[Summary] Interleukin-2 receptor antagonist (IL-2Ra,ie,basiliximab and daclizumab),a new antibody agent,is widely employed in lowering the risk of acute rejection after organ transplantation,but it meanwhile causes increasing concerns on the effect it exerts on glucose metabolism in transplant recipients,and so far the exact effect still remains controversial.New onset diabetes after transplantation (NODAT) is one of the most influential metabolic complications affecting graft survival and patients' long-term outcomes.Some of the current researches indicate that IL2Ra may improve glucose metabolism in the transplant recipients,some show just the opposite,yet others show no effects.Hence further investigations focusing this aspect are needed.

3.Meta-analysis of Olanzapine in the Prevention and Treatment of Chemotherapy-related Nausea and Vomiting

OBJECTIVE:To systematically review the effectiveness and safety of olanzapine in the prevention and treatment of chemotherapy-induced nausea and vomiting(CINV),and provide evidence-based reference for clinical treatment. METHODS:Re-trieved from Medline,PubMed,EMBase,Cochrane Library,Clinical Trials.gov,CBM,CJFD,VIP and Wanfang Database,ran-domized controlled trials (RCT) about olanzapine (test group) versus other drugs or conventional antiemetic regimen (control group)in the prevention and treatment of CINV were collected,Meta-analysis was performed by using Rev Man 5.3 software after data extraction and quality evaluation by modified Jadad. RESULTS:Totally 19 RCTs were included,involving 1 794 patients. Re-sults of Meta-analysis showed,olanzapine can significantly improve the complete control rate of patients with acute CINV [RR=1.12,95%CI(1.06,1.18),P<0.001],delayed CINV[RR=1.26,95%CI(1.14,1.39),P<0.001],overall CINV [RR=1.62,95%CI (1.32,1.99),P<0.001] and breakthrough CINV[RR=2.05,95%CI(1.47,2.86),P<0.001],there were significant differences be-tween 2 groups;there were no significant differences in the incidence of tiredness,dizziness,lethargy and constipation(P>0.05). CONCLUSIONS:Olanzapine is safe and effective in the prevention and treatment of CINV and can be recommended in treating breakthrough and refractory CINV. Besides,olanzapine in the prevention and treatment of CINV belongs to“off-label use”,so the clinicians should have a comprehensive consideration of chemotherapy and patients’consent.
4.Applications of PBL combined with role play teaching method in the clinical teaching of emergency internal medicine

Objective To investigate the application effect of Problem based learning (PBL) combined with role play teaching method in the training of emergency internal medicine.Methods Totally 60 medical students were randomly divided into two groups.The students in the observation groop were trained with PBL teaching method while those in control group were trained with traditional teaching method.The practical performances of the students in both groups were evaluated by virtual experience of dealing with patients in the emergency department through the role play teaching method.The grasp of basic knowledge was assessed by writing tests.Finally all the students in the observation group answered a survey about the teaching methods.Results There was uo difference in the scores of the basic knowledge test between the two groups ( P > 0.05 ).The practical ability of the observation group was much better than that of the control group in the role play practice ( P < 0.05 ).Almost all the students in the observation group accept the PBL teaching method.Conelusion PBL combined with role play teaching method is effective based on the characteristics of emergency medicine.
5.Modified baculovirus system for high expression of Bombyx mori bidensovirus NS1 in silkworm.

To improve the expression of heterologous genes using baculovirus expression system, we constructed a novel shuttle vector based on the Bm-Bacmid. In the Bm-Bacmid, partial sequences of Chitinase and Cystein Protease were replaced with a tandem cassette of Cm and egfp through homologous recombination. Bombyx mori bidensovirus (BmBDV) ns1 under the control of polyhedrin promoter was inserted into the modified Bm-bacmid by transposition. For comparison, BmBDV ns1 under the control of polyhedrin promoter was also cloned in the wild type Bm-bacmid. The resulting Bm-bacmids were transfected into the cultured BmN cells to prepare recombinant virus to infect silkworms for expression of BmBDV ns1. Total proteins of hemocyte from infected silkworms were subjected to Western blotting and ELISA analysis. The yield of BmBDV NS1 1 with the modified vector was three times as much as that with the unmodified vector. The method to improve the yield of BmBDV NS1 in silkworms will facilitate the function and three-dimensional structure study of BmBDV NS1.

