1.Analysis of related risk factors of neurological deterioration in patients with acute cerebral infarction

Objective Toinvestigatetherelatedriskfactorsofneurologicaldeterioration(ND)in patientswithacutecerebralinfarction.Methods Atotalof446patientswithacutecerebral infarction admitted to the Department of Neurology,Zhongda Hospital,Southeast University from January 2012 to December 2013 were analyzed retrospectively. ND was defined as the reevaluation of the National Institutes of Health Stroke Scale (NIHSS)scores at any time for the increased admission baseline score 2 (ND2)or 4 (ND4)within the first 72 hours. All subjects were divided into a ND2 group (n=107)and a non-ND2 group (n=339)or a ND4 group (n=62)and a non-ND4 group (n=384 ). The differences of general demography,vascular risk factors,imaging,and hematological dataamongthedifferentgroupswerecompared.Results Ofthe446patients,107cases(24.0%) were diagnosed as ND2 and 62 cases (13. 9%)were diagnosed as ND4. The result of univariate analysis showed that there were significant differences in the length of hospital stay,age,baseline NIHSS score,baseline systolic blood pressure on admission,guilty artery occlusion,and the levels of leukocyte,fasting glucose,and C-reactive protein between the ND2 patients and the non-ND2 patients (all P<0. 05). There were significant differences in sex,age,atrial fibrillation,baseline NIHSS score, baseline systolic blood pressure on admission,guilty artery occlusion,and the level of C-reactive protein between the ND4 patients and the non-ND4 patients (all P<0. 05). After adjustment for the confounding factors,the results of Logistic regression analysis showed that the baseline NIHSS score (OR,1.114, 95%CI 1. 0481-1.185,P=0. 001),C-reactive protein (OR,1. 014,95%CI 1. 004-1. 024,P=0. 004), and guilty artery occlusion (OR,2. 303,95%CI 1. 152-4. 606,P=0. 018)were independently correlated with ND2;while the age (OR,1. 040,95%CI 1. 011-1. 070,P=0. 006),systolic blood pressure (OR, 1.015,95%CI 1. 003-1. 027,P=0. 018),C-reactive protein (OR,1. 016,95%CI 1. 005-1. 026,P=0.003),and guilty artery occlusion (OR,2. 845,95%CI 1. 291-2. 269,P =0. 009)were independently correlatedwithND4.Conclusion TheearlyonsetofNDinpatientswithacutecerebralinfarctionare closely associated with age,stroke severity,baseline systolic blood pressure,C-reactive protein,and occlusion of guilty artery. In the clinical diagnosis and treatment,detecting the above indicators timely may contribute to identify the patients with acute cerebral infarction and early progressive deterioration.

3.Large arterial stenosis is associated with early neurological deterioration of middle cerebral artery infarction

Objective To investigate the correlation of large artery stenosis and early neurological deterioration (END) of middle cerebral artery infarction.Methods The patients with middle cerebral artery infarction were enrolled retrospectively.END was defined as that the National Institutes of Health Stroke Scale (NIHSS) scores increased ≥2 within 72 hours of admission compared with the baseline value.Middle cerebral artery and internal carotid artery were evaluated by using cerebral angiography.The degree of stenosis was divided into 4 grades:non-or mild (< 50%),moderate (50-70%),severe (71-99%) stenosis,and occlusion (100%).The clinical risk factors,degree of neurological deficits,size of lesion,and major arterial lesions were compared between the END group and the non-END group.Results A total of 256 patients with middle cerebral artery infarction were enrolled,and END occurred in 70 of them (27.34%).The age (P =0.045),infarct volume (P =0.045),baseline NIHSS score (P=0.007),and major arterial stenosis (P =0.038) of the END positive group were significantly higher or lager than those of the END negative group.Multivariate logistic regression analysis showed that the higher baseline NIHSS score (odds ratio [OR] 1.071,95% confidence interval [CI] 1.004-1.142; P =0.037),infarction diameter > 20 mm (OR 2.077,95% CI 1.077-3.736; P =0.028),and severe stenosis of the major artery (OR 2.521,95% CI 1.079-5.886; P=0.033) or occlusion (OR 3.074,95% CI1.262-7.489; P=0.013) were the independent predictor of END.Conclusions Severe stenosis or occlusion of the major artery may be an independent predictor of END in patients with middle cerebral artery infarction.
4.Experimental study of acute hemorrhagic necrotizing pancreatitis complicated with acute lung injury

