1.Clinical study on 67 cases with uterine rupture

Objective To investigate the incidence,etiology,diagnosis,treatment and outcome of uterus rupture.Methods From January 1999 to May 2013,clinical data of 67 cases with uterine rupture in Woman's Hospital,School of Medicine,Zhejiang University were studied retrospectively.Results A total of 67 cases of uterine rupture with 21 +2--39 +2 gestational weeks out of 128 599 deliveries were recorded giving an incidence of uterine rupture was 0.052 1 % (67/128 599).Cesarean scar rupture were found in 59 cases (88%,59/67) and noncesarean scar rupture were found in 8 cases (12%,8/67).The causes of uterine rupture include 60 cases of scar uterus (59 cesarean scar cases and 1 myomyectomy scar case),2 cases of assisted delivery operation trauma,2 cases of malformed uterus,3 cases of unknown causes (all with artificial abortion history).Non obstructive dystocia and improper oxytocin use were found to be related with uterine rupture.Fifty-two cases of cesarean scar spontaneous incomplete rupture were found and repaired during repeated cesarean delivery without maternal and fetal complications.The remaining 15 cases need emergency rescue operation for fetal distress or dead fetus,severe acute abdomen,prepartum or postpartum vaginal bleeding even maternal hypovolemia; 6/15 uterine rupture cases were diagnosed with the history,clinical symptoms and signs,3/15 cases with ultrasonic found dead fetus in the peritoneal cavity before exploratory laparotomy and 6 cases were diagnosed just during laparotomy.Hysterectomy was done in 10/15 cases and uterine repair in 5/15 cases; there was no maternal death and 12 perinatal fetal death (5 cases of mid-late pregnancy termination for deformed fetus) of the 15 uterine rupture cases.One case with hysterectomy was complicated with stress pancreatitis and dysfunction of liver and kidney and discharged 20 days after operation,the remaining 14 cases were discharged 5-7 days postpartum.One case with repaired malformed uterus got pregnancy 4 years later and delivered a 2 000 g healthy baby by cesarean section at gestational age of 33 +4 weeks.Conclusion Uterine scar caused by caesarean section or other operations became the leading cause of uterine rupture,assisted delivery operations,history of intrauterine manipulation and uterine malformations were the predisposing risk factors of uterine rupture.

Objective To prospectively evaluate changes in serum vascular endothelial growth factor (VEGF) levels in patients with hepatocellular carcinoma (HCC) before and after transcatheter arterial chemoembolization (TACE), and to investigate the association between serum VEGF levels and characters of imaging features. 3.Experimental study on the mechanism of GuiJi cream in dispelling effects on corrupt microorganism

Objective To study the mechanism of GuiJi cream in dispelling effects on corrupt microorganism.Methods A total of 90 full-layer skin injured rats were served as test model. The effects of Guiji cream on collagen type Ⅰ (C Ⅰ), matrix metalloproteinase-1 (MMP-1) and MMP-3 in granulation tissue of wound were observed with immunohistochemistry technique and image-analysis system at various stages of healing. Results The expression of C Ⅰ was obviously increased and the level of MMP-1 and MMP-3 increased at the early stage and kept at a high level from day 3 to day 10. Conclusion The mechanism of GuiJi cream for dispelling effects on corrupt microorganism was increasing the C Ⅰ content in granulation tissue of wound through inhibiting the secretion of MMP- 1.
4.Determination of niclosamide in water with extraction photometry

Objective To study the method and conditions of determination of niclosamide in water. Methods Niclosamide was extracted by mixed extractent, then striped by NaOH. Niclosamide in NaOH solution was determined with dual-wavelength photometry. Results The recovery of niclosamide was 93%. The linear ranger was 0-4. 0 g/m3 and the detectable limit for niclosamide was 0. 042 7 g/m3. The apparent molar absorptivity was 1. 26 ?105 L/(mol?cm). Conclusion Extraction photometry is suitable for determination of niclosamide in water in the field.
5.Use of Antidiabetics in Our Hospital During 2002 and 2005

