1.Expression of Interleukin-1 9 and Its Relationship with Angiopathy in 2 Diabetes Mellitus

Objective To explore the expression of interleukin-19 (IL-19)and to investigate the relationship between IL-19 and type 2 diabetes mellitus9 (T2DM)with angiopathy.Methods IL-19,FBG,FINS and HbA1c were measured in 120 T2DM (including 32 patients with macroangiopathy,52 patients with microangiopathy and 36 without angiopathy)and 50 healthy subjects,the results were compared.Results Compared with control group,the level of IL-19 in T2DM (41.9 ± 11.9pg/ml vs 16.2±8.5pg/ml)was significantly higher than those of healthy subjects,and the difference was signifecant (t=7.56,P<0.05).The levels of IL-19 in T2DM with macroangiopathy or microangiopathy increased significantly in com-parison with those without angiopathy (t=4.57,3.26;all P<0.05),and closely correlated with the number of angiopathy. The level of IL-19 in T2DM was significantly positive correlated with HOMA-IR and HbA1c (r=0.523 and 0.491,P<0.01).Conclusion IL-19 level was significantly increased in patients with T2DM,and was closely related with HbA1c,insu-lin resistance,IL-19 may play acertain role in the pathogenesis of in T2DM with vascular complications.
2.Effect of Qingqihuatan Decoction on pulmonary inflammation in asthmatic mouse

Objective To observe the interventional effect of Qingqihuatan Decoction on airway inflammation and inflammatory reaction of lung tissue in asthmatic mouse.Methods Forty-five BALB/c mice were randomly divided into the normal control group(CON),asthmatic model group(MOD),dexamethasone group(TRE),Qingqihuatan Decoction group(X1) and Qingqihuatan Decoction combined dexamethasone group(X2).The asthmatic mouse model was established by the sensitization and inhalation of OVA and aluminium hydroxide gel.The bronchoalveolar lavage fluid (BALF) was performed the cell counts and eosinophil counts,and the pathological changes of lung tissue were observed.Results Compared with CON group,BALF cell count and eosinophil count in the model group were increased obviously,which in the treatment group were significantly decreased.The effect of the X1 group and X2 group had statistical difference between on 5 d and 15 d.(P<0.05 or P<0.01);the treatment groups could reduce the pathogenical infiltration,lung bullae production and airway epithelial thickening of mouse lung tissue,especially the inflammation damage in X2 group was mild.Conclusion Qingqihuatan Decoction can relieve the airway inflammation and improves the lung tissue inflammatory response in model mouse.
3.Expression and Clinical Significance of Angiopoietin Like Protein 1 and 2 in Patients with Gastric Cancer

Objective To explore the expressions and clinical value of angiopoietin like protein 1 (Ang-1)and angiopoietin like protein 2 (Ang-2)in patients with gastric cancer.Methods 65 patients with gastric cancer and 50 healthy subjects for con-trol group were collected from June in 2012 to December in 2014.The level of Ang-1 and Ang-2 were determined by enzyme-linked immunosorbent assay (ELISA).The results were compared.Results Compared with control group (19.8±2.1 μg/L),the level of Ang-1 in patients with gastric cancer (19.8±2.1 μg/L)was no statistics difference (P >0.05),but Ang-2 (2.3±0.8 μg/L)was significantly higher than those of healthy subjects (0.8±0.2 μg/L,t=2.50,P <0.01).There was no significant difference of Ang-2 level in different pathological types of gastric cancer (P >0.05).The level of Ang-2 in TNM stage Ⅲ~Ⅳ (2.6±0.5 μg/L)was obviously higher than that in stage.Ⅰ~Ⅱ (1.6±0.4 μg/L).Ang-2 levels were signif-icantly higher in patients with lymph node metastasis (2.7±0.5 μg/L)or postoperative recurrence after one year (2.0±0.6μg/L)than those without lymph node metastasis (1.6 ± 0.5 μg/L)or postoperative recurrence (1.2 ± 0.5 μg/L,P <0.05).The level of Ang-2 in gastric cancer was significantly positive correlated with tumor stage and lymph node metastasis (r=0.31 and 0.33 respectively,P <0.01).Conclusion Ang-2 level was significantly increased in gastric cancer.Its level was significantly correlated with tumor stage,lymph node metastasis and prognosis,which has important application value for the cancer progression,clinical observation and prognosis evaluation of gastric cancer.
4.Drug resistance and molecular epidemiological analysis of KPC-producing Escherichia coli strains isolated in Hangzhou

