1.Clinical study on pelvic reconstruction surgery in the treatment of pelvic organ prolapsey

Objective To study the efficacy of pelvic reconstruction surgery in the treatment of pelvic organ prolapsey (POP).Methods 18 patients with POP underwent pelvic reconstruction surgery were selected.Clinical parameters during peri-operation were recorded.After 3 months and 6 months,pelvic organ prolapse quantitative examination(POP-Q) and anatomic improvement in these patients were analyzed.Comparisons of pelvic floor distress impact questionnaire short form 7 (PFIQ-7) and quality of sexual life of chinese women questionnaire in these patients before and after surgery were used to evaluate quality of life and quality of sexual life.Results Median operation time was (90.0 ± 13.2) minutes and median blood loss was (30.0 ± 1.2) ml,all the patients were followed.The objective cure rate was 100%.The mean score of post-operative PFIQ-7 were significantly lower than those of pre-operation (P < 0.01),and those of qualify of sexual life of chinese women questionnaire was no significantly difference (P >0.05).The rate of mesh crosion was 0,and there was no case of de novo urinary incontinence and de novo dyspareunia.Conclusion Pelvic reconstruction surgery is a safe and efficacious management.It can improve qualify of life remarkably and have no influence on sexual quality of life.
3.Novel drugs and novel regimens in treatment of advanced gastric cancer

There is a high prevalence of gastric cancer in China,but most of the patients are diagnosed at advanced stages,when chemotherapy is regarded as an important component of the multimodal treatment.Chemotherapy has demonstrated advantages over best suppor-tive care in prolonging survival and improving quality of life.Over the past 40 years,the clinical responses of two or three-drug combination chemotherapy regimens have been accepted,and 5-fluorouracil,cisplatin and epirubicin have been most frequently used in gastric cancer.Recently,the therapeutic effects of advanced gastric cancer treated by new regimen consisted of docetaxel,irinotecan,oxaliplatin and capecitabine have been further enhanced.The molecular targeted agent,cetuximab and bevacizumab,combined with chemotherapy,would be more promising therapeutic regimens in the treatment of advanced gastric caner.
4.Study on acceptability of interactive teaching mode

Objective To probe into the optometry students' acceptability of interactive teaching mode by the questionnaire survey.Methods Totally 207 optometry students including 42 five-year undergraduate students,17 seven-year program students,95 three-year postgraduate students and 53 three-year junior college students were investigated for their acceptability of interactive teaching mode by the questionnaire survey.Results The results showed that 63.3% of the students preferred traditional teaching mode; 70% of the students would like to accept interactive teaching mode,the others didn't accept it or hesitated to accept it.Lack of self-confidence was the major reason.Conclusions Most medical college students are willing to accept the interactive teaching mode.Interactive teaching mode should be combined with traditional teaching mode.
5.Observation on integrated differentiantion of disease and TCM syndrome for treaing 35 cases of second degree atrioventricular block

Objective:To observe the clinical effects of integrated disease and TCM syndrome on patients with second degree atrioventricular block.Methods:Because of the lentor cardiac rhythm characteristics of econd degree atrioventricular block,the herbs which had the effect in increasing cardiac rhythm were selected and were used in combination of clinical syndrome differentiation to treat 35 cases of second degree atrioventricular block.Results:The total effective rate was 88.6%.Conclusion:Integrated differentiation of disease and TCM syndrome had significant effects on second degree atrioventricular block.
6.DNA Microarray Probe Preparation Using cDNA Fragments Library from HCV Genotype 1

Objective To investigate the feasibility of preparing HCV-1 diagnostic microarray probes using the technique of cDNA fragments library construction. Methods The full-length cDNAs of HCV of subtypes 1a and 1b were digested with restriction endonuclease Sau3A I, and the resulted fragments were cloned into the pMD18-T vector. Positive clones were isolated and identified by sequencing. Results A total of 57 different fragments were obtained, and sequence analysis showed that all the fragments ranging from 200 to 1000bp were specific gene fragments of HCV genotype 1, which can be efficiently used as probes in microarray prepapration. Conclusion The method of preparing microarray probes by construction of cDNA fragments library was effective, quick and simple.
7. Clinical value of hMAM combined with CK19 detection for circulating tumor cells in patients with breast cancer

Objective: To explore the clinical value of cytokeratin 19 (CK19) and human mammaglobin (hMAM) in the detection of circulating tumor cells (CTCs) in peripheral blood of patients with breast cancer. Methods: Expressions of CK19 and hMAM in blood were identified by real-time fluorescent quantitative PCR, and the correlations between CK19, hMAM and the clinicopathologic features were analyzed. Results: In breast cancer patients, the rates of diagnostic positive detection of individual CK19 or hMAM and the combination of these two biomarkers were 45.5% (20/44), 38.6% (17/44) and 56.8% (25/44), respectively. The expression of combined biomarkers was correlated with lymph node metastasis (P = 0.031). The rates of diagnostic positive detection of CK19, hMAM and the combination biomarkers were significantly higher in breast cancer patients prior to treatment than those in the healthy control group (P = 0.000 3, P = 0.000 1 and P = 0.000 1, respectively). After two-cycle chemotherapy, CTCs which were positive at baseline presented as negative in 5 stage III patients and 4 stage IV patients (P = 0.025 3 and P = 0.045 5). However, the number of CTCs-positive patients at stagesI-II decreased no matter using CK19, hMAM or CK19 plus hMAM as biomarker, leaving more CTCs-positive patients in combined biomarker group than that in single biomarker group, but there was no statistical significance. Conclusion: The combined panel of both hMAM and CK19 may serve as representative biomarkers for CTCs, thus it presents potentially significant value for monitoring early metastasis, therapeutic efficacy and prognosis for the patients with breast cancer. 8.Surveillance on drug resistance of Staphylococcus aureus from bloodstream infection in Shandong, 2017-2020

