1.Clinical efficacy of the technique of reticular basket through cystic duct stone removal during laparoscopic common bile duct exploration

ObjectiveTo explore the efficacy of the technique of reticular basket through cystic duct stone removal during laparoscopic common bile duct exploration.MethodsFrom January 2008 to February 2011,185 patients received laparoscopic bile duct exploration in our hospital.These patients underwent intraoperative cholangiography (IOC) through the cystic duct with a reticular basket.With the help of a choledochoscope,a reticular basket was used to take out stones for patients who were shadow-positive.Re-cholangiography was then performed to ascertain that no stone was retained in the biliary tract.ResultsOf the 185 patients,60 were found to be positive on IOC.Stones were successfully removed by using a reticular basket through the cystic duct in 45 patients,and through the cystic duct and common bile duct after a micro-incision was made under choledochoscopic assistance in 10 patients.After laparoscopic common bile duct exploration re-cholangiography found no residual stones in these patients. In 5 patients,stones were removed after conversion to open surgery. Conclusions During laparoscopic common bile duct exploration,using reticular baskets via the cystic duct for intraoperative cholangiography (IOC) and removing stones is of great value.
2.Clinicopathological analysis of primary non-Hodgkin lymphoma of lung---28 cases report

Purpose To explore the clinicopathological characteristics of primary non-Hodgkin lymphoma ( PNHL) of lung. Methods The clinical features, morphology and immunohistopathological phenotypes were retrospectively studied in 28 cases of PNHL and re-viewed of the literature. Results The composition of this group of cases is 18 cases of male, 10 cases female. The a median age of pa-tients was 57 years old. According to the WHO classification, all of the 28 cases of in our PNHL series were diagnosed as non-Hodgkin lymphoma ( NHL) , including 17 mucosa-associated lymphoid tissue extranodal marginal zone lymphomas ( MALT) , 5 diffuse large B cell lymphomas ( DLBLC) , 2 NK/T cell lymphomas and 2 anaplastic large cell lymphomas, 1 mantle cell lymphoma ( MCL) and 1 pe-ripheral T cell lymphomas, unspecified, respectively. One-third PNHL patients presented with specific clinical symptoms such as cough, chest pain, dyspnea, and fatigue, Imaging examination showed unilateral or bilateral pulmonary infiltrate, single lesions or multiple nodules, and the lesions always involved with the trachea, bronchus and lung. Follow-up was completed in 16 patients ( range, 3 to 38 months) . 3 cases were conducted with pneumonectomy, among which 2 cases were given postoperative adjuvant chem-otherapy. Seven patients were given chemotherapy alone, and 5 patients did not give any treatment following initial diagnosis. At the time of last follow-up, 13 patients were alive with disease, 2 patients were died. The tumor metastasis in the left inguinal lymph node was found in one patient after 2 years by surgery. Conclusions It was shown that there was no specific clinical manifestations and fea-tures of pulmonary PNHL. Among of them, the MALT is the most common diseases, and the highly aggressive lymphomas many be oc-cur, such as the DLBLC, NK/T cell lymphoma. The diagnosis of PNHL depends on pathological examination. The immunohistochemi-cal staining and molecular pathological technology may helpful for the diagnosis and differential diagnosis of the tumors.

4.Studies on The Supercondensed pBR322 DNA Isolated From Escherichia coli topA-Mutant

The supercondensed DNA, a special kind of topological structure of plasmid DNA, was firstly found in E. coli SD108(topA+ gyrB225). Now, this structure is also found in E. coli DM800(topA- gyrB225). The result indicates that the formation of supercondensed DNA is related with decrease of the activity of gyrase in vivo. Topoisomerase Ⅳ was proved to relax the supercondensed DNA completely in vitro, which suggested that the supercondensed DNA and the supercoiled DNA could transform to each other in cells. The supercondensed DNA samples were analyzed by atomic force microscopy and compared to supercoiled DNA. The results showed that the length of supercondensed DNA decreased about 30% and the width and height of double-strand increased about 60%, which indicates that the structure of double-strand of supercondensed DNA is much more similar to A-DNA than B-DNA. The results also showed that chloroquine intercalation did not change the supercoiling level of supercondensed DNA, but made it knot and compact.
5.Clinical analysis of early postoperative oral enteral nutrition on immune function for colorectal cancer patients

