1.A Sociological Analysis of Plagues based on Habermas' Living-World Theories——Taking the Harbin murrain in the First Half Leaf of 20th Century as an Example

Based on the construction and its rational theories of Habermas's living-world,this paper analyzes the characteristics of plague and adjusts them according to Chinese social framework and realistic conditions during plagues.The social rescuing system and its sociological rational evolution style are also analyzed based on the functional framework of government,social culture and individuals.Individual's life style and behaviors have both been adjusted due to high pressures of plagues and government's standardization and interferences to the plagues.But seen in the long run,rational evolution of individual's life style depends on the accumulation of time,civilization,and the recognition of medical ethics among general public.
2.Effects of indomethacin on inflammatory process induced by focal cerebral ischemia-reperfusion in rats and its mechanism

Objective Cerebral ischemia would rapidly initiate structural and functional damages in brain, including blood-brain barrier disruption, inflammation, and angiogenesis. The purpose of present study is to investigate the effects of indomethacin, an agent that inhibits cyclooxygenase, on the inflammatory reaction after cerebral ischemia-reperfusion in transient middle cerebral artery occlusion(MCAO)rats and its possible mechanism. Methods Adult male SD rats weighed 250-250g were subjected to either sham surgery or middle cerebral artery occlusion (MCAO) for 2h of brain ischemia and 24h reperfusion. After 24h of reperfusion,the size of cerebral infarction and the neurological deficit were determined by the method of TTC (2,3,5-Triphenyltetrazolium Chloride) staining and Longa's score analysis.The contents of IL-8, IL-1?, TNF-? and MPO in brain tissue were assayed by ELISA. The expressions of ICAM-1 ande E-selectin were evaluated with Western-blot. 4.The effect of mitomycin,5-fluorouracil and tamoxifen on human breast cancer cell lines

ObjectiveTo evaluate the killing effects of MMC,5-Fu and TAM on human breast cancer cell lines (MCF-7 and MDA-MB-435S). MethodsThe MTT method and median-effect principle were used. ResultsKilling effects of drugs were enhanced as drug concentration increased while they were used alone or in combination. The median-effect concentration of TAM in conjunction with MMC was 1.65?10 -5 mmol/L,and that of TAM in conjunction with 5-Fu was 0.032 6 mmol/L. MMC was synergetic with 5-Fu at high concentration(CI1). The ratios of drug concentration and the sequence of administration were not related with the killing effects. ConclusionsA combination of MMC, 5-Fu and TAM has an increased killing effect on MCF-7 cell lines and questionable enhanced effect on MDA-MB-435S cell lines.
5.Perfusion augmentation in hypovascular tumor by ultrasound stimulated microbubble cavitation

Objective:To study the effects of different duty cycles [modulated by pulse repetition frequency(PRF)] of diagnostic ultrasound on enhancing blood perfusion in hypovascular tumors.Methods:Walker-256 tumor cells were injected subcutaneously into the inner thigh of 15 SD rats, and the rats were randomly divided into 3 groups according to PRF: group A(PRF=50 Hz), group B(PRF=1.0 kHz) and group C(PRF=2.0 kHz), with 5 rats in each group. The tumor-bearing rats in each group were treated with ultrasound combined with microbubbles for 10 minutes. Contrast-enhanced ultrasound was performed before treatment, immediately after treatment, and 4 hours after treatment, respectively, to compare the peak intensity(PI), area under curve(AUC), and tumor perfusion area in each group.Results:Immediately after treatment, PI and AUC values of group A and B increased compared with those before treatment (all P<0.05); 4 hours after treatment, PI and AUC values of group B continued to increase, superior to that immediately after treatment (all P<0.05), while PI and AUC values of group A and C were decreased compared with those immediately after treatment (all P>0.05). Immediately after treatment, the tumor blood perfusion area in group A and B increased significantly compared with that before treatment (all P<0.05); 4 hours after treatment, the tumor blood flow perfusion area of group B continued to increase, which was better than that immediately after treatment ( P<0.05), and the tumor blood perfusion areas of group A and C decreased compared with that immediately after treatment (all P>0.05). Conclusions:Low-intensity diagnostic ultrasound stimulated microbubble cavitation with PRF=1.0 kHz can enhance tumor blood flow significantly which can last for more than 4 hours.
6.Prognosis of subclinical Cushing's syndrome:comparison of surgical ablation of incidentaloma with conservative approach

Forty-eight patients with subclinical Cushing's syndrome(SCS)were evaluated.Eleven of them underwent adrenalectomy(Group 1)and the other 37 cases did not(Group 2).Serum and urine corticosol, plasma ACTH and parameters related to metabolic syndrome(such as waist circumference,blood pressure,blood lipids and fasting plasma glucose)were measured.The data at diagnosis were compared with those during the survey.The results indicated that patients with SCS had a significantly high prevalence of metabolic syndrome.The symptoms and signs of metabolic syndrome could be improved after removing the tumor.Otherwise there is no improvement,some patients will even develop into overt Cushing's syndrome.
7.Neutrophil gelatinase- associated lipocalin for diagnosis of acute kidney injury after cardiopulmonary bypass

