1.New advances in the treatment of multisystem Langerhans cell histiocytosis

Langerhans cell histiocytosis (LCH) is a rare proliferative disorder of dendritic cells and the reticular cells. It is divided into single-system langerhans cell histiocytosis (SS-LCH) and multi-system langerhans cell system (MS-LCH). MS-LCH has a very poor prognosis. The key of survival is timely and effective treatment. The treatment protocols include chemotherapy, immunotherapy and stem cells transplantation. In this review, the new advances in the treatment of MS-LCH were systemically reviewed.
2.Genotoxicity of Organic Extracts from Product Water of Water Plants and the Influencing Factors

A(D、B were negative); As for TA100(+S9), only E was positive. Conclusion The mutagenicity of product water of six water works is relatively high,the type of mutagenicity is mainly code-shifting. Taking river water as the water source,using chlorine dioxide and active carbon filtration and no using pretreatment of chlorination may reduce the mutagenic organics.

Objective To explore the clinical significance of urinary leukotriene E4(LTE4) in children with respiratory syncytial virus(RSV) bronchiolitis.Methods Forty infants with bronchiolitis were enrolled for the identification of RSV-Ag from the nasopharyngeal secretions.Forty cases who were found to be positive of RSV were put into RSV bronchiolitis group.According to the severity of illness,the infants in RSV bronchiolitis group were separated into 2 subgroups:mild illness group (24 cases) and moderate illness group (16 cases).Thirty healthy children were chosen as healthy control group.Urine of the patients in RSV bronchiolitis group was collected in acute phase (on the 1st day) and recovery phase (on the 7th day after the admission).Urine of children in control group was collected .Enzyme-linked immunosorbent assay was used to assay the level of urinary LTE4.Results The levels of LTE4 in healthy control group (M,Q) were 1.28 pmol?mmol-1,1.72 pmol?mmol-1,while the levels of LTE4 in acute phase in RSV bronchiolitis group (M,Q were 30.52 pmol?mmol-1,23.56 pmol?mmol-1,and the levels of LTE4 in recovery phase in RSV bronchiolitis group (M,Q) were 3.98 pmol?mmol-1,3.43 pmol?mmol-1.Urinary LTE4 levels in acute phase in RSV bronchiolitis group were significantly higher than those in healthy control group (P

5.An exploration of contributing factors to the oocyte utility rate of high responders in an in vitro fertilization cycle

Objective:To figure out the clinical factors contributing to the oocytes utility rate (OUR) of high responders in in vitro fertilization (IVF)-embryo transfer treatment.Methods:OUR was defined by the number of usable embryos for transfer and (or) cryopreservation divided by the number of oocytes retrieved in a freeze-all cycle. The cycles with ≥15 eggs were included from January 2013 to December 2019. Those with OUR at the top 10% (Group A) and the bottom 10% (Group B) were picked and compared for patients′ characteristics, parameters relating to ovary stimulation and pregnant outcomes. Multifactorial logistic regression was applied to reveal the risk factors affecting OUR in them.Results:A total of 43 patients were included in Group A (OUR: 77.4%, 601/776) and 47 for Group B (OUR: 11.9%, 104/874). Previous IVF/intracytoplasmic sperm injection (ICSI; OR=0.10, 95% CI: 0.01-0.81) and endometriosis ( OR=0.16, 95% CI: 0.03-0.84) were negative factors for OUR ( P<0.05); dual suppression protocol ( OR=3.74, 95% CI: 1.06-26.86) and longer days of stimulation ( OR=3.24, 95% CI: 1.25-8.42) were protective factors in terms of ovarian stimulation ( P<0.05), on contrary to that, any decline of estradiol during the stimulation led to poorer OUR ( OR=0.16, 95% CI: 0.04-0.64). Although two groups had similar quantities of eggs and metaphase of meiosis Ⅱ (MⅡ) oocytes, distinguished cumulative clinical pregnancy rate and cumulative live birth rate were seen in Group A and group B respectively [95.3% (41/43) vs 40.4% (19/47) and 90.7% (39/43) vs 31.9% (15/47), all P<0.01]. Conclusions:High responders with attempted IVF/ICSI and endometriosis should be considered as risk factors for OUR. Over ovarian stimulation, dual suppression and a slightly longer stimulating duration could be tried; besides, estradiol decline should be prevented for a better OUR.
6.Observation of the clinical effect of extracapsular cataract extraction and implantation of intraocular lens combined with vitreoretinal surgeryby samall incision

Objective To observe the clinical effect of extracapsular cataract extraction and implantation of intraocular lens combined with vitreoretinal surgery by samall incisionto to treat cataract merge vitreoretinal disease.Methods A retrospective study was managed on 18 cases(18eyes). Eyesight and complications after operation was study. Result Follow-up period was from 3 months to 18 months( the 8 months on average). The cases of corrected visual acuity less than 0. 02 was 2 eyes, among 0.02 to 0. 1 was 4 eyes, among 0. 2 to 0.3 was 8 eyes, then more than 0.3 was 4 eyes. The complications of surgery included tunicae uveae reaction, cornea reaction, ocular hypertension, vitreous hemorrhage,retinal detachment. Conclusion Extracapsular cataract extraction and implantation of intraocular lens combined with vitreoretinal surgery by small incision was safe and effective to treat Cataract combined with vitrcoretinal disease. The main factor affected the eyesight retrieval was posterior segment pathology.
7.Short-term curative effect of vacuum sealing drainage (VSD) combined with platelet rich plasma (PRP) for the treatment of the refractory wounds.

OBJECTIVETo observe the efficacy of the treatment of refractory wound by VSD combined with platelet-rich plasma.
METHODSFrom April 2010 to June 2012,15 patients with refractory wound were treated including 11 males and 4 females with an average age of 35.2 years old ranging from 18 to 45 years old. The formation time of wound was from 6 to 24 months, which was unhealed after long-term medication or repeated treatment. The VSD combined with autologous platelet-rich plasma was applied to treat the wound. The wound healing was an indicator and treatment and clinical features were summarized.
RESULTSAll the patients were followed up for 3 to 8 months (means 5 months). The wound of all patients were healed without recurrence.
CONCLUSIONThe curative effect of VSD combined with platelet rich plasma for treatment of refractory wounds is obvious. It could reduce the treatment course and the treatment cost.

8.Research progress on biochemical risk factors of stroke in young and middle-aged patients

The incidence of stroke is increasing year by year,about 75% of the patients with different levels of sequelae,and the age of onset also tends to be younger,to the community and the family to bring a heavy burden.Therefore,it is urgent to strengthen the study of the related risk factors of stroke in young people,so as to reduce the incidence of stroke.Traditional recognized risk factors such as hypertension,diabetes,high cholesterol,heart disease,obesity,and smoking can explain part of the stroke events,of these risk factors intervention treatment significantly reduced the incidence and mortality of stroke.However,cardio cerebral vascular system is not fully protected.In view of the biochemical indexes of blood of patients with specimens by convenience,detection index simple and precise,further studies of the relationship between biochemical risk factors and young stroke has made clear the necessity of and have become major challenges in current epidemiological study of cerebral stroke.