6.Expression of ns1 gene from Bombyx mori bidensovirus by a novel baculovirus expression system.

Baculovirus gene expression is the most popular method to make target protein in cultured insect cells. To fast determine the generation of recombinant virus in cultured cells, donor plasmid of pFastBacI was modified by introducing egfp cassette. In the modified vector, egfp cassette was under the control of ie1 promoter, and target gene cassette was under the control of polyhedron promoter. To evaluate the convenience of the genetically modified donor plasmid used in eukaryotic expression, ns1 gene from Bombyx mori bidensovirus was ligated into the donor plasmid to generate recombinant plasmid pFastBacI-[P(ie1)-egfp-sv40]-[P(polh)-ns1-sv40]. Then the plasmid was transformed into DH10B competent cells containing Bm-Bacmid vector to produce the final recombinant Bm-Bacmid with the help of transposase. The resulting recombinant Bm-Bacmid was transfected into BmN cells to generate recombinant virus, which was easily and rapidly judged by green fluorescent signal observed in BmN cells. After infection for 96 h, the BmN cells were harvested and the total protein extracted from the infected BmN cells was subjected to Western blotting analysis. The result showed that a specific protein band about 36 kDa was detected, indicating that NS1 protein was successfully expressed in the BmN cells. In conclusion, the expression of NS1 protein with the modified expression system is useful for further research on the function of NS1 protein.

7.Rescuing Bombyx mori bidensovirus in BmN cells in vitro.

Bombyx mori bidensovirus (BmBDV) has been identified as causing chronic densonucleosis in Bombyx mori specifically. The replication mechanism of BmBDV remains unknown. Its genome comprises two single stands DNA (VD1 and VD2). In order to rescue infectious virions in vitro, we obtained the total viral DNA extracted from the BmBDV-infected larvae midguts, subsequently cloned the full-length sequence of BmBDV genome fragments by PCR and constructed recombinant plasmids pMD18T-VD1 and pUC-VD2. The linear genome fragments were obtained by digesting recombinant plasmids with corresponding restriction enzymes, and then collectively transfected BmN cells by the method of liposome-embedding. We determined the replication of the virus gene by PCR with the template of demethylated total DNA extracted from the post-transfect BmN cells. Meanwhile, we collected the total proteins from the post-transfect BmN cells and the larvae midgut of feeding the post-transfect BmN cells to perform Western blotting analysis, and detected the expression of viral genes. Here we firstly confirm that infectious virions can be rescued in BmN cells by linear co-transfect method.

8. Study on public dose burden in Suzhou from medical exposure in X-ray digital radiography and computed tomography

Objective:
To estimate the effective dose burden to the public in Suzhoui induced by medical exposure from computed tomography (CT) and digital radiography (DR).
Methods:
Twenty-seven hospitals were selected by stratified random sampling. The application information was colleted from picture archiving and communication system (PACS) and radiology information system (RIS). For DR, DAP was measured by the dose-area product meter in different body parts, then the effective dose values were calculated by the DAP. For CT, effective dose was estimated by measuring CT dose index weighted (CTDIw) and scanning parameters in different parts of the body. The public dose burden caused by DR and CT medical exposure in Suzhou was estimated according to the scanning time and effective dose to each part.
Results:
The effective dose due to DR examination was abdomen AP 0.565 mSv, pelvis AP 0.280 mSv, skull LAT 0.016 mSv, skull AP 0.012 mSv, chest LAT 0.111 mSv, chest AP 0.060 mSv, thoracic spine LAT 0.100 mSv, thoracic spine AP 0.102 mSv, lumbar spine LAT 0.307 mSv and lumbar spine AP 0.152 mSv, respectively. The effective doses from CT scanning were 1.33 mSv for head, 5.75 mSv for thorax and 7.31 mSv for abdomen, respectively. The annual collective effective dose in Suzhou in 2017 from DR exposures and CT scans was 9 593.07 man·Sv, and the average annual effective dose was 0.898 mSv.
Conclusions
The contribution of CT medical radiation to the public dose is much greater than that of DR. Controlling the frequency of medical exposure and single scan dose is an effective way to reduce the public dose burden. The public dose burden from DR and CT medical exposure in Suzhou is at a high level and attention needs to be paid by relevant health administrative departments.
9.Protective effect of Liensinine on lipopolysaccharide ( LPS)-induced acute lung injury in mice

