2.Nosocomial Infection and Antimicrobial Resistance of Pseudomonas aeruginosa among Geriatrics Patients

OBJECTIVE To investigate nosocomial infection and antibiotic resistance of Pseudomonas aeruginosa among geriatrics patients and to instruct clinical monitoring and control infection.METHODS P.aeruginosa isolates from geriatrics patients were identified and the resistance to antibiotics was determined by K-B method.RESULTS The risk factors of nosocomial infection were underlying diseases,low immunological function,long time hospitalization,invasive treatment,and prolonged antibiotic therapy.The resistant rates to trimethoprim/sulfamethoxazole and cefotaxime in PA were 74.8% and 60.2%,respectively.The resistance rate to imipenem was 3.9%.CONCLUSIONS It is suggested to carry out proper measurement to control the spread of multiresistant strains in order to improve the treatment of PAE infection in geriatrics patients.
3.Effectiveness comparison of channel-assisted mini-incision and open Achilles shortening for treatment of healed Achilles tendon rupture

Objective To compare the clinical effectiveness between the channel-assisted mini-invasion and open Achilles shortening for treatment of the elongated Achilles tendon following previous rupture.Methods The clinical data of 19 patients admitted from Dec. 2013 to Dec. 2015 and met the inclusion criteria were analyzed retrospectively. Eight patients were treated with shortening operation by channel-assisted minimally invasive repair system, while 11 patients received dissection of Krackow Achilles tendon shortening. There was no significant difference between the two groups in gender, age, injury to operation time, preoperative calf circumference and preoperative AOFAS (American Orthopaedic Foot & Ankle Society) score (P>0.05).Results The operation time, incision length and postoperative hospital days were significantly less in min-invasion group than in incision group (P<0.05). After operation, wound healing by first intention was obtained in incision group except 1 case which got infection and healed by treatment 2 weeks later. No sural nerve and vascular injury and lower extremity deep vein thrombosis existed. MRI showed that the Achilles tendon recovered in both groups at the 8th week follow-up. The patients were followed up for 12-17 months in the mini-invasion group and 12-20 months in the incision group. The calf circumference and AOFAS score of both groups were higher in the last follow-up than that in pre-operation (P<0.05), but no significant difference existed between the two groups at the last follow-up (P>0.05).ConclusionChannel-assisted minimally invasive Achilles tendon shortening operation has not only similar effectiveness to the incision shorting operation for the treatment of elongated Achilles tendon following previous rupture, but also has the advantages of shortening operation time and stay in hospital and avoidance of sural nerve injury.
5.Effects of nociceptin/orphanin FQ on cross-organ sensitization in rat model with visceral hypersensitivity

Objective To investigate the effect of nociceptin/orphanin FQ (N/OFQ) on crossorgan sensitization and bladder function disorders in rat model with visceral hypersensitivity. Methods The model of cross-organ sensitization was induced by basic OVA-sensitization combined with colon mustard oil instillation in 44 female SD rats. All rats were then divided into control group (n= 14,infusion with 0.9% NaCl solution), model group (n= 14) and N/OFQ group (n= 16, infusion with N/OFQ). The anti-nociceptive effect of N/OFQ were evaluated by using the visceromotor response (VMR) to grade bladder distention (0. 5~2.0 ml, 20 s). Bladder function was evaluated with the cystometry by measuring voiding pressure and micturition frequency. Results The rats in model group had a significant increase of VMR response at 1.0 ml, 1.5 ml and 2.0 ml of pressure when compared to those in control group (P=0. 006,0. 015 and 0. 0331, respectively). However, N/OFQ treatment was able to reverse the increased VMR response in N/OFQ group at 1.5 ml and 2.0 ml of pressure (P=0. 016 and 0. 031, respectively). It was showed that mustard oil significantly reduced bladdervoiding pressure (P<0. 01) and significantly increased micturition frequency (P<0.01)in model group when compared to control group. The N/OFQ reversed the decrease of voiding pressure (P=0. 0038) and the increase of micturition frequency in N/OFQ group( P = 0. 001). Conclusions These findings indicate that the rat model of cross-organ sensitization was able to be established successfully by the basic OVA-sensitization combined with colon mustard oil instillation. N/OFQ has an antinociceptive effect on cross-organ hypersensitivity and attenuates changes in bladder urodynamic function.
6.Color Doppler ultrasonography diagnosis of congenital muscular torticollis

Objective To investigate the diagnostic value of color Doppler ultrasonography in congenital muscular torticollis (CMT). Methods The sonographic characteristics of 62 children with CMT confirmed clinically were retrospectively analyzed. Results Abnormal ultrasound features could be visualized in all of 62 children with CMT. According to the sonogram features, 62 children were divided into 2 types: mass-forming (32/62, 51.61%) and diffuse type (30/62, 48.39%). Arterial flow signals were detected in 18 children (18/62, 29.03%); RI was 0.73-0.88. Conclusion Ultrasonography has specific value for the diagnosis of CMT.
7.Serum Procalcitonin Levels for Differentiating Gram-positive and-negative Bacteria Infection in Patients of ICU