OBJECTIVE:To investigate the use and its developing trend of antidiabetics in our hospital.METHODS:A statistical analysis was conducted on the consumption sum,DDDs of antidiabetics used in our hospital from 2002 to 2006.RESULTS:Over the 5 years,the average annual incremental rates of oral antidiabetics in consumption sum,DDDs were 38.94% and 22.56%,respectively,with the kinds from joint venture and home showing the highest increasing range in consumption sum and DDDs,respectively.CONCLUSION:Antidiabetics used in our hospital has confirmed efficacy,and the kinds from home assume a predominant place,therefore,research and development for domestic drugs should be strengthened.

Objective To investigate the changes of bone mineral density (BMD) and its causing factors in menopausal female patients with type Ⅱ diabetes mellitus (DM2). Methods Dual energy X ray absorptimetry was used to measure the BMD of lumbar spines (L 2-4 ) and proximal femur in 82 female patients with type Ⅱ diabetes and 46 normal controls. According to BMD, DM2 patients were divided into two groups (DM A and DM B). Fasting plasma glucose (FPG), fasting plasma insulin (FINS), fasting plasma C peptide (FCP), glycated hemoglobin (HbAc1), urine albumin excretion rate (UAER), and urine ? 2 microglobin (? 2 MG) were measured and compared between the two groups. Objective To observe the incidence of critical illness-related corticosteroid insufficiency (CIRCI) after severe traumatic brain injury (sTBI) and investigate the relationship between CIRCI and prognosis.Methods This prospective cohort study enrolled 89 sTBI patients (68 males and 21 females;at age range of 15-80 years) hospitalized within 24 hours after sTBI from June 2014 to December 2015.The Glasgow coma scale (GCS) was ≤8 points.The causes of injury included extensive contusion of brain (44 cases),subdural hematoma (21 cases),epidural hematoma (11 cases),primary brain stem injury (8 cases) and diffuse axonal injury (5 cases).Adrenocorticotropic hormone (ACTH) stimulation tests were done within 36 hours after sTBI to identify CIRCI patients.The patients were divided into CIRCI group (50 cases) and non-CIRCI group (39 cases).Moreover,the patients were categorized into survival group (62 cases) and death group (27 cases) based on survival status.The GCS score,mechanical ventilation time,cerebral hernia,survival time and mortality within 28 days were observed in two groups.Results The incidence of CIRCI in sTBI patients was as high as 56% (50/89).Compared with the non-CIRCI group,the CIRCI group had lower GCS [(5.3 ± 1.7) points vs.(6.1 ± 1.4) points,P < 0.05],and sTBI patients with CIRCI were mechanically ventilated for a longer period of time [(9.9 ± 2.8) days vs.(7.5 ± 1.6) days,P < 0.05].In comparison with non-CIRCI patients,the incidence of brain herniation in sTBI patients with CIRCI was higher (58% vs.21%,P <0.01).The total fatality rate within 28 days was 30% (27/89).The survival time of CIRCI group was significantly shorter than that of non-CIRCI group (P < 0.05).The fatality rate in the CIRCI group was significantly higher than that of the non-CIRCI group [40% (20/50) vs.18% (7/39),P <0.05].The incidence of CIRCI in death group was significantly higher than that of the survival group [74% (20/27) vs.48% (30/62),P < 0.05].Conclusions The incidence of CIRCI in STBI patients is high.The sTBI patients with CIRCI has significantly higher incidence of brain hernia,longer mechanical ventilation time,higher 28-day mortality and shorter survival time compared with non-CIRCI patients.
8.Construction of a Novel Eukaryotic Expression Plasmid pcDNA6/myc-his-EGFP Band Its Applications in Expression of Recombinant Genes

