Chinese Journal of Primary Medicine and Pharmacy 2021;28(4):592-596
Objective:To investigate the efficacy of modified Zhigancao decoction on Qi/ Yin-deficiency acute viral myocarditis and its effect on serum level of high mobility group box chromosomal protein 1 (HMGB-1). Methods:Eighty-eight patients with Qi/ Yin-deficiency acute viral myocarditis who received treatment in Chun'an Hospital of Traditional Chinese Medicine from July 2017 to December 2018 were included in this study. They were randomly assigned to undergo either standard conventional intervention (control group, n = 43) or treatment with modified Zhigancao decoction based on standard conventional intervention (observation group, n = 45) for 4 weeks. After treatment, serum levels of cardiac troponin I (cTnI) and HMGB1, traditional Chinese medicine symptom score, and clinical efficacy were compared between the two groups. Results:After 2 and 4 weeks of treatment, serum level of cTnI in the observation group was (590.22 ± 78.42) ng/L and (461.36 ± 52.44) ng/L, respectively, which was concurrently significantly lower than that in the control group, respectively [(621.41 ± 86.12) ng/L, (587.62 ± 67.94) ng/L, ( t2 weeks = 1.574, t4 weeks = 9.785, both P < 0.05). After treatment, traditional Chinese medicine symptom score in the observation group was significantly lower than that in the control group ( tfever = 27.650, tsore throat = 27.210, tpalpitation = 14.560, tvexation and chest tightness = 15.370, all P < 0.01). Total effective rate in the observation group was significantly higher than that in the control group [93.33% (42/45) vs. 72.09% (32/43), χ2 = 8.41, P < 0.05). After 2 and 4 weeks of treatment, serum level of HMGB1 in the observation group was concurrently significantly lower than that in the control group ( t2 weeks = 9.144, t4 weeks = 11.534, both P < 0.01). Conclusion:Modified Zhigancao decoction combined with conventional western medicine intervention for the treatment of Qi/ Yin-deficiency acute viral myocarditis exhibits great therapeutic effects possibly through downregulating serum level of HMGB1.
2.The application of serum antibody to group A streptococcal polysaccharide in the diagnosis of atypical rheumatic fever
Chinese Journal of Rheumatology 2001;0(01):-
Objective To investigate the diagnostic value of antibody to group A streptococcal polysaccharide (ASP) in diagnosing atypical rheumatic fever (RF). Methods Fifty-seven cases including 33 atypical RF and 24 recurrent attack of RF with chronic rheumatic heart disease were included, and 57 patients suffered from other diseases were used as control. Nitrous acid extraction and molecular sieve chromatography was adopted to extract and purify the polysaccharide. The isolated portion of polysaccharide which possessed strongest antigenity was used to detect the level of serum ASP by enzyme-linked immunosorbent assay (ELISA). Erythrocyte sedimentation rate (ESR), antistreptolysin-O (ASO) and anti-deoxyribonuclease B (anti-DNase B) were tested with the same samples. Results The sensitivity and specificity of ASP in the diagnosis of atypical RF was 73.7% and 76.6%; while 47.4% and 46.4% for ESR respectively. As for the evaluation of streptococcal infection, the sensitivity and specificity of ASP, ASO and anti-Dnase B were 73.7%, 15.6%, 65.4% and 76.6%, 94.6%, 44.6%, respectively. ROC curve analysis and discriminant analysis were adopted to compare these laboratory measurements. The results demonstrated that ASP was superior to other methods. Conclusion ASP has a higher sensitivity together with a higher specificity than the other three methods for diagnosing the activity of RF and the infection of streptoccus. It should be a promising diagnostic test for atypical RF.

Chinese Journal of Primary Medicine and Pharmacy 2016;23(12):1832-1834
Objective To investigate the clinical effect of cefoperazone sodium and sulbactam sodium. Methods 56 cases who used cefoperazone sodium and sulbactam sodium for anti -infection were selected as the observation group, and 56 hospitalized patients who used cephalosporin ceftazidime were selected as the control group. The observation group was intravenously injectioned cefoperazone sodium and sulbactam sodium, the control group received intravenous ceftazidime. The clinical efficacy and adverse reactions of the two groups were compared. Results After treatment, the cure rate and total effective rate of the observation group were 55.36%,98.21%,which of the control group were 35.71%,91.07%.The cure rate of the observation group was significantly higher than that of the control group (χ2 =4.356,P <0.05),and the total effective rate between the two groups had no statistically signifi-cant difference (χ2 =2.817,P >0.05).The incidence rate of adverse reactions of the two groups had no significant difference. Conclusion Cefoperazone sodium and sulbactam sodium therapy has good clinical efficacy in the treat-ment of infectious diseases, and has low incidence of adverse reactions, good safety, which can be popularized and applied in clinic.
