Objective To access the feasibility of ueing "flexible Uitrasonic cystoscopy (FUCS)" (Uitrasonic bronchoscope Olympus BF-UC260F-OL8 as a subslitute) for staging of bladder cancet Methods From Jun 2010 to Nov. 2010,36 patierrts with piiimary bladder urotheiial carcinoma were examined by "FUCS" before operation and were rttaged by "FUCS" findings. All patients rectived IVP and enhanced pdvc CT Non-muscle-invasive bladder cancer patierrts rem-ved TURBT, and the tumor base was routinely bopied for pathology Pathological results, traditional imaging eraliiaiton, and "FUCS" evaluation were compared. Visual analog scores (VAS) were also compared between patierrts reccing "FUCS" and rcmime cystoscopy. Results Compared with conventional cyrtoscopy, the VAS of patients receiving intraoperative "FUCS" was significantly tower (1 3 ± 0. 5 vs 4. 3 ± 0. 9, P<0. 01). Of the 36 patierrts, 28 were diagrosed as non-muscle-invasive bladder cancer (Ta + T1) and 8 were diagrosed as muscle-invasive bladder career (T2 + T3). Based on the postoperative pathologic results, "FUCS" had a 100% accuracy in diagnoeing norem Uselelinvaeive and muscle-invasive bladder cancer. Conclution "FUCS" is painless and feasible in clinical practice; it can display the depth of bladder wall invaded by tumor, and can be used for preoperative staging of primary bladder tumor.
3. Application of ureteroscope-assisted laparoscopic varicocelectomy with two trocars only: A clinical report of 11 cases

Objective: To evaluate the application of ureteroscope-assisited laparoscopic varicocelectomy with two trocars only. Methods: We retrospectively analyzed the clinical efficacy and safety of ureteroscope-assisited laparoscopic varicocelectomy with only two trocars in management of 11 patients with grade II-III varicocele, who were treated in our hospital between Jun. 2008 to Jun. 2009 and were followed up for 3-15 months. Results: The operation was successful in all the 11 cases. The mean operation time was (19.4 ± 1.1) min (ranging 16-28 min). There were no complications after the procedure; the patients were allowed for ambulation on the next day after operation and were discharged on the third day. The sperm count was (2609 ± 208) × 104/ml three months after operation, which was significantly higher than that before operation (1463 ± 128) × 104/ml (P < 0.01). There was no recurrence or testis atrophy. Conclusion: Ureteroscope-assisited laparoscopic varicocelectomy with only two trocars is safe, effective, and easy-to-perform, and it has less complications and trauma.
4. Application of central venous catheter for refractory urinary retention in children: A report of 7 cases

Objective: To assess the value of central venous catheter in treatment of refractory urinary retention in children. Methods: Seven boys with acute urinary retention, who underwent failed routine drainage from Aug. 2007 to Jun. 2008, received paracentric suprapubic cystostomy via disposable central venous catheter. The patients were followed up for 2-11 months and the outcomes and complications were observed. Results: The catheterization processes were successful at the first try, with the mean operation time being (3.2 ± 0.4) min, ranging 2-5 min. The drainage outcomes were satisfactory and there were no complications. Conclusion: Suprapubic puncture and drainage with central venous catheter is safe and effective for children with urinary retention and is worth popularizing.
5.Present status in studying immunotherapy for acute leukemia and its perspective--Editorial.

One of the important approaches for further prolonging remission duration and eradicating minimal residual disease in acute leukemia is immunotherapy. Four kinds of immunotherapy for acute leukemia are under investigation: (1) monoclonal antibodies, among them, Mylotarg (cytotoxic antibiotic calicheamicin linked to CD33 Mab) is given for the treatment of refractory or relapsed acute myeloid leukemia and molecular relapse in acute promyelocytic leukemia with good results, Campath-1H (antiCD52 Mab) is administered in the treatment of prolymphocytic leukemia and Rituximab (anti-CD20 Mab) in B-PLL with high complete remission rates. Other Mabs under preclinical and clinical trials include anti-IL-2 receptor Mab for the treatment of acute T lymphocytic leukemia, anti-220 kD Mab-6G7 for acute leukemias, recombinant immune toxin BL22 (anti-CD22) for hairy cell leukemia and Mabs labeled with radio-isotopes for different types of acute leukemias; (2) adoptive cellular immunotherapy using cytokine-induced killer cell, alloreactive NK cells, allogeneic or autologous leukemic-specific CD8(+) cytotoxic T lymphocytes, and other immune effector cells; (3) cytokines and other immune modulators comprising IL-2, IL-12, GM-CSF, CD40L, FLT-3L and thalidomide and its derivatives; (4) leukemia vaccines of several different formulations including antigen-specific, leukemia cell-based, leukemia antigen-pulsed dendritic cell (DC) and leukemia-derived DC vaccines, the latter two formulations are more attractive. In conclusion, up to now, the most effective example of immunotherapy in acute leukemia is provided by the administration of Mabs, and the majority of other approaches in immunotherapy for acute leukemia although promising, need further studies.

