1.Method of Evacuated Suture of Reducing Tension to Prevent Incision Fat Necrosis

Objective To discuss the effect of preventing postoperative incision fat necrosis by the method of evacuated suture of reducing tension. Methods The method of evacuated suture of reducing tension was used in 158 cases. Age of them was 22~83 years old,mean 52 years old. There were thick fat tissues in the sub-skin of all patients.The fat average thickness was from 4 0~8 0cm.Results The treatment group had 158 cases, of which, lumbar incision had 62 cases, subcostal incision had 5 cases and hypogastric median incision had 91 cases. Among 158 cases,there were 4 cases of incision fat necrosis, 3 cases of incision infection and the others were one-stage healing.The control group had 97 cases. Among them, there were 18 cases of fat necrosis, 4 cases of incision infection and the others are one-stage healing. Conclusions Method of evacuated suture of reducing tension to prevent incision fat necrosis has satisfactory efficacy and worth of clinical popularization and application.
2.Hospital Infections among 246 Elderly Patients:Analysis and Countermeasures

90 years old patients was 13.04%.The longer the hospitalization time the incidence among inpatients longer than 60d was 43.51%,the higher was the hospital infection rate.CONCLUSIONS There are many risk factors inducing hospital infections among elderly,there fore the monitoring must be strengthened,and the disinfection isolation and asepsis must be strictly conducted,with limited invasive manipulations,reasonable use of antibiotics,and active treatment of the primary affection,all above can effectively reduce the occurrence of hospital infection among ellerly patients.
3.The prevalence of vitamin D deficiency in intensive care unit and its influence on prognosis

Objective To investigate the prevalence of vitamin D deficiency in medical intensive care unit (ICU) and its relationship with severity of disease and prognosis.Methods A prospective study was performed to evaluate vitamin D status in 216 patients admitted to the medical intensive care unit.The incidence of hypovitaminosis D was observed.Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score,days kept in ICU and on ventilator,main laboratory findings,and mortality rate were compared among patients with different serum 25-hydroxyvitamin D [25 (OH) D] levels.Potential risk factors for mortality were analyzed by multivariate logistic regression analysis.Results One hundred and fifty-three patients (70.8%) developed hypovitaminosis D.25 (OH) D deficiency was identified in 95 (44.0%),25 (OH) D insufficiency in 58 (26.8%),and 25 (OH) D sufficiency in 63 (29.2%) patients.APACHE Ⅱ score,positive blood culture,the incidence of multiple organ dysfunction syndrome(MODS),and mortality rate were higher in deficiency group compared with the other two groups [APACHE Ⅱ score:deficiency group 25 (20,28) score,insufficiency group 22 (17,26) score,sufficiency group 19 (18,20) score,P<0.01 ; positive blood culture:deficiency group 18.9%,insufficiency group 13.8%,sufficiency group 3.2%,P=0.015 ; MODS:deficiency group 48.4%,insufficiency group 43.1%,sufficiency group 25.4%,P=0.025; mortality rate:deficiency group 40%,insufficiency group 24.1%,sufficiency group 15.9%,P =0.003].25 (OH)D levels were negatively correlated with APACHE Ⅱ score and mortality rate (r =-0.325,P<0.01 ; r=-0.276,P<0.01,respectively).Analysis by multiple logistic regression demonstrated that 25 (OH) D deficiency (OR =3.005,95 % CI 1.321-5.875,P =0.008) was independent risk factor for mortality.Conclusions This study demonstrates that vitamin D deficiency is highly prevalent among patients admitted into ICU.Vitamin D deficiency is associated with disease severity,and seems to be an independent risk factor for mortality.
4.Control and testing methods for blood bump in heart assistant and replacement devices.

Using extracorporeal circulation for cardiopulmonary bypass is the first step for the successful cardiac surgery, and the blood pump is the key component in extracorporeal circulation devices, the control and measurement of mechanical-fluid parameters such as flow rate and pressure are very important to guarantee the successful cardiac surgery. This paper reviews several control methods of the blood pump based on the flow rate/pressure, heart rate, ventricular work and blood assist index, and also discusses the direct and indirect measurements of the flow rate and pressure. This review concludes with the main possible trends for the further development of the blood pump control and testing methods.

5.Magnetic field numerical calculation and analysis for magnetic coupling of centrifugal blood pump for extracorporeal circulation.

This paper mainly studies the driving system of centrifugal blood pump for extracorporeal circulation, with the core being disc magnetic coupling. Structure parameters of disc magnetic coupling are related to the ability of transferring magnetic torque. Therefore, it is necessary to carry out disc magnetic coupling permanent magnet pole number (n), air gap length (L(g)), permanent magnet thickness (L(m)), permanent magnet body inside diameter (R(i)) and outside diameter (R(o)), etc. thoroughly. This paper adopts the three-dimensional static magnetic field edge element method of Ansys for numerical calculation, and analyses the relations of magnetic coupling each parameter to transmission magnetic torque. It provides a good theory basis and calculation method for further optimization of the disc magnetic coupling.

According to the clinical requirements of cardiopulmonary bypass surgery, this paper established a simulation system for cardiac surgery which consists of venous reservoir, variable balance chamber, blood suction bag, ventricle suction bag, resistance valves, pressure gauges and tubings. Using the proposed system, perfusionists can mimic the implementation of pre-established surgery strategy, predict various abnormal conditions in the operation, and accordingly take the urgent actions so as to improve the success rate of surgery and to ensure the safety of patients.