Objective To investigate clinical effects of early postoperative oral enteral nutrition on immune function for colorectal cancer patients,to provide a reference for clinical treatment.Methods We selected 90 colorec-tal cancer surgery patients from January 2012 to April 2015,and they were divided into two groups by random number method.50 cases in the study group were given early oral enteral nutrition,and 40 patients in the control group were given postoperative parenteral nutrition.Before and after treatment,the quality of life,complications,immunology indi-cators,nutrition indicators were compared in two groups.Results In the observation group,the postoperative fever, exhaust time,hospital stay,hospital costs,quality of life scores were (54.29 ±5.76)h,(57.89 ±5.95)h,(10.48 ± 1.63)d,(41 432.85 ±2 165.46)RMB,(79.23 ±5.17)points,which were significantly lower than those in the con-trol group,the differences were statistically significant (t =8.69,P =0.00;t =8.61,P =0.00;t =9.49,P =0.00;t =5.54,P =0.00;t =4.16,P =0.01);7d after the treatment,the IgA,IgG,IgM,CD +4 ,CD +4 /CD +8 in observation group were (1.92 ±0.49)g/L,(11.31 ±2.24)g/L,(0.99 ±0.21)g/L,(41.21 ±5.51)%,(1.72 ±0.32),which were better than those in the control group,the differences were statistically significant (t =2.05,P =0.04;t =2.11, P =0.04;t =2.12,P =0.04;t =3.64,P =0.01;t =2.23,P =0.03).In observation group,postoperative 7d Hb, TRF,ALB,PAB were (113.28 ±13.36)g/L,(3.02 ±0.39)mg/L,(38.97 ±3.15)g/L,(333.15 ±18.35)mg/L, which were better than those in the control group,the differences were statistically significant (t =3.45,P =0.01;t =2.18,P =0.03;t =2.32,P =0.04;t =3.21,P =0.01).7d after operation,the CRP level of observation group was(7.29 ±2.05)mg/L,which was significantly lower than that of the control group,the difference was statistically sig-nificant (t =4.22,P =0.01 ).The incidence rate of postoperative complications in the observation group was 26.00%,that of the control group was 40.00%,the difference was statistically significant (χ2 =4.52,P =0.01). Conclusion Early oral enteral nutrition in patients with colorectal cancer will help to improve immune function, nutritional support,reduce the inflammatory response,which will help postoperative recovery.
6.Study on Quality Standard of Fufang Xiongdan Yinchen Granule

Objective To establish the quality standard for Fufang Xiongdan Yinchen Granule. Methods TLC was employed to identify Pulvis ellis urs, Capillary Wormwood Herbin, Atractylodis Macrocephalae Rhizoma and Glycyrrhizae Radix, and HPLC was used to determine the content of sodium tauroursodeoxycholic acid in Fufang Xiongdan Yinchen Granule. Kromail 100-5 C18 column (4.6 mm× 250 mm, 5 μm) in an oven at 25 ℃ was need, with a mobile phase consisting of methanol-0.03 mol/L sodium dihydrogen phosphate solution (60∶40, pH=4.4) and UV detector at 210 nm, the flow rate was 1.0 mL/min. Results Pulvis ellis urs, Capillary Wormwood Herbin, Atractylodis Macrocephalae Rhizoma and Glycyrrhizae Radix can be detected by TLC. The linearity of sodium tauroursodeoxycholic acid was obtained in the range of 0.182 1-1.821 mg (r=0.999 98). The average recovery was 100.39%, and RSD was 1.70% (n=6). Conclusion The method is simple, accurate, specific, and can control the quality of Fufang Xiongdan Yinchen Granule effectively.

Objective To study the effects of arsenic trioxide ( As2 O3 ) on inhibiting the proliferation of pancreatic carcinoma cell lines. Methods The inhibiting rate of As2O3 and Fluorouracin(5-Fu) ,Gemcitabine(GEM) on pancreatic carcinoma cell lines PC-3 were detected by using CCK-8 assay. Results As compared with 5-Fu、GEM,the inhibiting rate of As2O3 was the highest one( P < 0.01 or P < 0.05). Conclusion As2O3 can inhibit pancreatic carcinoma cell lines PC-3 effectively in vitro. The effects of As2O3 on inhibiting the proliferation of pancreatic carcinoma cell lines was stronger than 5-Fu and GEM. This is possibly due to the extensive and unique anticancer mechanism of As2O3.
8.Clinical efficacy of interventional therapy on children with congenital heart diseases

