1.THE SEGEMENTAL DISTRIBUTION OF THE AFFERENT NERVES OF THE GENITAL GLANDS AND SANYINCHIAO REGION IN RABBITS—A STUDY WITH HRP METHOD AND FLUORESCENCE METHOD

Eighteen adult rabbits(0 9and 0 9, body weights 1.5~2kg) were selected for the present study. A solution of 5~15% HRP(RZ=2.9) 10% EB (Evans Blue) and 7% TY (Titan Yellow)was respectively injected into the subserosa of genital glands in fourteen rabbits and a solution of 35% HRP into the tibial nerves of the Sanyinchiao regions in other four rabbits. The uptake and retrograde transmission of HRP and of EB and TY in the afferent fibres of both the somatic and visceral nerves were traced to the spinal ganglia. In addition, anterograde transport were verified by using ARG method in two cats. The range of segments where the neurons give off these two afferent fibres overlap each other. The results were as follows: 1. The labelled sensory neurons from the region of the testis where HRP was injected were observed more concentrated in the spinal ganglia T11~L4, and the ovary in the spinal ganglia T10~L14. 2. The labelled neurons from the region of Sanyinchial were found more concentratedly in the spinal ganglia L_4~S_3. 3. The ranges of distribution of labelled neurons from the two groups of afferent fibres overlap in segment T7~L7.
2.ORIGINS OF THE AFFERENTS TO THE HIPPOCAMPUS IN THE RAT——HRP METHOD

The afferent projections to the hippocampus were studied with HRP. Thirty three per cent solution of HRP was slowly injected into the dorsal or ventral hippocampus on its right side in 25 albino rats and HRP-labelled cells were found in the following nuclei:1. On the ipsilateral side: entorhinal cortex, medial septal nucleus, supramammillary nucleus, submammillothalamic nucleus, dorsal raphe nucleus, median raphe nucleus, locus coeruleus, diagonal band and nucleus of superior colliculus.2. On the contralateral side: areas CA_3 and CA_4 of hippocampus and locus coeruleus.

Ten adult rabbits (7 and ♀3; body weights 1.5~2 kg) were selected for thee present study. A solution of 2~10% HRP (RZ=2.9) was injected into the subserosa of the caecum in seven rabbits and a solution of 5~10% HRP into the deep peroneal nerve of Tsusanli (足三里) region in the other three. The uptake and retrograds transmission of HRP in the afferent fibres of both the somatic and visceral nerves were traced to the spinal ganglia. The range of segments where the neurons from which these two afferent fibres originate overlap each other. The results are a follows:1. Labelled sensory neurons from the region of the caecum where HRP was injected are observed in the spinal ganglia C_8~S_3 with a higher concentration in T_(11)~L_2.2. Labelled neurons from the region of Tsusanli are found in the spinal ganglia L_1~S_3 with a higher concentration in L_6~S_2.3. The ranges of distribution of labelled neurons from the two groups of afferent fibres overlap in the segments L_1~S_3.4. Most of the labelled cells are small and medium in size and the Iabelled cells are found more concentrated in the lumbosacral segments.
4.Diagnostic value of contrast-enhanced harmonic endoscopic ultrasonography for pancreatic cystic lesions

Objective:To evaluate diagnostic value of contrast-enhanced harmonic endoscopic ultrasonography (CEH-EUS) for pancreatic cystic lesions.Methods:Endoscopic and clinical follow-up data of patients with pancreatic cystic lesions diagnosed by EUS in Department of Gastroenterology of the First Affiliated Hospital of Naval Medical University with CEH-EUS video from March 2013 to April 2020 was retrospectively analyzed.Results:A total of 36 patients were included. There were 16 cases of serous cystadenomas (SCA), 10 cases of mucinous cystic neoplasm (MCN), 5 cases of intraductal papillary mucinous neoplasms (IPMN, 3 with complex type, 2 with main pancreatic duct type) and 5 cases of pancreatic pseudocyst (PPC). 87.5%(14/16) of SCA and 86.7%(13/15) of MCN+ IPMN had hyperenhanced cystic wall with obvious peak and similar washout as surrounding tissue, whereas only 20%(1/5) PPC had hyperenhanced cystic wall. The hyperenhancing effect of PPC was significantly lower than that of SCA and MCN+ IPMN ( P=0.0035 and P=0.0048, respectively ). Mural nodules were detected in 17 cases of pancreatic cystic lesions by EUS, of which 3 cases had hyperenhanced mural modules and 14 cases had hypoenhanced mural nodules by CEH-EUS. Patients showing hyperenhanced mural modules were all finally diagnosed as pancreatic malignancy (1 IPMN, 2 MCN), and the accuracy was 100%. Conclusions:CEH-EUS can have a obvious advantage of differentiating pseudocyst and other pancreatic cystic lesions, while not very useful for differentiating SCA and MCN. Pancreatic cystic lesions showing hyperenhanced mural nodules under CEH-EUS may imply malignancy potential.
5.A study of a trial withdrawing of the urethral catheter in patients with acute urinary retention caused by benign prostatic hyperplasia under the treatment of alpha-adrenergic receptor blocker

