2.Mutual Benefit and Win-win:the Economic Ethical Basis of Harmonious Physician-patient Relationship
Chinese Medical Ethics 1994;0(06):-
It was undoubted that doctor-patient relationship was becoming more and more worse, and this condition lead the doctors, patients, governments feel ve ry awkward.Every related side would be Faliures.Nowadays, the tensional doctor patient relationsh i p is a result of improper value choice and institutional arrangement in the back ground of market economy and high technology development.Building a harmonious doctor patient relationship and become double win is a key issue to affect medical service. It needs government, hospital, doctor and patient to work together. Therefore,we should promote the harmony of doctor-patient relationship in the respects of value choice and institutional arrangement. The action of benefiting others at the medical market includes benefiting for others unselfish and benefiting for others for self under the condition of market economy.Seeking patients' interests even at the expense of the professionals' own interests is the most essential moral criterion and professional obligation.

Childhood hypertensive crisis is a relatively rare emergency event, and is associated with a high risk of morbidity and mortality.Children in this situation need rapid, safe and effective treatment to release clinical symptoms, relieve injuries of target organs, improve quality of life, and reduce mortality. Many kinds of anti-hypertension drugs have been used in children for decades, but most data were extrapolated from studies in adult. There is limited number of randomized controlled trials in children. Therefore it is still difficult to choose drugs for pediatric hypertensive crisis based on evidence-based medicine. This paper sought to summarize the experience of medical management of childhood hypertensive crisis in children.

Objective To evaluate the diagnostic value of multi-slice spiral CT (MSCT)for traumatic spleen and liver rupture. Methods We made a retrospective analysis of 140 cases of liver and spleen injury with clinical manifestations confirmed by operation.MSCT examination results and clinical data were compared. Results Of the 140 cases, male patients outnumbered female ones, and the peak age was 10 - 30 years old.The injuries were most commonly attributed to traffic accident and falling. There were 69 (49%)cases of spleen injury,5 1 (36%) cases of liver injury, and 20 (14%)cases of both.Liver and spleen injuries showed on MSCT examination were liver and spleen laceration, hematoma within the liver and spleen as well as hematoma beneath the envelop. Conclusion MSCT has an important diagnostic value for traumatic liver and spleen rupture and thus can guide clinical treatment choice.

Six cases of Paget′s disease of bone, including 5 males and 1 female, aged (57. 7 ±11. 8) years old, were recruited. Mean duration of disease was (7. 5±6. 5) years. Clinical manifestations were bone pain and bone deformity. The lesions mainly reside in the pelvis and femur. X-ray film showed typical lesion of Paget′s disease of bone, such as impaired bone trabecular structure with coarseness and disorder, cortical thickening, medullary cavity narrowing and skeletal deformation. Bone scan revealed abnormal radionuclide concentration in the involved bone. Serum alkaline phosphatase ( ALP) in 6 patients was increased ( median 235 U/ L). 5 patients received zoledronic acid sodium intravenous infusion therapy. Bone pain was relieved obviously in 5 patients after treatment for 2-3 months. Physical activity was greatly improved, and serum ALP levels significantly decreased.

Chinese Journal of Endocrinology and Metabolism 2014;(8):697-700
A 47-year-old Chinese man with hypophosphatemic osteomalacia suffered from bone pain, difficulty in walking and pseudo-fractures. Biochemical examination showed a lowered serum phosphorus level and an elevated alkaline phosphatase level. The X-ray examination showed pseudo-fractures of multiple ribs, low bone density, biconcave deformities of the lumbar vertebrae, and pseudo-fracture of the right superior ramus of the pubis. A diagnosis of hypophosphatemic osteomalacia was made. The case was treated with oral neutral phosphate solution, calcitriol and Caltrate D. After treatment for two years, biochemical indicators were improved, and pseudo-fractures healed. Clinically, hypophosphatemic osteomalacia is often overlooked or misdiagnosed. In addition, the duration and dosage of these drugs should be appropriate, in order to avoid tertiary hyperparathyroidism or poor response to treatment.

Clinical Medicine of China 2011;27(6):642-644
Objective To evaluate the clinical effect of local POME optical treatmeat in patients with severe vulvovaginal candidiasis (VVC) . Methods Ninety-eight patients, who were diagnosed as severe vulvovaginal candidiasis from January to December 2009 in Beijing Wuzhou Women's Hospital gynecologicalclinic, were enrolled into this study and divided into control and physiotherapy groups randomly. The patients of control group (re =48) were treated with oral Fluconazole and had Miconazde Nitrate suppositories settlement into vagina, while in the physiotherapy group (n =50) the patients accepted local POME optical treatment combined with the same medications as control. The therapeutic effect were evaluated and compared according to the subjective symptoms and VVC scores in both groups. Results The clinical symptoms of physiotherapy group were relieved within 4 hours after the treatment, which were significantly depressed compared to before the treatment (itching score 1.4 ±0.3 vs. 2. 8 ±0. 5,P <0. 05;pain score 1.5 ±0.4 vs. 2. 8 ±0. 3,P <0. 05),while obvious improvements were observed in control group untill 48 hours later (itching score 1. 8 ± 0. 6 vs.2.9 ±0.3,P<0. 05;pain score 1.5 ±0.5 vs. 2. 8 ±0.4,P <0. 05). The VVC scores were significantly lower in physiotherapy group than control after treatment at different timepoints (24 h VVC Score 5. 3 ± 1. 6 vs. 9. 5 ±2. 1,P<0. 05;48 h Score 4. 8 ±1.0 vs. 7.9 ±2. 3,P<0. 05),but no significant difference was observed in the comparison of VVC Scores between two groups 72 h after treatment (P > 0.05). Conclusion Local POME optical treatment combined with drug treatment in severe VVC patients not only has better effect than drug treatment alone, but also relieves clinical symptoms more quickly.