Objective To summarize clinical outcomes of interventional therapy on children with common congenital heart diseases(CHD).Methods A retrospective study was conducted.One hundred and fourteen patients with CHD were selected as our subjects,who underwent catheter interventional therapy in the Second Affiliated Hospital of Zhengzhou University from Jan.2004 and Dec.2012.The size of occluder was chose according to intraoperative echocardiography or cardiac imaging measurements,and occluder was released under assisted monitoring by subtraction angiography or cardiac ultrasound.Results There are all together 112 patients got the therapy successfully,2 cases failed(occluder detachment),and the success rate of operation was 98.2%.After the success of interventional,echocardiography examination showed that 11 cases were with star point across shunt,but the function of the around valve was not affected.One months after operation,echocardiography examination showed star point across shunt of 11 cases were disappeared,and no occluder was shifted as well as no thrombosis formed.Three months after operation,chest radiograph showed pulmonary congestion decreases and heart shadow was shrink.Thirty-eight cases were with three tricuspid regurgitation before operation and 32 cases were without reflux at 3 months after operation,and 6 cases relieved significantly.The patients were followed up for 6 months or 3 years,activity endurance was significantly improved than that before operation.No occluder was shiftand hemolysis and arrhythmia occurred.Meanwhile,No thrombosis or embolism occurred.Conclusion Interventional treatment for children with congenital heart disease is proved as a safe,effective methods and it have broad prospects in clinical application.
9.MicroRNA expression profiling in male and female model mice after renal ischemia-reperfusion injury

BACKGROUND:Renal ischemia-reperfusion injury has been shown to exhibit gender difference, but its precise mechanisms deserve further investigations. OBJECTIVE:To investigate the differential expression of microRNAs in the kidney between female and male mice in order to study the effects and mechanisms of microRNA in pathogenesis of ischemia-reperfusion injury between different genders. METHODS:Male and female mice received kidney ischemia for 45 minutes and reperfusion injury for 24 hours. Simultaneously, male and female sham surgery groups served as controls. The microRNA gene chip technology was used to detect the differences of microRNA expression in the kidney of male and female mice at 45 minutes after ischemia and 24 hours of reperfusion as wel as after sham surgery. The threshold of difference in expression among samples was double. RESULTS AND CONCLUSION:Five microRNAs were up-regulated between female and male ischemia-reperfusion injury groups. Twenty-nine microRNAs differential y expressed in the female ischemia-reperfusion group and female sham surgery group, including 25 up-regulated microRNAs and 4 down-regulated microRNAs. Thirty-eight microRNAs differential y expressed in male ischemia-reperfusion injury group and male sham surgery group, including 9 up-regulated microRNAs and 29 down-regulated microRNAs. 102 microRNAs differential y expressed in the female sham surgery group and male sham surgery group, including 22 up-regulated microRNAs and 80 down-regulated microRNAs. Results suggested that there was differential expression in microRNAs in the kidney before and after renal ischemia-reperfusion in male and female mice. These differential y expressed microRNAs may be lead to different sensitivity and tolerance to the ischemia-reperfusion injury in the kidney of male and female mice.
10.Effects of thymopeptides on cell immune function of patients with malignant tumor under radiotherapy

Objective:To study effects of large dose thymopeptides on T cell subpopulations of patients with malignant tumor under radiotherapy.Methods:Fifty one patients with malignant tumor under radiotherapy were divided into 2 groups with 100 mg and 200 mg thymopeptides respectively.The patients we re given thymopeptides,100 mg/d or 200 mg/d,iv, for 10 days.The positive percent ages of CD4,CD8,CD25 and CD56 in T cells of peripheral blood before and after thymopeptide treatment were determined by flow cytometry.Results:The positive percentages of CD4 and CD25 in T cells of peripheral bl ood after 100 mg/d thymopeptide treatment were significantly higher than those befor e thymopeptide treatment (P<0.05),while those of CD4,CD8,CD25 and CD56 in T cells of peripheral blood after 200 mg/d thymopeptide treatment all increased significantly (P<0.05 or P<0.01). Conclusion:These results suggest that large dose of thymopeptides can increase i mmune function of patients with malignant tumor under radiotherapy,and the curat ive effect of 200 mg/d thymopeptides is better.
