1.Risk factors analysis for the peptic ulcer bleeding in elderly patients

Objective To explore the risk factors for peptic ulcer bleeding in elderly patients.Methods The 414 patients with upper gastrointestinal ulcer bleeding in Xuanwu Hospital from January 2001 to January 2006 were enrolled.The patients were divided into elderly group (≥ 60years,n= 183 ) and non-elderly group ( < 60 years,n= 231 ).The coexisting diseases and hemorrhage causes were compared and analyzed.Results The detection rate of coexisting diseases was significantly higher in elderly group than in non-elderly group (68.9% vs.10.0% ).The hemorrhage causes included the taking of drugs for cardiovascular and cerebrovascular diseases or osteoarthropathy in elderly group.And the fatigue,stress and dietary upset were the main causes in non-elderly group.Helicobacter pylori infection rate was 35.0% in the elderly and 58.0% in young patients.Conclusions It is very important to promote rational use of anticoagulant drugs and analgesic agents in elderly patients for managing peptic ulcer complication.
2.Outcome of three-year follow-up for elderly people with dementia in urban and rural areas of in Beijing

Objective To analyze the influence of elderly dementia living in urban and rural areas of Beijing on their survival rate. Methods The elderly people with dementia aged 60 years and over lived in Beijing was investigated since 1997 by two step method and the mortality of the same population was followed up in 2000. Results A total of 2 788 patients were collected in 1997, and 2 721 of them were visited in 2000. Sixty seven(2 4%) of them were lost for follow up due to making a move and other reasons. Among the followed 2 721 cases , 2 307 were still alive (82 7%) and 414 died (14 9%), with the annual fatality rate being 3 79%(standardized). For all 205 cases diagnosed as dementia in 1997, the accumulated fatality rate was 42 46%(standardized), being 5 folds of the fatality for the non dementia. No significant difference was found in the fatality rate between male and female dementia. The fatality of dementia in rural area and over 75 years of age increased. Age and living conditions influenced significantly the fatality, and the fatality rate increased in patients with dementia and dysfunction of body moving. Conclusions Dementia is a disease impacting seriously the survival of elderly people.
3.Prevalence of the Mild Cognitive Impairment among Elderly in Beijing

Objective:To study the prevalence of mild cognitive impairment (MCI) among elderly people and associated factors to it.Method:A sample of 1865 elderly people aged 60 or above living in Beijing metropolitan area was investigated by a two-stage survey.In the first stage, a door-to-door visit to the subjects was performed by a trained interviewer to conduct general questionnaire including activity daily living assessment, memory and cognitive test.On the second stage, neurologists examined all screened subjects to collect family history and to finish general and neurological examination as well as neuropsychological testing.MCI was diagnosed by the consensus of 2 neurologists.Result:Among the 1865 subjects visited, 217 (11.6%) were defined to have MCI.After adjusted for age and gender the standardized prevalence reaches 8%.Within the people with MCI 22.6% was classified as vascular MCI.The prevalence of MCI was higher in people living in rural area, in people with lower educational attainment, and in people who were labor workers.The prevalence increased with age advancing.Conclusion:Similar to dementia, people with advance aged, with low education and living in rural area are those at high risk of developing MCI.

5.Study on the HPLC Fingerprint of Abrus mollis Seed

OBJECTIVE:To establish the HPLC fingerprint of Abrus mollis seed. METHODS:HPLC was performed on the column of Inertsil ODS-3 C18 with mobile phase of acetonitrile-0.1% phosphoric acid(gradient elution)at flow rate of 1.0 ml/min, detection wavelength was 270 nm,column temperature was 30 ℃ and volume injection was 10 μl. With the reference of abrine, 10 batches of A. mollis seed were analyzed,and similarity evaluation was performed by using chromatographic fingerprint of TCM. RESULTS:There were totally 12 common peaks,and the similarity was more than 0.9. According to the verification,the finger-print of A. mollis seed has good consistency with the reference fingerprint. CONCLUSIONS:The established fingerprint can pro-vide reference for the identification and quality evaluation of A. mollis seed.
6.Investigation on Strengthening the Supervision of Drug Stocktaking in Our Hospital