Objective To observe the influence of using alpha-adrenergic receptor blocker and catheterization time on the success rate of a trial without catheter (TWOC) in patients with acute urinary retention (AUR) caused by benign prostatic hyperplasia (BPH).Methods The 124patients from January 2007 to December 2009,aged 62-90 years (mean age 71.3 years),diagnosed as AUR caused by BPH in emergency room,were enrolled in this study.They were randomized to two groups:(1) Medication group:60 cases,who were given alpha-adrenergic receptor blocker 4 mg daily for 2-3 days before TWOC.Among them,there were 30 cases with catheterization for 3 days,and the rest 30 cases for 7 days;(2)Control group:64 cases,who were without alpha-adrenergic receptor blocker.Among them,there were 30 cases with catheterization for 3 days,and the rest 34 cases for 7days.The urine culture was made when the catheter was drawn out.Results (1)The success rate of TWOC was higher in medication group than in control group (71.7% vs.53.1%,x2 =4.523,P=0.033).Both in medication group and control group,the success rate was higher in patients with catheterization for7 days than for 3 days (83.3% vs.60.0%,x2=4.022,P=0.045;67.7% vs.36.7%,P=0.013).(2)After follow-up for 6 months,the patients with catheterization for 7 days had lower incidence rate of second AUR and selective operation (x2 =4.538 and 4.709,P=0.033 and 0.030).(3)There was no significant difference in rate of urinary infection between the patients with catheterization for 7 days and for 3 days (20.3% vs.16.7%,P = 0.603).Conclusions Alpha-adrenergic receptor blocker could increase the success rate of TWOC,and the success rate of TWOC is much higher in patients with catheterization for 7 days than for 3 days,while the urinary infection rate is not significantly increased.
6.Establish of the orthotopic human pancreatic cancer model in mice and high frequency probe of endoscopy ultrasonographic monitoring

Objective To establish the orthotopic human pancreatic cancer model in mice and study the method to detect the tumor growth. Methods Human pancreatic cancer cell lines SW1990 in logarithmic phase was made into cell suspension and in situ injected into the envelope of pancreatic tail of BALB/c nude mice. High-frequency endoscopy ultrasound was used to observe the growth of tumor mass and its imaging characteristics were studied. Results 20 nude mice were successfully implanted, and 1 died 25 h after implantation. 14 days after implantation, the tumor of pancreatic cancer on EUS was (8.09 ± 2.61) mm3, the tumor appeared as homogeneous hypoechoic mass with clear boundary, and envelope as well as sound halo was present, and the shape was regular; there was low speed circular vasculature signal in around 30% of tumor.The tumor size increased to (12.40 ± 3.51)mm3, and the shape of 70% tumor was irregular, and some appeared as lobulated, and the tumor appeared as heterogeneous hypoechoic mass, no necrosis or liquefaction was found 28 days after implantation. There was low speed circular vasculature signal in around 70% tumors.Conclusions The orthotopic pancreatic cancer model in nude mouse can be established by in situ injection and this method is relatively ideal because it is simple and effective. The high frequency probe of endoscopic ultrasonograph is a reliable method for monitoring implanted pancreatic cancer.
7.Nursing of peripherally inserted central career for patients with colorectal cancer undergoing chemotherapy

Objective To summarize the nursing experience of orthophoria peripherally inserted central catheter(PICC).Methods Patients who failed percutaneous intravenous puncture(23 cases)underwent orthophoria HCC.The operation technique,nursing points and prevention of complications were summarized.Results All 23 patients received successful first-time puncture.Accurate anatomical position,strict aseptic manipulation and change of dressing were required during nursing.One patient failed in transmission of the tube after successful puncture.One patient appeared local red swelling of skin.Conclusions Orthophoria PICC is suitable for patients with colorectal cancer undergoing chemotherapy who failed percutaneous intravenous puncture.Selective nursing has pivotal significance for successful operation.
8.Monitoring of drug resistance of common bacteria in hospital

