1.Using machine learning to construct the diagnosis model of female bladder outlet obstruction based on urodynamic study data

Purpose:
To intelligently diagnose whether there is bladder outlet obstruction (BOO) in female with decent detrusor contraction ability by focusing on urodynamic study (UDS) data.
Materials and Methods:
We retrospectively reviewed the UDS data of female patients during urination. Eleven easily accessible urinary flow indicators were calculated according to the UDS data of each patient during voiding period. Eight diagnosis models based on back propagation neural network with different input feature combination were constructed by analyzing the correlations between indicators and lower urinary tract dysfunction labels. Subsequently, the stability of diagnostic models was evaluated by five-fold cross-validation based on training data, while the performance was compared on test dataset.
Results:
UDS data from 134 female patients with a median age of 51 years (range, 27–78 years) were selected for our study.Among them, 66 patients suffered BOO and the remaining were normal. Applying the 5-fold cross-validation method, the model with the best performance achieved an area under the receiver operating characteristic curve (AUC) value of 0.949±0.060 using 9 UDS input features. The accuracy, sensitivity, and specificity for BOO diagnosis model in the testing process are 94.4%, 100%, and 89.3%, respectively.
Conclusions
The 9 significant indicators in UDS were employed to construct a diagnostic model of female BOO based on machine learning algorithm, which performs preferable classification accuracy and stability.
2.Laparoscopic simple prostatectomy for large volume benign prostate hyperplasia: a report of 30 cases

【Objective】 To investigate the feasibility of laparoscopic simple prostatectomy (LSP) in the treatment of large volume benign prostate hyperplasia (BPH). 【Methods】 Clinical and follow-up data of 30 patients with large volume BPH treated with LSP in our hospital during Feb.2019 and Dec.2021 were retrospectively analyzed. All patients underwent extraperitoneal LSP operation. The perioperative and 1-12 month postoperative follow-up data were analyzed. 【Results】 The average prostate volume was (92.4±38.9) mL, operation time (125±45) min, and weight of resected prostate (60.25±16.90) g. The hemoglobin decreased by (12.21±7.25) g/d after operation. No blood transfusion was needed. There was no need for bladder irrigation after operation in 21 cases (70%), and 9 cases (30%) had bladder irrigation time of (0.95±0.49) d. The postoperative catheter indwelling time was (6.92±2.51) d, and hospital stay (5.36±1.63) d. During the follow-up of (9.25±5.4) months, there was 1 case of postoperative intestinal obstruction (Clavien-Dindo grade II), 1 case of transient urinary incontinence (Clavien-Dindo grade I), and 1 case of delayed hematuria (Clavien-Dindo grade I). No urethral stricture occurred. The maximum urinary flow rate (Qmax), post-void residual urine volume (PVR), International Prostate Symptom Score (IPSS) and quality of life (QoL) 3 months after operation were significantly improved compared with those before operation (P<0.05). There was no significant difference in sexual function before and after operation (P>0.05). 【Conclusion】 LSP is safe and effective in the treatment of large volume BPH. It has advantages of complete resection of glands, minor bleeding and short postoperative bladder irrigation time. However, it still needs to be confirmed by a prospective control study of large samples.
3.Research progress of mechanical power in ventilator-induced lung injury

ICU acute respiratory distress syndrome has a high morbidity and mortality rate, and these patients usually need mechanical ventilation to maintain their respiratory function during treatment. However, improper setting of mechanical ventilation parameters may lead to ventilator-induced lung injury (VILI). In order to effectively prevent the occurrence of VILI, ARDSnet recommends the use of a protective ventilation strategy with low tidal volume and limited airway plateau pressure. However, from the perspective of ventilator energy transfer, VILI is actually the result of a combination of respiratory parameters such as tidal volume, airway pressure, and respiratory rate. The mechanical power well reflects the combined effect of the above parameters and is increasingly becoming a hot topic in clinical research. In this review paper, the definitions of mechanical energy and mechanical power were introduced, and the calculation methods of mechanical power under different respiratory modes are summarized. Moreover, the clinical studies related to mechanical power and VILI and further exploration of the safety threshold of mechanical power are reviewed. It is expected to provide new ideas for the future clinical development of personalized mechanical ventilation strategies and the effective prevention of VILI.
4.Clinical observation of warm acupuncture combined with external application of Tibetan medicine Baimai Ointment in the treatment of low-back pain with cold-dampness type