Objective To evaluate the value of neutrophil gelatinase - associated lipocalin (NGAL) for diagnosis of acute kidney injury (AKI) after cardiopulmonary bypass established. Method 120 children undergoing cardiopulmonary bypass were enrolled in this prospectively study. Serial urine samples were analyzed by ELISA for NGAL expression. The primary outcome measure was AKI, which was defined as a 50% or greater increase in serum creatinine from baseline. Result 21 children ( 17.5% ) developed AKI. Urine concentrations of NGAL increased after cardiopulmonary bypass had been established for 2 hours. The concentration of NGAL in urine after cardiopulmonary bypass had been established for 2 hours was used to diagnose AKI. The area under the receiver-operating characteristic curve was 0. 96, the sensitivity was 92%, and the specificity was 91% for the cutoff value of 100ng/mg Cr. Conclusion Concentrations of NGAL in urine might be used as a sensitive and specific biomarker for early prediction of AKI after cardiopulmonary bypass had been established.
8.A pilot study on Lauren classification of advanced gastric carcinoma using virtual touch tissue quantification

ObjectiveTo explore the value of virtual touch tissue quantification (VTQ) in evaluating Lauren classification of advanced gastric carcinoma.MethodsForty-one patients with gastric cancer proved by endoscopic biopsy performed preoperative VTQ examination,and the findings were compared with postoperative pathologic results via hematoxylin -eosin and Alcian Blue-Periodic Acid Schiff (AB-PAS) staining.ResultsIn 41 patients,26 cases were diagnosed as diffuse type and 15 cases as intestinal type by pathology after operation.The shear wave velocity(SWV) of diffuse type was higher than that of intestinal type [(1.72± 0.83)m/s vs (1.05± 0.66)m/s,t =2.819,P=0.008] measured by virtual touch tissue quantification (VTQ).According to the area under the ROC curve,the cut- off value of SWV in gastric cancer tissues for assessing the diffuse type was 1.045 m/s with a sensitivity of 80.8% and specificity of 73.3 % respectively.ConclusionsVTQ could be considered as a promising method to distinguish the Lauren classification in patients with advanced gastric carcinoma.
9.Citrate lock versus heparin lock for the prevention of catheter related infections in hemodialysis patients with tunneled catheters: a Meta-analysis of randomized controlled trials

Objective To study whether citrate lock is superior to heparin lock in the prevention of catheter related infections,bleeding complications and catheter malfunctions among hemodialysis patients with tunneled catheters.Methods By searching in Pubmed,the Cochrane Library,EMBASE,Ovid,WanFang,VIP,CNKI and CBM databases as well as related journals,qualified randomized controlled trials were included in a Meta-analysis using Revman 5.0 and STATA 10.0 software.The endpoints included catheter related infection,bleeding complication,thrombolytic treatment,catheter removal for malfunction,catheter thrombosis and all-cause death.Results Fifteen randomized controlled trials were included with 1621 patients involved.Eight studies compared citrate alone with heparin lock,while 7 trials focused on citrate in combination with other antimicrobials.Pooled analysis demonstrated that incidence of catheter related infections in patients receiving citrate lock decreased by 47% compared with those on heparin (RR=0.53,95%CI 0.36-0.77,P < 0.01).Subgroup analysis by types of citrate lock indicated that all combined lock solutions of citrate and other antimicrobials (citrate + gentamicin,citrate + taurolidine,citrate + methylene blue +methylparaben + propylparaben) were superior to heparin lock in preventing catheter-related infections (P =0.01,0.04,0.01,respectively); citrate alone seemed to reduce catheter-related infection risk (RR =0.68),but no statistically significant difference was observed (95%CI 0.38-1.21,P=0.19).There were fewer patients with bleeding complications in citrate group (RR =0.53,95% CI 0.34-0.S4,P < 0.01),while citrate showed no advantage over heparin lock in terms of thrombolytic treatment (P =0.93),catheter removal for malfunction (P =0.35),catheter thrombosis (P =0.64) and all-cause death (P =0.35).Conclusions For hemodialysis patients with tunneled catheters,combined lock solutions of citrate and other antimicrobials,rather than citrate alone,are superior to heparin in preventing catheter related infections.Citrate locks are associated with less bleeding complications,and are comparable to heparin in the maintenance of catheter patency.
10.Risk factors of chronic disease for high-risk population and its application in community

The individual intervention control of risk factors in high-risk population is one of key preventive measures of chronic disease.A total of 312 volunteers consulted physicians in one selected community.And 23 high-risk individuals of chronic disease were screened.The physicians customized the limit or requirement of diet,activity,tobacco and alcohol for each individual and made interventions of life behaviors according to the plans.The results showed that physical state of 20 (87%) of them converted from high-risk to health after interventions.Therefore customized control plan of individual risk factors is an effective control method of chronic disease.
