1.Anesthesia for surgery of spinal tumors

[Objective]To summarize the clinical experience on anesthesia techniques for surgery of spinal tumors.[Method]A total of 546 patients undergoing surgery of spinal tumors were retrospectively studied,and the complications of surgery were also collected.[Result]There was no perioperative death.The average degree of blood loss was 1997ml per patient who usually required a large volume of blood transfusion.Acute pumonlary edema were occurred in 3 patients who were suspected to develop larynx edema immediately after operation.A perioperative visual loss following cervical spine surgery was also founded.[Conclusion]It is important to prevent and minimize blood loss by varied techniques during operation.Peventing adverse effect of operative position on patients and protecting their airway are also crucial for spinal surgery.
2.The MRI diagnosis of tangential osteochondral fracture of the patella

Objective To investigate the value of MRI diagnosis in tangential osteochondral fracture.Methods 1 7 patients with tangential osteochondral fracture were examined by MRI,and the related signs were analyzed,including the subluxation or disloca-tion of patellofemoral joint,the injury or fracture of bone and cartilage,the injury of meniscus,the injury of medial patella retinacu-lum and ligament,the effusion articular capsule.we investigated the role of X-ray film,CT,MRI,compared with arthroscopy.Re-sults MRI showed subluxation of patellofemoral joint(Ⅰtype 12 cases、Ⅱtype 3 cases、Ⅲ type 1 case)in 1 6 cases,1 case had mild move of patella.All of 1 7 cases were found fracture in bone or cartilage of the inferior medial part of patella and the anterior lateral part of lateral femoral condyle.All cases were found the injury of medial patella retinaculum(Ⅰtype 7 cases、Ⅱtype 9 cases、Ⅲ type 1 case),3 cases were found the injury of MCL,2 cases were found the injury of ACL.3 cases were found the injury or tear in anteri-or horn of meniscus.1 1 cases were found articular capsule filling with simple effusion,5 cases were found articular capsule filling with hemorrhagic effusion,1 case was found articular capsule filling with hemorrhagic effusion and fat granule.Conclusion MRI can clearly show tangential osteochondral fracture and its accompanying changes:the subluxation or dislocation of patellofemoral joint, the injury or fracture of bone and cartilage,the injury of meniscus,the injury of medial patella retinaculum and other ligament,the articular capsule effusion.
3.The clinical characteristics of 169 cases of severe fever with thrombocytopenia syndrome

Objective To analyze the clinical characteristics of severe fever with thrombocytopenia syndrome (SFTS) so as to improve the recognition of the emerging infectious disease.Methods A retrospective analysis was performed upon clinical manifestations,laboratory test results and prognostic features of 169 patients with SFTS admitted to the 154 Hospital,Chinese People's Liberation Army from October 2010 to May 2011.The patients were divided into moderate disease group and severe disease group according to the prognosis.The differences between two groups were compared to explore the prognostic indicator of severe type.Results All patients with SFTS inhabited in hilly ground with history of field work.The main clinical symptoms were severe fever (98.8%,167/169),headache (52.1%,88/169),muscle soreness (95.9%,162/169),nausea (73.4%,124/169),vomiting (67.5%,114/169),coughing (61.5%,104/169),etc.Superficial lymph node enlargement with haphalgesia was observed in 45.0%(76/169) patients.Petechia or ecchymosis was observed in 36.7% (62/169) patients.Critical patients were mainly aged > 60 years,associated with nerve and circulatory system syndrome,prominent hemoptysis,dyspnea,and nearly 73.3% (22/30) of severe thrombocytopenia.Serum levels of aspartate aminotransferase,creatinine and urea nitrogen were significantly higher in severe disease group than those in moderate disease group [235 (47-1750) U/L vs 88 (14-2000) U/L,997 (281-2601) U/L vs 399 (26-2633)U/L,101 (62-291) μmol/L vs 70(26-205) μmol/L,7.0(2.2-20.0) mmol/L vs 4.8(1.4-18.5) mmol/L,all P values <0.001].Conclusion Nerves system syndrome,transaminase and urea nitrogen are risk factors of prognosis of severe SFTS to whom deserves paying attention.
4.Establishment of a mouse model of chronic pulmonary infection with Pseudomonas aeruginosa and analysis of its inflammatory response

Objective To research the chronic respiratory infectious diseases caused by Pseudomonas aeruginosa need to objectively the reflection of the real environment in the body, so established a mouse model of chronic pulmonary infection with Pseudomonas aeruginosa and analyzed it's inflammatory response. Methods Establishment of chronic bronchial infection mice model that were inoculated with Pseudomonos aeruginosa-laden agarose beads, and analyzed its inflammatory response by detected the cytokines and MMP-2, and a differential cell count and a total leukocytes count were performed as well. Results Pseudomonas aeruginosa had been detected after infection, and there were changes in pathological. Pulmonary inflammation appeared in 1 d and reached near baseline levels by 7 d after inoculation. It was verified that the peak of inflammatory reaction in Pseudomonas aeruginosa infection is in 2-3 d; the mice did have detectable levels of circulating matrix metalloproteinase-2 ( MMP-2 ) after infection with Pseudomonas aeruginosa. MMP-2 concentrations in the blood serum peaked at 3 d after inoculation. It is indicated that Pseudomonas aeruginosa can initiate a certain degree of pulmonary fibrosis on the basis of the pulmonary inflammation. Conclusion In this study, chronic bronchial infection animal model affected by Pseudomonas aeruginosa was established successfully. Base on this animal model, we can do the pathogenicity and drug resistance of Pseudomonas aeruginosa further study.
5.Construction of student-centered learning system for undergraduate clinical training in otolaryngology-head and neck surgery