Objective To understand the drug resistance situation of clinical common bacteria in hospital.Methods VITEK-2 was adopted to perform the identification and antimicrobial susceptibility test.The drug resistance data were analyzed by adopting the WHONET 5.5 software.Results 13 841 strains of bacteria isolated for the first time from the patient were collected from Jan-uary 2009 to December 2012,in which Gram positive bacteria accounted for 29.3% and Gram negative bacteria accounted for 70.7%.Meticillin-resistant Staphylococcus aureus (MRSA)and meticillin-resistant coagulase-negative staphylococci (MRCNS) accounted for 78.3% and 88.4% of staphylococci aureus and coagulase-negative staphylococci,respectively.No vancomycin and tigecycline resistant strains of staphylococci were found.4.3% of enterococcus faecalis and 7.5% of enterococcus faecium were re-sistant to vancomycin.The detection rates of E.coli and Klebsiella spp (Klebsiella pneumoniae +Klebsiella bacteria producing acid )producing extended spectrum beta-lactamases (ESBLs)were 58.5% and 24.4%,respectively.Enterobacteriaceae strains were still more sensitive to carbapenem antibiotics.The resistance rates of pseudomonas aeruginosa and Acinetobacter spp strains to imipenem were 24.9 % and 42.6%,respectively.Conclusion Bacterial drug resistance still constitutes a serious threat to the clini-cal anti-infection therapy.So the antibacterial agents should be rationally used for reducing the bacterial drug resistance.
9.Effects of changing temporal resolution of spiral computed tomography on perfusion of cervical cancer and radiation dose

Objective To study the effects of different temporal resolution of spiral computed tomography (CT)on perfusion parameters and perfusion curve of cervical cervical cancer.Methods Ten cases of cervical cancer were clinically confirmed with CT perfusion scanning.The original data were acquired using temporal resolution of 0.75 s.Then the original data were grouped according to different temporal resolution,namely,1.5 s group,2.25 s group,3 s group,3.75 s group,4.5 s group,5.25 s group,and 6 s group (experiment group). According to the same mathematical model and ROI of the same part,perfusion parameters (BF,BV,MTT,and PS)in each group were calculated respectively and compared with the original data.Results BF and MTT were relatively sensitive to the change of temporal resolution.When the temporal resolution was 3 s,it had a significant impact.PS and BV were not so sensitive to the change of temporal resolution.Temporal resolution of 4.5 s had a significant effect on PS. There was a significant effect on BV until the temporal resolution was 5.25 s. Conclusion Changing the temporal resolution will lead to corresponding changes of perfusion curve and perfusion parameters.Under the premise that it does not affect the diagnosis,properly decreasing temporal resolution (circu-lar scanning temporal ≤2.25 s)of CT perfusion scanning of cervical cancer can reduce the radiation dose effectively.
10.Clinical features and genetic sequences of severe fever with thrombocytopenia syndrome bunyavirus in a new-epidemic region of Zhejiang Province, China

ObjectiveTo investigate the clinical characteristics,epidemiology of patients with severe fever with thrombocytopenia syndrome bunyavirus (SFTSV) infection and genetic sequences of SFTSV.MethodsClinical data of five cases of severe fever with thrombocytopenia syndrome (SFTS)from Zhoushan Hospital during May 2011 to July 2011 were retrospectively analyzed.SFTSV gene was amplified by polymerase chain reaction (PCR).CD3+ CD4+ and CD3+ CD8+T lymphocytes were detected by flow cytometry (FCM).The sequences of isolated SFTSV strains were compared with those in GenBank. ResultsThe symptoms of continuous high fever,sore muscles,enlarged superficial lymph nodes,abdominal pain,diarrhea with gastrointestinal hemorrhage were observed.The white blood cells,platelets and CD3+ CD4+ T lymphocytes were progressive decreased in acute phase with the minimum of (0.97-2.00) × 109/L,(12-42) × 109/L and 7.52%-20.39%,respectively.The SFTSV was isolated from the sera of two patients.The sequences were compared with SFTSV sequences in GenBank.The homology of RNA-dependent RNA polymerase gene was 96% compared with BX-2010,L-WWG,LN3,JS4,SD4,HN6 and AH12; the glycoprotein gene was 94% ; N protein gene was 95% compared with JS4,SD4 and LN4.The homology of the above three genes between two isolates was 99%.ConclusionsOur results suggest that SFTSV is sporadic in Zhejiang Province which is probably from native epidemic focus.SFTS is progressive and severe with acute onset.Multiple organ dysfunction is common in severe eases.
