Chinese Journal of Orthopaedic Trauma 2004;0(12):-
The appearance of theoretical medic ine is an inevitable result of develo pment of medicine. The development of molecular biology an d molecular immunology has laid the b asis of theoretical medicine, which studies various systems in life so as to reveal the essential laws of life phenomen a.The mystery of fracture healing wi ll be explained only from the viewpoint of theoretical medicine. The primary c allus response(PCR)is the basic event of fracture healing that is a process of numerous repetition of PCR and accum ulation of the surplus repair of PCR.The pathologic process of fracture healing shares very similar cellular and molecular mechanisms with the physiologic process of bone remodeling. This theory has been referred to as monistical theory of fracture healing.[

Objective To evaluate the feasibility and advantage of using withdrawable Sigma stent for the treatment of tracheo-esophageal fistular. Methods The stents were placed into trachea or/and esophagus by interventional or/and endoscopic technique. Results Esophageal cancerin 17 and benign disease in 2. Totally 38 stents were placed in 19 cases of patients (trachea 19, main bronchus 1, esophagus 18). Only one tracheal stent was placed in 2 cases. Two stents(one in trachea, another in esophagus) were placed in 15 cases ( 11 cases with 2 tubular type stent, 4 cases with one tubular and one bifurcated type stent). Three stents were placed in 2 cases. One stage placement of the stent in 35, withdrawed the stent and reinserted again in 3. All the patients have normal meal 2~4 days postoperatively. Only one patient had a little contrast in the trachea during X-ray exam but without symptoms, the fistulae completely sealed in 18 cases. Follow-up was fron 3 months to 3 year. 10 patients were still alive; the longest survival is 18 months. There were 9 deaths. The causes of death were pulmonary infection in 1, hemorrhage in 1, and systemic metastasis in 7 cases. Conclusion Sigma stent is can effectively treat tracheal or/and esophageal stenosis or fistular.

To discuss the advantages and disadvantages of fibular osteosynthesis and osteotomy in the treatment of tibial fractures. Medical literature was searched for articles concerning “fibular fracture”and“fibular osteotomy”on Medline to have a comprehensive idea of experiences of others. The results of others and of ours were analyzed and interpreted. Fractures of distal tibia and fibula with displaced distal tibial articular fractures were classic indications of fibular osteosynthesis, but distal tibiofibular syndesmosis and stability of ankle joint were seldom affected by the fibular fracture when tibia was fractured. It is not necessary to perform routine osteosynthesis for distal fibula fracture. Fibular osteotomy may be considered as an adjuvant procedure when atrophic nonunion or deformity union of tibia occurs. It is wise in treatment of tibial fractures not to perform fibular osteosynthesis or osteotomy whenever possible.

Objective To discuss the characteristics of renal cell carcinoma with regional lymph node metastasis at diagnosis. Methods The data of 19 patients diagnosed with renal cell carcinoma with regional lymph node metastases at diagnosis from January 2004 to December 2008 were reviewed. The median age was 57 years (29-77).The study group included 15 males and four females. The primary tumor was located in the left kidney in 12 patients and fight in seven patients. The median maximam diameter of retroperitoneal lymph nodes was 2.8 cm(1.5-5.0).The lymph nodes in four patients were not detected by the preoperative image examination, but were confirmed by intraoperative exploration.Eleven cases had enlarged retroperitoneal lymph nodes resected and eight had regional lymph nodes dissected. Results The patients with regional lymph node metastases at diagnosis of renal celI carcinoma accounted for 1.6% (19/1213) of the total renal cell carcinoma cases. With a median follow-up of 34 months, six patients were survival without progression, and seven were survival with progression.giving a 5-year survival rate of 68.4%.The survival and recurrence rates after surgery were not significantly different by Fisher test(P=0.644 and 0.319 respectively) between the patients who underwent retroperitoneal regional lymph node dissection and those who underwent enlarged lymph node resection. Condmiom Renal cell carcinoma with regional lymph node metastasis at diagnosis is uncommon. Some patients may achieve long-term tumor-free survival through regional lymph node dissection or enlarged Iymph nodes resection.

Objective To prepared the docetaxel nano liposome (L-DOC) for the therapy of liver cancer HepG2 cells in vitro and in vivo. Methods The film-ultrasonic dispersion method was used to prepare the L-DOC.The diameter and Zeta potential of L-DOC were determined by Nanosizer and the encapsulation efficiency was further measured.CCK-8 method was used to determine the cell viability of HepG2 cell after treating with various concentration of DOC and L-DOC respectively and the cell death type was detected by Flow cytometer.Next, we have studied the relative tumor volume change of tumor-bearing mice and the toxicity in vivo. Results The average diameter of L-DOC was 104 nm and the Zeta potential was about -35.1 mV.The Zeta potential of L-DOC was almost unchanged after standing for 96 hours. The encapsulation efficiency of L-DOC was ( 71.2 ±1.6 )%.The CCK-8 results showed that the cell viability was decreased after treating with various concentration of DOC and L-DOC, but the inhibition effect of L-DOC was better than that of DOC after treating with the same dose, especially for 20μg/mL.It was found that the cell death was induced by apoptosis. The in vivo study results showed that 6mg/kg L-DOC could inhibit the tumor volume better than that of same dose of DOC.In addition, 6mg/kg L-DOC and DOC didn’ t induce in vivo toxicity. Conclusion The L-DOC is prepared by film-ultrasonic dispersion method which has small diameter, great biocompatibility. And it could inhibit the HepG2 cells in vitro and in vivo, especially for no in vivo toxicity.

