1.Pulmonary perfusion for preparation of the acellular matrix

BACKGROUND:The extracelular matrix with removal of cels and soluble proteins can maintain the normal shape of organs and matrix components. OBJECTIVE:To prepare the acelular matrix of lung tissue using perfusion method. METHODS:Forty Wistar rats were randomly divided into two groups: under routine anesthesia, the chest was open to obtain complete lung tissue for construction of rat lung acelular matrix scaffold using Langendorff perfusion model in experimental group, and there was no treatment in control group. Lung tissue color and shape were observed and recorded dynamicaly. A smal tissue from different sites of the lung was taken from each group and observed histologicaly under electron microscope. The elastic fibers and connective tissues were highlighted by a Weigert or von Gieson staining, respectively. Hematoxylin-eosin staining was also employed. RESULTS AND CONCLUSION:After perfusion with 1% sodium deoxycholate, the lung tissue of the experimental group gradualy appeared with a piecewise and lobulated translucent appearance in white color from the inside to the outside, and the lung structure was clear. Eventualy, the lung tissue became uniform white translucent. The Weigert-Von Gieson staining and hematoxylin-eosin staining showed that there were a large amount of cels in fresh lung tissue sections of the control group, including capilaries, fibroblasts and endothelial cels; the cels were arranged neatly, and had complete alveolar structure; the elastic fiber structure was clear, and the colagen fiber was arranged neatly and compactly. In the experimental group, the lung cels nearly disappeared, and the alveolar structure was stil intact but in a loose state; the elastic fibers were preserved wel and the colagen fibers were loosely arranged. These findings indicate that the perfusion method can be used to effectively construct the lung matrix scaffold in rats.
2.Genetic polymorphism of FMO3 and its role in drug metabolism and toxicity.

The flavin-containing monooxygenase 3 (FMO3) is an important hepatic microsomal enzyme. Numerous mutations of FMO3 gene have been reported, and polymorphic varients of the gene have been identified. Several studies indicated that variability in the expression of FMO3 involved in some nitrogen, or sulfur-containing durg metabolism. This review summarizes the genetic polymorphism of FMO3 and its role in drug metabolism and toxicity.

3.Risk Management Evaluations and Countermeasures in Sterilization Supply Department

OBJECTIVE To investigate the application of risk management in the practice of the sterilization supply department.METHODS Through the risk management theory to identify and evaluate the latent risk factor and achieve the purpose of preventing and controlling.To improve the awareness and ability of risk guarding among nurses by strengthening professional training and establish and perfect the regulations,we insisted on "zero fault" management idea and adopted irregular spot-checking and quality overruled system.We also bought the results into bonus-penalty system and continue for the quality improvement.RESULTS It strengthened the risk and quality cognition and the responsibility of the nurses and reduced the occurrences of the small defects and defaults.CONCLUSIONS The risk and quality management is binding with each other.Through the applications of risk management the most high quality sterilized goods are offered to our clinical practice due to the decreasing in risk factors.
4.Diagnoses on Pharmacology Cognizing Among Five-year and Three-year Systems Students

Objective:To analyze and compare pharmacology cognizing between five-year and three-year systems students in medicine to provide materials for teaching innovation.Method:To gather feedbacks from the students and to analyze and induce the cases by EXCEL software.Results:The effectiveness of medicament is the focus.But five-year and three-year systems students concern different problems.57.1% questions raised by three-year system students are about the effectiveness and the effective factors of medicament,while 53.2% by five-year system students are about the effectiveness and the mechanism of medicament.Conclusion: There have been certain differences of pharmacology cognizing between five-year and three-year systems students.
5.The efficacy of superselective renal artery embolization for iatrogenic renal hemorrhage

Objective To evaluate the efficacy of superselective renal artery embolization in the treatment of iatrogenic renal hemorrhage. Methods The clinical data of 31 iatrogenic renal hemorrhage patients who had underwent superselective renal artery embolization were retrospectively analyzed. Results All patients were found to have bleeding site by contrast examination, including pseudoaneurysm in 21 cases, renal arteriovenous fistula in 3 cases, extravasation of contrast media in 4 cases, and pseudoaneurysm and renal arteriovenous fistula in 3 cases. Microcoil embolization was used in 20 patients, and microcoil embolization combined with gelatin sponge was used in 12 patients. Thirty patients got successful embolization once, and success rate was 96.8%(30/31). One patient got successful embolization in the second time. There were no serious complications and no recurrence of renal hemorrhage. Conclusions Superselective renal artery embolization in the treatment of iatrogenic renal hemorrhage has the advantages of exact hemostasis, less trauma and fewer complication, and can retain the normal renal tissue maximumly.

7.A modified technique of total cystectomy and ileal orthotopic neobladder(report of 25 cases)

Objective To present a modified technique of total cystectomy and ileal neobladder. Methods Radical cystectomy and ileal neobladder were performed for 25 male patients. The dome, the upper part or the lateral walls and the base of the bladder were dissected antegradly. After cutting the ureters, the mobilization was then made retrogradly.For the construction of the reservior, the isolated 40 cm ileal segment was opened along its antimesenteric border and then arranged into a W shape. The wall of ileal pouch was created by side-to-side blanket suture of the incised ileum. The ureters were implanted at the two corners of W shape with antireflux nipple. Results 24 of 25 cases of ileal neobladder have been followed up for 2 to 84 months with a mean of 24 months.The operation took 150 to 310 minutes with a mean of 240 minutes. The intraoperative blood transfusion was 0 to 1200 ml.1 patient died of cancer metastasis. 23 patients have achieved excellent daytime continence,whereas only 14 had nighttime continence. The mean bladder capacity for the entire group was 310 ml. Renal functions of 2 patients were mildly abnormal.Hypokalemia occurred in 2.There was no metabolic acidosis.There were 3 cases of hydroureter or hydronephrosis on one side. 1 calculus of neobladder was discovered on BUS. On voiding cystography, no reflux has been noted in all.And no patient suffered from urethral recurrence. Conclusions The modified total cystectomy and ileal neobladder takes less time and is more feasible.
8.AFFINITIES OF PEPTIDE (BNEP) MIMICKING BACTERICIDALITY/PERMEABILITY-INCREASING PROTEIN FOR LIPOPOLYSACCHARIDE AND Lipid A BY BIOSENSOR TECHNOLOGY