Objective To study the pathological changes of acute hemorrhagic necrotizing pancreatitis(AHNP) complicated with acute lung injury(ALI). Method The model of AHNP with ALI was established in rats. The changes of function and structure of pancreas and lung were observed. Results One hour after induction of the model , pancreas showed mild edema and congestion . 12 hours after introduction of the model , the typical pathological changs of AHNP were found . The lung extravascular water volume and levels of PaCO 2 increased significantly, and the PaCO 2 decreased obviously. Morphological examination demonstrated that inflammatory cell , insterstitial edema , intra-alveolar hemorrhage ,desquamate and disintegration occurred in the lungs. Conclusions The pancreatic and pulmonary morphologic changes in this model is similar to the changes in clinical AHNP with ALI , which suggests that the model can be used to study the mechanism of AHNP with ALI and evaluate the effect of drugs for AHNP with ALI.
5.Changes of CD64 expression in succus in patients with chronic bacterial prostatitis

Objective To observe the levels of neutrophil CD64 expression in prostatic fluid of patients with chronic prostatitis and different degrees of infection,to evaluate the value of CD64 in diagnosis and treatment of chronic bacterial prostatitis.Methods 100 subjects were divided into two groups according to chronic prostatitis symptom index (CPSI) established by the National Institutes of Health (NIH):CPSI high score group and CPSI low group,bacterial infection group,and 30 healthy controls.CD64 expression of neutrophils in prostatic fluid was detected by flow cytometry and prostate fluid routine microscopy inspection.Results The expression of CD64 in severe group (high score) was (4857.25±278.45) molecules / cell,which was significantly higher than that in the mild group (P<0.05).There was significant difference between the mild group and the nonbacterial infection group or the healthy control group (P<0.01).There was no significant difference between nonbacterial infection group and healthy control group.The expression of CD64 was positively correlated with some white blood cells in the bacterial infection group,but not the whole.ROC curve was drawn according to the data of the subjects.When CD64 was 2135.2 molecules/cell,the sensitivity was 92.9% and the specificity was 91.7%.Conclusion Quantitative detection of CD64 in prostatic fluid can be used as a sensitive index for diagnosis of chronic bacterial prostatitis and has clinical value.

Objective:To research the clinical effect of HeDanPian combined with edaravone in the treatment of acute cerebral in farction and the influence on serum levels of interferon-γ (IFN-γ),promote angiogenin Ⅱ (Ang-2),homocysteine (Hcy).Methods:102 cases of patients with acute cerebral infarction were selected and randomly divided into the control group and the experimental group,with 51 cases in each group.The control group was treated with edaravone,intravenous infusion of30mg edaravone,1 time in the morning and night;The experimental group was treated based on the control group treated with HeDanPian,oral HeDanPian 14.6 g,1 time in the early to middle.The curative effect,nerve function defect score (NIHSS),serum IFN-γ Ang-2,Hcy,propylene glycol (MDA),superoxide disproportionation alcohol (SOD),interleukin 6,10 (IL-6,IL-10),tumor necrosis factor-α(TNF-α) levels,incidence of adverse reactions were compared between two groups.Results:After treatment,the total effective rate,serum Ang-2,SOD,IL-10 levels of experimental group were higher than those of the control group (P<0.05),the NIHSS,serum IFN-γ,Hcy,MDA,IL-6,TNF-α levels of experimental group were lower than those of the control group (P<0.05).No significant difference was found in the incidence of adverse reactions was found between the two groups (P>0.05).Conclusion:HeDanPian combined with edaravone was effective in the treatment of acute cerebral infarction,which could regulate the expression of serum IFN-γ,Ang-2 and Hcy,improve the oxidative stress and inflammation.
7.Effects of lentiviral-delivered CDC25B2 siRNA on biological behaviors of human pancreatic cancer cell line CFPAC-1

Objective To establish CFPAC-1 cell lines deficient in CDC25B2 by recombinant lentivirus, and to investigate the role of this gene. Methods After CFPAC-1 cells were transduced with recombinant lentivirus producing CDC25B2 siRNA, stably transduced cells with green fluorescent protein were selected by flow cytometer. The mRNA and protein expression of CDC25B2 was examined by RT-PCR and Western blot analysis. The effect of the lentivirus on the cell proliferation, cell cycle, clone-forming, migration and invasion ability was analyzed by MTr method, flow cytometer, plate clone-forming assay and Transwell chamber method respectively. Results CDC25B2 siRNA knocked down CDC25B2 expression in CFPAC-1 cells significantly. The silencing efficiency of siRNA transduction by recombinant lentivirns was very high. Proliferation, cloneforming, migration and invasion ability of human pancreatic cancer cell line CFPAC-I were significantly in-creased, while cell cycle was not affected. Conclusion CDC25 B2 plays an important role in cell proliferation, clone-forming, migration and invasion of pancreatic cancer. This research provides experimental evidences for targeting CDC25B2 in gene therapy against pancreatic cancer.
8.An Electrochemiluminescence Method for Determination of Manganese (Ⅱ)

