1.Application of endotracheal intubation in the rescue of children with severe trachea and bronchial foreign body.

OBJECTIVE:
To discuss the application of endotracheal intubation in the rescue of children with severe trachea and bronchial foreign body. By observing the patient's clinical pathological characteristics, the diagnosis and treatment experience were summarized.
METHOD:
A retrospective analysis of 10 children with severe tracheal and bronchial foreign bodies were performed. They were given emergency intubation and cardiopulmonary resuscitation to relieve respiratory and circulatory failure after reaching hospital. After the restoration of spontaneous respiration, oxygen saturation increased by more than 90 percent and vital signs stable, the foreigh body was removed with bronchoscopy. The role of endotracheal intubation before and after the treatment of tracheal and bronchial foreign bodies in children was observed.
RESULT:
Ten cases of children were rescued successfully. No one died and the complications such as cerebral palsy, pneumothorax and pneumomediastinum didn't happened.
CONCLUSION
For the children with severe tracheal and bronchial foreign body, endotracheal intubation can establish effective ventilation in a shorter period of time. The earlier endotracheal intubation, the shorter time required and the higher success rate. Endotracheal intubation can increase the survival rate of children with cardiopulmonary resuscitation, and can reduce disability.

2.Study on the influence of the postoperative hepatic function in hemihepatectomy for hepatoma

Objective To explore the clinical significance and safety in hemihepatectomy for hepatoma without inflow occlusion.Methods A total of 62 patients with liver cancer were divided into 2 groups.The inflow was occluded in group A (n = 28) and not occluded in group B (n = 34) during hepatectomy.Subsequently,the influence of the two approaches on the parameters including intraoperative blood loss,postoperative hepatic function and complications incidence rate was comparatively analyzed.Results Hepatectomy was successfully performed in the two groups.There was no significant difference between the two groups in intraoperative blood loss (P > 0.05).Significant difference existed between the two groups in postoperative hepatic function and complications incidence rate (P < 0.05).Conclusions The hemihepatectomy for hepatoma without inflow occlusion can reduce complications incidence rate and profit the recovery of postoperative hepatic function parameters.
3.Metrological analysis of literatures on stem cell transplantation for treatment of diabetic foot from 2004 to 2009 in China

Objective To summarize the literature of autologous stem cell transplantation (ASCT) for the treatment of diabetic foot from 2004 to 2009 in China through analyzing its development, the main operative methods as well as the future development tendency. Methods Papers about breast reconstruction from 1987 to 2008 on Chinese Medical Current Content (CMCC) and Chinese Bio-medical Literature Database (CBMdisc) were metrologically analyzed according to journal, institute, operative method, time, content and so on. Results In all 105 papers collected, the first place in terms of journal, institute, author, area, operative method was as follows:Chinese Journal of Clinical Rehabilitation (6 papers), Beijing Capital Medical University (10 papers), Gu Yongquan (9 papers), Beijin (17 papers). Autologous bone marrow stem cell transplantation (ABMSCT) and autologous peripheral blood stem cell transplantation (APBSCT) are the main operative methods. Conclusions This paper provides a credible resource for further study through analyzing papers published in China on stem cell transplantation for treatment of diabetic foot in recent years. ABMSCT and APBSCT must be the main trends in the future. Before formulating strict indications, ABMSCT and APBSCT will be the main operation modes of ASCT. Quantifying indications of ASCT and coupling with other drugs in order to make better role of ASCT will be the main subject of the study.
4.Disease perception and awareness investigation of secondary osteoporosis in patients with chronic obstructive pulmonary in Urumqi

