1.Mantle cell lymphoma primarily occured in lymph nodes:A clinicopathologic study

Purpose To study the clinicopathologic and immunohistochemical characteristics of mantle cell lymphomas(MCL). Methods Six cases of MCL in lymph nodes were collected and immunohistochemical stains for CD45,CD20,CD79,CD45RO,CD30,CD68,CD43,CD5,cyclinD1,bcl-2,c-myc,IgD and IgM performed. Results According to the histological pattern, MCL could be divided into 4 subtypes, mantle zone type(1 case), nodular type (1 case), diffuse type (2 cases) and blastoid type (2 cases). The tumor cells were pan-B+, IgD+, CD43+, cyclinD1 + (5/6),CD5+(4/6). Conclusions MCL is a kind of special immunophenotypic B cell lymphoma. The different histiological subtype may have a different prognosis. The differential diagnosis includes the others B cell lymphomas, for instance, marginal zone B cell lymphomas (MZL),follicular lymphomas(FL), and choronic lymphocytic leukemia/small lymphocytic lymphomas (CLL/SLL).
2.The expression of T-cell receptor Ⅴβ subfamily in hepatitis B virus-related acute-on-chronic liver failure patients and its clinical significance

Objective To investigate the expression and clinical significance of T-cell receptor (TCR) Ⅴβ subfamily in hepatitis B virus (HBV)-related acute-on-chronic liver failure (HBV-ACLF) patients.Methods Twenty-eight patients with HBV-ACLF (HBV-ACLF group) and 32 patients with chronic hepatitis B flare (CHB-F group),who were treated in The Second People's Hospital from Oct.2010 to Mar.2012,and 20 healthy controls (HC group) were included in the study.Reverse transcriptase-polymerase chain reaction was used to detect the levels of TCR Ⅴβ subfamily and enzymelinked immunosorbent assay was used to detect the levels of serum cytokines [interleukin (IL)-2,IL-4,IL-6,IL-10,interferon (IFN)-γ and tumor necrosis factor (TNF)-α)] in the three groups.The comparison among three groups was done by one-way analysis of variance and the comparison between two groups was done by LSD-t test or rank sum test.Results The three groups had similar gender and age distribution (all P>0.05).The HBV-ACLF group had significant different profiles of total bilirubin,albumin,prothrombin activity,international normalized ratio and cholesterol tatol compared with the CHB-F group (all P<0.05).For patients in the HBV-ACLF group,the serum IL-2,IL-4,and IL-10 levels were lower(all P=0.000),and the IL-6 and IFN γ levels were higher than those of the HC group (all P=0.000).The IL-4,IL-10,and TNF-α levels in the CHB-F group were also significantly lower than those of the HC group (all P=0.000).Compared with the CHB-F group,the HBV-ACLF group had significantly lower IL-2,IL-10,and TNF-α levels (P=0.003,0.002,0.004),and higher IL-6 and IFN-γ levels (P=0.015,0.006).By one-way analysis of variance,there were significantly differences of △Ct1,△Ct5,△Ct7,△Ct12,△Ct15,△Ct20,△Ct22,and △Ct23 among the three groups (H=20.368,14.368,19.500,31.532,19.985,19.116,41.752 and 20.649,all P<0.05).Conclusion The expression levels of TCR Ⅴβ subfamily and cytokines are changed in HBV-ACLF patients.
3.Rapid Screening and Confirmation of Perfluorinated Compounds and Their Precursors in Fish Muscle by Solid Phase Extraction Purification and Liquid Chromatography Coupled with Quadrupole/Exactive Orbitrap Mass Spectrometry

A method was developed for the rapud screenung and confurmatuon of 18 perfluorunated compounds ( PFCs) and 21 precursors un fush muscle by solud phase extractuon and luquud chromatography coupled wuth quadrupole/exactuve orbutrap mass spectrometry (LC-Q Exactuve Orbutrap MS). In thus method, the sample was furstly extracted wuth acetonutrule-water (90∶10, V/V), cleaned-up by solud-phase extractuon usung Oasus PRuME HLB column. Then, 39 target compounds were separated on a BEH C18(2. 1 mm×100 mm, 1. 7 μm) column, and the background unterference caused by LC system was elumunated by a short C18 HPLC column located between the muxer and the auto sampler. In the process of quantufucatuon and qualufucatuon, a full MS/dd-MS2 experument was adopted un mass spectrometry acquusutuon, accompanued wuth the full scan MS date for quantufucatuon, as well as the date dependent MS2 product uon spectra for qualufucatuon. The mass accuracy error was less than 3×10-6, and the calubratuon curves were lunear well wuth correlatuon coeffucuent over 0. 99. The lumuts of detectuon ranged from 0. 02 to 0. 50 μg/kg. The average spuked recoverues for 39 target compounds were between 61 . 7% and 122 . 0%, wuth relatuve standard deruvatuons ( RSDs ) from 6 . 9% to 18 . 8%. The developed method was successfully applued to the sumultaneous determunatuon of 18 PFCs and 21 precursors un fush muscle wuth satusfactory results.
4. Reconstruction of extensive soft-tissue defect of leg using free anterolateral thigh flap with the contralateral leg vessels as the recipient vessels