9.Effects of Different Anesthesia Techniques on the Balance of Th1/Th2 in Patients Undergoing Radical Resection of Gastric Cancer
Acta Medicinae Universitatis Scientiae et Technologiae Huazhong 2014;(4):405-408
Objective To explore the optimal method of anesthesia in radical resection of gastric cancer by comparing the effects of two anesthesia techniques on the balance of Th1/Th2 in order to provide evidence for clinical anesthesia manage-ment.Methods Forty patients who underwent elective radical resection of gastric cancer were randomized into two groups:in-halation anesthesia group(group S)and total intravenous anesthesia group(group P),in which sevoflurane and propofol were ad-ministered for the induction and maintenance of anesthesia, respectively. The general data of the patients were collected, the tis-sue type of tumors obtained and the fluid infusion and blood loss volumes recoded. The VAS score was obtained 1 h,6 h and 5 d after the surgery. Before the induction of anesthesia,6 h and 5 d after surgery, the blood samples were drawn to determine the serum levels of IL-1β,IFN-γ,TNF-α,IL-2,IL-6 and IL-10.Flow cytometry was used to detect the proportion of CD3+CD8-IFN-γ+ and CD3+CD8-IL-10+ cells and the Th1/Th2 ratio was calculated. Results The level of IL-6 was increased post-sur-gically in groups S and P(P<0.05).There were no significant changes in the levels of IL-1β,IL-2 and IL-10 in groups S and P and in the levels of TNF-αand IFN-γ,and the Th1/Th2 ratio in group S between before the induction of anesthesia and different time points after the surgery(P>0.05).The levels of TNF-αand IFN-γ,and the Th1/Th2 ratio were significantly increased in group P 5 d after the surgery(P<0.05).Conclusion Total intravenous anesthesia with propofol can enhance Th1 response and it is an ideal anesthetic method for radical resection of gastric cancer.
10.Effect of angiotension Ⅱ on Apelin mRNA and APJ mRNA expression in pulmonary microvascular endothelial cells in rats
Chinese Journal of Anesthesiology 2008;28(9):824-827
Objective To investigate the effects of angiotensinⅡ (AngⅡ) on the Apelin mRNA and APJ mRNA expression in pulmonary microvascular endothelial ceils (PMVECs) in rats. Methods Pulmonary microvascular endothelial ceils obtained from 24 h old neonatal SD rats were cultured in DMEM liquid culture medium. The 2nd-4th generation PMVECs were inoculated on 6-well plates (5×105). The experiment was performed in two parts. In part Ⅰ different concentration of AngⅡ 0, 10-9, 10-8, 10-7, 10-6mol/L (group Ⅰ- Ⅴ) were added into the PMVECs. The expression of Apelin mRNA and APJ mRNA was determined at 24 h after addition of Ang Ⅱ by RT-PCR. In part Ⅱ the cells were exposed to 10-7 mol/L Ang Ⅱ. The expression of Apelin mRNA and APJ mRNA was determined immediately and at 1, 6, 12, 24, 48 h(group Ⅰ-Ⅵ) after addition of Ang Ⅱ by RT-PCR. Results In part Ⅰ Apelin mRNA expression was significantly higher in group Ⅱ (Ang Ⅱ 10-9 mol/L) but lower in group Ⅲ-Ⅴ (AngⅡ 10-8, 10-7, 10-6 mol/L) than in group Ⅰ (control, Ang Ⅱ 0 mol/L). The APJ mRNA expression was significantly lowered in group Ⅱ-Ⅴ in a dose-dependent manner as compared with control group (group Ⅰ). In partⅡ beth Apelin mRNA and APJ mRNA expression exhibited a bi-phasic response to AngⅡ 10-7 mol/L, increased at first and was then decreasing with time. The Apelin mRNA and APJ mRNA expression reached the peak at 1 h of incubation with Ang Ⅱ respectively. Conclusion Ang Ⅱ decreases both Apelin mRNA and APJ mRNA expression in PMVECs in a dose and time dependent manner. The down-regulation of Apelin mRNA and APJ mRNA expression may be involved in the mechanism of injury to PMVECs.
