2.Laparoscopic repair of common hepatic duct defect caused by Mirizzi syndrome

Objective To evaluate the repot of common hepatic duct defect caused by Type Ⅱ or Type Ⅲ Mirizzi syndrome laparoscopically.Methods Eight cases of Type Ⅱ or Type Ⅲ Mirizzi syndrome treated under laparoscopy were analyzed retrospectively.Among them six cases were identified as Mirizzi Ⅱ and a defect on the lateral wall of common hepatic duct was found and repmred by suturing the stubble of cystic duct.The two Mirizzi Ⅲ cases had a defect on front and lateral wall or back and lateral wall of common hepatic duct respectively.Both defects were repaired by a patch from the infundibulum of galllbladder. Results All the 8 cases underwent laparoscopic subtotal cholecystectomy followed by repair of common hepatic duct defect.The operation lasted for 80~150 min(mean 110 min).There was no recurrence of the symptoms after 6~24 months follow up.Conclusion The familiarity of the anatomical characters of Mirizzi syndrome and the skillful suture technique under laparoscopy are both necessary.
3.Clinical analysis of Laparoscopic combined fiber choledochoscope common bile duct cholelithotomy in 25 cases

Objective To evaluate the Laparoscopic combined fiber choledochoscope common bile duct cho-lelithotomy for my own clinical experience in rencent five years.Methods Clinic data of 25 cases with choledocholi-thiasis undergoing laparoscopic common bile duct cholelithotomy from June 2010 to March 2014 were retrospectively analyzed.Results The operation was performed successfully in 24 cases,a case conversions to laparotomy,21 cases with T-tube drainage,3 cases with primary ductal submucosa closure.The operative time was 105 to 305min,mean (184.0 ±12.2 ) min, postoperative hospitalization time was 6 to 10 day, mean ( 7.7 ±0.2 ) day, a case residual stones,no bile leakage.Conclusion The beginning of Laparoscopic combined fiber choledochoscope common bile duct cholelithotomy,one-stage suture of common bile duct should be selected carefully, timely transfer laparotomy, should not aim for laparoscopic surgery success rate.
4. Correlation analysis of gout with body mass index and waist-hip ratio

Abstract: Objective To explore the relationship between body mass index (BMI), waist-hip ratio (WHR) and the risk of gout in Urumqi. Methods A total of 516 male patients with gout in a third-class hospital in Urumqi from 2015 to 2019 were randomly selected as the gout group and 516 male healthy subjects in the same hospital as the control group. The relevant blood biochemical indexes were examined and analyzed. Blood pressure, waist circumference and hip circumference were measured. Body mass index and waist-to-hip ratio were calculated. Logistic regression model was used to analyze the relationship between overweight / obesity, waist-to-hip ratio and the risk of gout. The test level is α = 0.05. Results Uric acid, glucose, urea nitrogen, creatinine, triglyceride, low-density lipoprotein, systolic blood pressure, weight and waist circumference in gout group were higher than those in control group, and the differences were statistically significant (P<0.05); There were no significant differences in age, height and diastolic blood pressure between the two groups (P<0.05). There was a positive correlation between BMI and WHR and the occurrence of gout (r=0.272, 0.345, P<0.05). There were significant differences in BMI, WHR and waist circumference between the gout group and the control group(χ2= 55.338, 54.928, 54.153, P<0.05). After adjusting for age, aerobic exercise and other confounding factors, the results of multi-factor unconditional Logistic regression analysis showed that the odds ratio (OR) of gout in patients with BMI of 24.00-27.99 kg/m2 and ≥28.00 kg/m2 was 2.005 (1.337-3.006) and 2.677 (1.668-4.296) times higher than that of patients with normal BMI, respectively. The OR value of gout in patients with WHR≥0.90 was 1.668 times higher than that in patients with normal WHR, and the difference was statistically significant. The results of subgroup analysis according to age are generally similar. Conclusions The BMI and WHR of man with gout in Urumqi are higher than those of normal people, and BMI, waist circumference and WHR are all associated with the incidence of gout. The risk of gout increases with the increase of BMI and WHR.
5.Effect of Qingyi Huaji formula for inhibition of pancreatic cancer cell growth through down-regulating Ski expression.