Objective To investigate the effect of Liensinine on lipopolysaccharide ( LPS)-induced acute lung injury (ALI) in mice. Methods BALB/c mice were randomly divided into six group: control group, LPS group, LPS+Liensinine (2 mg/kg, 4 mg/kg, 8 mg/kg) groups, and dexamethasone group. Acute lung injury in mice was induced by nasal instillation of LPS. After 12 h, the pathological changes of lung tissue were observed. The levels of TNF-α, IL-6 and IL-1β in bronchoalveolar lavage fluid (BALF) were detected by ELISA. The number of neutrophils in BALF was detected using Wright-Giemsa staining. Total protein content was detected by BCA protein quantification assay. The pulmonary capillary permeability was examined with Evans blue. The MPO activity, MDA content, SOD activity, and GSH content in lung homogenate supernatant were detected by spectrophotometry. The content of ROS in lung tissue was detected by flow cytometry. Results The LPS group showed inflammatory cell infiltration, thickening of bronchial alveolar wall and pulmonary congestion in the lung tissue, while Liensinine improved the lung injury. In the LPS group, the contents of TNF- α, IL-6 and IL-1β in BALF were significantly increased, the number of neutrophils and the content of total protein were significantly increased, pulmonary capillary permeability, MPO activity and MDA content were increased, SOD activity and GSH content were decreased, the content of ROS was increased; while the Liensinine group reduced the contents of TNF-α, IL-6, IL-1β in BALF, reduced the number of neutrophils and total protein content, decreased the pulmonary capillary permeability, attenuated MPO activity and MDA contents and increased SOD activity and GSH content, and reduced ROS content in the LPS-challenged lung tissue. Conclusions Liensinine protects mice from LPS-induced acute lung injury by its anti-inflammatory and anti-oxidative activities.
10.Treatment of bromhidrosis by cutting off the apocrine glands under direct vision combined with micro electrocoagulation

Objective:To investigate the efficacy of treatment of bromhidrosis by cutting off the apocrine glands under direct vision combined with micro electrocoagulation.Methods:From January 2017 to December 2017, 25 patients (50 sides) with axillary osmidrosis admitted to the First People’s Hospital of Yuhang District were treated with direct vision cutting and micro electrocoagulation to destroy the apocrine sweat glands, including 13 males and 12 females, aged 14-38 years, with an average age of 23 years. According to Young-Jin Park′s criteria for judging axillary osmidrosis, 21 cases were classified as grade 3 and 4 cases as grade 2. After tumescent anesthesia, the subcutaneous apocrine sweat glands, fat and hair follicles were cut off under direct vision. The subdermal vascular network was preserved, and the basement was destroyed one by one by using high-frequency electrocoagulation mode. After the operation, the evaluation was conducted according to the criteria for judging axillary osmidrosis formulated by Young-Jin Park.Results:The symptoms of axillary osmidrosis reached the standard of cure in 24 patients after operation, 1 case failed to reach the cure standard. Among the patients who reached the standard, there were 2 cases of necrosis of the skin flap and 1 case of hematoma after operation unilaterally, which were given actively dressing change, secondary debridement, suturing and hematoma removal, The follow-up period was 6 months to 1 year. During the follow-up period, 24 cases were cured, including 21 cases of grade one and 3 cases of grade zero. The treatment effect was stable.Conclusions:The treatment of axillary osmidrosis by cutting off the apocrine sweat glands under direct vision combined with micro electrocoagulation, exposing the apocrine sweat glands as much as possible, effectively removing the apocrine sweat glands and hair follicles, and reducing the recurrence rate of axillary osmidrosis. It is a simple, safe and effective treatment technology for axillary osmidrosis.