8.Neuroprotective effect of paracrine extracts derived from human adipose stem cells on white matter injury of neonatal rats

Objective To explore the effect of paracrine extracts derived from human adipose stem cells on white matter injury of neonatal rats and to compare the difference of therapeutive effect between the cerebellum medulla oblongata pool injection and the jugular vein injection.Method A total of 73 three-day-old SD rats were chosen to establish the model of white mater injury.After 24 hours,the 73 rats were randomized into the experimental group (n =46) and the control group (n =27).Then the experimental group was reclassified into ventricular group (n =23) and intravenous group (n =23).In the ventricular group,the paracrine extracts of human adipose stem cells was injected locally into the cerebellum medulla oblongata pool injection,while the extracts was injected into the jugular vein in the intravenous group.The control group was reclassified ventricular control group (n =15) and intravenous control group (n =12),and equivoluminal saline was injected the same way as the experimental group.Frozen sections of the brain tissue from 3 rats of each experimental group one day after injection were stained with fluorescein-conjugated streptavidin to study the distribution of the extracts.The brain tissue of 3 rats from each subgroup 3 days after injection were stained with hematoxylin eosin (HE) to observe the pathomorphological changes.While 7 days later,myelin basic protein (MBP) of white matter which was obtained from 7 rats of each group was detected by immunofluorescence staining.28 days after injection,the remaining rats were assessed by neurobehavior tests.For the rats that died during the experiment,the same number of the rats would be substituted in this study.Result The paracrine extracts were found to transfer to the brain lesion area,and the amount of the extracts was more in the ventricular group.The results of the HE staining showed that the white matter injury was more severe in the ventricular control group,and extensive area of infarction were found in this group.White matter injury was mild in the experimental group,and the structure of the corpus callosum was more complete in the ventricular group.MBP semi-quantitative scores of the ventricular group (0.7 ± 0.3) and intravenous group (1.7 ± 0.3) were lower than those of ventricular control group (3.4 ± 0.4)and intravenous control group(3.3 ±0.3).And the MBP scores of ventricular group was significantly lower than that of intravenous group (P < 0.05).The scores of the neurobehavioral tests of the experimental group were significantly higher than those of the control group,while the scores of the ventricular group were significantly higher than those of the intravenous group (P < 0.05).Conclusion The paracrine extracts derived from adipose stem cells could improve the prognosis of white matter injury,and cerebellum medulla oblongata pool injection showed better curative effect than the jugular vein injection.
9.Effects of non-invasive limb ischemic preconditioning on endothelial function after myocardial ischemia-reperfusion injury

Aims To investigate the protective effects of noninvasive limb ischemic preconditioning (LIPC) on myocardial ischemia-reperfusion (I /R)injury,and to explore the mechanism.Methods Healthy male Wistar rats were divided randomly into I /R,I /R +LIPC,I /R +5-Hydroxydecanoate (5-HD)and I /R +LIPC +5-HD groups.The I /R +LIPC and I /R +LIPC-5-HD groups of rats were subjected to three cy-cles of LIPC induction per day with 5 min of reperfu-sion after occlusion for 5 min at the left hind limb for 3 days.All rats were subjected to myocardial I /R injury on the fourth day.The I /R +5-HD and I /R +LIPC-5-HD groups of rats were given the inhibitor of ATP-sen-sitive potassium channel 5-HD before and during myo-cardial I /R injury. Results Compared with I /R group,LIPC reduced myocardial infarct size (P <0.05),lowered cardiocyte apoptosis index and Fas, FasL positive cell number (P <0.01 ),increased the reduced nitric oxide (NO)/endothelin (ET)-1 ratio (P <0.05)in serum in I /R +LIPC group.5-HD a-bolished the protective effects induced by LIPC in I /R+LIPC-5-HD group.Compared with normal myocardi-al tissue,expression of mir-30a-3p was increased in I /R group (P <0.01 )and was decreased in LIPC group (P <0.01 ).Conclusion LIPC alleviates myocardial I /R injury and improves endothelial function. The mechanism may be related with the opening of ATP-sensitive potassium channel,regulating the balance be-tween NO and ET-1 and decreasing the expression of myocardial mir-30a-3p.
10.The Infulence of Factors on Auditory and Speech Performances in Preschool Children with Unilateral Cochlear Implantation

Objective To investigate the affecting factors on auditory and speech performances in preschool children with unilateral cochlear implantation (CI) .Methods The clinical data of the preschool children (n=165) with unilateral cochlear implantation in the Second Xiangya hospital from January 2006 to April 2013 were collected . These children received rehabilitation according to the method recommended by the China Rehabilitation Research Center for Deaf Children ,and the data were analyzed retrospectively .The categories of auditory performance (CAP) and speech intelligibility rating (SIR) were used to assess their auditory and speech performances .The relationships between the performance and gender ,implanted age ,genotype ,inner ear malformation ,history of hearing aid were evaluated .Results Implanted ages and genotypes were associated with the auditory and speech performance of par‐ticipants (P<0 .05) ,while genders ,hearing aid experience ,and inner ear malformations(enlarged vestibular aque‐duct syndrome ,EVAS)were not significant related (P<0 .05) .Children were found to have achieved better CAP and SIR growths when CI was implanted during 1~3 years old and 2~4 years old ,respectively (P<0 .05) .The outcomes of CI recipients with GJB2 mutation were significantly better than those of the GJB2-nonrelated CI recipi‐ents (P<0 .05) .Conclusion This study provides evidence that CIs during first 1~3 years old having better auditory rehabilitation results than those of during 4~6 years old ,and CIs during 2~4 years old obtaining a better speech development in the first 12 months after operation .Deaf children with GJB2 mutation show better auditory and speech performances after CIs than those of the peers without GJB2 mutation .CIs can be effectively performed in deaf children associated with EVAs as in those without EVAS .