OBJECTIVETo explore the action mechanism of action of Qingyi Huaji Formula (QYHJ), whether its inhibition on the growth of the tumor is by way of down-regulating Ski expression.
METHODSSW1990 tumor cell with low Ski expression was created by lenti-virus mediated RNA interfering technique. Tumor cells with different levels of Ski expression were subcutaneously transplanted to nude mice, and the response of cancer cells to QYHJ treatment was detected.
RESULTSAfter QYHJ treatment, the tumor growth slowed-down significantly, with the Ski mRNA and protein expressions in tumor reduced by 39.6% and 41.3% of that in untreated tumor respectively (P < 0.05). Ski mRNA and protein expressions in the created high and low Ski expression tumor cells were 105%, 123% and 46%, 30% respectively of that in parental cells (P < 0.05). The tumor weight inhibitory rates of QYHJ on high Ski expression cells were 29.6% and 32.2%, while on low Ski expression cells, it was 16.0% to 17.8% (P < 0.05).
CONCLUSIONSki acts as an important therapeutic target of QYHJ in treating SW1990 pancreatic cancer, and its expression status mediates different response of tumor cells to QYHJ treatment.

Photoaffinity labeling is widely applied to demonstrate targets of small molecule ligands. In this paper, biotin photoaffinity labeled molecule with propargyl group 1 has been designed and synthesized, followed it's labeling of N2-acetyl-2'-O-propargyl guanosine 9 by "click chemistry". This technology presents delight development potential in labeling of second messenger cyclic nucleotide, antisense oligonucleotide or siRNA.

8.Migration and homing of bone marrow mesenchymal stem cells in segmental nerve injury

BACKGROUND:A large number of studies have confirmed that tissue-engineered stem cel therapy is feasible to repair peripheral nerve injury, but the repair mechanism is unclear.
OBJECTIVE:To observe the differentiation and homing of bone marrow mesechnymal stem cel s under local nerve microenvironment by exploring the migration and effect of bone marrow mesenchymal stem cel s in the repair of damaged nerve.
METHODS:Male SD rats, aged 8 weeks, were selected to establish segmental nerve injury models by freezing the sciatic nerve. Thirty-six model rats were randomized into three groups (n=12):frozen nerve injury group, cel injection into the nerve group, cel injection around the nerve group. Before modeling and at 4, 8, 12 weeks after cel implantation, the sciatic nerve function index was measured. Electrophysiological test, contractility recovery rate, wet weight recovery rate of the triceps surae were detected and Masson staining was performed;toluidine blue staining of the distal nerve injury and immunofluorescence staining of the damaged nerve were performed.
RESULTS AND CONCLUSION:At 4, 8, 12 weeks after cel implantation, the sciatic nerve function index was ranked as fol ows:frozen nerve injury group

Objective To determine,the clinical significance of serum myocardial enzymes (Mb,cTNI, CK,CK-Mb,AST,LDH) in the classification of the disease severity of non-cardiogenic critically-ill patients. Compared with APACHEⅡscore concerned as the standard diagnosis of the critical ills,these biomarkers were investigated for the evaluation possibility of the degree and the prognosis of the critical ills.Method Patients admitted to our EICU were consecutively collected for the research from April to December in 2005 and the myocardial enzymes,and routine serum biochemical test and APACHEⅡscore were detected simultaneously.All the patients were classified to three groups according to the APACHEⅡscore (mild group,APACHEⅡ25) and two groups (survive group and death group) according to the prognosis.All the patients were followed up till recovery/discharge or death. 10.The study on activity of platelet-activating factor acetylhydrolase in asthmatic children

Objective Platelet activating factor(PAF),which has been implicated in the pathophysiology of inflammation in asthma,is degraded and inactivated by PAF acetlhydrolase(PAF AH).To investigate the association of PAF AH activity with genotype in asthmatic children.Methods We studied 57 asthmatic children and 30 normal controls. The plasma PAF AH genotype was detected as representative case with 3 different genotypes (Val/Val,Val/Phe and Phe/Phe) by allele specific polymerase chain reaction(AS PCR).The PAF AH activity in plasam was examined by the changes of substrate assay.Results In severe asthmatic individuals plasma PAF AH activities were lower than those of mild or moderate groups and control group,and plasma PAF AH activition was absent 15.4 %.In another three groups plasma PAF AH activation were absent 2 %-3 %.There was significant difference of plasma PAF AH activity among 3 groups of genotype(Val/Val,Val/Phe and Phe/Phe).In the similar genotype, there was no significant difference of plasma PAF AH activity between the groups of control and asthma.Conclusions There was imbalace of PAF/PAF AH in asthmatic children. In severe asthmatic individuals plasma PAF AH activities were lower than those of mild or moderate groups and control group. PAF AH(Val279Phe) gene mutation was related with plasma PAF AH activity.