OBJECTIVE:To strengthen the supervision of drug stocktaking,and to improve drug management. METHODS:The data of drug stocktaking were extracted from our hospital from 2011 to 2014,and was analyzed in respects of accounts coinci-dence rate,drug profit and loss account rate,the entry and exit account between departments and original record,etc. A series of supervision strategies were established,and the data of drug stocktaking was analyzed during Jan.-Jun. in 2015 statistically to evalu-ated supervision effects. RESULTS:The supervision of drug stocktaking was strengthened through limiting the right of drug profit and loss on stocktaking,strengthening original stocktaking record management,establishing Key Drug List,setting up drug stock-taking supervision institutions,etc. During Jan.-Jun. in 2015,there was obvious improvement in the drug supervision skills,in terms of account coincidence rate,profit and loss account,material safety,cash flow rate,staff responsibility and stocktaking dura-tion,etc. More specially,the accounts coincidence rate had rose from about 82.8% to more than 95%;both the key drug accounts coincidence rate and the entry and exit account between departments coincidence rate had reached to 100%;the profit and loss ac-count rate had reduced from about 7.6% to less than 0.3%;the stocktaking time had reduced from 10 h to 3 h. CONCLUSIONS:Strengthening drug stocktaking supervision can not only improve the stocktaking efficiency and quality markedly,but also contrib-utes to the development of other pharmacy work in hospital.

8.Research on the health care needs of the elderly in Beijing

Objective To find out about the current health situation of the elderly in the Beijing area and their health care needs so as to provide basis for the formulation of health care policies for the elderly by the government and relevant departments and for the development of health care programs for the elderly in accordance with their needs. Methods A sample survey, covering such topics as the health situation, medical treatment, nursing, rehabilitation and health care needs of the elderly and community health service, was made of 2 487 elderly people aged 60 and above in different parts of Beijing, including the city proper, the part linking city and country, and the suburbs. An analysis was then made of the difference in the health situation and health care needs of the elderly between various parts and populations. Results Chronic diseases constituted the major health problem for the elderly, with hypertension, coronary heart disease, osteoarthrosis, and cerebrovascular disease ranking in the first four places and the incidence of diabetes increasing rapidly. About 10 percent of the elderly, who suffered from somatic dysfunction, had reduced daily life skills and needed assistance from others. An increased number of elderly people also meant a greater demand for medical and nursing services. Community health service, the advantages of which bacame increasingly manifest, was thus popular with the elderly. Financial difficulty was the chief factor affecting the elderly in their seeking medical service. Conclusion The elderly is a weak population in health. With the aging of the general population, the health care needs of the elderly are on the rise. As health is the major factor affecting the life quality of the elderly, enhanced efforts in the prevention and treatment of chronic diseases ought to be taken as the central task in the health care of the elderly. They are also the key to improving the life quality of the elderly.
9.Effects of breviscapine on the renal structure, function and PKC-mRNA and its protein expression in brain-dead BA-Ma mini pigs

Objective To investigate the effects of breviscapine on the renal structure, function and PKC-?mRNA and its protein expression in brain-dead BA-Ma mini pigs. Methods Fifteen BA-Ma mini pigs were randomly divided into 3 groups: brain-dead group (group A), breviscapine pretreatment group (group B), and control group (group C), 5 pigs in each group. The brain-dead models were established by increasing intracranial pressure in a modified, slow and intermittent way. At 3, 6, 12, 18 and 24 h after the initial brain death, serum BUN, Cr, TNF-?, IL-1?, and IL-6 were determined. At 3, 6, 12, and 24 h, the changes of renal tissues were observed by HE staining, and the expression of PKC-?mRNA and protein was detected by RT-PCR and immnohistochemistry respectively. The ultrastructural changes of hepatic cells were observed under electron microscopy. 10.Content Determination of Taxol in Taxus Yunnanensis of Different Parts and Growth Time Limits

Objective To study the content of taxol in different parts of Taxus Yunnanensis and the effects of different growth time on taxol content. Method A HPLC method was developed for the detetmination of taxol content in bark, needle, twig, root from Taxus Yunnanensi, and the effects of different growth time. Results Taxol content in bark was the highest, that in root was the lowest and that in needle and twig were higher. Taxol content was higher with the growth time in needle and twig of Taxus Yunnanensi. Conclution It was significant to take taxol from needle and twig of Taxus Yunnanensis which is more than five years growth time.
