1.Hereditary nonpolyposis colorectal cancer:a report of 27 cases in 10 families

Objective To study the clinical characteristics of hereditary nonpolyposis colorectal cancer (HNPCC). WT5”HZMethods Ten kindreds of HNPCC in South Zhejiang area were analyzed. ResultsWT5”BZ Among the 10 families, 27 persons had colorectal cancer with age ranging from 19 to 71(average 45 3 years),74 percent of cases being diagnosed before the age of 50. The total of the colorectal cancer foci in 27 cases were 37, 40 percent of focis located proximal to splenic flexure, 30 percent of cases had multiple primary colorectal cancer, among which 14 8 percent of cases had synchronous multiple primary colorectal cancer, 18 5 percent of cases had metachronous multiple primary colorectal cancer. Thirteen cases died during follow-up of 1~23 years, 14 cases were still alive during follow-up of 1~28 years. KG2Conclusion HNPCC has definite charactistics that are of much help to early diagnosis.
2.Bone tunnel positions in anterior cruciate ligament reconstruction evaluated by three-dimensional CT reconstruction based on Mimics software:modified transtibial versus anteromedial portal technique.

OBJECTIVE:
To compare the femoral and tibial tunnel positions of anterior cruciate ligament reconstruction using the modified transtibial (MTT) technique and anteromedial (AM) portal technique.
METHODS:
Between January 2017 and September 2020, 78 patients with anterior cruciate ligament rupture underwent single-bundle reconstruction with the modified transtibial technique in 39 cases (group MTT) and through anteromedial approach in 39 cases (group AM). There were 25 males and 14 females in group MTT, with an average age of (37.0±2.3) years old; 27 males and 12 females in group AM, with an average age of (37.5±2.2) years old. CT scan of the affected knee was conducted one week after the surgery to measure and compare the femoral tunnels positioning (Fx, Fy), tibial tunnels positioning in the frontal plane(Tx1), tibial tunnels positioning in the sagittal plane (Ty1), and tibial tunnels positioning in the axial plane (Tx2, Ty2) in patients undergoing anterior cruciate ligament reconstruction through Mimics software.
RESULTS:
Three-dimensional CT reconstruction after the surgery showed that the average Fx and Fy were(25.2±2.1)% and (34.9±3.0)% respectively and the Tx1 and Ty1 were (45.5±3.3)% and (44.7± 3.0)% respectively, while the Tx2 and Ty2 were (47.0±3.0)% and (39.9±4.2)% respectively in group MTT. In group AM, the average Fx and Fy were (26.0±2.0)% and (36.1±3.9)% respectively and the Tx1 and Ty1 were (46.5±3.1)% and (45.6± 3.1)% respectively, while the Tx2 and Ty2 were (47.4±2.5)% and (39.6±3.9)% respectively. There were no statistically significant differences in the femoral and tibial tunnels between the two groups (
CONCLUSION
Both the MTT and AM technique can achieve good anatomical positioning of the femoral and tibial tunnels, without significant differences in the positioning of the bone tunnels.

3. Pharmacokinetics and tissue distribution characteristics of sunitinib

AIM: To establish a HPLC-UV method to determine sunitinib in rat plasma and mouse tissues, and to study its pharmacokinetics in rats and tissue distribution characteristics in mice. METHODS: The biotic samples were prepared by protein precipitation followed by a stereoselective analysis of sunitinib was achieved on Waters XBridgeTM C18 (4.6 mm×250 mm, 5 μm) with a mobile phase composing of methanol-0.02 mol/L sodium dihydrogen phosphate (70:30) at a flow rate of 1.0 mL/min. The detection wavelength was 310 nm, and the column temperature was 25 ℃. RESULTS: The calibration curve for rat plasma sunitinib was linear in the range of 0.019 2-15.34 μg/mL. The linear ranges in mice brain and kidney were 0.038 3-11.50 and 0.038 3-69.00 μg/mL, respectively. After intragastric administration of sunitinib at a dose of 20 mg/kg to rats, the pharmacokinetic characteristics were Tmax=9.0 h, Cmax=0.194 mg/L, t1/2=18.4 h, AUC(0-∞)=6.8 mg•L-1•h. And the absolute bioavailablity was 47.1%. It was indicated that sunitinib could permeate the blood brain barrier, but the concentration was lower in brain and higher in kidney. CONCLUSION: A HPLC-UV method for the determination of sunitinib in rat plasma and mouse tissues was established. The method is simple, rapid, reliable, and provides a reference for the clinical application of sunitinib.
5.Treatment of ankylosing spondylitis with thoracolumbar fractures by robot-assisted internal fixation in lateral decubitus position.