ObjectiveTo evaluate perception and awareness of disease knowledge in patients with chronic obstructive pulmonary disease (COPD) in rural area of Urumqi city.MethodsA survey was made to collect 121 patients with chronic obstructive pulmonary disease in out-patient and hospitalization of Traditional Chinese Medicine Hospital of Xinjiang Medical University in 2011.Questionnaire data were coded and entered into Microsoft EpiDate by two persons independently,with SPSS 17.0 version statistical package was used for statistical analysis.ResultsAmong 121 sampling subjects,104 participants completed the questionnaire qualifiedly.Among these patients,Male occupied 65.4% (68/104),female occupied 34.6% (36/104).Patients who know that post-menopausal women were easier to suffer osteoporosis totaled 42 cases (40.38%),patients who know that bedridden persons were easier to suffer osteoporosis totaled 42 cases (40.38%),patients who know that female was more likely to suffer than male totaled 65 cases (62.5%),who think the disease is related to the gender totaled 76 cases (73.08%),who think it is unlikely to develop osteoporosis after fracture healing totaled 13 cases (12.5%).Patients who do not think hat low back pain is the common manifestation of osteoporosis totaled 23 cases (22.12%),Patients who do not think hump and fracture are the common osteoporosis performance totaled 14 cases (13.46%) and 4 cases (3.85%) ; patients who know that osteoporosis is often accompanied by bone spurs totaled 33 cases (31.3%),patients who know that bone mineral density is a reasonable detection of osteoporosis totaled 62 cases (59.61%).Patients who know about alendronate phosphate (such as Fosamax,etc.) totaled 12 cases (11.76%),patients who know that commonly used TCM medicines for treating osteoporosis are Eucommia and Epimedium totaled 5 cases (4.90%).ConclusionCOPD is prone to cause osteoporosis,but patients are lacking the awareness of osteoporosis.

Objective:To explore the effect of revision endoscopic sinus surgery (RESS) on quality of life in the patients with chronic rhinosinusitis with nasal polypsis (CRSwNP).Method:To survey and evaluate 60 cases of RESS patiens (treatment group) and 120 normal ones with physical examination (Control group) through the medical outcomes survey short form questions (MOS SF-36) and the Sino-Nasal Outcome Test-20 (SNOT-20), and comparison and analysis of the two groups results which we got were carried out.Result:With SF-36 scales for evaluation of quality of life, the results show that: the scores of CRSwNP patients (treatment group) without surgical treatments with RESS were significantly lower than that of the control group(P<0.05); while use of SNOT-20 scales in evaluation of the properative treatment group patients, the results were significantly higher than that of the control group(P<0.05). While the CRSwNP patients with surgical treatments with RESS for 6 monthes, not only with SF-36 scales but also with SNOT-20 scales,there was no significant difference between the two groups(P>0.05 ).Conclusion:RESS may obviously improve the clinical symptom of CRSNP patients. The SF-36 and SNOT-20 assessment scales could reflect the patient's QoL change.
6.Roles of probiotics in regulating serum ghrelin levels and protecting pulmonary function of children with acute lung injury

Objective To investigate the regulatory effects of probiotics on ghrelin and their pro-tective effects on pulmonary function of children during acute lung injury.Methods Fifty children with acute lung injury were recruited form Henan Provincial People′s Hospital during 2010 to 2013 and randomly divided into two groups.In addition to regular therapy, those who received probiotics treatment were set up as the probiotics treatment group and the others were considered as the control group.Levels of ghrelin, sur-factant protein A( SP-A) , TNF-αand IL-6 in serum samples were measured before and 10 days after the cor-responding treatments.Each patient was analyzed by pulmonary function test and echocardiography.The vol-ume to peak expiratory flow ( VPEF) , ratio of time to reach peak tidal expiratory flow to total expiratory time ( TPTEF/TE) , mean arterial pressure ( MAP) and pulmonary artery pressure ( PAP) were recorded for sta-tistical analysis.Results The levels of SP-A, TNF-αand IL-6 in serum samples from the probiotics treat-ment group were decreased on day 10 as compared with those of the control group [(195.9±54.3) ng/ml vs (245.5±56.4) ng/ml, (382.0±56.4) pg/ml vs (440.7±46.8) pg/ml, (221.5±62.6) pg/ml vs (303.5 ±83.9) pg/ml;P<0.05] .Children from the probiotics treatment group showed higher values of VPEF and TPTEF/TE on day 10 as compared with those without probiotics treatment [(25.3±4.5) ml vs (23.8±4.6) ml, (29.5±4.3) %vs (26.8±4.7) %;P<0.05].Treatment of children with probiotics increased MAP and decreased PAP [(74.5±6.7) mmHg vs (71.9±6.9) mmHg, (25.5±6.4) mmHg vs (29.1±6.2) mmHg] .The levels of ghrelin in patients received probiotics treatment were higher than those of children without probiotics treatment [(329.3 ±49.6) pg/ml vs (296.6 ±48.5) pg/ml].The levels of ghrelin showed negative correlations with the levels of SP-A, TNF-α, IL-6 and PAP on day 10, but positive correla-tions with TPTEF/TE and MAP (P<0.05).Conclusion Use of probiotics to children with acute lung inju-ry could regulate the level of ghrelin, alleviate the inflammation of lungs and improve the recovery of lung function and circulation.Therefore, probiotics treatment had positive effects on the protection of pulmonary function.
7.Clinical effect of Radical TUR-BT combined with intra-arterial chemotherapy for localized muscle-invasive bladder cancer