Objective To analyze the epidemiological characteristics of Klebsiella pneumoniae car-bapenemase(KPC)-producing Escherichia coli(E. coli)strains isolated in Hangzhou,China. Methods A total of 25 KPC-producing Escherichia coli strains were collected from four hospitals in Hangzhou from July 2012 to January 2014. Antibiotic susceptibility of the isolates to 22 common antimicrobial agents was deter-mined by using Kirby-Bauer(K-B)disk diffusion method. PCR analysis and gene sequencing were used for bla KPC gene screening. The modified Hodge test was performed to detect the production of carbapenemase. Pulsed-field gel electrophoresis(PFGE)and multi-locus sequence typing(MLST)were used for homology analysis. Results All of the 25 clinical isolates were confirmed to be KPC-producing E. coli strains,harbo-ring the blaKPC-2 gene. These KPC-producing isolates showed high drug resistance rates and were resistant to almost all β-lactam antibiotics. PFGE typing classified the 25 isolates into three main homologous clone groups,including clone group A(4 isolates),clone group B(5 isolates)and clone group C(2 isolates), and some single clones(14 isolates). MLST typing classified the isolates into eight ST types,including ST131(14 isolates),ST167(3 isolates),ST2003(3 isolates),ST410(1 isolate),ST457(1 isolate), ST1463(1 isolate),STnew1(1 isolate)and STnew2(1 isolate). The typing results of PFGE and MLST were consistent with each other. Conclusion The prevalent KPC-producing E. coli strains in Hangzhou, China were ST131 type,which were resistant to multiple antibiotics and had been detected in several hospi-tals. The epidemic of KPC-producing E. coli strain often occurred at some special wards,such as Intensive Care Unit(ICU)and emergency ICU.

ObjectiveTo investigate the state of rehabilitation professionals in knowledge about chronic obstructive pulmonary disease(COPD).Methods60 rehabilitation professionals had received a 3-hour education delivered by four pulmonary rehabilitation experts. The trainees were assessed by Bristol COPD Knowledge Questionnaire (BCKQ) after the course.ResultsThe score of BCKQ was from 42 to 80 while the average score was (60.00±9.33).ConclusionThe rehabilitation professionals' knowledge about COPD was poor.
6.Expression and clinical significance of pro-gastrin-releasing peptide and carbohydrate antigen 72-4 in patients with gastric cancer

