1.Clinical analysis of autologous serum injection cobined with urokinase through continuous drainage in lumbar occipital angle on encephalocoele hemarrage patients

Objective To study the clinical effects of autologous serum injection combined with urokinase through continuous drainage in lumbar occipital angle on encephalocoele hemarrage patients. Methods 106 cases of encephalocoele hemarrage were randomly allocated to treatment group (n = 53) and control group (n = 53). Urokinase injection were applied to both group through double occipital angle puncture, and additional autolagous serum were given to these in treatment group. Results There were 39 cases(73.6%) with effectiveness, 6 cases(11.3%) without any effects and 8 (15.1%) deaths in the control group and in the treatment group, these were 47 (88.7%), 4 (7.5%) and 2 (3.8%) (P < 0.05). The ratio of Hemarrage disappearing in both group are 77.4% and 92.5% respectively (P < 0.05). Conclusion Autologous serum injection combined with tradition treatment is a better alternative.
2.The influence of ovarian artery on the outcomes of uterine fibroid embolization

Although many studies have confirmed that uterine arterial embolization has good short-term and long- term effects for the treatment of uterine fibroids, certain failure rate does exist. Many researches have indicated that the surgical failure is mainly related to the collateral arterial supplies, among which the ovarian artery supply is the commonest cause. This review aims to make a further understanding of the influence of ovarian artery on the outcomes of uterine fibroid embolization.
3.Cytological and histological analysis of samples obtained by endoscopic ultrasound guided fine needle aspiration

Objective To evaluated diagnosis yield of cytological and histological analysis of endoscopic ultrasound guided fine needle aspiration (EUS-FNA).Methods A total of 43 patients who were clinically diagnosed as having mediastinal lesions,retroperitoneal lesions or pancreatic lesions by CT were recruited to the study.EUS-FNA was performed with standard 22G needle.Specimens were placed in formalin for histological analysis,and residual materials were prepared for cytologic analysis.Results Lesions were located in the mediastinum (n =9),the retroperitoneum (n =1),and pancreas (n =33).Cytologic diagnoses included malignancy (n =22),suspicious malignancy (n =2),atypical cell (n =3),absence of malignancy (n =16).Histologic diagnoses included malignancy (n =30),suspicious malignancy (n =1),sarcoidosis (n =2),chronic inflammation (n =10).The positive rates,sensitivities and accuracies of cytology and histology were 51%,59%,65% and 74%,81%,84%,respectively.Conclusion The positive rate,sensitivity and accuracy of histology of EUS-FNA is superior to cytology.
5.Biochemical and pharmacological characteristics of a fibrinolytic enzyme from Agkistrodon halys Pallas venom in Jiangzhe region

BACKGROUND: A fibrinolytic enzyme (FLE) was purified from the venom of Agkistrodon blomhoffi brevicaudus by ion-exchange chromatography;meanwhile, it can dissolve thrombotic anti-thrombus drugs during onset of myocardial infarction and apoplexy.OBJECTIVE: To investigate the purification of FLE from the venom of Agkistrodon blomhoffi brevicaudus and research some biochemical and pharmacological characterizations.DESIGN: Controlled observational study.SETTING: Snake Venom Research Institute, Guangxi Medical University.MATERIALS: The experiment was carried out in Snake Venom Research Institute of Guangxi Medical University from July 2003 to June 2005.Kunming mice, weighing 18-25 g, of mean body mass of (20±2) g, aged 3-4 months were provided by Experimental Animal Center of Guangxi Medical University. Agkistrodon blomhoffi brevicaudus was supplied by Snake Venom Research Institute of Guangxi Medical University.METHODS: FLE was purified with DEAE Sepharose CL-6B ion exchang columns and HiPrep Sephacryl S 100; purity and relative molecular mass were measured with high-effective chromatograph of liquid and polyacrylamidedel electrophoresis (PAGE); isoelectric point was measured with isoelectric focusing electrophoresis; and then, components and partially pharmacological characteristics were also measured.MAIN OUTCOME MEASURES: Relative molecular mass, conformation of isoelectric point and amino acid, effect of inhibitor on FLE, hemorrhagic activity, and anticoagulant effect of FLE.RESULTS: All 30 rats and 12 rabbits were involved in the final analysis.Relative molecular mass was 59 100 and isoelectric point was 4.98. SDSPAGE acted as a zone in vitro. High-effective chromatograph of liquid showed a simple peak of FLE. This group belonged to metallo proteinases.Analysis of components of amino acid suggested that it contained a lot of acidity amino acids, had mild hemorrhagic toxicity and strongly thrombolytic effect.CONCLUSION: A simple acidity metallo proteinases, which is characterized by mild hemorrhagic toxicity and strong anti-coagulation, can be purified from the venom of Agkistrodon blomhoffi brevicaudus.
6.The influence of different method of mediastinal lymph node excision on perioperative immune function of lung cancer patients

