1.Atherosclerosis:Role of the scavenger receptor CD36

CD36 is involved not only in the metabolism of lipids,but also in the inflammatory reaction,in the formation of foam cells and in the pathogenesis of atherosclerosis.Monocyte/macrophage CD36 has been shown to play a critical role in the development of atherosclerotic lesions by its capacity to bind and en-docytose oxidized low density lipoproteins(oxLDL),and it is implicated in the formation of foam cells.CD36 expression and regulation mechanism already provides a new understanding and the relevant drug research has attracted more attention of scholars.This article is to carefully review the importance of CD36 as an essential component in the pathogenesis of atherosclerosis.
2.Prospective Application of Automated External Defibrillator in Inferior Troops

3.Application of Defibrillation Techniques in Cardiopulmonary Resuscitation

Objective To investigate the techniques of electrical defibrillation and their application to cardiopulmonary re-suscitation(CPR).Methods On the basis of reviewing the definition of electrical defibrillation,the main defibrillation tech-niques and their developments were introduced,as well as the selection of shock energy.The role that defibrillation played during CPR was defined according to the consensus.Results As an unfashionable technique,monophasic waveforms was inclined to be replaced by present biphasic waveform including biphasic truncated exponential(BTE) waveform and recti-linear biphasic waveform(RBW).The selected shock energy varied with the waveform that a defibrillator made use of,i.e.a monophasic defibrillator required a higher energy level as usual,whereas a biphasic one did not.It was deemed that biphasic technique presented an excellence in less damage to heart in addition to higher rate of successful defibrillation.In the critical care for sudden cardiac arrest with a shockable rhythm,the protocol of "1 shock + 5 cycles of CPR" should be recommended.Conclusion Electrical defibrillation techniques consist mainly of monophasic waveform and biphasic wave-form,between which the latter gains an advantage over the former.On the grounds of modern emergency medicine,defib-rillation has to be performed in combination with CPR.
4.Research progress in the diagnosis and treatment of adenoid cystic carcinoma of the salivary gland

Adenoid cystic carcinoma(ACC) of the salivary gland constitutes approximately 21.9%-24.0% of all salivary gland malignant tumors,and is described as a tumor with indolent,but persistent and recurrent,growth and late onset of metastases,leading eventually to death.This review focuses on the advances in the studies of the diagnosis,treatment,and prognosis of ACC in the recent years.
5.Clinical study on the effect of Shugan Tongluo Decoction on upper limb swelling after breast cancer surgery

Objective To study the effect of Shugan Tongluo Recipe on the patients with upper limb swelling after breast cancer surgery, and to explore the efficiency of treatment.Methods84 patients with breast cancer were selected from June to December 2015 in the traditional Chinese medicine clinic of Ge zhen,, were randomly divided into control group and observation group, control group using beta seven yezao sodium and conventional rehabilitation therapy, observation group based on the combined application of Shugan Tongluo Decoction, and compared the effectiveness of treatment.ResultsThe effective rate of the observation group was 92.8%, significantly better than the control group 78.6%, the difference between the two groups significantly, with statistical significance (P<0.05).At the same time, the observation group VAS score, IL-6 score and CCL-18 score were lower than the control group, EORTC QLQ score is higher than that of the control group, the treatment of feedback better(P<0.05).In addition, the incidence of adverse reactions was 4.7%, 9.5% in the control group, the observation group treatment.ConclusionThe prescription of Shugan Tongluo decoction has a good effect on the treatment of upper limb swelling after breast cancer surgery, and it has high safety and good patient satisfaction.

7.Endoscopic ultrasonography and endoscopic treatment of pancreatic diseases

The principle of Endoscopic Ultrasonography (EUS) is similar to abdominal ultrasonography. The difference between them is the installment of a high-frequency ultrasound miniature probe at the tip of the endoscope for real-time ultrasound scanning. The pancreas is a retroperitoneal organ. The insertion of the ultrasonic endoscope into the stomach and duodenum can clearly demonstrate the structure of the pancreas on ultrasound scanning. With the extensive application of EUS and development of endoscopic accessories, EUS is becoming more important and it plays an important role in the minimally invasive treatment of pancreatic diseases. EUS-FNA and injection technique, EUS-CPN,EUS-guided radiation seed implantation, endoscopic ultrasound-guided puncture and drainage of pancreatic pseudocyst broaden the application of EUS techniques. EUS will become another important minimally invasive endoscopic treatment for pancreatic diseases following ERCP.
8.Multi-strategic mass spectrometry technological confirmation of poisoning causes in a patient with unexplained multi-organ damage

Abstract: Objective To investigate the cause of poisoning leading to multi-organ damage in a patient with an unexplained condition, to confirm the type and exposure amount of the pathogenic factor, to identify the sources of pathogenic factors, and to provide references and bases for the clinical treatment and the prevention of such events. Methods Starting with the unknown traditional Chinese medicine taken by patients for a long time, the targeted screening strategy was initially used to screen for alkaloid poisoning. Subsequently, a non-targeted screening strategy using Information-dependent Acquisition Mode (IDA) was used to screen the patient's blood, urine, and drug samples. Combining the toxicological effects of suspected compounds with the patient′s clinical manifestations, the main pathogenic factors were determined. Quantitative methods were established according to standard substances to quantify the pathogenic factors in all the samples. An epidemiological investigation was conducted to obtain the patient's medication history, and combined with the examination of the medication's specifications and content, the exposure dose of each pathogenic factor was determined. Finally, a Data data-independent acquisition (DIA) mode termed Sequential Window Acquisition of All Theoretical Fragment-Ion Spectra (SWATH) was used to screen all samples, unbiasedly collecting secondary mass spectrometry information of compounds, thereby verifying and supplementing the confirmed pathogenic factors. Results Targeted screening ruled out common alkaloid poisoning. Tadalafil was detected in the patient's blood and urine. Tadalafil, sildenafil, chloropretadalafil, acetaminophen, and diclofenac were detected in unknown traditional Chinese medicine. The side effects of these compounds were consistent with the clinical manifestations of the patient. The highest daily average exposure doses of tadalafil, sildenafil, chloropretadalafil, acetaminophen, and diclofenac were 30.5 mg, 15.8 mg, 0.05 mg, 45.6 mg, and 3.2 mg respectively, which were seriously excessive. The SWATH mode also screened out the above five drugs. In addition, palmatine chloride was also detected. Conclusions Combining the patient's clinical manifestations and epidemiological data, this study integrated targeted screening, untargeted screening, targeted quantitative strategies, data-dependent, and DIA mode to confirm that this case is a drug poisoning event caused by long-term overdose consumption of traditional Chinese medicine adulterated with chemical components. This study provides insights for the diagnosis and investigation of patients with poisoning for unknown causes, offering references for emergency detection and management of related poisoning incidents.