Objective:To evaluate the clinical efficacy of warm acupuncture combined with external application of Tibetan medicine Baimai Ointment in the treatment of low-back pain with cold-dampness type.Methods:Randomized controlled trial. Totally 60 outpatients in Tibetan Medicine Hospital of Cuona County from May to July of 2021 were selected as the observation objects, and they were divided into two groups by random number table method, with 30 cases in each group. The control group was treated with Baimai Ointment, and the treatment group was treated with warm acupuncture and Baimai Ointment. Both groups were treated for 2 weeks and followed up for 3 months. VAS scale and Oswestry disability index (ODI) were used to evaluate the low-back pain and dysfunction, and the clinical efficacy was evaluated.Results:The VAS scores of the treatment group were lower than those in the control group immediately after treatment and at the last follow-up ( t=-18.17, -6.05, P<0.01). The ODI score of the treatment group was lower than that of the control group at the last follow-up ( t=-15.86, P<0.01). The total effective rate was 96.7% (29/30) in the treatment group and 93.3% (28/30) in the control group, without statistical significance ( χ2=0.001, P=1.000). Conclusion:Warm acupuncture combined with Tibetan medicine Baimai Ointment can effectively improve the clinical symptoms of low-back pain with cold-dampness type, improve the quality of life of patients, and the clinical effect is satisfactory.
5.The pathological role of hypersplenism in the course of liver cirrhosis

Hypersplenism is an important complication of cirrhotic portal hypertension, and splenectomy is an important means to treat hypersplenism in cirrhosis. It is realized that hypersplenism played a pathological role in the course of cirrhosis. This article analyzes and compares the changes in the condition of patients with cirrhosis between splenectomy with and without hyperfunction, and comprehensively discusses the pathological role and mechanism of hypersplenism in the course of cirrhosis, in order to strengthen the clinical prevention and treatment of hypersplenism in cirrhosis and to better improve the condition and prognosis of patients with cirrhosis.
6.Design of SF 6 measurement system based on Visual TFT

Objective:To design a Visual TFT-based SF 6 gas measurement system. Methods:Based on Visual TFT, an SF 6 gas measurement system is designed, including system hardware design and system software design. The system hardware mainly includes gas sensors, a Visual TFT display, a signal processing circuit, and a power supply circuit. The system software mainly includes data acquisition, data display, SF 6 monitoring system, and alarm program. The Visual TFT display is controlled by Keil code to display other parameters such as gas flow rate, concentration, and waveform, and calculate the error (? E). The measurement system extracts the gas flow rate and concentration data through the waveform graph and gets the change curve of functional residual gas volume (FRC) data. Results:The proposed Visual TFT-based SF 6 gas measurement system can detect SF 6 with an arithmetic mean value of concentration ? E of 8.906%, which is less than 10% of full scale and meets the design requirements. Conclusions:The Visual TFT-based SF 6 gas measurement system can monitor the concentration of SF 6 gas in real-time, which helps to control the release of SF 6 gas and reduce the impact of inert gas emission on the human body under test. At the same time, the system has an intuitive display interface and alarm function, which helps healthcare workers diagnose and treat lung and respiratory diseases more conveniently and quickly, and improves the efficiency and safety of healthcare workers.
7.Clinical Significance and Method of Measuring Lung Fluid Viscosity and Surface Tension

Pulmonary fluid refers to a thin and continuous liquid layer mainly consisting of airway mucus and lining liquid on the inner surface of alveoli, which is essential for maintaining a healthy lung. The clinical significance of rheological properties of pulmonary fluid in airway stability, pulmonary barrier and clearance functions, ventilator induced lung injury and surfactant replacement therapy in infant respiratory distress syndrome was introduced in this paper. Furthermore, a variety of classical methods for measuring surface tension and liquid viscosity, such as Langmuir-Wilhelmy balance, captive bubble method, glass capillary viscometer and rotational viscometer, and emerging techniques (e.g. particle tracking microrheometer and axisymmetric drop shape analysis) were reviewed, and their advantages and drawbacks were also compared, in order to provide an important reference for the assisted diagnosis and treatment of lung diseases in clinical practice.
8.Correlation analysis between the chemical contents and bioactivity for the quality control of Alismatis Rhizoma.