Objective To explore the expressions and clinical values of pro-gastrin-releasing peptide(ProGRP) and carbohydrate antigen 72-4 (CA72-4) in patients with gastric cancer.Methods Ninety patients with gastric cancer and fifty healthy subjects were selected from January 2014 to December 2016 in our hospital.Serum levels of ProGRP and CA72-4 were detected by electrochemiluminescence.The relationships between ProGRP and clinicopathological characteristics,postoperative recurrence and CA72-4 were analyzed.The diagnostic values of ProGRP and CA72-4 in gastric cancer were analyzed by receiver operating characteristic (ROC) curve.Results The expressions of ProGRP and CA72-4 in patients with gastric cancer were (249.3 + 28.9) pg/ml and (148.8 + 33.5) U/ml respectively,which were significantly higher than those of healthy subjects [(14.4 ± 7.6) pg/ml and (3.8 ± 1.4) U/ml],and the differences were statistically sigificant (t =56.320,P < 0.001;t =30.504,P < 0.001).The expression of ProGRP in TNM stage Ⅲ-Ⅳ [(269.1 ±30.9)pg/ml] was obviously higher than that in stage Ⅰ-Ⅱ [(198.5 +23.9)pg/ml],with a significant difference (t =11.200,P < 0.001).The expression of ProGRP in patients with lymph node metastasis [(259.9 ±31.4)pg/ml] was significantly higher than that in patients without lymph node metastasis [(190.3 ±26.8)pg/ml],with a significant difference (t =9.500,P < 0.001).The expression of ProGRP in patients with postoperative recurrence after one year [(181.3 ± 21.7)pg/ml] was higer than that in patients without postoperative recurrence [(26.1 ± 12.8)pg/ml],with a significant difference (t =31.830,P < 0.001).There was a positive correlation between serum ProGRP and CA72-4 (r =0.792,P =0.012).According to the ROC curve,the cut-off point of ProGRP was 23.6 pg/ml,and the diagnostic sensitivity was 80.0%,the specificity was 70.0%.The cut-off point of CA72-4 was 11.2 U/ml,and the diagnostic sensitivity was 60.0%,the specificity was 89.0%.The sensitivity and specificity diagnostic value of combined detection were 89.7% and 94.8%,better than those of individual detection (x2 =6.028,P =0.009;x2 =4.675,P =0.031).Conclusion ProGRP and CA72-4 are highly expressed in the serum of gastric cancer patients,with a positive correlation.The combined detection of ProGRP and CA72-4 can improve the diagnostic sensitivity and specificity.ProGRP is significantly correlated with tumor stage,lymph node metastasis and prognosis,which may be a mew target for prevention and treatment of gastric cancer.
8.Single-center experience on anti-thrombotic strategies for left ventricular assist device patients

Objective:Preliminary explore the safety and efficacy of using only vitamin K antagonists without antiplatelet therapy after left ventricular assist devices (LVAD) implantation.Methods:This is a cohort study. Patients who underwent HeartCon LVAD implantation in TEDA International Cardiovascular Hospital from September 2020 to September 2022 were included. Oral warfarin sodium was given on postoperative days 1 to 2, with the target international standardized ratio (INR) of 2.0 to 2.5. Follow-up until September 2022, survival, INR level and occurrence of bleeding and thrombosis were recorded. Survival analysis was performed by the Kaplan-Meier method (censored for heart transplantation).Results:A total of 22 patients, including 16 male patients (72.7%), aged (51.0±13.3) years, were included. The duration of HeartCon LVAD support was (458±166) days and the INR during support was 2.28±0.26. One patient underwent the heart transplant at 307 d after implantation. One patient (4.5%) occured cardiac tamponade, two patients (9.1%) occured hemorrhagic stroke, five patients (22.7%) occured gastrointestinal bleeding, four patients (18.2%) occured gingival hemorrhage, two patients (9.1%) occured epistaxis, one patient (4.5%) occurred ischemic stroke, one patient (4.5%) occured pump thrombosis, and one patient (4.5%) occured aortic valve thrombosis. The survival rates were 100%, 95%, 95%, and 95% at 3 months, 6 months, 1 year, 2 years after implantation respectively.Conclusion:The single antithrombotic strategy using warfarin (target INR 2.0-2.5) without antiplatelet for patients with implantations of HeartCon type LVAD may be safe and effective.
9.Biochemical disturbances, nutritional assessment, and interventions in patients with glycogen storage disease in a Beijing-based general hospital