Objective To evaluate the clinical efficacy of Radical TUR-BT combined with intra-arterial chemotherapy in the treatment of stage T2 N0 M0 localized muscle-invasive bladder cancer (MIRC).Methods The clinical data of 91 patients with T2 N0 M0 localized myometrial invasive bladder cancer admitted from January 2010 to December 2015 in the people's Hospital of Xinjiang Uyghur Autonomous Region were retrospectively analyzed,76 cases were males,15 cases were female,average age was 63.2 years old,age range 39-91 years old.Patients were divided into BPT group (n =34) and RC group (n =57) according to different surgical methods.Patients in the BPT group underwent Radical TUR-BT + intra-arterial chemotherapy,and patients in the RC group underwent radical cystectomy + urinary diversion.The median follow-up time,2-year progression-free survival,5-year disease-specific survival rate,overall survival and Karnofsky performance status (KPS) were compared between the two groups.Measurement data were expressed as mean ± standard deviation (Mean ± SD),t test was used for comparison between groups;and Chi-square test was used to compare the count date between groups.Using KaplanMeier method to depict disease-specific survival time in two groups.Quality of life evaluation using Karnofsky score,Evaluate the quality of life and health status of the two groups 3 months before and after treatment.Results The median follow-up time of BPT group and RC group were 35 months and 33 months.The 2-year progression-free rates in the BPT and RC groups were 73.5% and 75.4%,respectively,and the difference was not statistically significant (P =0.758).The 5-year disease-specific survival rates of the two groups were 72.4% and 74.8%,respectively,and the difference was not statistically significant (P =0.780).The overall survival rates of the two groups were 61.8% and 63.2%,respectively,and the difference was not statistically significant (P =0.103).There was no significant difference in preoperative KPS score between the two groups (P =0.652).The KPS score of the BPT group was significantly higher than that of the RC group at 3 months after surgery.The difference was statistically significant(P =0.034).Conclusions For patients with localized MIBC with clinical stage of T2 N0M0.Radical TUR-BT combined with intra-arterial chemotherapy has similar effects to RC.This treatment can effectively improve the prognosis,preserve the bladder,and improve the quality of life for patients.

OBJECTIVE:

9.Influence of metoprolol on elderly patients with chronic heart failure and serum IL-12 and BNP levels

Objective To explore the clinical efficacy of metoprolol on elderly patients with chronic heart failure (CHF)and its influence on serum interleukin-12 (IL-12)and brain natriuretic peptide (BNP)levels.Methods A total of 86 patients with CHF served as study objects and ran-domly divided into observation group and control group,43 cases for each.Routine medicines were given to control group while observational group added with metoprolol for 12 weeks.Clinical effica-cy and changes of serum IL-12 and BNP levels were observed.Results Total response rate in obser-vation group was 93.02%,evidently higher than in control group (P <0.01),and there were sig-nificant differences before and after treatment and between two groups in blood pressure,heart rate and left ventricular ejection fraction (LVEF)(P <0.01).Conclusion Basic therapies concomitant with metoprolol have better clinical efficacy than single application of basic therapies,and can more effectively regulate serum IL-12 and BNP expressions and improve the internal environment of body of elderly patients with CHF.
