Colorectal cancer is one of the most common malignant tumors. Though the development of multidisciplinary therapy has largely improved the therapy effects, many patients still died of local recurrence and metastases after surgery. Circulating tumor cell (CTC) originates from primary tumor tissues and has a close relationship with cancer metastases and prognosis. This review summarizes the CTC detection methods and relevant clinical research on CTC in recent years.
3.Research progress in Caveolin-1s' role in gastrointestinal cancer

Caveolin-1 is an important surface structural marker of Caveolae. It is closely involved in the proliferation, apoptosis, invasion, metastasis, angiogenesis, signal transduction and multi-drug resistance of various tumor cells. Recent studies have found that altered Caveolin-1 expression has dual roles, as a tumor suppressor and promoter, in different stages of gastrointestinal cancer development and progression. The exact mechanisms of Caveolin-1s' involvement in the development of gastrointestinal cancer remain to be clarified and have attracted the attention of a lot of researchers.
6.THE PREOPERATIVE AND POSTOPERATIVE MANAGENMENT IN LUGN VOLUME REDUCTION SURGERY FOR PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Objective:To evaluate the preoperative and postoperative management in lung volume reduction surgery for patients with chronic obstructive pulmonary disease.Methods:All 7 patients were male.Age ranged from 65~76 years.Preoperative dyspnea index was grade 4 in 4 and grade 3 in 3.FEV1 were all less than 50%.We performed unilateral Lung volume reduction surgery on the lung that was estimated to have poorer lung function.The resected lung volume was 20%~30% of the operated.Results:2 tracheotomy were performed.Postoperative oxygen supply lasted 6~8d.The symptoms such as palpitaion,dyspnea were improved significantly.FEV1 and dyspnea index were respectively increased by 30%~45% and 1~2 grade.All patients recovered.Conclusions:Preoperative adjustment of the balance of water and electrolyte,proper respiratory exercises,resasonable postoperative oxygn supply and maintenance of unobstructed respiratory tract,all these are important.Moreover,tracheotomy shoulde be perfored promptly in acute cases.
7.Comparative study of efficacy and stability of small incision lenticule extraction, FS-LASIK and LASIK for myopia with a follow-up of 6 months

AIM:To evaluate the efficacy and stability by comparing acuity and diopter of small incision lenticule extraction ( SMILE) , femtosecond laser in situ keratomileusis ( FS-LASIK ) and laser in situ keratomileusis ( LASIK ) in treating myopia with a follow-up of 6mo.
METHODS: A retrospective study, 42 cases ( 84 eyes ) received SMILE, 37 cases ( 74 eyes ) received FS-LASIK and 31cases (62 eyes) undergone LASIK in our hospital during Apr. 2014 to Jun. 2014 were involved. The follow-up data of 6mo was analyzed. The preoperative spherical equivalent was -5. 91±1. 83D, -5. 89±1. 96D, -5. 88±1. 68D in SMILE, FS-LASIK and LASIK group, respectively. The differences of preoperative best corrected visual acuity ( BCVA ) , pupil diameter ( PD ) and central corneal thickness ( CCT ) had no statistically significant between three groups. The postoperative uncorrected visual acuity ( UCVA) , BCVA and diopter were comparative analyzed at 1wk, 1, 3 and 6m after surgery.
RESULTS:1) No patients lost to follow-up of 1wk and 1mo. A total of 10 eyes (5 cases), 10 eyes (5 cases) and 8 eyes (4 cases) lost to follow-up of 3m in SMILE、FS-LASIK and LASIK group, respectively, and raised to 18 eyes (9 cases), 12 eyes (6 cases) and 14 eyes (7 cases) in follow-up of 6m. 2) At 1wk follow-up, the differences of UCVA between SMILE group, FS- LASIK group vs LASIK group was statistically significant respectively ( t=4. 098, P=0. 000;t=2. 493, P=0. 004). 3) In LASIK group, the differences of UCVA between 1wk vs 3, 6m follow-up was statistically significant respectively (t=3. 410, P=0. 001;t=3. 771, P=0. 000), the differences of UCVA between 1m and 6m follow-up was statistically significant (t=2. 283, P=0. 026). 4) The differences of diopter were not statistically significant among three groups at 1wk, 1, 3 and 6mo follow-up respectively (χ2=0. 119, P=0. 942;χ2=1. 504, P=0. 471;χ2=0. 949, P=0. 622; χ2=0. 277, P=0. 871). 5) the differences of eyes with UCVA≥5. 0 was statistically significant between SMILE group vs FS-LASIK group, LASIK group at 1wk follow-up (χ2=9. 249, P=0. 002<0. 05/3;χ2=12. 906, P=0. 000<0. 05/3), there was no significant statistical difference between FS-LASIK group and LASIK group (χ2=0. 500, P=0. 604). 6) there was no significant statistical difference of eyes with SE (±0. 50D) at any time post operation among three groups (χ2=0. 809, P=0. 697;χ2=1. 176, P=0. 634;χ2=0. 871, P=0. 736;χ2=0. 683, P=0. 770).CONCLUSION: All of SMILE, FS-LASIK and LASIK are effective and stable on treating myopia according to follow-up of 6mo. However, in this study, SMILE group shows more effective than FS-LASIK and LASIK at 1wk, which could enhance postoperative UCVA more rapidly.
8.Changes of Ultrastructure of Muscle in Rats with Spastic Palsy after Injected with Botulinum Toxin