To explore the mechamisms of bactericidal neutralizing endotoxin peptide(BNEP), a synthetic peptide mimicking bactericidality/permeability-increasing protein (BPI). The affinities of BNEP for LPS and Lipid A were determined with biosensor technology, and the ability of BNEP neutralizing LPS in vitro was tested by quantitative limulus amoebocyte lysate assay. The results showed that BNEP had high affinities for both LPS and Lipid A. The Kd value for LPS was at the level between 25.8 and 48.8nmol/L and for Lipid A from 11.8 to 21.8nmol/L. When 8?g/ml of BNEP was used, it could completely neutralize the concentration of 2ng/ml of LPS in vitro. It is concluded that BNEP has high binding affinities for both LPS and Lipid A. Our results also suggest that the binding site of LPS is at the glucosaminyl-?1'-6-glucosamine disaccharide of Lipid A. The binding activity of BNEP for LPS is in accord with its neutralizing activity for LPS.
9.Anterior cervical discectomy and intervertebral fusion using endoscopic procedure

Objective To report the results using endoscopic techniques in ce rvical discectomy and intervertebral fusion. Methods From October 2002 to August 2003, 26 patients underwent cervical discectomy and intervertebral fusion using endoscopic techniques. 16 patients had been followed-up more than 3 months, inc luding 4 females and 12 males. The average age was 53.2 years (range, 23 to 65 y ears). The disorders lasted from 3 to 14 months before surgery. There were 3 cas es of cervical injury associated cervical disc herniation (CDH), 8 of cervical s pondylotic myelopathy (CSM), 2 of solitary ossification of the posterior longitu dinal ligament (OPLL), and 3 of radiculopathy. The mean preoperative ADL of Japa nese Orthopedic Association (JOA) score was 7.2. The working channel was inserte d through a 2 cm long incision, the protruded discs or ossified posterior longit udinal ligaments were excised for complete decompression, then an appropriate in tervertebral PEEK fusion cage was implanted. Results All patients obtained endos copic cervical discectomy and interbody fusion successfully. None of cases was c onverted to open procedures. The surgery lasted an average of 120 min (range, 50 to 150 min), mean blood loss was 110 ml (range, from 40 to 140 ml). There was n o complication during operation and no any stimulating symptoms on laryngopharyn x after surgery. However, postoperative hemorrhage of the incision occurred in 1 case, then an injury of a thyroid vessel was found during immediate exploration . The follow-up period extended from 3 to 8 months (mean, 6.5 months), the aver age preoperative ADL of JOA score was 13.1 while the improvement rate was 60.2%. Conclusion The cervical discectomy and intervertebral fusion through endoscopi c approach is able to reduce the soft tissue injury and the incidence of stimula ting symptoms on laryngopharynx, which makes the surgery more safe. The indicati ons for this procedure include cervical disc herniation, cervical myelopathy, ra diculopathy, and traumatic cervical disc injury on C3,4 to C5,6 segments.
10.The comparison of feasibility of different plasma substitutes during preoperative acute hypervolemic hemodilution.

Objective To compare the effects of three different plasma substitutes, 6% hydroxyethyl starch (HES 200/0.5), 4% Gelofusine and 5% Polygeline, on blood coagulation and arterial blood gases and electrolytes during acute hypervolemic hemodilution (AHHD) . Methods Seventy ASA Ⅰ - Ⅱ adult patients undergoing elective orthopedic surgery with the intraoperative blood loss predicted to exceed 500 ml were entered in this study. Radial artery was cannulated for BP monitoring and blood sampling before induction of anesthesia. Anesthesia was induced with fentanyl 3 ?g?kg-1, propofol 2 mg?kg-1 and atracurium 0.5 mg? kg-1 and maintained with enflurane (1-2 MAC) . The patients were mechanically ventilated ( VT= 10 ml? kg-1 , RR = 12 bpm) after tracheal intubation. Internal jugular vein was cannulated after induction of anesthesia for CVP monitoring. After induction of anesthesia the patients were randomized to receive 20 ml?kg-1 of either 6% HES (group H, n = 20), 4% Gelofusine (group G, n = 20), 5% Polygeline (group P, n = 20) or lactated Ringer's solution (group R, n = 10) within 20-40 min. Arterial blood samples were taken before AHHD, at the end of and 30 min after the infusion of substitute for determination of activated coagulation time (ACT), thromboelastography (TEG) , blood gas analysis and plasma electrolytes. Blood volume expansion rate was calculated [ blood volume expansion rate = (Hct before AHHD - Hct after AHHD) / Hct before AHHD] .Results The four groups were comparable with respect to sex, age and body weight. Lactated Ringer' s solution was significantly less efficient in expanding intravascular blood volume than the 3 plasma substitutes, but there was no significant difference in blood volume expansion rate among group H, G and P. CVP increased significantly after AHHD compared with the baseline value before AHHD in group H, G and P (P