Objective To investigate the biochemical disturbances,nutritional assessment and nutritional interventions in hospitalized patients with glycogen storage disease (GSD) in a general hospital in Beijing and provide examples of advanced nutritional management for these patients.Methods We retrospectively reviewed 22 consecutive hospitalized patients with GSD diagnosed from January 1,2013 to January 1,2016 in Peking Union Medical College Hospital and analyzed their medical data,nutritional assessments,and interventions.Results These 22 GSD patients included 2 cases with GSD-Ⅰa,3 cases with GSD-Ⅰb,and 17 cases with GSD-Ⅲ.They were composed of 21 children (including a 6-month old infant and a 7-month old infant) and one 18-yearold man.Biochemical abnormalities:hypoglycemia (5/5),hyperlactacidemia (4/5),hyperlipidemia (4/5),hyperuricemia (4/5) were common in GSD-Ⅰ patients,while hypoglycemia (17/17),hyperlactacidemia (5/17),hyperlipidemia (7/17),and hyperuricemia (3/17) were also present among patients with GSD-Ⅲ.Growth and development assessments:among 5 patients with GSD-Ⅰ,the body weight was below the 10th percentile in of 4 patients and the height was below the 3rd percentile in 5 patients;among 17 patients with GSD-Ⅲ,the body weight was below the 10th percentile in 4 patients and the height was below the 10th percentile in 9 patients.Nutritional management condition:all the patients received nutritional assessment,dietary modulation and nutritional support.Twenty children or adolescents were prescribed with raw corn starch (RCS) every 4-6 hours,with a mean initial dose of (1.58±0.28) g and (1.21±0.33) g for patients with GSD-Ⅰ and GSD-Ⅲ,respectively;the other 2 infant aged 6-month old and 7-month old were regularly administered amino acid-based enteral nutritional agents.In addition,all the patients with GSD-Ⅲ were recommended high protein intake of 3 g/(kg · d) and received dietary guidance and modulation.Follow-up condition:5 cases with GSD-Ⅰ and 2 cases with GSD-Ⅲ stayed in track after discharge from our hospital.The mean follow-up duration was (480.40±246.16) d and (373.00± 108.89) d for patients with GSD-Ⅰ and GSD-Ⅲ,respectively.All the patients reported alleviated symptom of hypoglycemia,and the examinations also indicated improved fasting blood glucose,uric acid,lipid profile,and growth status.Conelusions Hypoglycemia,metabolic disturbances,and growth retardation are prevalent in both GSD-Ⅰ and GSD-Ⅲ patients.Potential biochemical abnormalities should not be ignored in GSD-Ⅲ patients,especially in young patients.Comprehensive nutrition assessment,regular administration of RCS,sufficient protein intake,and continuous patient follow-up and surveillance can help to alleviate symptoms,correct metabolic disturbances,and improve growth status of patients with GSD.
10.Delayed series casting after botulinum toxin injection is more effective for equinus children with spastic cerebral palsy

Objective:To compare the efficacy of series casting with different timings after botulinum toxin injection among equinus children with spastic cerebral palsy.Methods:Sixty-three equinus children with spastic cerebral palsy were randomly divided into a control group and an experimental group. After being injected with botulinum toxin type A, 4 weeks of series casting was applied immediately in the control group, while in the experimental group it was applied 2 weeks later. Both groups were given basic rehabilitation therapy including heel walking, kicking balls, single-leg standing, standing platform training and balance training. The modified Tardieu scale, three-dimensional gait analysis, gross motor function measure 88 (GMFM-88) and a self-made questionnaire were used to evaluate muscle tone, spasticity, gait parameters, standing and walking before the treatment and 2 and 6 months afterward.Results:Immediately after the treatment there was a significant improvement in both groups′ average MTS scores (R1, R2, R1-R2 with knee extension and flexion), dorsiflexion at initial contact, peak dorsiflexion in the stance and swing phases, and dimension D (standing), and also in their average GMFM-88 (walking) scores. Two and six months later the experimental group′s average MTS scores, peak dorsiflexion in the stance phase, and dimension D (standing) score in the GMFM-88 (walking) had improved significantly compared with the control group′s averages. However, no significant differences were observed between the two groups in any of the other measurements after the treatment.Conclusion:Delaying series casting after botulinum toxin A injection can significantly reduce tone and spasticity in the plantarflexors, improve the range of motion and functioning of the ankle, and alleviate the discomfort and pain caused by series casting.