In the presence of silver ion, Mn2+ could be electro-oxidized to potassium hypermanganate in phosphoric acid solution, which could effectively react with pyrocatechol in acid solution and luminol in sodium hydroxide solution to produce chemiluminescence. On the basis of this, a novel indirect approach for the detection of Mn2+ was established. The effect of silver ions on the electrochemical oxidation of Mn2+was studied. when 1. 5 ×10-5 mol/L Ag+ and 0. 01 mol/L phosphoric acid solution were used in the process of electrochemical oxidation, the CL intensity could be up to the maximum value after the above solution was electrolyzed for 2 min. The relation of CL intensity and Mn2+concentration in the solutions at different pH and the selectivity were also investigated. when the pyrocatechol was used as luminescent reagent in the acidic medium, the CL intensity was linearly to the Mn2+concentration in the range of 1. 82×10-7-7. 27×10-5 mol/L with excellent selectivity. Common ions had little interferences in the determination of Mn2+. The method was successfully applied to the determination of Mn2+ in surface water and drinking water with satisfactory results.
9.Mutations in hepatitis B virus genome involved in immunoprophylaxis failure against vertical transmission

Objective To determine the factors responsible for failed postnatal immunoprophylaxis for hepatitis B virus(HBV) in Qidong, China. Methods Eleven children who developed into chronic HBV infection after receiving HBIG and HBV recombinant vaccines were recruited into the study. Eleven paired mothers with chronic hepatitis and other 6 mothers whose children successfully generated anti-HBs after im-munoprophylaxis were included as the control in the study. Full-length HBV DNA was amplified through ser-um sample by PCR method and underwent cloning and sequencing. HBV DNA level was quantified by real-time PCR. Results The mean levels of HBV DNA in mothers who had HBV DNA positive children and healthy children were ( 1.2 ×107± 3.1 × 106 ) copies/ml and ( 1.6× 107±8.8×106 ) copies/ml, respec-tively. There was no significant difference between the groups (P >0.05). Meanwhile, viral load in chil-dren was unrelated to that in their mothers (r2 =0.2429). In 11 HBV DNA positive children, 4(36.4% ) demonstrated amino acid substitutions in HBsAg "a" determinant region with 6 different types, I.e. T125A, I126T, Q129H, M133V, D144V and G145A. All of the mothers showed the wild-type sequence in "a" epitope, indicating surface escape mutants were not acquired from the initial infection, but developed under the immune pressure. The mutation rates after immunoprophylaxis for preS1, preS2, S, X, preC/C and P genes were 0.38%, 0. 22%, 0.27%, 0.17%, 0.11%, and 0.11%, respectively, nt2999-3157 in preS1, nt529-677 in S, nt1955-2016 in C, nt923-1001 and nt2489-2602 in P genes were among the hottest muta-tional spots throughout the HBV genome. Conclusion HBV mutation may occur in all the open readingframes after passive and active immunoprophylaxis. In addition to S gene, HBV preS and P genes could alsoassociate with the escape mutants.
10.Therapeutic effects of Thesium chinensis extracts on IgAN rats

Objective To evaluate the therapeutic effects of Thesium chinensis Turcz extracts on IgA nephropathy (IgAN) rats .Methods The hot water extracted solution of Thesium chinensis Turcz flow through the AB‐8 macroreticular res‐in column repeatedly ,then being eluted with different contents of ethanol to get four part of extract .The IgAN model of SD rats were established by oral intake of bovine serum albumin(BSA ,400 mg/kg) together with injection of lipopolysaccharide(LPS) and CCl4 .Beginning with 8th week ,the positive group(LGT ,10 mg/kg) ,water extract group(TT ,1 .0 g/kg) ,water soluble group(TTW ,0 .4 g/kg) ,high polarity group(TT20 ,0 .4 g/kg) and medium polarity group(TT50 ,0 .4 g/kg) rats were intragas‐tric administrated drug daily ,the normal group control and model control group rats were given with saline(n=8) .During the next 5 weeks treatment process ,the 24 h urine volume ,24 h urine protein concentration ,and quantity of erythrocyte in urine were observed .The levels of TP ,ALP ,ALT ,AST ,Scr and BUN in serum were determined .The expression of IgA deposition was measured by immunofluorescence staining ,and the pathological changes of kidney tissue were assayed after the therapy process .Results After 5 weeks therapy ,the 24 h urine protein concentration ,the quantity of erythrocyte in urine ,and the level of Scr and BUN of TT and TT50 groups were significantly lower than model control .The fluorescence intensities in the me‐sangial area were weaker than model control .Also ,the degrees of renal capsule expansion and mesangial matrix proliferations , and the kidney tubules damage of TT and TT50 groups were weaker than those of model control .Conclusion The water extract and its medium polarity fraction of Thesium chinensis Turcz show good therapeutic effects on IgAN rats .
