5% (13/44) achieved a partial response; 29.5% (13/44) had stable disease; and 27.3% (12/44) progressed. The average time to PSA progression was 37.8 weeks (12~101 weeks) in the responsive and stable disease patients. The 12 patients with progressive disease were given MP as a salvage therapy, and 16.7% (2/12) achieved a partial response, 25.0% (3/12) had stable disease. Only 2 patients died of disease aggravation. Mitoxantrone plus prednisone were given to 39 patients, and 7.7% (3/39) of them got a complete response; 25.6% (10/39) achieved a partial response; 25.6% (10/39) had a stable disease; and 41.0% (16/39) of patients progressed. The mean time to PSA progression was 25.3 weeks (8~61 weeks) in the responsive and stable disease patients. The 14 patients with progressive disease were administered DP as a salvage therapy, and 7.1% (1/14) achieved a complete response, 35.7% (5/14) got a partial response, 21.4% (3/14) had stable disease and from the new baseline. Four patients died at the last follow-up.Conclusions:In Chinese patients, docetaxel plus prednisone is better than mitoxantrone plus prednisone in PSA response rate and PSA control, but there occurs a bit more toxicity. When the tumor is resistant to one regimen, the other might be still effective in controlling the disease from progression.
8.Treatment of bone defect in hip revision.

To investigate the option of treatments for bone defect in femoral revision after total hip replacement (THR), 29 patients (29 hips) who had undergone hip replacement 13.7 years previously were hospitalized again to undergo revisions. 3 patients were given cement protheses and the other 24 patients were given non-cement protheses. In 3 cases were used Synergy+Reflection protheses, and in the others were used CLS +ALLOFIT. All of the patients have got primary healing without early infection symptoms post-operatively. We have been following up 25 cases (25 hips) for 58 months averagely. All the hip joints functioned well. Harris score increased from preoperative 36.8 to postoperative 93.2. In our experience, if there exist significant bone defect in acetabular bone and the coverage of the cup by the graft is more than 80%, bone graft is not needful. If the coverage is between 50% and 80%, bone grains combining bone segment grafts are needful. If the coverage is less than 50%, bone segment grafts be likely to give good effect. In summary, compacted bone grains grafts combining allograft osintegumentale plate could gain satisfactory outcome for femoral bone defect.

9. Analysis of infection composition and drug resistance to Gram-negative bacilli in children′s respiratory tract in Suzhou from 2007 to 2016

Objective:
To analyze the infection composition and drug resistance to Gram-negative (G-) bacilli in children′s respiratory tract in Suzhou, in order to provide evidence for rational use of antibiotics clinically.
Methods:
G- bacilli culture samples were collected from 21 561 cases of nasopharyngeal secretions from patients with respiratory tract infection admitted at the Department of Respiratory, Children′s Hospital of Soochow University from January 2007 to December 2016, including 21 246 cases in general wards, and 315 patients who were transferred to the respiratory department after treatment in the Intensive Care Unit(ICU), and the children were divided into the general ward group and the ICU group, and the pathogens were compared and the changes in bacterial susceptibility were dynamically observed between the 2 groups.
Results:
The primary G-bacteria for respiratory infection was Haemophilus influenzae, followed by Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and Acinetobacter baumannii.The detection rates of Klebsiella pneumoniae and Pseudomonas aeruginosa in the ICU group were 16.8% (21/125 strains)and 14.4%(18/125 strains), respectively, which were significantly higher than those in the general ward group [10.0%(208/2 071 strains), 9.2%(190/2 071 strains)]. The detection rates of G-bacteria in the ICU group were 33.7%(106/315 cases), which were significantly higher than those in the general ward group [9.4%(1 997/21 246 cases)], and the difference was statistically significant(
10.Clinical Efficacy of Niuhuang Chengqitang in Treatment of Acute Gastrointestinal Injury in Patients with Severe Traumatic Brain Injury and Its Effect on MTL, DAO, and NSE

ObjectiveTo evaluate the clinical efficacy of Niuhuang Chengqitang in the treatment of acute gastrointestinal injury (AGI) in patients with severe traumatic brain injury (sTBI) and observe the effect on serum motilin (MTL), diamine oxidase (DAO), and neuron-specific enolase (NSE). MethodBy the prospective randomized controlled trial (RCT) method, 86 sTBI inpatients with AGI were randomly divided into an observation group(43 cases)and a control group (43 cases). All patients were given conventional western medicine treatment, including actively targeting the primary diseases and combined diseases, supply of enteral nutrition as soon as possible, and gastrointestinal agents, while those in the observation group received additional Niuhuang Chengqitang by nasal administration. The treatment course of both groups was 7 days. The clinical efficacy of the two groups was compared before treatment and 7 days after treatment. Gastrointestinal failure (GIF) score and intra-abdominal pressure (IAP) of the two groups were recorded, and serum MTL, DAO and NSE levels were detected before treatment, 3 days and 7 days after treatment. The Glasgow Coma Scale (GCS) score, duration of mechanical ventilation, length of ICU stay, and 28-day mortality were recorded. ResultDuring the treatment and observation, 4 cases fell off and 82 cases were finally enrolled, including 41 in the observation group and 41 in the control group. After treatment, the total effective rate in the observation group was 80.49%(33/41), higher than 56.10%(23/41) in the control group (χ2=9.137,P<0.05). GIF and IAP scores in the observation group were significantly improved after 3 and 7 days of treatment (P<0.01), and the observation group was superior to the control group after 7 days of treatment (P<0.05). The serum levels of MTL, DAO, and NSE in the two groups were improved after 3 and 7 days of treatment (P<0.05, P<0.01). The levels of MTL and NSE in the observation group were better than those in the control group after 3 days of treatment (P<0.05, P<0.01), and the levels of MTL, DAO, and NSE in the observation group were superior to those in the control group after 7 days of treatment (P<0.05, P<0.01). The 28-day GCS score in the observation group was higher than that in the control group (P<0.01), and the duration of mechanical ventilation and ICU stay in the observation group were significantly shorter than those in the control group (P<0.05). There was no significant difference in 28-day mortality between the two groups. No adverse reactions related to Niuhuang Chengqitang occurred in the observation group. ConclusionNiuhuang Chengqitang can effectively improve the gastrointestinal function, regulate MTL, DAO, and NSE levels, shorten the ICU stay, and promote the recovery of sTBI patients with AGI.
