1.X-ray Features of Heart and Lung in Chronic Renal Failure

Objective To improve the knowledge of X-ray findings of heart and lung in chronic renal failure ( CRF ) . Methods X-ray findings of heart and lung in 120 cases with CRF were analysed retrospectively.Results The X-ray appearances of lung and heart in CRF included augment and blur of hilum, increased, tortuous and disorder lung markings, accretion of heart, and the abnormal shadows in lung were irregular, inhomogeneus and changing fast in morphology. Conclusion Radiography can supply valuable messages and is ofconsiderable value in the diagnosis of CRF.
2.Treatment of portal vein tumor thrombus of hepatocellular carcinoma with percutaneous laser ablation

Objective To evaluate the efficacy of percutaneous laser ablation in the management of portal vein tumor thrombus(PVTT)of hepatocellular carcinoma(HCC).Methods Under ultrasound guidance,a needle was percutaneously punctured into the portal vein,through the axial of the tumor thrombus,and until the proximal end.An optic fiber was inserted through the needle and repeated pulse laser ablation was given.Results Complications included 102 cases(94.4%)of pyrexia 1~3 d after treatment(37.5~39.5 ℃),84 cases(77.8%)of incisional pain,and 3 cases(2.8%)of upper gastrointestinal bleeding.There were three types of findings of PVTT in 53 cases surviving over 1 year:①the tumor thrombus was atrophied and disappeared in 20 cases;②the thrombus was atrophied and the portal vein exhibited a honeycomb-like appearance in 18 cases;③the thrombus kept on growing and the portal vein was enlarged in diameter in 15 cases.Among 95 cases of totally occluded portal vein,color blood flow signals appeared in all of them on the first postoperative day.The signals could still be seen in 76 cases at 1 month after treatment,in 64 cases(out of 91 cases)at 3 months,in 52 cases(out of 71 cases)at 6 months,in 36 cases(out of 42 cases)at 1 year,in 10 cases(out of 14 cases)at 2 years,and in 2 cases(out of 2 cases)at 3 years.The 1-,2-,and 3-year survival rates were 55.56%,35.20%,and 20.30%,respectively.Conclusions Percutaneous laser ablation is a feasible novel option for the treatment of portal vein tumor thrombus of hepatocellular carcinoma.
3.Effects of radiofrequency ablation combined with percutaneous vertebroplasty by the injection of artificial bone for metastatic spinal tumor

Objective To explore the effects of radiofrequency ablation ( RFA) combined with percutaneous vertebroplasty ( PVP) by the injection of artificial bone for metastatic spinal tumor. Methods Fifty-eight patients with metastatic spinal tumors were divided into bone cement group 1 (n=12),bone cement group 2 (n=16), artificial bone group 1 (n=16), artificial bone group 2 (n=14). The patients in bone cement group were arranged RFA combined with PVP by injection of bone cement,and the artificial bone group were treated with RFA combined with PVP by injection of artificial bone. The vertebral body modality,fractures, and visual analogue scale ( VAS) ,spinal ODI score were compared before operation,1 week,1,3,6,9,12 months of after operation. Results There were 12 patients (42. 9%) with vertebral body deformation in bone cement group,which was more than those in artificial group (6. 7%) (χ2 =8. 4768,P=0. 0036). The VAS or DOI score decreased after operation than those pre-operation in the 4 groups (P<0. 01),but increased in bone cement group after operation of 6, 9 and 12 months than those in artificial group (P<0. 05). Conclusion The combination of RFA and PVP by injection of bone cement or artificial bone is effective to metastatic spinal tumors. The effects of PVP injection with artificial bone maintains longer duration compared to bone cement.
4.Preparation of Anti-EGFR Monoclonal Antibody Immunoconjugate (EQ75-ADR) and It's Inhibitory Effect on Human Epidermoid Carcinoma

Objective: To prepare an immunoconjugate of anti-EGFR monoclonal antibody (ETS) and adriamycin(ADR) and to investigute it's inhibitory effect on human epidermoid carcinoma (A431) in vitro and in vivo and it's side effect on C57BL/ 6J mice.Methods:The immunoconjugate(ETS-ADR) was prepared with an improved glutaraldehyde conjugate method and the cytotoxicity effects of EQ75-ADR on A431 cells and Wish cells were measured by MTT assay. A431 tumor-bearing nude mice and C57BL/6J experimental model were established and were used for testing the inhibitory edicts of EQ75-ADR. The side effect of EQ75-ADR was compared with ADR. Results:The EQ75-ADR showed specific cytotoxicity on A431cells,the ytotoxicity of EQ75-ADR was 50-fold, 14-fold and 10-fold more efficient than that of EQ75, ADR and mixture of EQ75 and ADR (EQ75+ ADR) respectively.The effect of EQ75-ADR on with cells was 10-fold less efficient than that of ADR and EQ75+ADR. As compared with the cytoxicity effect of EQ75-ADR on Wish cells with A431 cells, it showed 300-fold less efficient.Similar cytotoxicity results were shown in 2 h treatment experimental group. 5.Middle-column preserved pedicle subtraction closing-opening wedge osteotomy for the treatment of stiff kyphosis