OBJECTIVE:
To explore the effect of robot-assisted internal fixation in lateral decubitus position for the treatment of ankylosing spondylitis (AS) complicated with thoracolumbar fractures.
METHODS:
The clinical data of 26 patients with ankylosing spondylitis complicated with thoracolumbar fractures treated from January 2018 to June 2020 was retrospectively analyzed. According to different surgical methods, these patients were divided into observation group and control group. There were 8 patients in observation group, which were treated with robot-assisted percutaneous screw fixation in lateral decubitus position, including 4 males and 4 females, aged form 55 to 85 years old with an mean of (66.25±9.42) years, the course of disease was (4.00±0.76) days on average, 2 cases were T11 fracture, 2 cases were T12, 3 cases were L1 and 1 case was L2. And there were 18 patients in control group, which were treated with conventional percutaneous screw fixation in prone position, including 6 males and 12 females, aged from 48 to 81 years old with a mean of (61.22±9.53) years, the course of disease was (4.11±0.83) days on average, 2 cases were T10 injury, 3 cases were T11, 4 cases were T12, 7 cases were L1, and 2 cases were L2. The intraoperative blood loss, operation time, position time and postoperative neurological complications were compared between two groups. Postoperative visual analogue scale (VAS) at 1d and 3 months, and Oswestry Disability Index (ODI) before and 3 months after operation were observed. According to Gertzbein-Robbins standard to evaluate the accuracy of pedicle screw placement.
RESULTS:
There was no nerve injury due to pedicle screw placement in both groups. The intraoperative blood loss in observation group and control group was (34.13±4.61) ml and (78.17±22.02) ml, operation time was(92.13±9.82) min and (106.22±11.55) min, position time was(10.00±2.14) min and (15.17±2.66) min, the differences was statistically significant(P<0.05);VAS of the two groups were (2.38±0.52) points and (4.56±0.98) points one day after surgery, respectively, with statistically significant differences (P<0.05), while VAS and ODI three months after surgery showed no statistically significant differences (P>0.05). The screw accuracy was 96.88%(62/64) in observation group and 81.48%(88/108) in control group, the difference was statistically significant(P<0.05).
CONCLUSION
Robot-assisted internal fixation in lateral decubitus position for the treatment of ankylosing spondylitis complicated with thoracolumbar fractures can shorten the position time and operation time, significantly improve the accuracy of internal fixation screw placement, relieve the early postoperative pain, reduce intraoperative blood loss and postoperative complications, and facilitate the fast track rehabilitation of patients.

6.Assessing the significance of platelet count to in-vitro hemolytic test by roller pump.

This study was directed at estimating the value of platelet count in the course of in-vitro hemolytic tests by roller pump. Five paired in-vitro tests were conducted using the POLYSTAN pump(group A) and COBE pump(group B). The pumps ran 16 hours. The samples for platelet count and plasma-free hemoglobin were taken before pumping, and every 2 hours after 4 hours of pumping. The 2 groups' platelet count and plasma-free hemoglobin levels were found to be linearly increasing with the hours of pumping and linear regression analysis showed the platelet count and plasma-free hemoglobin were positively correlated. These data suggested that platelet count could be used as an index in evaluating the in-vitro hemolytic properties of blood pumps.

7.Study on mechanism of tea polyphenols in inducing human lung cancer cell apoptosis in vitro.