Abstract: Objective To analyze the drug resistance of Staphylococcus aureus in blood samples of children and adults from 50 hospitals in Shandong Province, and to understand the drug sensitivity characteristics of Staphylococcus aureus bloodstream infection (BSI), so as to provide reference for clinical experience. Methods The distribution and drug resistance of Staphylococcus aureus isolated from blood samples from 50 hospitals in Shandong province from 2017 to 2020 were analyzed based on the Cooperative Research Network of Pediatric Bacterial and Fungal Resistance Monitoring in Shandong Province. Meanwhile, the drug sensitivity characteristics of Staphylococcus aureus were divided into children group (<14 years old) and adult group (≥14 years old). The data were analyzed by Whonet 5.6 and SPSS 22.0 with reference to CLSI 2021 M100 document standard judgment results. Results A total of 3 661 Staphylococcus aureus strains were collected from 50 medical institutions in Shandong Province, including 675 in 2017, 870 in 2018, 1 080 in 2019, and 1 036 in 2020. The drug resistance rates of multiple antibiotics in blood culture methicillin-resistant Staphylococcus aureus (MRSA) group and methicillin-sensitive Staphylococcus aureus (MSSA) group were significantly different (P<0.05). There were significant differences in antibiotic resistance rates between adult group and children group (P<0.05). The overall detection rate of MRSA was 27.5%, and no staphylococcus aureus strains resistant to vancomycin, linezolid and tigecycline were found. Conclusion The detection rate of MRSA strains decreased continuously and increased by 2020. The detection rate of MRSA in adult group was lower than that in children group, suggesting that we should pay attention to the monitoring of bacterial resistance in children group, to the management of multiple resistant bacteria and rational use of antibacterial drugs.
9.Results of developmental screening among children at ages of 0 to 3 yearsin Tongzhou District, Beijing Municipality

Objective:
To learn the status and influencing factors of 0-3-year-old children's development screening in Tongzhou District, Beijing Municipality, so as to provide the reference for early intervention measures.
Methods:
Data of children who were born from September 1, 2020 to August 31, 2022 and had received Denver Developmental Screening Test (DDST) in Tongzhou District Community Health Service Center before September 1, 2023 were collected through the Beijing Maternal and Child Healthcare Information System, including basic birth information, mother's situation and DDST results. The screening positive rates of DDST among children were analyzed, and factors affecting positive DDST were identified using a multivariable logistic regression model.
Results:
A total of 27 783 children aged 0-3 years were included, with a median age of 0.91 (interquartile range, 0.71) years. There were 14 320 boys (51.54%) and 13 463 girls (48.46%). There were 2 471 preterm births (8.89%) and 2 248 mothers with health problems during pregnancy (8.09%). The positive rate of DDST was 2.25%. Multivariable logistic regression analysis showed that gender (boy, OR=1.228, 95%CI: 1.045-1.444), preterm birth (OR=2.417, 95%CI: 1.890-3.092), low birth weight (OR=2.583, 95%CI: 1.980-3.371) and maternal health problems during pregnancy (OR=2.012, 95%CI: 1.591-2.545) were factors affecting positive DDST among children at ages of 0 to 3 years.
Conclusions
The positive rate of DDST among children at ages of 0 to 3 years was 2.25% in Tongzhou District. Gender, preterm birth, low birth weight and maternal health problems during pregnancy may affect positive screening for developmental delay.
10.Infective endocarditis with positive anti-neutrophil cytoplasmic antibodies: three cases report with literature review

Objective To analyze the clinical features of infective endocarditis with positive antineutrophil cytoplasmic antibodies ,and compare with ANCA associated small vessel vasculitis(AASV). Methods Three IE patients with positive ANCA were analyzed, and 13 cases from literatures were reviewed. Results Sixteen patients had positive anti-PR3 ANCA, in which 2 cases had both positive (anti-PR3 and anti-MPO ANCA) ANCA. All patients had some clinical manifestations mimic AASV, including fever ( 13/16, 81% ), rash (8/16, 50% ), rapidly progressive glomerulonephritis (7/16, 44% ), splenomegaly (6/16, 38% ). Streptococcal species were identified in 12 patients, and cardiac valvular abnormalities were demonstrated in all patients. All patients except 2, who died of cerebral hemorrhage followed by cerebral infarction, recovered with antibiotic therapy. Conclusion Infective endocarditis sometimes can have the same clinical features as AASV, so physicians should carefully differentiate between them when dealing with patients with positive ANCA antibodies.
