1.Clinical significance of N-terminal pro-brain natriuretic peptide and D-dimer in patients with chronic obstructive pulmonary disease

Objective To explore the clinical significance of serum N-terminal pro-brain natriuretic peptide (NT-proBNP) and plasma D-dimer (D-D) in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and stable chronic obstructive pulmonary disease (COPD).Methods The clinical data of 76 patients with AECOPD (AECOPD group) and 68 patients with stable COPD (stable COPD group) were analyzed retrospectively.Serum NT-proBNP,plasma D-D levels and arterial blood gas analysis were measured and compared.Results The levels of serum NT-proBNP and plasma D-D in AECOPD group were (918 ± 271) ng/L and (0.269 ±0.048) mg/L,in stable COPD group were (264 ± 32) ng/L and (0.223 ± 0.042) mg/L.The levels of serum NT-proBNP and plasma D-D in AECOPD group were significantly higher than those in stable COPD group (P < 0.01).The levels of serum NT-proBNP and plasma D-D were negatively correlated with arterial blood oxygen partial pressure (r =-0.181,P =0.030;r =-0.166,P =0.047),and were positively correlated with arterial blood carbon dioxide partial pressure (r =0.763,P < 0.01;r =0.424,P < 0.01).Conclusion Combined detection of serum NT-proBNP and plasma D-D has important clinical significance in evaluating severity of COPD,and may indicate AECOPD.
2.Risk factors and prevention strategies of perinatal brain injury in premature infants

Life quality of the premature babies after rescuing successfully was gained more attention by perina-tal medical field and society with the more success rate of premature rescue. The brain injury is one of the main factors affecting the long term prognosis of the premature infants. The major risk factors of brain damage include gestational age and birth weight,intrauterine hypoxia and infection,the mode of delivery and whether the resuscitation and later treat-ment skills proper and skilled,and so on. So the key point of preventing the brain injury is avoiding premature delivery and extending the gestational age as much as possible. If the premature delivery could not evitable,the obstetrician should use adrenal cortex hormones before delivery to promote fetal lungs maturition,since the increasing application of cortical hormone prenatally and decreasing using that after birth have been shown good effects on long term prognosis of the nervous system in the preterm infants. It is also confirmed that application of magnesium sulfate prenatally could prevent the premature brain injury. There are still no agreements on the relationship between the delivery modes and the brain injury of extremely low birth weight infants. But prolonging the stage of labor is sure to increase the incidence of intracranial hemorrhage in the premature infants thereby affecting the long term prognosis. After birth,we should imple-ment the correct resuscitation processes by skilled and proper technical methods,in order to decrease the mortality and to improve the long term prognosis.

4.Risk aversion and evaluation of science research projects in military hospital

To investigate an acceptable formula of evaluation of science research projects and its risk aversion in military hospitals and the important role of nosocomial science fund in development of military hospitals.The relation of the methods and systems whith had been used to evalualing science research projects in hospitals and risk aversion were analyzed. 5.Further improving the result of multi-modality treatment for gastric cancer by optimizing scheme and measures.

The incidence of gastric cancer has declined worldwide in the past decades. However, due to the large number of population in mainland of China, the absolute number of newly-diagnosed patients in this developing country is still very high. Most of the major clinical centers have established multi-disciplinary team, but the strategy in treating this life-threatening disease still needs to be optimized, including the precise preoperative staging by various techniques, perioperative chemotherapy for locally advanced diseases, role of minimally invasive surgical approaches in treating patients with different stages, translational research to fill the gap between clinical trials and basic research, as well as the individualized treatment for gastric cancer. We believe that the biological behaviors and molecular events of gastric cancer in eastern countries are different from that of Western countries, so the clinical evidences derived from Eastern countries need to be strengthened to enrich the unmet demands in the clinical practice in China.

6.All-round administrating scientific research grants to ensure their quality

We also discussed a new model of subject construction, thesis and achievements administration leaded by grant administration, which should basically and greatly raise the level of scientific research in the hospital.
7.Clinical evaluation of relationship between dry eye and bubbles in the inferior conjunctiva fornix

Background Instability of tear film,increase of ocular surface temperature and tear evaporation are the associated factors of both bubbles and dry eye.But the relationship between dry eye and ocular surface bubbles is unclear.The bubbles are easily retained and checked in the inferior conjunctiva fornix.Objective This study aimed to study the relationship between dry eye and bubbles in the inferior conjunctiva fornix.Methods Sixty-four eyes of 32 patients with binocular irritative symptoms and bubbles or without bubbles in the inferior conjunctiva fornix were collected in Affiliated Tianyou Hospital of Wuhan University of Science and Technology from January to May,2012.The bubbles in the inferior conjunctiva fornix were examined by slit lamp microscope,and the break-up time (BUT) and Schirmer Ⅰ test (S Ⅰ t) were performed.The differences of BUT and S Ⅰ t between the two groups were compared using independent sample t test,and abnormality rate of examination indexes was statistical analyzed by CHI-Square test using a SPSS 11.5 software.Written informed consent was obtained prior to the clinical survey.Results In the 64 eyes of the bubbles group,incidence of dry eye was 75.00%,and the incidence of both BUT<5 seconds and S Ⅰ t<5 mm/5 minutes was 46.88% ;while the incidence of BUT<5 seconds or S Ⅰ t<5 mm/5 minutes was 53.13% and 57.81%,respectively.In 64 eyes without bubbles,incidence of dry eye was 31.25%,the incidence of both BUT<5 seconds and S Ⅰ t<5 mm/5 minutes was 18.75%,that of BUT<5 seconds or S Ⅰ t<5 mm/5 minutes was 21.88% and 20.31%,respectively.The incidences of dry eye,both BUT<5 seconds and S Ⅰ t<5 mm/5 minutes,BUT <5 seconds,S Ⅰ t<5 mm/5 minutes were significantly higher in the bubbles group than those of the without bubbles group (all at P=0.00).In the 68 eyes determined as dry eyes,48 eyes (48/68) were in the bubbles group and 20eyes (20/68) were in the without bubbles group,showing a significant difference between the two groups (x2 =24.60,P<0.01).The incidences of both BUT<5 seconds,S Ⅰ t<5 mm/5 minutes were not significantly different between the two groups (x2 =0.01,P=0.95;x2 =1.06,P=0.30).Conclusions The bubbles in the inferior conjunctiva fornix is highly correlated with dry eye.
8.In vitro study on inhibition of expression of MHC II molecules by adenovirus-mediated gene transfer of CIITA mutant gene