The author proposed student-centered learning system in otolaryngology-head and neck surgery for undergraduate clinical training after exploration and intentions.Four mutual impacted frames were built including integration of teaching philosophy,visualization of training methods,diversification of educational targets and interaction of training courses.Endoscopic navigated learning and multimedia aided training were applied,respective teaching purposes were set and various clinical training courses were introduced to students in their learning of otolaryngology,which were believed to help develop more medical talents with higher comprehensive qualities and better clinical skills.
6.Teaching experience on experiment course of the human dermatoglyphics

Based on the teaching practice of human dermatoglyphic the opinions about the teaching reform and curriculum construction have been made in this paper.

To improve the quality of postgraduate students,school of medicine of Shanghai Jiao Tong University have offered medical molecular genetics course to postgraduates and obtained good teaching efficiency. More than ten professors gave lectures on the academic foreland,leading students to find and solve academic problems and training practical and innovative ability of students. Accord-ing to the questionnaires of evaluation of the course and suggestions from postgraduates in the recent two years,some recommendations for the further teaching reform were proposed,such as using multi-plex teaching models,strengthening communication for teaching information with the students,em-ploying comprehensive evaluation methods and strengthening coordination and management of the course.
8.Cloning, Expression, Purification of a Fusion Protein of SARS-CoV Nucleocapsid and Spike Protein Fragments and Its Immunological Characteristics

The spike (S) and nucleocapsid (N) proteins, which are responsible for viral binding to cell surface receptors and the formation of ribonucleoprotein complexes during virion assembling, are major structure proteins of severe acute respiratory syndrome coronavirus (SARS-CoV). The expression of recombinant protein may give more accurate result for detecting SARS-CoV infection. A novel fusion protein,comprising of two fragments of N and S proteins from SARS-CoV, was prepared. Our computer-assisted analysis suggested that the immunodominant domains were located in the amino acid residues 1-227 of N protein and 450-650 of S protein, further the fusion of the two fragments did not change the immunochemical characteristics. The complementary DNA(cDNA) encoding N1-227 fused with S450-650 was obtained by sequence overlapping extension (SOE), and named NLS. It was cloned into pET-28a ( + ), an expression vector for His-tag fusion protein. This new constructed fusion protein was prokaryotic expressed in E. coli,and purified by metal chelate affinity chromatography with the purity over 95 %. The purified fusion protein was identified by anti-His monoclonal antibody and convalescence SARS patients serum. The NLS protein based ELISA showed that NLS maintained appropriate antigenicity and specificity to react with the sera of convalescent SARS patients. The functional NLS protein were successfully expressed and purified. And the fusion protein based ELISA can be used for detection of antibodies (Abs) against the S and N proteins of SARS-CoV. It may provided a novel diagnostic tool and have the potential application in developing of anti-SARS vaccine.
9.Surgical and orthodontic management of maxillary palatal impacted canines guided by cone-beam compu-ted tomography

Objective:To assess the guidance of 3D reconstruction in the surgical and orthodontic management of maxillary palatal impacted canines with cone-beam computed tomography (CBCT).Methods:22 adolescent patients with 26 impacted maxillary conines were examined by CBCT.CBCT 3D reconstruction was used to observe the impaction of the canines.Measurements were taken to evaluate spatial relationship between the canine and adjacent structures and resorption of neighboring incisor roots.Results:CBCT was effective to localize the impacted canines.All of the maxillary palatal impacted canines inclinated mesially,angles of impacted ca-nines to occlusal plane were 61.1°±18.1°,there were 8,5,3 and 10 impacted canines found in classⅠ,Ⅱ,Ⅲ and Ⅳ mesial mal-position respectively.3 adjacent lateral incisors and 2 central incisors were extracted because of severely root resorption.Normal occlu-sion was achieved for 26 impacted canines by the traction of the canines into dental arch and reaching the occlusal plane.Conclusion:CBCT can spatially show the palatal impaction of maxillary canines and the relationship to adjacent tooth,can guide the treatment plan and traction direction,therefore increase therapeutic effect.
10.Analysis of 18 cases which is the nasal sinuses adenoid cystic carcinoma.

OBJECTIVE:
To explore the treatment and possible prognostic factor of nasal sinuses adenoid cystic carcinoma.
METHOD:
Retrospectively analysed the records of 18 patients with complete clinical and pathological data,which including 4 patients given up treatment, 5 patients taken surgical treatment and 9 patients taken surgical treatment as well as radiotherapy and chemotherapy.
RESULT:
Fifty percent of the patients got 2-year survival and 3 cases of death due to intracranial tumor invasion and 2 patients died of the disease distant metastasis.
CONCLUSION
If patients got Nasal sinuses adenoid cystic carcinoma, they should take comprehensive treatment based on surgery, in order to improve the survival rate. The prognosis depends on the tumor early detection and early treatment, the sooner the treatment, the better.