Objective To explore the influence of different method of mediastinal lymph node excision on the immune function of non-small cell lung cancer patients.Metbods Clinical data was collected continuously from 415 cases of inhospital patients with non-small cell lung cancer from March 2009 to March 2012,who were divided into system lymph node dissection group (SLND,216 cases) and lymph node sampling group (LNS,199 cases).The total lymphocytes,NK cells,CD4+ T cells and CD8+ T cells in blood samples taken preoperatively and at 3rd and 7th day postoperation were analyzed.Results Total lymphocytes,NK cells and CD8+ T cells were reduced significantly in the SLND group compared with those in LNS group at 3rd day and 7th day postoperation (3rd day:LC (0.95±0.57) × 109/L vs (1.10±0.65) × 109/L,CD8+ T cells (19.53± 6.48) % vs (20.93±6.70) %,NK (17.36±6.06) % vs (18.57±5.97) %,P < 0.05; 7th day:LC (0.86±0.53) × 109/L vs (1.00±0.60) × 109/L,CD8+ T cells (17.27±5.64) % vs (18.40±5.26) %,NK (13.11±4.84) % vs (14.20± 5.30) %; P < 0.05).CD4+ T cells in the SLND group were reduced significantly at 3rd day postoperation compared with LNS group ((29.59±6.53) % vs (31.19±6.32) %,P < 0.05).Conclusion LNS can reduce immunosuppression compared with SLND for postoperative patients.
7.Relationship of the fragmented QRS and the fragmented QRS time limit with ventricular arrhythmia in old myocardial infarction

ObjectiveTo explore the relationship of the fragmented QRS (fQRS) and the fQRS time limit with ventricular arrhythmia in old myocardial infarction (OMI) patients through contrasting the incidence of ventricular arrhythmia in OMI patients whether fQRS or not and ventricular arrhythmia in different fQRS time limit.MethodsAccording to the routine electrocardiogram,321 OMI patients were divided into group A (fQRS appearance,167 cases) and group B(fQRS non-appearance,154 cases).The lead with fQRS extense was ehosen and traced another 50 mm/s electrocardiogram,and 3 consecutive fQRS time limit were measured and them average was taken in group A.According to the fQRS time limit,the patients in group A were divided into 3 groups: group X( ≤0.100 s,96 cases),group Y (0.101-0.119 s,54 cases) and group Z( ≥0.120 s,17 cases).All the patients were continuously monitored with 24 hours dynamic electrocardiogram,and the incidence of ventricular arrhythmia was analyzed.ResultsThe incidence of ventricular arrhythmia in group A [ 78.4% ( 131/167 ) ] was higher than that in group B [ 63.6%(98/154) ] (P< 0.01 ).The incidence of premature ventricular contraction(PVC) > 720/24 hours in group A [ 28.7%(48/167 ) ] was higher than that in group B[ 17.5%(27/154) ] (P < 0.05 ).The incidence of multifocal PVC,coupled PVC,nonsustained ventricular tachycardia and Lown 3-5 grades PVC was 16.2% (27/167),33.5% (56/167),12.0% (20/167),34.1% (57/167) in group A,7.8% (12/154),21.4% (33/154),4.5%(7/154),23.4%(36/154) in group B,there were significant differences between two groups (P< 0.05 ).The incidence of ventricular arrhythmia in group Z [ 100.0%( 17/17 ) ] was significantly higher than that in group Y [79.6%(43/54)] and group X [74.0%(71/96)](P< 0.05).The incidence of Lown 3-5 grades PVC in group Z[ 70.6%( 12/17 )] was significantly higher than that in group Y[ 42.6%(23/54)] and group X [ 22.9%(22/96) ],and the incidence of Lown 3-5 grades PVC in group Y was significantly higher than that in group X (P< 0.05).ConclusionsOMI patients with fQRS have higher incidence and severe degree in ventricular arrlhythmia than those without fQRS.With the fQRS time limit widened,PVC and Iown 3-5 grades PVC significantly increased.So fQRS is a new predicting index of OMI,and fQRS time limit has definite value in predicting the heart event for OMI patients.
8.Treatment of superior sulcus tumors using the improved antero-cervical parasternal approach