In order to clarify regions of production and to discriminate processing methods, quantitative and qualitative analyses for saccharides and terpenes in 35 batches of Alismatis Rhizoma were performed. Methodologies included HPLC-PDA, HPLC-VWD and UHPLC-MS , combined with principal component analysis (PCA) and partial least squares regression techniques (PLSR). The inhibitory effects of triterpenes and Alismatis Rhizoma extracts on lipase activity were evaluated . PLSR analysis revealed significant positive correlations ( = 0.5795) between the contents of triterpenes , , , and and the inhibitory effects of Alismatis Rhizoma. The present study establishes an effective method for simultaneous determination of multiple components, and identifies key bioactive triterpenes. These results can be used for systematic and novel analytical strategies for the quality control of Alismatis Rhizoma production.
9.A cross-sectional survey of receiving no methadone maintenance treatment in HIV infected injecting drug users in Dehong Dai and Jingpo autonomous prefecture, Yunnan province

Objective To understand the current status of receiving no methadone maintenance treatment (MMT) and influencing factors in HIV infected injecting drug users (IDUs) in Dehong Dai and Jingpo autonomous prefectures,Yunnan province.Methods Data of survival of IDUs with AIDS in Dehong were collected from "Chinese National Comprehensive HIV/AIDS and Care Information System" in December,2014.Results There were 987 IDUs who should receive MMT,the majority of them were males (94.6%,934/987),aged 35-44 years (53.0%,523/987) and farmers (77.2%,762/987).Among the 987 IDUs,60.2% (592/987) received no MMT.Multivariate logistic regression analysis showed that being female (OR=2.66,95%CI:1.21-5.87),in Jingpo ethnic group (OR=3.05,95%CI:1.97-4.71) were the major risk factors for receiving no MMT;not being farmers (OR=0.46,95%CI:0.31-0.70),in Dai ethnic group (OR=0.53,95%CI:0.36-0.79),diagnosed HIV infection history ≥ 10 years (OR=0.60,95%CI:0.45-0.81) were the major protective factors for receiving no MMT.The reasons for receiving no MMT included long distance journey (289,48.8%),fear of exposure (124,20.9%),poor daily medication compliance (59,10.0%),fear of side effects (47,7.9%),others (73,12.3%).Conclusions The proportion of receiving no MMT in IDUs with AIDS in Dehong was high.Being female and farmer,in Jingpo ethnic group,low educational level,short diagnosed HIV infection history were influencing factors for receiving no MMT.The effective intervention measures should be taken to further improve MMT coverage according to the different characteristics of the patients.
10.Therapeutic effect of rosuvastatin combined clopidogrel bisulfate on patients with coronary heart disease

Objective:To explore therapeutic effect of rosuvastatin combined clopidogrel bisulfate tablet on patients with coronary heart disease (CHD).Methods: A total of 124 patients, who were diagnosed as CHD by coronary angiography, were selected, randomly and equally divided into routine treatment group (received routine treatment,including clopidogrel bisulfate tablet) and combined treatment group (received rosuvastatin based on routine treatment group), both groups were treated for six months.Cardiac function, serum levels of angiotensin II (Ang II), vascular endothelial growth factor (VEGF) and nitric oxide (NO), and therapeutic effect were measured and compared between two groups before and after treatment.Results: Total effective rate of combined treatment group was significantly higher than that of routine treatment group (96.77% vs.83.87%, P=0.015).Compared with routine treatment group after treatment, there were significant rise in left ventricular ejection fraction [(61.89±7.02)% vs.(68.96±8.23)%] and NO level [(75.25±9.45) μmol/L vs.(82.25±10.22) μmol/L], and significant reductions in wall motion score index [(1.35±0.39)% vs.(1.11±0.29)%], levels of Ang Ⅱ [(102.25±6.93) ng/L vs.(52.99±5.36) ng/L] and VEGF [(328.25±23.41) ng/L vs.(228.69±22.69) ng/L] in combined treatment group, P<0.01 all.Conclusion: Rosuvastatin combined clopidogrel bisulfate tablet can effectively improve heart function, serum levels of Ang II, VEGF and NO in patients with coronary heart disease, the therapeutic effect is significant, which is worth extending.