Objective:To investigate the effectiveness and safety of middle-column preserved pedicle subtraction closing-opening wedge osteotomy for the treatment of stiff kyphosis.Methods:From January 2016 to April 2018, 12 patients with stiff kyphosis in our department were treated with middle-column preserved pedicle subtraction closing-opening wedge osteotomy. The patients' operative time, intraoperative blood loss, postoperative drainage, surgical complications, low back pain and leg pain visual analogue scale (VAS), Oswestry dysfunction index (ODI) score, and SF-36 were recorded.These parameters were compared at preoperative, postoperative, and at the final follow-up. Coronal parameters included lumbar scoliosis Cobb angle, C 7 vertebral body center to humeral vertical line distance (C 7PL-CSVL), whilesagittal parameters includedlumbar Lordosis (LL), sacral slope (SS), pelvic tilt (PT), and sagittalvertical axis (SVA). Results:All of 12 patients successfully completed the operation.The mean operation time was 238.20±65.95 min, the mean intraoperative blood loss was 440.50±133.60 ml.The patients’ODI score was 65.92%±6.96% at the preoperative, and 21.00%±3.19% at the final follow-up. The difference was statistically significant ( t=20.32, P<0.0001).The VAS score of back pain was 6.00±0.95 at preoperative, 2.33±0.89 at 3 months postoperatively, and 1.42±0.51 at the final follow-up. The VAS score of leg pain was 6.91±1.24 at preoperative, 2.50±1.00 at 3 months postoperatively, and1.50±0.52 at the final follow-up. There was significant difference in SF-36 at preoperative and at final follow-up ( P<0.05). The differences in LL, SS, PT and SVA at the preoperative and at final follow-up were statistically significant ( F=17.47, P<0.001; F=5.015, P=0.0125; F=14.66, P<0.001; F=81.11, P<0.001) . There was significant difference in lumbar scoliosis Cobb angle and C 7PL-CSVL at the preoperative and at final follow-up ( F=87.19, P<0.001; F=100.9, P<0.001) . Conclusion:The advantages of this surgical procedure includesimple operation, reducedsurgery time, and shorten intraoperative bleeding, which can effectively relief clinical symptoms, improve the quality of life, correctkyphosis, and maintain the patient's spinal-pelvic balance.
6.Effect of early nursing intervention on severe complication of radical cystectomy and orthotopic ileal neobladder

Objective To investigate the effect of early nursing intervention on severe complication of radical cystectomy and orthotopic ileal neobladder.Methods A total of 30 patients undergoing radical cystectomy with orthotopic ileal neobladder were randomly divided into the intervention group and the control group with 15 cases in each group,the control group received routine nursing,while the intervention group received psychological counseling,health education,sufficient preoperative preparation,postoperative strict tube care,nutritional support according to the serious complications after operation.The postoperative complications,indwelling time of tubes,hospitalization days between two groups were compared.Results Serious complications rate after surgery of the intervention group was lower than that in the control group.The indwelling time of tubes,length of stay in the intervention group were less than those of the control group,the two groups had significant difference.Conclusions Early predictive nursing intervention can reduce compli-cations rate after radical cystectomy and ileal neobladder,shorten the hospitalization time and improve the success rate of operation.
8.Expression and the correlation of AQP5, HIF-1alpha and VEGF in human nasal polyps.

OBJECTIVE:
To detect the expression of AQP5, HIF-1alpha and VEGF in human nasal polyps, and to observe the relationship of AQP5 with HIF-1a and VEGF, and to investigate their role in the pathogenesis of nasal polyps.
METHOD:
Using the techniques of Real-time Quantitative Polymerase chain Reaction and Western blot, the expressions of AQP5, HIF-1alpha, VEGF mRNA and protein were examined. The specimens were obtained from patients underwent endoscopic surgery at the same time, including eighteen nasal polyps and ten inferior turbinate tissues.
RESULT:
(1) The result of RT-PCR showed that the expressions of the AQP5 mRNA were lower in nasal polyps than those in inferior turbinate tissue (P < 0.01); there was no significant difference between the expression of HIF-1alpha mRNA or VEGF mRNA in nasal polyps and inferior turbinate tissues (P > 0.05); (2) According to the Western blot, there was no significant difference between the expression of AQP5 in nasal polyps and inferior turbinate tissues (P > 0.05); the expression of HIF-la or VEGF were higher in nasal polyps than those in inferior turbinate tissues (P < 0.05); (3) According to the Western blot, there was a positive correlation between the expression of AQP5 and that of HIF-1alpha (r = 0.633, P < 0.05), and the correlation was also existed between AQP5 and that of VEGF (r = 0.611, P < 0.05).
CONCLUSION
AQP5, HIF-1alpha, VEGF are all involved in the edema formation of nasal polyps. The changes of AQP5's distribution can cause accumulation of water in nasal polyps, HIF-1alpha, VEGF can induce new blood vessels and increase vasopermeability in nasal polyps; they have separate regulatory mechanism in edema formation, but might have some relationships in some ways.