ObjectiveTo investigate the experience of resection of superior sulcus tumors using the improved antero-cervical parastemal approach.MethodsThe clinical data of 3 patients with superior sulcus tumor from July 2005 to May 2010 were analyzed retrospectively.Three patients with superior sulcus tumor underwent en bloc resection using the improved antero-cervical parastemal approach.The 1st to 3rd rib were excised,1 case with 1/5 centrum vertebra excision,1 case with the 1st transverse process of thoracic vertebra excision,1 case with the 1 st and 2nd transverse process of thoracic vertebra excision,1 case with T1 nerve root and sellate ganglion excision.ResultsAll the patients recovered well after operation.There was no serious complication except for 1 case with secondary Horner syndrome.The mild paradoxicalbreathing was found in 3 cases postoperative dressing change,but they had no dyspnea and tolerance well.The paradoxical breathing was disappeared when the mediastinum was fixed after 14 d thoracic wall pressure dressing.The pathological diagnosis after operation:2 cases with squamous-celled carcinoma,1 case with adenosquamous carcinoma,there was no mediastinal lymph node metastasis,cutting edge was negative.Stage:2 cases with T3N0M0,1 case with T4N0M0.Three cases were treated with chemotherapy (NP plan) for 4 cycles after operation without radiotherapy.Three cases were followed up for 53,37, 13 months after operation,they were all survival without recurrence and good quality life.ConclusionsFor the treatment of especially anterior and middle seated superior sulcus tumors,improved antero-cervical parasternal approach provides a safe and effective exposure.The improved antero-cervical parasternal approach is worthy of promotion.
9.An Untargeted Urinary Metabolomics Strategy for Investigation of Therapeutical Mechanism of Schisandra chinensis on Complications of Diabetes Rats

An untargeted urinary metabonomics method based on ultra-performance liquid chromatography coupled with quadrupole time-of-flight mass spectrometry ( UPLC-Q-TOF-MS ) has been established to investigate the mechanism of Schisandra chinensis in treating diabetes and its complications. The urinary biomarkers related to the therapeutic effects of Schisandra chinensis on the diabetes rats were analyzed. In urine, 28 kinds of endogenous metabolites were identified as potential biomarkers, including 13 endogenous metabolites in positive ion mode, 15 endogenous metabolites in negative ion mode, and hippuric acid detected both in positive and negative ion modes. The results revealed that Schisandra chinensis mainly affected the pathways of pentose and glucuronate interconversions, riboflavin metabolism, pantothenate and CoA biosynthesis, arginine and proline metabolism, intestinal bacteria metabolism, ascorbate and aldarate metabolism and tryptophan metabolism in diabetic rats. Combined with biological analysis of these pathways, the therapeutic mechanism of Schisandra chinensis on diabetes and its complications was verified. Based on the biological function of each pathway, the effect of Schisandra chinensis on diabetic nephropathy is stronger. Moreover, it also has the effects of protecting liver, decreasing fat and antioxidant activity.
10.Analysis of activated brain areas in uncertain rewarding processing of decision making on healthy volunteers

Objective To research the activated brain areas of decision making under uncertainty reward processing on healthy volunteers.Method The E-Prime programs were presented 3 kinds of tasks of the decision under uncertain reward processing.15 right-handed healthy volunteers made a response after receiving the task.At the same time,the GE 3.0T magnetic resonance scanner scanned the brains areas of subjects.Individual analysis and group analysis was done with SPM8 software,then the brain activating regions and the peak intensity were gotten.Results Orbitofrontal cortex was activated in certainty,peak intensity 2.4328 ± 0.1949 (P < 0.05).Prefrontal cortex,occipital lobe,parietal lobe,posterior lobe of cerebellum,limbic lobe and midbrain were activated under the risk reward processing,peak intensity 2.4228 ± 1.3762 (P < 0.05).When under ambiguity reward processing,prefrontal cortex,temporal lobe,occipital lobe,left inferior frontal gyrus and posterior lobe of cerebellum were activated,peak intensity 2.4056 ± 0.4222 (P < 0.05).Compared with the task under certainty,posterior lobe of cerebellum,gyri subtemporalis and gyri fusiformis,inferior parietal lobule,anterior central convolution,orbitofrontal cortex,ventrolateral prefrontal cortex,both frontopolar and supramarginal gyrus were activated in task under risk (P <0.05) ;and both frontopolar were activated in task under ambiguity (P < 0.05).Compared with the task under risk,dorsolateral prefrontal cortex and posterior lobe of cerebellum were activated in task under ambiguity (P < 0.05).Conclusion There are differences in different types of reward processing of decision making.