OBJECTIVE To evaluate the value of serum procalcitonin(PCT) levels for differentiating Gram-positive and-negative bacteria infected critically ill patients in intensive care unit(ICU). METHODS Serum PCT levels were measured in 80 patients with sepsis and severe sepsis in ICU.Of the 80 patients,40 were infected with Gram-positive bacteria and 40 were infected with Gram-negative bacteria.Another 40 patients with fungi or viruses infection or non-infection from ICU were measured in parallel as control. RESULTS Serum levels of PCT in patients with Gram-negative bacteria were significantly higher than in those with Gram-positive bacteria and control ones(18.5?5.8ng/ml vs 3.9?2.1ng/ml and 0.3?0.2 ng/ml,respectively,P
8.Effect of bifidobacterium-mediated CTP-NPRL2 on growth and apoptosis of nude mouse renal carcinoma

Objective To evaluate the effect of bifidobacterium-mediated CTP-NPRL2 on the growth and apoptosis of nude mouse renal carcinoma.Methods Recombinant plasmid pET15b-CTP-NPRL2 was constructed and transfected into bifidobacterium by electroporation,and then the expression of fusion protein CTP-NPRL2 was verified by Western blot.The nude mouse renal carcinoma model was constructed by subcutaneous injection of renal carcinoma cells suspension.The nude mice with renal carcinoma were equally and randomly divided into the observation group and control group(8 mice in each group).The mice in the observation group were treated with bifidobacterium containing recombinant plasmid pET15b-CTP-NPRL2 through tail vein injection,and the mice in the control group were treated with normal saline.All mice were treated once a week for four weeks,and then executed for evaluating the weight of mice and bearing tumors.Finally,the apoptosis of renal carcinoma was detected by TUNEL staining.Results The mass of nude mice was(26.24±1.98) g in the observation group and(23.28±2.17) g in the control group.The mass of bearing tumors was(1.37±0.12) g in the observation group and(1.68±0.18) g in the control group,and the differences were statistically significant(P<0.05).The TUNEL detection results showed that the apoptosis index of renal carcinoma cell in the observation group(23.27±5.14)% was significantly higher than that in the control group(10.37±2.58)%,and the difference was statistically significant(P<0.05).Conclusion Bifidobacterium-mediated CTP-NPRL2 has an inhibitory effect on the renal carcinoma growth of nude mouse,and significantly increases the apoptosis of renal carcinoma cells.
9.Effect of Id1 knockdown on formation of osteolytic bone lesions by prostate cancer PC3 cells in vivo.

The formation of osteolytic bone lesions is a key process for osteolytic cancer to metastasize to the bone and is under the control of a set of transcription factors. Recently, the inhibitor of differentiation 1 (Id1) has been linked with angiogenesis, tumorigenesis, metastasis and bone formation. However, the function of Id1 during the process of bone destruction caused by cancer in vivo has not yet been elucidated. We, therefore, examined whether and how Id1 affects the ability of cancer to form osteolytic lesion in vivo. The study used a lentiviral vector overexpressing short hairpin RNA (shRNA) targeting Id1 gene. PC3 cells, a prostate cancer cell line, were transduced with Id1 shRNA or negative control (NC) shRNA before implantation in BALB/c mice. Cells were implanted in a tibial injection model. Tumor formation in bone was monitored by X-ray. The relationship between parathyroid hormone-related protein (PTHrP), an osteolytic factor, and Id1 was analyzed by using immunohistochemistry in tissue sections from osteolytic lesion of the BALB/c mice. Our results showed that Id1 shRNA delivery to PC3 cells by lentivirus caused efficient and stable Id1 gene silencing. In the intratibial model, PC3 cells produced primarily osteolytic lesions in the bone. Eleven of 14 mice in Id1 shRNA group but only 4 of 14 mice in the NC shRNA group developed osteolytic lesions with cortical destruction at 4th week. Mice treated with Id1 shRNA had larger tumor volume in the bone and larger cortical destruction. The expression of PTHrP protein in PC3 cells was not affected by Id1 knockdown in vivo. These results indicate that Id1 may down-regulate the ability of PC3 cells to form osteolytic lesions in vivo and the signal pathway needs to be further investigated.
10.Mechanical response numerical analysis of bone tissue based on liquid saturated biphasic porous medium model.

A biphasic porous medium model based on the mixture theory in continuum mechanics frame was used to depict the distributions of osseous stress field, distortion field and pore pressure when the bone tissue was subject to various dynamic loads. In the model, the bone tissue was considered as a transversely isotropic, liquid saturated porous material. The coupling relationship among the distortion, fluid flow and the streaming potential is studied. The Galerkin weighted residual method was used to derive the finite element formulation for dynamic response and the streaming potential calculating formulation of bone tissue, the penalty finite element formulation was obtained via introducing the ratio term of pressure p and penalty parameter beta in the continuity equation and, in turn, eliminated the pressure term in governing equation sets. The computational results show that the viscoelastic behavior and the energy dissipation property in the bone tissue, especially in the cancellous bone, is caused to a great extent by the pore liquid flow and diffusion. Meanwhile, because of the existence of electrical double layer between the solid phase and liquid phase, when the liquid constituents in the pore diffuse, the streaming potential appears, and the growth and absorption of bone tissue is expedited.