8.Three dimensional conformal radiotherapy for metastasis of retroperitoneal lymph node

Purpose:To evaluate the feasibility of irradiat ion to the metastatic retroperitoneal lymph nodes using three-dimensional confo rmal radiotherapy (3-DCRT) and to assess the irradiation side effects. Methods:From Jun. 1997 to Sep. 2000, 52 patients of metastatic retroperitoneal lymph nodes were treated with 3-DCRT.These patients had receive d radical therapy for their primary tumors. 50 of them had been proved by pathol ogical examination. Radiotherapy was designed by Elekta company’s 3-D treatmen t plan. Dose volume histography (DVH) was used to verify and optimize the radiat ion plan. Using 5~6 conformal portals,the total irradiation dose was 40 Gy an d delivered at 4~5Gy per fraction every other day,to a total of 8~10 fractions in 16~20 days. The immediate responses and side effects were evaluated accordi ng to the WHO and RTOG criterion system. Results:According to Karnofsky criterion system the median scor e was raised 21.3. Three months after irradiation, the complete ,partial and tot al response rates were 38.6%(27/70), 54.3%(38/70)and 92.9%(65/70),respect ively. The early response was related to the target volume and pathological clas sification. The smaller the target volume, the better the response(? 2 = 1 5.211,P=0.004).The anaplastic carcinoma cases had better results than the squamous carcinomas and adenocarcinomas(? 2 =13.455,P=0.009).The 1- ,2- and 3- year survival rates were 59.6%(31/52)、23.1%(12/52)and 9.6%(5/ 52),respectively. Conclusions:Three dimensional conformal radiotherapy can be con sidered as an effective and feasible approach for the treatment of metastatic r etroperitoneal lymph nodes,and no serious complications were observed.
9.An evaluation of the efficacy of long pulsed Nd:YAG laser in the treatment of onychomycosis

Objective To evaluate the efficacy and safety of long pulsed Nd:YAG laser in the treatment of onychomycosis.Methods Thirty-five patients with onychomycosis were treated by long pulsed Nd:YAG laser with a wavelength of 1064 nm,power of 30-40 J/cm2,pulse width of 35 milliseconds,diameter of light spot of 4 mm.One treatment session included 4 times of irradiation at an interval of 2 minutes.Patients were treated once a week for 4 weeks followed by once a month for 6 months.Clinical and mycological efficacy was evaluated 9 months after the beginning of treatment.Side effects were recorded and analyzed.Nails were divided into various groups according to causative fungal species,clinical phenotypes,and distribution of affected nails.Data were processed by using SPSS 17.0 software.Chi-square test was performed to compare the response and recurrence rate between these groups.Results A total of 79 nails were treated by long pulsed Nd:YAG laser in this study.At 9 months after the first treatment,clinical response was observed in 67.1% of these nails,mycological response in 73.4%,and recurrence in 19.0%.The Trichophyton rubrum group showed a better clinical (x2 =10.913,P < 0.05) and mycological response (x2=13.532,P < 0.05),but a lower recurrence/reinfection rate (x2 =10.980,P < 0.05) compared with the other-species group.No significant difference was observed between the Trichophyton rubrum group and Candida albicans group in the clinical and mycological response rate or recurrence/reinfection rate.The clinical response in the distal lateral subungual onychomycosis (DLSO) group was significantly poorer than that in the white superficial onychomycosis (WSO) group (x2 =11.935,P < 0.05),but better than that in the proximal subungual onychomycosis (PSO)/total dystrophic onychomycosis (TDO) group (x2 =17.515,P < 0.05).Increased clinical response rate was observed in the second-fifth finger nail group compared with the thumb nail/second-fifth toe nail group (x2 =13.437,P < 0.05)and hallux nail group (x2 =10.595,P< 0.05),while no statistical difference was observed in clinical response rate between the hallux nail group and thumb nail/second-fifth toe nail group (x2 =3.030,P> 0.05),or in mycological response rate or recurrence/reinfection rate among the second-fifth finger nail group,thumb nail/second-fifth toe nail group and hallux nail group.There was no obvious adverse reaction but pain during the treatment.Conclusions Long pulsed Nd:YAG laser appears to be an effective and safe approach to the treatment of onychomycosis,and the treatment outcome is affected by clinical types,location,and causative fungal species of onychomycosis.
10.The emergency treatment of iliac artery rupture in the process of iliac artery angioplasty

Objective To evaluate covered stent placement in the treatment of iliac artery rupture in the process of iliac artery angioplasty.Methods Clinical data of 9 patients' iliac artery rupture in the process of iliac artery angioplasty were retrospectively reviewed.Results Covered stent was successfully placed in all cases.The patency of involved iliac artery was identified in 8 cases by angiography and there was no contrast extravasation.Type Ⅰ endoleak occurred in one patient in the proximal end of the stent graft and iliac artery graft replacement was done by open surgery immediately.Postoperatively clinical symptoms were relieved in all these 9 patients.One case lost to follow up.8 patients were followed up for 14 to 45 months,average (22 ±9) months.Stenosis on the distal end of the covered stent (external iliac artery)was found in one patient who underwent external iliac artery balloon-expandable stent angioplasty.One patient with artificial vessel placement was symptoms free with patency of the involved artery as reviewed by ultrasonography on 18 month followed up.Conclusions When iliac artery rupture in the process of iliac artery angioplasty should develop,the first choice is immediately covered stent placement therapy,while open surgery remains as the last resort.