Objective To observe the ultrastructural changes of end-plate and gastrocnemius muscle of rats with spastic palsy after injected with botulinum toxin type A in order to provide scientific base of histomorphology for clinical use of botulinum toxin type A.Methods Wistar rats′ pyramidal tracts in experiment group were injured by electric current 2.5 mA for 30 s,twice,but 36 normal control rats were only drilled hole in skull(control group).Seventy-two rats were successful in building spastic palsy model.Botulinum toxin type A (6 U/kg) were injected into right gastrocnemius muscles of 36 spastic palsy rats (group B),and physiological saline were injected into the same muscles in 36 spastic palsy rats as control(group A).Control group were not injected.The rats of group A,group B were sacrificed on 3,7,15,30,60 and 90 days after botulinum toxin therapy,and right gastrocnemius muscles was separated for histological analysis.Results Under the electron microscope,the structure of muscle fiber in group B had changed(Z line rupture,disorder and abolition,myofilament lysis,triad tract diso-rder,vacuolar degeneration) and these changes in group B occurred earlier and were severer than those in group A.There was compensatory hypertrophy of muscle fiber at later stages in group B.During the first 15 days after botulinum toxin type A injection,there were many synaptic vesicle without pre-synaptic membranes near terminal portion of nonmedullated nerve fibers.Following this,between 15 to 30 days,there were many folds similar to postsynaptic membranes in the cellular membranes and there were many grains in it.But there was no synaptic vesicle and pre-synaptic membranes near them.Compensatory hypertrophy and atrophy of muscle fiber coexist at 90 days after injection in group B.Conclusions Injection of botulinum toxin type A maybe induce the sprout of nerve and degeneration of the partial postsynaptic where there are many grains.It also accelerates muscle atrophy,but it induces compensatory hypertrophy of muscle fiber at later stage.It implies that injection of botulinum toxin type A can improve spasm symptom of the gastrocnemius muscles and this is good for muscle fiber repairing.
9.Efficacy of ropivacaine combined with fentanyl for patient-controlled epidural analgesia after suprapubic prostatectomy

Objective To investigate the efficacy of patient-controlled epidural analgesia(PCEA)with ropivacaine plus fentanyl for postoperative analgesia after suprapubic prostatectomy(SPP).Methods Thirty-six ASAⅡorⅢpatients aged 62-78 yrs weighing 52-84 kg undergoing SPP were randomly divided into 3 groups with 12 patients in each group:groupⅠreceived PCEA with 0.12% ropivacaine and fentanyl 1?g?ml~(-1);groupⅡreceived PCEA with 2% ropivacaine and fentanyl 1?g?ml~(-1) and groupⅢreceived patient-controlled intravenous analgesia(PCIA)with morphine 0.2 mg?ml~(-1).Postoperative pain was assessed using VAS(0=no pain,10= worst pain).The degree of motor blockade(modified Bromage scale)was evaluated at 6,24,48 and 72 h after operation(T_(1-4)).Bladder spasm episodes were recorded.Side effects including nausea,vomiting and pruritus and the total number of RBC in rinse solution were also recorded.Results(1)Bromage score(0=no motor block,3 =inability to flex ankle joint)was significantly higher in groupⅠandⅡthan in groupⅢat T1(6 h after op.), but significantly lower in groupⅠandⅢthan in groupⅡat T_(2-4)(24-72 h after operation).(2)VAS score(0= no pain,10=worst pain)was not significantly different among the 3 groups without bladder spasm but significantly higher in groupⅢthan in other 2 groups during episodes of bladder spasm.(3)The number of bladder spasm episodes was not significantly different among the 3 groups within 6h after operation,but was significant larger in groupⅢthan in the other two groups during 6-72 h after operation.During 6-24 h after operation more bladder spasm attacks occurred in groupⅠthan in groupⅡ.(4)The incidence of side effects was significantly lower and the total number of RBC in rinse solution was significantly smaller in groupⅠandⅡthan in groupⅢConclusion PCEA with 0.12% or 2% ropivacaine plus fentanyl can effectively reduce the number of bladder spasm attack and postoperative bladder bleeding.Different concentrations of ropivacaine should be used for PCEA during different postoperative periods.
10. Effect of Semen Euphorbiae and Semen Euphorbiae Pulveratun extracts on intestinal flora in rats

Objective To study the effect of Semen Euphorbiae and Semen Euphorbiae Pulveratun on the microflora in rats gastrointestinal from the aspect of the intestinal microecology, so as to provide reference for further revealing mechanism of the attenuation after processing of Semen Euphorbiae. Methods Rats were fed with high, medium and low dose of Semen Euphorbiae and Semen Euphorbiae Pulveratun, and the plate-counter method was used to examine the four types of intestinal flora in the feces of rats which are Bifidobacterium, Lactobacillus, Escherichia coli, and Enterococcus. Results After intragastric administration, the rat had the alteration of intestinal flora, and the degree of alteration of intestinal flora in the Semen Euphorbiae Pulveratun group was lower than the group of Semen Euphorbiae. In the low doses of group, the number of Enterococci and Escherichia coli was decreased. Conclusion Semen Euphorbiae and Semen Euphorbiae Pulveratun could affect the intestinal microbial diversity and the balance of intestinal flora. The effect of Semen Euphorbiae Pulveratun on four kinds of intestinal flora was weakened, and the degree of intestinal flora disturbance was reduced, which was consistent with the results of alleviation of the effect of Semen Euphorbiae Pulveratun. It was suggested that the intervention effect of Semen Euphorbiae on intestinal microflora could be clarified from the perspective of intestinal microecology, which could reveal possible correlation between Semen Euphorbiae Pulveratun and the changes of intestinal flora.
