1.Mechanism Study Progress of Gastrointestinal Bypass Operation on Treatment of Type 2 Diabetes Mellitus

Objective To study the mechanism of gastric bypass operation on treatment of type 2 diabetes mellitus,recognize the etiology and pathogenesy of the disease and frame therapy strategy for type 2 diabetes mellitus.Methods The literatures about gastric bypass operation on treatment of type 2 diabetes mellitus,including clinical cases reports and evidence-based studies were reviewed.Results Gastrointestinal bypass operation was regarded as an effective treatment for type 2 diabetes mellitus.There were three hypotheses of therapy mechanism: early delivery of nutrients to the distal intestine,exclusion of the proximal intestine and incretin/anti-incretin.Conclusion Gastrointestinal bypass operation is now considering as an effective treatment,there is still a lack of basic experimental studies to clarify the mechanism.
2.Clinical analysis of arrhythmia after total pneumonectomy

Objective To approach the reason and treatment of arrhythmia after total pneumonectomy. Methods 94 arrhythmic cases after total pneumonectomy surgery were reviewed, the arrhythmia's clinical types, developing reasons and treatment process were summarized. Results There are 34 arrhythmic cases (36.2%) in all the 94 patients, most of them are sinus tachycardia. The incidences of arrhythmic are about 22.5 % and 78.3 % for normal and abnormal ECG patients before operation. The incidences of arrhythmic are also about 24.2% and 28.1% for using PCEA or not using any pain killers after surgery. Conclusion Arrhythmia after total pneumonectomy was influenced by patients' age, previous medical history, suffered hypoxemia during operation and high cardiac irritability. Using interventional treatment for patients with cardiovascular disease before operation, give enough oxygen, keep respiratory tract ease and smooth and using analgesia can significantly decease the arrhythmic incidence after total pneumonectomy.
3.Analysis of the tasks of and demands for health human resources in the laboratory specialty of the hospital

Objective To determine the tasks that health human resources of technicians in the medical laboratory specialty ought to fulfill, the factors that affect performance and the estimation of demands. Methods Such methods as analyzing medical records, tasks and health demands were used. Results The current situation of health human resources of technicians is as follows: Those with intermediate, junior or no professional titles respectively account for 46.5%, 36.5% and 11.6%; those with a master's or a bachelor's degree or a degree from a polytechnic college or school respectively account for 2.0%, 8.2%, 42.9% and 40.8%; and others account for 6.1%. The average utilization ratio of laboratory service by inpatients and outpatients is respectively 96.7% and 49.5%; and the average utilization of laboratory service per inpatient and per outpatient is respectively 12.5 and 2.2 times. Of the average daily service volume, emergency, outpatient and inpatient, microbiological, immune serum, and biochemical tests respectively account for 8.3%, 3.0%, 8.1%, 7.8% and 72.8%. The chief tasks of the technicians are performing laboratory operations and assisting the physicians with the reporting of the laboratory results. The demand for health human resources of technicians in the laboratory specialty in the hospitals across the country numbers 89 920 people. Conclusion The results of the study will provide health administrative departments, medical institutions and medical education departments with basis for strengthening human resources management and formulating educational plans.
5.The changes of serum FSH and E_2 levels in women after simple hysterectomy

Objective To determine the levels of serum FSH and E 2 in the patients with uterus preserved or hysterectomy so as to guide the selection of operative modality for the patients with hysteromyoma and hormone replacement therapy for the patients with hysterectomy. Methods Radioimmunoassay (RIA) was used to detect the levels of serum FSH and E 2 in the patients with uterius preserved or after hysterectomy and control group. There were 60 subjects in each group. Results Before operation, there was no significant difference in the levels of serum FSH and E 2 among myomectomy group, hysterectomy group and control group. 6.The promotion of nerve regeneration in peripheral nerve by progesterone after terminolateral neurorrhaphy: an experiment study

