Objective To study the effect of expansion ex vivo on the hematopoietic reconstitution of umbilical cord blood (UCB) hematopoietic cells, and to find a probable way of UCB transplantation (UCBT) to be used on adults. Method Three groups of UCB hematopoietic cells were transplanted to lethally irradiated BALB/C mice: fresh cells, a half part of fresh cells and a half part of cells expanded for a week, cells expanded for a week. Results All the three groups of cells could reconstitute hematopoiesis. The cells in the latter two groups could more rapidly accelerate the recovery of early-period hematopoiesis. The death rate of the mice in second group was the lowest among three groups. Conclusion The way transplanted with a half part of fresh UCB hematopoietic cells and a half part of UCB hematopoietic cells ex vivo expanded for a week may be a probable one for adult UBCT.

Objective To analyze the negative experience from medical practice of medical students by Balint group case study, and to explore new approach for training doctor-patient communication skills. Methods 10 times of Balint group activities were held in Zhongshan Hospital of Fudan University, with one case each time.Grounded theory approach were applied for analyzing discussion record. Results The cases were occurred in doctor-patient interaction, teamwork, conflict between one's personal life and vocational development planning.Trigger events involved doctor-patient conflict, conflict of values, decision making problem while conffronted with incurable patients, role ambiguity, unfair treatment.Topics of discussion included emotional experience with patients, career development orientation, medical teamwork, clinical education of medical student. Conclusion Through practice of Balint group, medical students in teaching hospital can improve their awareness of complicating psycho-social problem of patients and themselves,as well as handling it.And it can improve the professional identity, professional growth and professional self-identity.

Objective To evaluate three internal fixation methods in treatment of type C3 fractures of distal humerus. Methods From February 2002 to January 2005,73 cases of humeral intercondylar fractures were treated with single plating (Group A,21 cases),Y-shape plating (Group B,33 eases) and dual plating (Group C,19 cases).According to the AO/ASIF classification,73 cases belonged to type C3.The posterior midline incision and the approach of liguliform flap of musculus triceps brachii were used for all the cases. The recorded clinical data were reviewed to analyze effects of internal fixation methods, functions of the elbow and the complications. Results All the cases were followed up.The follow-ups ranged from 12 to 36 months, with an average of 22.3 months.By the end of the twelfth month, according to the Jupiter scoring system, the elhow function was excellent and good in 57.1% of cases in Group A,81.8% in Group B and 89.5% in Group C,the difference between Gronp A and Groups B and C being statistically significant(P<0.05).Meanwhile,according to Sodergard's criteria for failure of internal fixation system for intercondylar fracture of humerus, the failure rate was 33.3% in Group A,15,2% in Group B and 5.3% in Group C.The loosening or breakage rate in Group C was significantly lower than in Groups A and B (P<0.05),and that in Group B was obviously lower than in Group A (P<0.05).No incision necrosis or deep infection occurred. Conclusions The type C3 fractures of distal humerus should not he treated with single plating, because the functional outcome of elbow joint is poor, and the rate of loosening or breakage of internal fixation is higher than other fixation methods.Y-shape plating and dual plating are good for them, but Y-shape plating is not good for severe intercondytar comminnted fractures of the humerus, because its peculiar shape tends to lead to a higher rate of loosening.

Objective To investigate the effect of hot compress with traditional Chinese herbs with wine and vinegar on lumbago after cesarean section. Methods Ninety patients with lumbago after cesarean section were divided into the control group and the observation group equally according to their admission number. Both groups were cured by the injection at the pressure pain point, the observation group by hot compress of traditional Chinese herbs of Reyanbao with wine and vinegar. The treatment effect between both groups were observed. Results The effective rates of the experiment group and the control group were 93.3%and 68.9%, respectively. The difference was statistically significant (χ2=12.433, P<0.05). Conclusions Based on injection on the pressure pain point, lumbago after cesarean section can be effectively relieved with the extra treatment by hot compress with traditional Chinese herb of Reyanbao with wine and vinegar. The treatment is simple and safe and worthy of being applied clinically.

Background The special pathological change of diabetic retinopathy(DR)is microvascular disorder.Angiopoietin-like protein 2(Ang-2)is a new protein associated with genesis of blood vessels.Quercetin has multiple pharmacological action, including improving the microcircularion and the permeability of blood capillary. However, the action mechanism of Ang-2 on DR was unclear. Objective The present study was to investigate the effects of quercetin on Ang-2 and its receptor Tie2 expression in retina with diabetes mellitus. Methods Sixty clean male Wistar rats were randomized into 7 groups and 10 rats for each group, and 10 rats served as blank control group.Streptozotocin of 35 mg/kg was intraperitoneally injected in 60 rats to establish the diabetic models.Quercetins encapsulated by liposome with the doses of 50,150 and 250 mg/(kg · d)(3-5 ml)were used to gavage in different groups of models for 12 weeks, and normal saline solution and calcium dobesilate were used at the same fashion as the negative control group and positive control group, respectively.Twelve weeks later, the animals were sacrificed and retinas were isolated.Expressions of Ang-2 protein and Tie2 mRNA in retinas were detected by ELISA and RT-PCR,respectively.The usage and rearing of the animals complied with the Regulations for the Administration of Affair Concerning Experimental Animals by State Sciences and Technology Commission. Results ELISA showed that the A450 of Ang-2 in 150 and 250 mg/(kg · d)quercetin groups was 0.796±0.057 and 0.842±0.043 respectively and was lower than that negative group(1.012±0.046),showing statistically significant differences(q =2.95,2.698,P<0.05).RT-PCR assay showed that expression of Tie2 mRNA(Tie2 mRNA/GAPDH mRNA)in retinas was 0.712±0.092 and 0.821±0.087,presenting statistically significant differences in comparison with negative group(1.182±0.098)(q =3.497,2.852,P<0.05).The expression levels of Ang-2 and Tie2 mRNA in retina were lowest in 150 mg/(kg · d)quercetin group. Conclusions Quercetin can improve the retinal microcirculation by downregulating the expressions of Ang-2 and its receptor in early period of diabetic rats.

Objective To explore the mobile changes of nitric oxide(NO) and the relationship between nitric oxide and glomerular hyperfiltration in experimental diabetic rats. Methods Experimental diabetes mellitus(DM) was induced in rats with streptozotocin(STZ).The levels of NO and NOS in renal tissue homogenate were assayed after establishment of diabetesat the 4 th,8 th,12 th week.At the same time, renal morphology in diabetic rats was examined by light microscope and image of computer. Results The contents of NO and NOS in renal homegenates were evidently increased at 4 th week, and decreased gradually from 8 th week(P