Objective:To construct adenovirus containing murine MHC class II transactivator (CIITA) mutant gene and to observe expression of this gene mediated by adenovirus as well as function of the expression product in vitro.Methods:The type IV CIITAcDNA gene was cloned from peritoneal macrophage of BALB/c mouse by conventional way of molecular biology, and then it was cloned into expression vector pIRES. CIITA mutant gene was constructed by the way of overlap extension by PCR (OE-PCR), and then it was also cloned into expression vector pIRES. By using of pAdEasy-1 system we acquired the deficient recombination adenovirus Ad-CIITAm, containing CIITA mutant gene, which had the ability of infection and the no-loaded control adenovirus Ad-GFP.Then the two adenoviruses strains were processed for a great deal amplification,purification and titer determining. The Ad-CIITAm and Ad-GFP were infected into HeLa cells and Raji cells.Inducible or constitutive expression of HLA-DR molecule and the following changes were observed by the use of flow cytometry.Results:The murine CIITA gene was cloned successfully and the recombinantant adenovirus Ad-CIITAm containing murine CIITA mutant gene was also constructed successfully. It was proved by flow cytometry that expression of HLA-DR molecules on the surface of HeLa cells and Raji cells infected by Ad-CIITAm were all decreased remarkably in comparison with those of control cells infected by Ad-GFP.Conclusion:This experiment proves that expressed murine CIITA mutant mediated by adenovirus could inhibit the expression of MHC II molecules efficiently.
9.Healing of gingiva after electrosurgical gingivectomy

Objective:To compare the healing of gingiva after electrosurgical gingivectomy with different power outputs and electrodes. Methods:15 rabbits were randomly assigned to 3 groups which used different electrodes: needle electrode, long loop electrode and loop electrode. 1 animal was chosen as control in each group. Free gingiva of anterior teeth had been removed 2 mm in height with different power output and electrosurgical knife. Gingival specimens were harvested for histopathological study on days 1,7,14,and 21 after operation. Statistical analysis was performed on wound healing. Results:Electrical knife with medium power output caused significantly higher degree of inflammation than other techniques(P0.05). Conclusion:When electrosurgery is used on gingiva, there is no significantly difference with different electrodes. Longer observation is needed after gingival electrosurgery with medium power output electrical knife.
10.Effects of transverse arch reconstruction by modified Youngswick osteotomy on young and middle-aged hallux valgus

[Objective]To evaluate the effect of transverse arch reconstruction by modified Youngswick osteotomy on young and middle-aged hallux valgus. [Methods]From November 2005 to November 2008,25 female(36 feet) hallux valgus patients with the average age of 37.7years old(range 22 to 49 years old) were treated surgically.All feet had hypemobility of the first tarsometatarsal joint and collapse of the transverse arch of the foot.In the operation,the first metatarsal head was pushed laterally,downward and rotated downward.Modified McBride procedure was used on the correction for hallux valgus deformity.The X-ray films of all patients were obtained before operation,and 12 months after operation.The hallus valgus angle and intertatarsal angle were measured and analyzed on radiographs.The surgical outcome was evaluated combined with the Gu Xiang-jie's score.All pamameters were statistically analyzed.All cases were followed-up regularly.[Results]Twenty-one cases(28 feet) were followed-up for averaged 15.9 month.Hallax valgus angle reduced from 26.8??1.07? to 11.1??0.29?,intertatarsal angle reduced from 14.9??1.36? preoperatively to 8.3??1.19? postoperatively.The painful callus beneath the forefoot was alleviated or diminished obviously in all cases.Based on Gu Xiangjie'score,the subjective assessments were given as excellent in 16 feet,good in 7 feet,and fair in 5 feet.The good to excellent result was 82.1%.[Conclusion]For the hallux valgus with hypermobility of the first tarsometatarsal joint and transversal arch collapse,modified Youngswick surgery can recover the transverse arch,correct first metatarsal adduct abnormality,and regain the weight-bearing function of the first metatarsal head.Modified Youngswick surgery is an ideal choice for young and middle-aged hallux valgus because of less complications and reliable surgery outcomes.