Objective To evaluate the effect of progesterone (PROG) on promoting peripheral nerve collateral growth and nerve regeneration after terminolateral neurorrhaphy (TLN). Methods Thirty-six female SD rats were from nine to ten weeks old and weighed from 250 to 270 grams. They were randomly divided into two groups by different drugs they were administrated, the PROG group and the control group. Both groups were subdivide into three groups respectively, PROG four weeks group, PROG eight weeks group, PROG twelve weeks group, control four weeks group, control eight weeks group and control twelve weeks group. There were six subjects in every subgroups. First, both groups were performed ovariectomy. Then, two weeks later, TLN was operated. In the PROG group, the left peroneal nerve was transected, then an 1mm epineukral window was created on the neighbouring. The distal end of peroneal nerve was sutured to the windowed tibial nerve by means of end-to-side attachment. In the control group, nerve TLN was performed as in the PROG group, 0.2ml (10mg/ml) PROG and tea-oil (the solvent of PROG) was injected by subcutaneous injection for days in PROG group and control group respectively. Electrophysiological, histolgicial and morphological examinations were measured at 4,8, and 12 weeks after the operation. Results Regeneration of peroneal nerve occurred in both groups. All the data were analysed by factorial designed analysis of variance. 7.Efficacy on hemiplegic spasticity treated with plum blossom needle tapping therapy at the key points and Bobath therapy: a randomized controlled trial.

OBJECTIVETo evaluate the efficacy on hemiplegic spasticity after cerebral infarction treated with plum blossom needle tapping therapy at the key points and Bobath therapy.
METHODSEighty patients were collected, in compliance with the inclusive criteria of hemiplegic spasticity after cerebral infarction, and randomized into an observation group and a control group, 40 cases in each one. In the control group, Bobath manipulation therapy was adopted to relieve spasticity and the treatment of 8 weeks was required. In the observation group, on the basis of the treatment as the control group, the tapping therapy with plum blossom needle was applied to the key points, named Jianyu (LI 15), Jianliao (LI 14), Jianzhen (SI 9), Hegu (LI 4), Chengfu (BL 36), Zusanli (ST 36), Xiyangguan (GB 33), etc. The treatment was given for 15 min each time, once a day. Before treatment, after 4 and 8 weeks of treatment, the Fugl-Meyer assessment (FMA) and Barthel index (BI) were adopted to evaluate the motor function of the extremity and the activity of daily life in the patients of the two groups separately. The modified Ashworth scale was used to evaluate the effect of anti-spasticity.
RESULTSIn 4 and 8 weeks of treatment, FMA: scores and BI scores were all significantly increased as compared with those before treatment in the two groups: (both P<0. 05). The results in 8 weeks of treatment in the observation group were significantly better than those in the control group (all P<0. 05). In 4 and 8 weeks of treatment, the scores of spasticity state were improved as compared with those before treatment in the patients of the two groups (all P<0. 05). The result in 8 weeks of treatment in the observation group was significantly better than that in the control group (P<0. 05). In 8 weeks of treatment, the total effective rate of anti-spasticity was 90. 0% (36/40) in the observation group, better than 75. 0% (30/40) in the control group (P<0. 05).
CONCLUSIONThe tapping therapy with plum blossom needle at the key points combined with Bobath therapy effectively relieves hemiplegic spasticity in the patients of cerebral infarction and improves the motor function of extremity and the activity of daily life.

8.Dynamic changes of liver cancer stem cell markers and inflammatory factors during the induction of liver cancer in rats