9.Epidemiological survey on prevalence of alcohol dependence in Guangxi Zhuang Autonomous Region

Objective To understand the prevalence of alcohol dependence in individuals living in Guangxi Zhuang Autonomous Region,China. Methods From July 2007 to December 2007,21290 subjects aged more than 15 were randomly selected for 6 Prefecture-level cities and 10 counties by stratified multi-stage cluster sampling method. All subjects were investigated by face to face interview. The Chinese version of Composite International Diagnostic Interview (CIDI3.0) Provided by the Chinese Center for Disease Control And Prevention was used as the screening tool and the diagnoses were made with the 10th edition of international classification of Disease Criteria. Results 18 219 subjects completed the screening,the Completion rate was 85.58‰. The general point prevalence and the general lifetime prevalence of alcohol dependence in individuals living in Guangxi was both 4.50‰. The general lifetime prevalence was higher in rural (6.17‰) than in urban (1.85‰) ( RR =0.31,95% CI =0. 17 ~0.57, P=0. 000) ,higher in men (8.59‰) than in women (0. 33‰) ( RR =0.04,95% CI =0. 01 ~ 0.13, P = 0.000 ) and higher in Zhuang (9.63‰) than in Han(1.95‰) ( RR = 5. 01,95% CI = 3.02 ~8.34, P = 0.000). The lifetime prevalence of alcohol dependence tends upwards as age increasing (P<0.05 ) and it differs in different marital status,higher in divorce groups (P < 0.05). Conclusion Schizophrenia, Depression disorder, Alcohol dependence disorder are the most common mental disorders and have become the public health problem in Guangxi. To do this,it is necessary to carry out targeted research in prevention and rehabilitation.
10.Middle-column preserved pedicle subtractionclosing-opening wedge osteotomy to treat thoracolumbar kyphosis deformity in ankylosing spondylitis

Objective To investigate the effectiveness and safety of middle-column preserved pedicle subtraction closingopening wedge osteotomy for the treatment of ankylosing spondylitis (AS)-related thoracolumbar kyphosis.Methods From January 2010 to December 2016,eleven patients (9 males and 2 females) who underwent one-level middle-column preserved pedicle subtraction osteotomy with an average age of 40.8 years (21-68 years) were reviewed.The average thoracolumbar kyphosis angle (TLK) was 50.5°±9.6° and the average lumbar lordosis angle was-2.5°±24.1°.The thoracolumbar kyphosis,sagittal vertical axis (SVA),lumbar lordosis,pelvic tilt,sacral slope,thoracic kyphosis,Scoliosis Research Society (SIRS) 22,Oswestry disability index (ODI) score,the length of anterior column and posterior column at the pre-and post-operation,operation time,perioperative blood loss and complications were recorded.Results The osteotomy sites of 11 cases included 7 in L2,2 in L3 and 2 in T12.Theaverage operation time was 156.8 min (120-220 min).The average blood loss was 604.5 ml (350-900 ml).The average time of followup was 24 months (12-42 months).Complications were encountered in 2 patients.There were 1 casewith transient neurological deficits,another case with incision infection.No serious neurological or vascular complications occurred in all cases.The thoracolumbar kyphosis (TLK),lumbar lordosis,pelvic tilt,sacral slope,thoracic kyphosis,SRS 22 and ODI score were improved significantly.The preoperative TLK cobb was 50.5° (36°-66°),which improved to 5.1 ° (1 °-11°) at the final follow-up with a mean correction rate of 89.9%.Sagittal migrationwas improved from 184.6 mm preoperatively to 79.2 mm atthe final follow-up with an average correctionrate of 49.3%.The differences of the length of posterior column of osteotomy site after operation showed no statistical significance.All patients had solid fusion at osteotomy site and no instrumentational failure and loosening were found over the follow up.Conclusion Middle-column preserved pedicle subtraction closing-openingwedge osteotomy is safe and effective for correction of the thoracolumbar kyphosis deformity occurring in ankylosing spondylitis,resulting in satisfactory out comes with acceptable complications.