Enhanced green fluorescent protein( EGFP), myc epitope and polyhistidine metal-binding tag are often used as a marker for recombinant fusion protein in many gene expression vectors, each marker has its own function, EGFP emits green fluorescence for direct detection, myc epitope facilitates recombinant fusion protein detection using its antibodies, polyhistidine tag allows purification of recombinant fusion protein using resin.Hitherto, no a plasmid vector can integrate all of these functions. In this study we constructed a novel eukaryotic expressive plasmid, designated as pcDNA6/myc-his-EGFP B, which integrated the functions of EGFP, myc epitope and polyhistidine tag. Importantly, a linker octo - peptide in N terminal of EGFP was designed using LINKER program. A DNA fragment encoding a putative protein containing a signal peptide of human interleukin 2(IL-2) in N terminal was cloned into pcDNA6/myc-his-EGFP B in frame with the C-terminal peptide to construct pMHES. 2.2.15 Cells were transfected with pcDNA6/myc-his-EGFP B and pMHES, and Balb/c mice were intravenously injected with pcDNA6/myc-his-EGFP B by tails, results revealed that both of the plasmids worked in 2.2.15 Cells and livers of Balb/c mice. Assuming gene of the IL-2 was inserted into pcDNA6/myc-his -EGFP B in frame with EGFP, myc and 6 × His, three-dimensional structure for this putative expression product was simulated using Modeller8V2, results revealed that IL-2, EGFP, myc and 6 × his did not interfere each other and octo- peptide linker owned certain flexibility. The results suggest that pcDNA6/myc-his-EGFP B may be useful as a genetic tool for mammalian cells and a vector for gene therapy.
9.Effects of lentiviral-delivered CDC25B2 siRNA on biological behaviors of human pancreatic cancer cell line CFPAC-1

Objective To establish CFPAC-1 cell lines deficient in CDC25B2 by recombinant lentivirus, and to investigate the role of this gene. Methods After CFPAC-1 cells were transduced with recombinant lentivirus producing CDC25B2 siRNA, stably transduced cells with green fluorescent protein were selected by flow cytometer. The mRNA and protein expression of CDC25B2 was examined by RT-PCR and Western blot analysis. The effect of the lentivirus on the cell proliferation, cell cycle, clone-forming, migration and invasion ability was analyzed by MTr method, flow cytometer, plate clone-forming assay and Transwell chamber method respectively. Results CDC25B2 siRNA knocked down CDC25B2 expression in CFPAC-1 cells significantly. The silencing efficiency of siRNA transduction by recombinant lentivirns was very high. Proliferation, cloneforming, migration and invasion ability of human pancreatic cancer cell line CFPAC-I were significantly in-creased, while cell cycle was not affected. Conclusion CDC25 B2 plays an important role in cell proliferation, clone-forming, migration and invasion of pancreatic cancer. This research provides experimental evidences for targeting CDC25B2 in gene therapy against pancreatic cancer.
10.Hepatolithiasis concurrent with intrahepatic cholangiocarcinoma

Objective To investigate the clinical features and management of hepatolithiasis associated with intrahepatic cholangiocarcinoma. Methods Data of 84 patients of hepatolithiasis associated with intrahepatic cholangiocarcinoma in our hospital from 1990 to 2009 were retrospectively analyzed.Results The incidence of intrahepatic cholangiocarcinoma in patients of hepatolithiasis was 4. 6%(84/1840), among them only 47 patients got a definite diagnosis before operation. All cancer located in the bile duct containing cholelith. In 20 patients intrahepatic cholangiocarcinoma was identified 6 - 16 years after lithotomy. The clinical manifestation of hepatolithiasis associated intrahepatic cholangiocarcinoma included:refractory hepatic abscess, incurable infection of intrahepatic biliary tract, and progressive obstructive jaundice. Only 35 patients received radical excision, 26 patients received palliative excision, 4 patients received radiofrequency ablation therapy, 19 patients received biopsy only. Conclusions There has been a considerable high coincidence between intrahepatic cholangiocarcinoma and hepatolithiasis. Resection of the lobe containing intrahepatic stones may help to prevent the development of intrahepatic cholangiocarcinoma.