BACKGROUND:Many liver cancer stem cel markers have been found in liver cancer tissues and cel lines such as CD133, acetaldehyde dehydrogenase (ALDH), CD90, CD44, EpcAM, CD13, OV6, K19, c-kit and ABCG2. Of them, CD133, CD90 and CD44 have been shown to be strongly associated with the recurrence and metastasis of liver cancer. OBJECTIVE:To explore the dynamic changes of liver cancer stem cel markers and inflammatory factors during the induction of liver cancer in rats and their correlation. METHODS:Diethyl nitrosamine solution was given to Sprague-Dawley rats for 24 hours to induce rat models of liver cancer. Rats that were given common water were considered as the healthy control group. RESULTS AND CONCLUSION: Immunohistochemical staining revealed that Kupffer cels-related ED2 expression showed a gradual increase in the model group. Compared with the healthy control group, ED2 expression was significantly higher at 12, 16, 20 and 24 weeks after induction in the model group (P < 0.05). Quantitative PCR demonstrated that CD90 showed a gradualy increased trend during induction (P < 0.05). Compared with healthy tissue, CD90 increased significantly in the liver cancer tissue (P < 0.05). CD133 showed an increased trend, but one-way analysis of variance did not show significant differences (P > 0.05). During induction, no significant change was found in other liver cancer stem cel markers (P> 0.05). During the induction, tumor necrosis factor α, transforming growth factor β, MCP-1 and interleukin-6 expression levels were significantly increased (P < 0.05). Compared with healthy tissue, transforming growth factor β, MCP-1 and interleukin-6 expression levels were significantly higher in the liver cancer tissue (P < 0.05). Other inflammatory factors did not exhibit significant alterations during the induction (P > 0.05). Pearson correlation analysis demonstrated that MCP-1, transforming growth factor βand interleukin-6 expression levels were significantly positively correlated with CD90 expression (P < 0.05). These findings suggest that partial inflammatory factors released from Kupffer cels have a certain correlation with liver cancer stem cels. Kupffer cels can promote the occurrence of liver cancer.

10.The research about bcl-2/bax/fas/fasL mRNA of splenic lymphocytes in HIBD rat model

Objective To obtain the change of bcl-2/bax/fas/fasL in splenic lymphoctyes with different lasting time of hypoxicischemic brain damage (HIBD). Methods The newborn rat were divided into 6 groups by the time of being HIBD model randomly, includes 1/6/12/24/48/72 hour(s) (8 for every group),and control groups were established at the same time point. The following four apoptosis related genes bcl-2/bax/fas/fasL were tested by real time PCR. Results ( 1 ) bcl-2: the mRNA expressions of HIBD groups were lower than control groups at the same time ( P<0.01 ). Eliminated the control effects, the mRNA expressions of HIBD groups were differernt by the modeling time(P <0.01 ). (2)bax: the mRNA expressions of HIBD groups were higher than control groups at the same time( P <0.01 ), and in control group the expression of 6 h was much higher than any other groups (P<0.01 ). Eliminated the control effects, the mRNA expressions of H IBD groups were different by the modeling time( P<0.01 ). (3)bcl-2/bax: the ratios of HIBD groups were lower than control groups at the same time( P <0.05 ), the ratios in control groups were higher than 1 ( except for 1 h); while in HIBI) groups the ratios were lower than 1; Eliminated the control effects, the ratios were different in all the groups. (4)fas: the mRNA expressions of HIBD groups were higher than control groups at the same time ( P <0.01 ), and both were maximum at 6 h. (5)fasL: the mRNA expressions of HIBD groups were higher than control groups in 1 h and 6 h ( P<0.01 ), while lower than control group at other time points( P<0.01 ),the expression of 24 h was the maximum of control groups and 12 h was the maximum of HIBD groups. (6)fas/fasL: the ratios of HIBD groups were higher than control groups( P <0.01 ) (except for 6 h), and the ratios in control groups were lower than 1 ( P<0.01 ) ( except for 6 h), and not concentrated, while in HIBD groups were higher than 1 ( except for 24 h), between 0.69 to 5.65. Conclusion Pro-apoptosis genes ( include bax/fas/fasL) were promoted by HIBD, while anti-apoptosis gene(bcl-2) was inhibited. The maximum of pro-apoptosis genes became early in HIBD. Both the pro- and anti-apoptosis genes got their maximum at 6 h and 12 h of HIBD. The apoptosis suppression was the main effects in control groups from the ratio of bcl-2/bax, which was lower than 1. The apoptosis promotion was the main effects in HIBD groups from the ratio of bcl-2/bax, which was higher than 1, especially at 12 h. Thefas/fasL effect which is the major way of lymphocytes apoptosis was strengthened in HIBD.
