Chinese Journal of Internal Medicine 2011;50(2):140-143
Objective Research the effect of anti-inflammatory and anti-oxidation drugs on acute stroke patients. Methods 128 patients with acute ischemic stroke are divided into two groups- combined treatment group (71) and control (57). Test serum biomarkers and evaluate neurological function and living ability before and after therapy, compare between groups and intra-group. To observe the effects of combined therapy on serum biomarkers and prognosis in acute stage. Results ( 1 ) MMP-9 of combined treatment group (3.23 ±0. 99) ng/L is higher than control (2. 82 ± 4. 21 )ng/L before therapy (P <0. 05). (2) The level of pre-treatment ox-LDL-Ab in combined treatment group ( 0. 08 ± 0. 01 ) U/ml higher than posttreatment (0. 07 ±0. 01 ) U/ml (P < 0. 05 ). The level of pre-treatment NIHSS in control group (5.76 ±6.61) score higher than post-treatment (4.22 ±5.45) score,P<0.05. Conclusion Combined treatment can degrade serum MMP-9 and ox-LDL in acute ischemic stroke patients, but it can't obviously improve the earlier prognosis.

Objective To assess the incidence, types and risk factors of atherothrombotic events (AT) within 7 days after transient ischemic attack (TIA) with ABCD2 Score in Chinese patients. Methods With ABCD2 Score, we retrospectively reviewed the medical records of 198 TIA patients ,which met the inclusion criteria of our study. They were divided into a low risk group (ABCD2 ≤3) and a moderate-high risk group(ABCD2≥4) and the incidence, types and risk factors of AT within 7 days after TIA were evaluated and compared respectively with χ2 test between the two groups. Statistic significance was considered as P < 0.05. Results The order of the frequency of accompanying diseases was hypertension 68.18% (135/198), diabetes 23.74% (47/198), coronary artery disease 21.72% (43/198), ischemic stroke 15.66% (31/198) and hyperlipemia 12. 63% (25/198). AT within 7 days after TIA was observed in 6.19% (6/97) of the patients in the low risk group, all of them were suffering from TIA recurrence. In the moderate-high risk group, 14. 85% (15/101) of the patients experienced AT, including 2 eases of TIA recurrence, 10 cases of ischemic stroke, 1 case of both TIA recurrence and ischemic stroke, 1 case of angina pectoris and 1 case of myocardial infarction. There was no peripheral vascular disease or death from vascular disease. Incidence of AT within 7 days in the moderate-high risk group was significantly higher than that in the low risk group (51.70% vs 27. 27%, P < 0.05). Conclusion TIA patients with ABCD2 ≥4 are at higher risk of AT within 7 days after TIA onset.

PURPOSE: To investigate the neuroprotective effect of growth hormone(GH) on neuronal cell necrosis and apoptosis at 1 week and 3 weeks after hypoxic ischemic brain injury. METHODS: Sprague-Dawley rats, seven-day-old, were used. Rats were anesthetized with ether less than 5 minutes. The right carotid artery was cut between double ligation. And then, rats were allowed to recover for 30 minutes followed by exposure to 8% oxygen at 37 degrees C for 2 hours for hypoxic ischemic insult. The study group was divided into 2 groups, control group(N=3) and GH treated group(N=3). GH treated group received intraperitoneal injection of GH 1 IU 2 hours after hypoxic ischemic insult following daily adminstration as same dose for 5 days. Rats were decapitated at 1 week and 3 weeks after hypoxic ischemic brain injury. After then, right hippocampal CA1 and CA3 neurons of rat brains were examined. RESULTS: Necrosis was significantly less in GH treated group than control group, and was more prominent at 3 weeks in both groups. The apoptosis was not found in GH treated and control group. CONCLUSION: GH has a neuroprotective effect on neuronal cell deaths(especially necrosis) from 1 week to 3 week after hypoxic ischemic insult in neonatal rat.

Objective: To establish the quality standard for Erxiaqingxin Tablets(Cordyceps, Rhizoma Pinelliae, Radix Puerariae, Caulis Bambusae in Taenia, Fructus Aurantii Immaturus, Pericarpium Citri Reticulatae, etc.). Methods:TLC was performed to identify Pericarpium Citri Reticulatae, Fructus Aurantii Immaturus and Radix Puerariae. HPLC was used to determine the content of puerarin. Results:The study on the quality control showed that the characteristic of identification by TLC was distinct and hightly specific. The quantification method had the linear range of 0.04092~2.4552?g. The average recovery was 99.36% and RSD was 2.32%. Conclusion:The method for identification and quantification was simple, realizable and reproducible. It can be used effectively for the quality control of Erxiaqingxin Tablets.

Objective To investigate the safety and efficacy of structured triglycerides in parenteral nutrition in elderly patients with acute biliary tract infection. Methods 62 elderly patients with acute biliary tract infection under conservative therapy were randomly divided into structured triglycerides (ST) group and medium-chain triglycerides plus long-chain triglycerides (MCT/LCT) group.Patients underwent parenteral nutrition for 5 days.Levels of hepatic enzymes, blood triglycerides, nitrogen balance, fasting blood insulin and glucose were compared between the two groups after 5 days of parenteral nutrition. Results There were no significant differences in levels of hepatic enzymes, blood triglycerides and fasting blood glucose between ST group and MCT/LCT group.Plasma total bilirubin (T-Bill) levels were increased in the two groups, and T-Bill level dropped faster in the ST group than in the MCT/LCT group after 7 days of parenteral nutrition(P< 0.05).The difference in total cholesterol level dropping was significant between ST group and MCT/ LCT group (P<0.05).The cumulative nitrogen balance for 5 days had significant difference between ST and MCT/LCT groups [(4.7-±-2.2) g vs.(2.8±0.7) g,P<0.05].STG group versus MCT/LCT group had less fluctuation of fasting blood insulin level after 1,4 and 7 days of parenteral nutrition. Conclusions In the acute phase of biliary tract infection in elderly patients,ST-based parenteral nutrition can provide energy and help maintain nitrogen balance, with a little fluctuation of fasting blood insulin level.Short-term nutritional support may have better tolerance in the liver and has little effect on the change of hepatic enzymes.

Objective To explore the effect of dietary fat acids on incretin and islet function in healthy adults .Methods Before each test, healthy subjects received a 1-week pre-experiment eucaloric diet .Fifteen subjects consumed two meals containing different fat acids , including high saturated fat acid ( HSF) and high monounsaturated fat acid ( HMF) .On two separate occasions, they underwent a minimum of 1-week washout between meals .At 0,30,60,120,180 and 240 min following meal intake, the plasma concentrations of gastric inhibitory polypeptide (GIP), glucagon-like peptide-1(GLP-1) and serum concentrations of glucose, insulin, triglycerides ( TG) and free fatty acid ( FFA) were measured. Results Postprandial glucose did not increase significantly following HSF and HMF meals (P>0.05).Compared with HMF meal, significant increase in AUCins240min,AUCTG240min and AUCFFA240minwas observed following HSF meal (P<0.05).Compared with HMF meal,a significant decrease in AUCGIP30min, AUCGIP240min, AUCGLP-1 30min and AUCGLP-1 240min was found following HSF meal (P<0.05).No significant difference was observed inΔI30/ΔG30 following HSF and HMF meals (P>0.05). AUCI/AUCG was significantly lower following HMF meal as compared with HSF meal (P<0.05).Conclusion This study demonstrates that the function of GIP ,GLP-1 andβcell is affected by the dietary fat acids in healthy adults .The HMF meal may stimulate GIP and GLP-1 secretion to a greater extent than HSF meal .