OBJECTIVETo investigate the apoptosis inducing effect of tea polyphenols (TPP) on human lung cancer cell (LCC) and its associative mechanism.
METHODSThe apoptosis inducing effect of TPP on LCC in vitro, and its influence on expression of the related gene were determined by MTT assay, laser scanning confocal microscopy and flow cytometry.
RESULTSTPP in different concentration (50,100,200 and 400 microg/ml) had dose-dependent inhibitory effect on LCC, the inhibitory rate was 28.69+/-1.27% ,46. 19+/-1.79% ,64.61+/-1.29%, 75.90+/-1.96%, respectively. The inhibited LCC were blocked in (G0/G1 phase, and could not transferred to S and G2/ M phase of cell cycle. Meanwhile, TPP could induce apoptosis of LCC, the apoptotic rate being 4.76+/-0.11 %, 5.78+/-0.38 %, 10.06+/-0.67 %, 24.44+/-0.44 %, respectively. Morphologic changes of cells were seen in laser scanning confocal microscopy observation. Compared to the control group, intracellular Ca2+ concentration, Annexin V expression, phospatase and tensin homologe deleted on chromosome ten (PTEN) protein and expression gradually increased, while Cyclin D1 protein expression gradually decreased in the TPP treated groups along with the increasing of TPS concentration.
CONCLUSIONTTP can induce LCC apoptosis, the mechanism is related to the change of intracellular Ca2+ concentration, PTEN protein and Cyclin D1 protein expression.

8.Event-related potentials P₃₀₀ with memory function and psychopathology in first-episode paranoid schizophrenia.

OBJECTIVETo investigate the variability of event-related potentials P(300) and the relationship with memory function/psychopathology in patients with first-episode paranoid schizophrenia.
METHODSThirty patients with first-episode paranoid schizophrenia (patient group) and twenty health subjects (control group) were enrolled in the study. The auditory event-related potentials P₃₀₀ at the scalp electrodes Cz, Pz and Wechsler Memory Scale (WMS) were examined in both groups, Positive And Negative Syndrome Scale (PANSS) was evaluated in patient group.
RESULTSIn comparison with control group, patients had longer latency of P₃₀₀ [(390.6 ± 47.6)ms at Cz and (393.3 ± 50.1)ms at Pz] (P<0.01), lower amplitude of P₃₀₀ [(7.7 ± 3.4) μV at Cz and (8.5 ± 3.9)μV at Pz] (P<0.05-0.01). The memory quotient (88.1 ± 10.0) scores and short-term memory, immediate memory in patient group were damaged significantly (P<0.05-0.01). In patient group, the latency of P300 was correlated positively with PANSS scores and negatively with WMS scores (P<0.05-0.01).
CONCLUSIONFirst-episode paranoid schizophrenia has memory deficit, which can be evaluated comprehensively by P₃₀₀ and WMS. The longer latency of P₃₀₀ might be associated with the increased severity of first-episode paranoid schizophrenia.

9.Internal fixation with AO distal humerus plates for the treatment of distal humeral fractures in elderly osteoporotic patients.

OBJECTIVETo study the clinical effects of AO distal humerus plate (DHP) for the treatment of distal humeral fractures in elderly osteoporotic patients.
METHODSFrom September 2008 to January 2010, 18 elderly osteoporotic patients with distal humeral fractures were treated with open reduction and internal fixation with DHP. There were 3 males and 15 females, ranging in age from 62 to 83 years (averaged, 71.4 years). According to AO classification, 1 patient was Type A2, 3 patients were Type A3; 2 patients were Type B1, 2 patients were Type B2; 3 patients were Type C1, 4 patients were Type C2, 3 patients were Type C3. The surgical approaches were either bilateral or via olecranon process of ulna. Early mobilization was initiated after surgery. Functional results were evaluated according to the Mayo elbow performance score (MEPS).
RESULTSThe average duration of follow-up was 14.6 months (ranged from 11 to 24 months). All the patients had a complete healing of their fractures. There was no infection, hardware failure or loss of reduction after the operations. The average Mayo elbow performance score (MEPS) was (92.2 +/- 9.6) (65 to 100 points), among which the score of pain was (42.5 +/- 5.8) (30 to 45 points), the score of motion range was (19.2 +/- 1.9) (15 to 20 points), the score of stability was (9.2 +/- 1.9) (5 to 10 points), and the score of function was (21.4 +/- 2.9) (15 to 25 points). According to MEPS, 11 patients got excellent results, 5 good and 2 fair.
CONCLUSIONTreatment of distal humeral fractures in elderly osteoporotic patients with AO distal humeral plates can obtain immediate stabilization which facilitates early mobilization of the elbow.