Objective:
To explore the clinical effects of the reconstruction of extensive leg defects using the free anterolateral thigh flap with the contralateral leg vessels as the recipient vessels.
Methods:
From January 2012 to January 2018, ten patients were treated with severe and extensive leg defects in the department of orthopedics of the First Affiliated Hospital of Nanchang University. There were 7 males and 3 females with an average age of 35, from 17 to 56. There were no main vessels for angiogenesis around the wounds in all cases. The size of defects ranged from 20 cm×13 cm to 29 cm×15 cm. The position of defects were anterior of shank in 5 cases, medial in 3 cases and medial posterior in 2 cases. The various flaps were harvested from the anterolateral thigh region of healthy leg and transferred to repair the leg defects. The healthy vessels of the contralateral leg were chosen as the recipient vessels. The musculocutaneous flap, fascia flap or perforator was removed according to the size of the defect and whether it was necessary to fill the dead space of the wound. The limbs were placed in parallel position and was fixed by external fixator. The pedicle division training was started 1 week after operation, the period of pedicle division and external fixator removing was from 21 days to 32 days. When the pedicle was divided, the vascular end of the limb and the distal end were anastomosed to re-established the continuous vessels.
Results:
All 10 flaps survived completely after surgery. The size of flaps ranged from 23 cm×14 cm to 32 cm×16 cm. The recipient vessels that were used included the posterior tibial vessels in 5 cases and anterior tibial vessels in the remaining 5 cases. All the vessels in flap pedicle were anastomosed to the recipient vessels in an end-to-end fashion. The anastomotic sites and vascular bundles were covered by using a local flap in 2 cases, skin tension reducer in 1 cases, and free skin graft in the remaining 7 cases. Very mild infection occurred in one case and was controlled by dress changing. A small-sized necrosis of the grafted skin occurred in another patient. All patients were followed up for 6 to 18 months with an average of 12 months. The function of the lower extremities almost recovered. All patients were happy with the final functional and aesthetic outcomes.
Conclusions
Although there some drawbacks of the technique, such as long-term immobilization of the lower extremities, multiple staged surgeries, for strictly selected patients, the healthy vessels of the contralateral leg could be served as recipients vessels when a free myocutaneous, fasciocutaneous, or perforator flap was used to reconstruct the extensive and severe injury of the leg, particularly in the absence of usable vessels in the ipsilateral leg.
5.The efficacy of SLAP repair and biceps tendon tenotomy and fixation for type Ⅱ SLAP lesions in shoulder arthroscopic:a Meta-analysis

Objective To evaluate the effects of SLAP repair and biceps tendon tenotomy and fixation in treating type Ⅱ superior labrum from anterior to posterior in shoulder arthroscopy.Methods PubMed,CNKI,VIP,and Wanfang database were searched electronically to screen randomized controlled trials and non randomized controlled trials that comparing two surgical methods for type Ⅱ SLAP lesions.In addition,we also retrieved the domestic orthopedic magazine and searches references that we had included in the present study.According to the criterion,screening literatures,data extraction and quality assessment were conducted by two reviewers independently.Meta-analysis was performed by using RevMan 5.3 software to evaluate UCLA scores,ASES scores,VAS scores and SST scores.Results A total of 7 articles were included in the present study,including 3 literatures in Chinese and 4 in English.There were 2 randomized controlled trials and 5 non randomized controlled trials.Two hundred and fifty-six patients were enrolled in the present study,including one hundred and twenty-five patients with SLAP repair and one hundred and thirty-one patients with biceps tendon tenotomy and fixation.Meta-analysis showed that there was significant difference in total score of UCLA [MD=2.55,95%CI(0.57,4.53),P=0.01] and shoulder function in the UCLA scoring system [MD=0.62,95% CI(0.21,1.02),P=0.003] compared with SLAP repair.There was no difference in the UCLA scoring system,including pain [MD=0.99,95%CI(-0.15,2.14),P=0.09],flexion [MD=0.16,95%CI(0.01,0.32),P=0.06],strength [MD=0.03,95%CI(-0.15,0.22),P=0.74],satisfaction [SMD=0.18,95% CI(-0.57,0.93),P=0.64] compared with SLAP repair.There was significant difference in ASES scores [MD=6.32,95%CI(2.55,10.08),P=0.001] between two groups.There was no difference in VAS scores [MD=0.54,95%CI (-0.64,1.72),P=0.37] and SST scores [MD=0.81,95%CI(-0.23,1.86),P=0.13] between two groups.Conclusion The UCLA scores,ASES scores and surgical results of biceps tendon tenotomy and fixation are superior to SLAP repair in treating type Ⅱ SLAP lesions.However,there is no significant advantage in pain,flexion,strength and satisfaction between the two groups.