Objective Direct impact is a common mechanism of injury for blunt thoracic injury, and if resulting thoracic injury is severe the mortality may be as high as 10% ~ 25% . We aim to develop a mechanical device for animal impact injury experiment, so as to establish a rodent model of severe thoracic injury. Method A spring operated mechanical device for animal impact injury experiment was developed. The device allowed for accurate controlled delivery of impact force to specific areas of the chest well, at specific velocities and degrees of chest compression. Eghty-four male Sprague-Dawley rats were anaesthetized and underwent left carotid artery cannulation. They were randomly divided into seven groups and given the following treatment: group A (Control group) were subjected to sham impact; group B to G animals were subjected to impacts on the right lateral superior chest at different velocities and degrees of chest wall compression. ( B 3 m/s, 20%; C 3 m/s 40%; D 6 m/s 20%; E 6 m/s 40%; F 9 m/s 20%; G 9 m/s 40%). Arterial blood gas samples were taken just before injury, and at 2 and 12 post injury. All rats were sacrificed at 12 hours and their degree of thoracic injury rated. Pathological examination of injured lung tissue was also performed. Results The device was able to deliver impact forces accurately, with < 4% deviation from desired velocity and < 3 mm deviation from target area of impact. Other than the control group, all animals experienced significant hemodynamic changes immediately post impact. Arterial blood gas analysis detected significant hypocapnia in groups B and C. Significant hypoxemia and hypocapnia was detected in groups D, E and F. In groups B,C,D and F, die impact produced a mild thoracic injury with low mortality rate at 12 hours. In group E, the impact produced severe thoracic injury with mortality rate of 33.33% at 12 hours. Group C animals sustained the most serious thoracic injury with mortality rate of 83.33% at 12 hours. Pathological examination revealed injuries from direct trauma as well as secondary lung injuries. Conclusions Our device was able to repetitively deliver accurate and precise impact forces to rats and allows us to establish a rodent model of severe thoracic injury firm blunt trauma. We found that with our device, impact force at velocity of 6 m/s and 40% chest compression produced the most severe lung injury in rats. This helps us establish a rodent model of severe thoracic injury which can be use for future research in severe blunt thoracic trauma and the secondary lung injuries.

Objective To establish the orthotopic human pancreatic cancer model in mice and study the method to detect the tumor growth. Methods Human pancreatic cancer cell lines SW1990 in logarithmic phase was made into cell suspension and in situ injected into the envelope of pancreatic tail of BALB/c nude mice. High-frequency endoscopy ultrasound was used to observe the growth of tumor mass and its imaging characteristics were studied. Results 20 nude mice were successfully implanted, and 1 died 25 h after implantation. 14 days after implantation, the tumor of pancreatic cancer on EUS was (8.09 ± 2.61) mm3, the tumor appeared as homogeneous hypoechoic mass with clear boundary, and envelope as well as sound halo was present, and the shape was regular; there was low speed circular vasculature signal in around 30% of tumor. The tumor size increased to (12.40 ± 3.51)mm3, and the shape of 70% tumor was irregular, and some appeared as lobulated, and the tumor appeared as heterogeneous hypoechoic mass, no necrosis or liquefaction was found 28 days after implantation. There was low speed circular vasculature signal in around 70% tumors. Conclusions The orthotopic pancreatic cancer model in nude mouse can be established by in situ injection and this method is relatively ideal because it is simple and effective. The high frequency probe of endoscopic ultrasonograph is a reliable method for monitoring implanted pancreatic cancer.

Objective To evaluate the application of Sigma stent in treatment for esophagn-tracheal fistula. Methods Clinical data of 22 cases of esophago-tracheal fistula treated with Sigma stent were retrospectively analyzed, including 17 cases of esophageal cancer and 5 cases of benign esophageal diseases, with length of 4 days to 2 months. One single stent was placed in 5 cases, two stents ( both in trachea and esophagus each) in 15 cases, and three stents in 2 cases, and 18 eases with tube-type trachea stent, 3 with obtuse angle L-type and 1 with Y-type. Placement of stent was by interventional method or/and endoscopy. Results Forty-one stents were successfully placed in 22 patients with no death in the procedure. And, all patients with fistula were healed successfully and could have normal eating 2 to 4 days after operation, except 1 patient who received a tracheal stent inserted first, but an esophageal stent inserted again due to incomplete seal of the fistula and another benign case who received surgical operation 1 year after stent placement due to protracted unsealed fistula, with an overall cure rate of 91% (20/22). Conclusions Placement of Sigma stent in the esophagus or/and trachea could effectively heal esophago-tracheal fistula. All the patients can eat normally after the procedure. Stent can be recycled and formation of secondary esophago-traeheal fistula can be prevented effectively with such procedure.
